이미지 가이드 시스템 기반 초음파 검사 교육 기법 개발: 예비 연구 (A Study on the Development of Ultrasonography Guide using Motion Tracking System)

  • 유방암은 현대 여성들에게 가장 많이 발생하는 3대 암 중 하나로, 발생률이 급격하게 증가하고 있다. 가족력이 높고, 15% 정도의 사망률이 있어 고위험군에 속하므로 조기 검진 후 꾸준한 관리가 필요하다. 암을 진단할 수 있는 여러 장비 중 초음파는 위험성이 적고, 실시간으로 진단할 수 있다는 장점이 있다. 그러나 초음파 검사는 검사자의 기술에 따라 결과가 크게 달라진다. 이를 보완하기 위해 모션트래킹 기술을 접목하고자 한다. 모션트래킹은 삼차원의 공간에서 대상의 움직임에 따라 위치를 특정하고 분석하는 기술이다. 그렇기에 실시간 제어가 가능하고, 복잡하고 빠른 움직임도 실시간으로 기록할 수 있는 장점이 있다. 이러한 장점을 활용한 초음파 검사용 이미지 가이드 시스템 제작을 목표로 하였다. 이를 위해서 초음파스캐너의 위치를 3차원적으로 추정할 수 있는 자체 제작 완드(wand)를 설계하였고, 제작된 완드를 기반으로 초음파 영상의 위치를 추정하는 벡터 연산 알고리즘을 개발하였다. 이후, 연속 촬영을 통하여 3차원 공간에서 프로브의 위치와 초음파 영상의 위치를 나타내는 것까지 완료하였다. 이러한 실험은 Optitrack 사의 Primex 41 카메라를 초당 120 frame (Hz)으로 영상을 획득하며, 성공적으로 초음파 영상의 3차원 위치를 추정할 수 있었다. 이 실험 과정을 통해 초음파 검사와 모션트래킹의 접목으로 가이드 제작의 가능성을 확인하였다. 차후 추가적인 연구를 통해 초음파 검사 가이드를 제작하여 검사자의 기술과 상관없이 질 좋은 영상을 획득할 수 있기를 바란다.

중년여성의 정량적 초음파(Quantitative Ultrasound)로 측정한 골다공증 정도와 위험인자 (Osteoporosis Measured by Quantitative Ultrasound and Its Risk factors in Middle-aged Women, Gyeonggi-do, Korea)

  • Purpose: To identify Osteoporosis and the related risk factors in middle-aged women, the descriptive survey was done. Method: The subjects were measured in 465 residents who were 40-60 aged healthy women. They underwent ultrasound measurement and health examination in G city's Health Center in Gyeonggi-do, Korea from July 2000 to March 2001. The self-reported questionnaire consisted of total 17 items about risk factors related to osteoporosis. The evaluation of bone density was based on ultrasound measurements of right heel. All data were analyzed by SAS-PC Program. Result: Mean age of the subjects was 46.7. Mean T score related to bone density was -1.30, ranged from -3.52 to 3.06. In diagnosis classification according to T score, 74.8% of subjects was normal, 12.9% was osteopenia, 12.3% was osteoporosis. Among risk factors, there were significant differences by age (t=15.35. p=0.000), parity (F=12.81, p=0.000), menopause status (t=22.05, p=0.000), period after menopause (F=5.20, p=0.006). The higher frequency of delivery, postmenopausal and longer period after postmenopause of subjects had the lower the bone density. Conclusion: It would be necessary to develop and apply the community-based health promotion program for middle-aged women to prevent osteoporosis.

소아 복부 종괴의 감별진단 (Differential diagnosis of abdominal masses in children)

  • Enlargement of organs or other solid tissues usually presents as an abdominal mass. Often, abdominal masses in children are found by an unsuspected parent or by a physician during a routine examination. Most masses have no specific signs or symptoms. Abdominal masses in children require immediate attention. History and physical examination may provide clues to the diagnosis. Ultrasound examination is the most useful screening test in most cases and may identify the organ involved and clarify whether the mass is solid or cystic. CT may be necessary to make a more precise diagnosis, especially solid masses. MRI is occasionally is helpful for specific abnormalities.

