The ability to directly harvest thin and superthin perforator flaps without jeopardizing their vascularity depends on knowledge of the microsurgical vascular anatomy of each perforator within the subcutaneous tissue up to the dermis. In this paper, we report our experience with ultrahigh-frequency ultrasound (UHF-US) in the preoperative planning of thin and superthin flaps. Between May 2017 and September 2018, perforators of seven patients were preoperatively evaluated by both ultrasound (using an 18-MHz linear probe) and UHF-US (using 48- and 70-MHz linear probes). Thin flaps (two cases) and superthin flaps (five cases) were elevated for the reconstruction of head and neck oncologic defects and lower limb traumatic defects. The mean flap size was 6.5×15 cm (range, 5×8 to 7.5×23 cm). No complications occurred, and all flaps survived completely. In all cases, we found 100% agreement between the preoperative UHF-US results and the intraoperative findings. The final reconstructive outcomes were considered satisfactory by both the surgeon and the patients. In conclusion, UHF-US was found to be very useful in the preoperative planning of thin and superthin free flaps, as it allows precise anticipation of very superficial microvascular anatomy. UHF-US may represent the next frontier in thin, superthin, and pure skin perforator flap design.

Ultrasound diathermy is widely used for the treatment of musculoskeletal disorders and other soft tissue injuries. Its use as a therapeutic modality is believed to be safe, with very few reported complications. Here, we report two patients who developed focal bone marrow abnormalities after receiving ultrasound diathermy. Both patients' magnetic resonance (MR) evaluations revealed linear subchondral bone lesions of the superolateral humeral head similar to those in osteonecrosis. The patients' symptoms subsequently improved, and available follow-up MR evaluation revealed near complete resolution of bone lesions. These findings suggest that ultrasound diathermy, and its interaction with bone tissue through thermal mechanisms, can cause focal bone marrow abnormalities. Furthermore, the bone marrow abnormalities seem to be transient, resolving upon cessation of ultrasound diathermy, therefore osteonecrosis should be differentiated from this temporal lesion.
In order to measure the distance from the bottom in the ocean we use ultrasound altitude sonars. The manganese nodule pick-up device developed by KRISO is also using an altitude sonar to control the gap between the pick-up head and sea bottom. This paper describes the performance of the altitude sonar by an experimental method. The experiment was performed with four ground models in a small basin, Manganese nodule models and water-bentonite mixture was used for setting up the ground models. Buttorworth filter was applied to remove the noise caused by a servo motor and its controller. The results show that the altitude sonar gives a good estimation of the types and slopes of the bottom as well as the distance.

The expanding scope of interventional musculoskeletal procedures has resulted in increased pressure on general radiologists. The confidence of general radiologists in performing ultrasound-guided musculoskeletal procedures varies with their clinical exposure. This didactic review provides a methodologically and clinically oriented approach to enhancing user understanding and confidence in performing ultrasound-guided musculoskeletal procedures. The body of the text is accompanied by figures depicting the procedural approach, injection site, and labeled ultrasonography images. This paper aims to provide a teaching and bedside aid for education on and the execution of musculoskeletal procedures to ensure the provision of quality health care.
Acoustical imaging has made brilliant progress in the medical science field, and has also made much progress in the nondestructive testing and under water acoustics applications since doctor Dussik brother has studied about possibilities of making images of brain by recording variations in the intensity of ultrasonic beam from head in 1937. In this paper an acoustical image is reconstructed with the power spectra analysed by impulse ultrasound wave generated by electrodynamic transducer(EDT). The EDT generates the impulse ultrasound of 77KHz in center frequency and 120KHz in bandwidth at -20dB by 1200V exciter in this experiment. The impulse ultrasound has the dominant frequency components of 47KHz, 177KHz, 110KHz and 155KHz. The U shape object is adopted in making an acoustical image. The resulted spectral acoustical images are different from the optical view of the U shape object. However the image reconstructed from 110KHz spectrum is very similar to the original optical shape of the object. Even KHz level impulse sound of 70$\mu$sec pulse width is found to be useful in reconstructing acoustical imaging improvement.
Purpose: This paper proposes proper and effective neck exercises by comparing the deep and superficial cervical flexor muscle activities and thickness according to the pressure level during cranio-cervical flexion exercises between a normal posture group and forward head posture group. Methods: A total of 20 subjects (8 males and 12 females) without neck pain and disabilities were selected. The subjects' craniovertebral angles were measured; they were divided into a normal posture and a forward head posture group. During cranio-cervical flexion exercises, the thickness of the deep cervical flexor neck muscle and the activity of the surface neck muscles were measured using ultrasound and EMG. Results: The results showed that the thickening of the deep cervical flexor was increased significantly to 28 and 30 mmHg in the forward head posture group. The sternocleidomastoid muscle activity increased significantly to 24, 26, 28, and 30 mmHg in the forward head posture group. The anterior scalene muscle activity increased significantly to 26, 28, and 30mmHg in the forward head posture group. A significant difference of 26, 28, and 30 mmHg in the sternocleidomastoid and anterior scalene muscles was observed between two groups. Conclusion: To prevent a forward head posture and maintain proper cervical curve alignment, the use of the superficial cervical flexor muscles must be minimized. In addition, to perform a cranio-cervical flexion exercises to effectively activate the deep cervical flexor muscles, 28 and 30 mmHg for normal posture adults and 28 mmHg for adults with forward head postures are recommended.
Intravenous pyogenic granuloma, commonly known as intravenous lobular capillary hemangioma, is a rare benign tumor of the vein. It rarely occurs in the neck, and its character is not enough to diagnosis clinically. It could be diagnosed with preoperative radiologic examinations such as ultrasound and computed tomography and typical pathologic findings that demonstrate lobules of multiple capillaries lined with flattened endothelial cells admixed with fibromyxoid stroma. The authors report a case of a 32-year-old male who presented with a palpable neck mass for one month with a review of the literature. He was successfully treated with resection, including the tumor and normal external jugular vein, without any complications.
Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.
Background: The purpose of this study was to identify the effects of the blood flow on carotid and vertebral artery during the forward shift on head. Methods: A 20-year female healthy subject participated in this study. This study was set up the forward shift on head at the 3cm and 6cm in a shoulder midline (acromion). Measurement method were using duplex ultrasound with colour doppler imaging for the blood flow on carotid and vertebral artery. Results: Carotid artery was increased the blood flow and vertebral artery was decreased the blood flow during forward shift on head. Conclusions: These findings suggest that carotid and vertebral artery changes to the blood flow during forward shift on head. Therefore, we should be consider that hemodynamic factor when apply to the therapeutic exercise for patients of forward head posture.
Objective: The aim of this study was to determine the effect of abdominal-compression belt in one leg standing on balance in normal adult. Background: With the effects of increased intra-abdominal pressure, the abdominal-compression belt is contributing to a static balance control. However, specific study is still insufficient. Method: Forty subjects were randomly allocated to two groups: control(n=20) and experimental group(n=20), respectively. The experimental group used an abdominal-compression belt, whereas the control group did not that. All subjects were educated using pressure biofeedback unit and ultrasound imaging for exact application by abdominal-compression belt. Main outcome measurement was used a general stability index, fourier harmony index, weight distribution index, and fall index in tetrax balance system. Results: Experimental group improved significantly on general stability, only 2 factors(eyes closed with head turned forward and eyes closed with head turned backward) among fourier harmony index, and fall index, However, weight distribution index did not revealed significant difference. Conclusion: The findings suggest that application of abdominal-compression belt could be effective on improving balance ability in one leg standing of normal adults. Application: The results of the abdominal-compression belt might help to control balance in workers.