구순열 태아에서 3D 산전 초음파를 이용한 치조열 및 구개열의 동반 유무 진단 및 산전상담 (Prenatal Diagnosis of Accompanying Alveolar Cleft and Cleft Palate in Fetuses with Cleft Lip Using Prenatal 3D Sonographic Identification and Antenatal Counseling)

  • Purpose: Cleft lip and/or palate is the most common congenital facial anomaly whose incidence is about 1 in 500~1000 live births. As this anomaly may be associated with the serious chromosomal anomalies or the multiple organ abnormalities resulting in the fetal loss or perinatal maternal morbidity and mortality, careful prenatal counseling with early and accurate detection is important. Although conventional prenatal ultrasound(US) examination in midterm pregnancy has been applied for screening of cleft lip, there are definite limitations in the diagnosis of accompanying cleft palate or alveolar cleft. We applied high-resolution 3D US along the serial axial, coronal and sagittal plane so that we could diagnose the cleft palate and/or alveolar cleft in fetuses with cleft lip. Methods: From May 2005 to September 2005, 20 fetuses with cleft lip were examined with prenatal 3D US. Average maternal age was 28.8 years old(24-35 years old), and average gestational age was 24.8 weeks(17.6 to 34.2 weeks). Consecutive axial, coronal and sagittal multislice view were obtained via prenatal 3D US examination and diagnosis of cleft palate and/or alveolar cleft in cleft lip fetuses was followed. Results: With noninvasive and safe prenatal 3D US examination, 17 of 20 cleft lip fetuses were demonstrated to have cleft palate and/or alveolar cleft. Prenatal counseling according to the result was made. Conclusion: Existing prenatal US examination is suitable for screening the cleft lip fetuses but has limitation in identifying the related existence of cleft palate and/ or alveolar cleft. Authors verify the presence of cleft palate and/or alveolar cleft acquiring the successive multislice axial, coronal, and sagittal view with prenatal 3D US examination. Therefore, prenatal 3D US examination could be regarded as a noninvasive and secure screening modality in fetuses with cleft lip for confirming whether cleft palate and/or alveolar cleft is accompanied.

뉴로-퍼지를 이용한 스펙클 제거 (Speckle Reduction based on Neuro-Fuzzy Technique)

  • 의용 초음파 장비는 X-Ray, CT, MRI 등 다른 의료 장비보다 휴대성과 안전성면에서는 장점이 있지만 진단 시 해상도와 화질 저하를 유발하는 스펙클이 생기는 단점이 있다. 그러나 단순한 스펙클 잡음의 제거는 경계선 정보의 손실을 발생시킬 수도 있다. 이에 본 논문에서는 효과적인 스펙클 제거와 손실 없는 경계선 검출을 위해 뉴럴네트워크와 퍼지 클러스터링을 이용한 뉴로-퍼지 스펙클 제거 방법을 제안하였다. 제안된 방법은 입력된 의용 초음파 영상에 대해 먼저 퍼지 클러스터링을 적용하여 세 영역으로 나누고 이후 각 영역별로 별도의 뉴럴 네트워크를 적용하는 방법이다. 실제 실험 및 기존 방법들과의 정성적?정량적 비교 분석을 통해 제안 방법의 유용성을 검증하였다.

초음파 생체 현미경을 이용한 증상이 없는 홍채모양체 종양의 진단 1 증례 (Diagnosis of an Asymptomatic Iridociliary Tumor using Ultrasound Biomicroscopy in a Dog)

  • A 10-year-9-month-old spayed female Shih Tzu was presented with ocular discharge and corneal opacity to Veterinary Medical Teaching Hospital of Seoul National University. Complete ophthalmic examination revealed bilateral keratoconjunctivitis sicca accompanied by the severe corneal neovascularization and edema in the right eye (OD). Ultrasound biomicroscopy incidentally showed an iridociliary mass located at 6~7 o'clock in the OD. No evidence of metastasis was observed on thoracic and abdominal radiography. Enucleation was selected and performed on the OD, considering the risk of metastasis or recurrence. Non-invasive and pigmented iridociliary adenoma was confirmed on histopathological evaluation.

The Therapeutic Value of Ultrasound-Guided Irrigation in Sialodochitis: A Case Report

  • In the present case study, we reported a female patient with sialodochitis fibrinosa in which a conservative ultrasound (US)-guided irrigation method was used to relieve her symptoms. A 30-year-old female patient visited Kyung Hee University Dental Hospital with chief complaints of persistent swelling and discomfort in the left facial area. Her primary symptom was facial swelling that was accompanied by neuropathological symptoms, such as itching, tingling, and warmth that usually continued for 3 to 4 days. During clinical examination of orofacial area, mild swelling and fever were noted in the left face including parotid region, and mild induration could be felt at the corresponding site; sialadenitis of the left parotid gland was tentatively diagnosed. Herein, we performed intraductal irrigation of the left parotid gland in three times per month and prescribed some medication. Thus, her signs and symptoms have been improved, and she did not experience a recurrence for 12-month follow-up to date. This study can support the usefulness of the US-guided irrigation method to treat the sialodochitis fibrinosa without remarkable side effects.

의인성 신경손상을 모방한 손뒤침근내 결정종으로 인한 뒤뼈사이신경병증 증례 (A Case of Posterior Interosseus Neuroapathy due to Intrasupinator Ganglion Mimicking Iatrogenic Injection Nerve Injury)

  • Posterior interosseus (PI) nerve compression is a rare form of compressive neuropathy. It can cause pain along the radial nerve course and weakness of radial nerve innervating muscles without sensory symptoms. A 65-year-old man visited our institution complaining weakness of finger extension and thumb abduction after 2 times of injections at the right elbow in local clinic. The patient's clinical history and physical examination implied an iatrogenic radial nerve injury caused by the injection. The electrophysiologic study revealed of posterior interosseus neuropathy (PIN) with incomplete conduction block. However, the ultrasound study showed that the PI nerve was compressed by an anechoic cyst. The magnetic resonance imaging also confirmed of a ganglion cyst, not a hematoma. After repeated aspirations and a steroid injection, the electrophysiologic study showed recovery of motor weakness. Despite of the clue which implying an iatrogenic injury, clinician should consider other possibilities such as ganglion cysts and ultrasound guided aspiration and steroid injection could be an effective option for conservative management.

  • 정형외과 영역에서 초음파를 이용한 진단과 치료가 광범위하게 사용되고 있다. 특히 족부 및 족관절에서의 초음파를 이용한 진단 족부 및 족관절의 해부학적 특성상 매우 유리한 것으로 알려져 있다. 족부 및 족관절에 발생하는 다양한 질환의 원인과 치료를 시행하기 위해서는 족부 및 족관절의 해부학적, 생역학적 특징을 잘 이해하여야 한다. 족부 및 족관절은 총 28개의 뼈와 많은 수의 인대, 건으로 이루어져 있으며 이들간의 유기적인 결합을 통해 신체를 지탱하는 정적 기능과 걷는 동적 기능을 동시에 수행할 수 있는 복잡한 구조 물이다. 그리고 작은 공간에 여러 구조물이 밀집되어 있어서 질환이 있는 부위의 정확한 이학적 검사가 요구된다. 그러므로 해부학적, 생역학적 지식을 바탕으로 이학적 검사를 체계적으로 시행함으로써 초음파를 이용한 진단 및 치료에 정확성을 기할 수 있다.

Ultrasonic Measurement of Tissue Motion for the Diagnosis of Disease

  • Ultrasonic pulsed Doppler velocimetry has become a standard international method of classifying carotid disease. Because the measured angle adjusted velocity increases as the Doppler angle increases, examinations should be performed at a convenient standard Doppler examination angle. An angle of 60 degrees is achievable throughout most examinations. Multiple Doppler viewing angles allow the acquisition of velocity vectors during the cardiac cycle, revealing the complex velocity patterns. Ultrasonic velocimetry (whether Doppler or time domain) is based on changes in the phase of the ultrasound echo. Other examinations can be done based on the echo phase. Slow motions of organs such as the brain can be used to monitor changes in edema. Measurements of tissue strain due to the pulsatile filling of the arterioles. This plethysmographic imaging method can display differences in tissue perfusion because of different tissue types and changes in autonomic activity.
