A Study on the Safety and Performance Test Guideline of Low Intensity Therapeutic Ultrasound Device (저강도 초음파 치료기기의 안전성 및 성능평가 가이드라인 수립을 위한 연구)

  • In this study, we suggested the performance and safety testing guideline for low intensity pulsed ultrasound (LIPUS) represented by the ultrasound fracture treatment device and cartilage treatment device and low intensity focused ultrasound (LIFU) represented by ultrasonic face lifting device. For these study, the international standards and management regulations of Korea, Japan and United State were analyzed. And the usefulness and applicability were evaluated by testing with commercial equipment and reflecting the views of the industry and experts. As a result of this study, the safety and performance test guidelines for low intensity therapeutic ultrasound device were proposed by presenting the 10 items for LIPUS and 12 items for LIFU. The suggested guidelines are considered a high utilization in the domestic testing and approval authorities. And they are also thought to be useful to new technology development.

Comparison of the Effect in ROM Increase and Pain Reduction on Stretching During Ultrasound Treatment in Patients With Frozen Shoulder (동결견 환자의 초음파치료 시 신장여부에 따른 관절가동범위 회복과 통증감소 효과 비교)

  • A single subject experimental design (alternating treatment design) was used to compare the effects of only ultrasound and ultrasound combined with stretching of the joint capsule on the ROM increase and pain reduction in patients with frozen shoulder. Two subjects were included in each group. In the only ultrasound treatment sessions, ultrasound was applied at the pain point of the shoulder joint in supine position. In the ultrasound combined with stretching treatment sessions, ultrasound was applied at the pain point of the shoulder joint positioned in external rotation and abduction in sitting position. Only ultrasound treatment and ultrasound combined with stretching treatment were alternately performed on each patient. Pain and disability was measured by shoulder pain and disability index (SPADI), and range of motion (ROM) was measured by scratch test. The results of this study showed that ultrasound combined with stretching treatment were more effective than only ultrasound treatment in ROM increase and pain reduction. However, disability score was not significantly different.

  • We had applied Doppler ultrasound guided modified Allen's test for a Volkmann's ischemic contracture patient who had ulnar artery constriction which was not detected with acoustic Doppler and CT-angiogram preoperatively. We report this case of Volkmann's ischemic contracture with brief review of literatures for awareness of the Doppler-ultrasound guided modified Allen's test.

The clinical usefulness of non-invasive prenatal testing in pregnancies with abnormal ultrasound findings

  • Purpose: This study aimed to evaluate the clinical usefulness of non-invasive prenatal testing (NIPT) as an alternative testing of invasive diagnostic testing in pregnancies with ultrasound abnormalities. Materials and Methods: This was a retrospective study of pregnant women with abnormal ultrasound findings before 24 weeks of gestation between April 2016 and March 2017. Abnormal ultrasound findings included isolated increased nuchal translucency, structural anomalies, and soft markers. The NIPT or diagnostic test was conducted and NIPT detected trisomy 21 (T21), T18, T13 and sex chromosomal abnormalities. We analyzed the false positive and residual risks of NIPT based on the ultrasound findings. Results: During the study period, 824 pregnant women had abnormal ultrasound findings. Among the study population, 139 patients (16.9%) underwent NIPT. When NIPT was solely performed in the patients with abnormal ultrasound findings, overall false positive risk was 2.2% and this study found residual risks of NIPT. However, the discordant results of NIPT differed according to the type of abnormal ultrasound findings. Discordant results were significant in the group with structural anomalies with 4.4% false positive rate. However, no discordant results were found in the group with single soft markers. Conclusion: This study found different efficacy of NIPT according to the ultrasound findings. The results emphasize the importance of individualized counseling for prenatal screening or diagnostic test based on the type of abnormal ultrasound.

The Use and Educational Effect of Ultrasonic Phantom in Korean Medicine Obstetrics and Gynecology Clinical Practice Education (한방부인과 임상실습교육에서 초음파 팬텀의 활용과 효과)

  • Objectives: The purpose of this study is to analyze educational effect of ultrasound examination training with ultrasonic phantom in the department of Korean Medicine Obstetrics and Gynecology. Methods: All 4th grade students in 2021 and 2022 of school of Korean Medicine, Dong-Shin University must be trained in the department of Korean Medicine Obstetrics and Gynecology according to ultrasound examination training guideline including Objective Structured Clinical Examination (OSCE). After completing ultrasound examination training, we distributed questionnaires to them about a confidence before OSCE and after OSCE, difficulty in using ultrasound, ultrasound reading, difficulty with probe manipulation and computer operation, ultrasound understanding, clinical usefulness. And then, we analyzed the related factors including descriptive statistics, frequency analysis, student's t-test and paired t-test by SPSS 12.0. Results: Confidence in using ultrasound showed differences among students by gender and year, but it was not statistically significant. The difficulty of ultrasound use and ultrasound reading was at a moderate level, and the gender difference was not significant, and the difference between the practical students by year was statistically significant. The difficulty of the operation of the ultrasonic probe was at a normal level, and the difference between the students in practice by gender and year was not statistically significant. Although they answered that they had a very good understanding of the use of ultrasound, there was a gender difference and it was statistically significant, and practical students by year was not statistically significant. They answered that the effect on clinical use was very sufficient, but it was not statistically significant. The appropriateness of the practice time was evaluated at a moderate level. Conclusions: The use and educational effect of ultrasound examination using ultrasonic phantom in the department of Korean Medicine Obstetrics and Gynecology clinical practice education was very effective.

Effect of Ultrasound on the Properties of Biodegradable Polymer Blends of Poly(lactic acid) with Poly(butylene adipate-co-terephthalate)

  • This study investigated the effect of ultrasound irradiation on the blend of poly(lactic acid) (PLA) and poly(butylene adipate-co-terephthalate) (PBAT). The blends of PLA/PBAT(50/50) (PBAT50) were prepared in a melt mixer with an ultrasonic device attached. Thermal, rheological, and mechanical properties, morphology, and biodegradability of the sonicated blends were analysed. The viscosity of the sonicated blends was increased by the ultrasound irradiation owing to the strong interaction. The morphology of the sonicated blends was significantly dependent on the duration o the ultrasound irradiation. For PBAT50, the phase size reduction was maximized when the blends were ultrasonically irradiated for 30 sec. At longer duration of ultrasound irradiation, the PBAT phase underwent flocculation. Measurement of the tensile properties showed an increased breakage tensile stress and an enhanced Young's modulus when the blends were properly irradiated. This improvement was ascribed to better adhesion between the PLA matrix and the PBAT domain and to better dispersion of the PBAT phase. However, the tensile properties were maximized after excessive energy irradiation, which was ascribed to an emulsifying effect leading to coalescence of the PBAT phase. Impact strength was increased to reach a peak with the ultrasound irradiation, and was higher than the untreated sample for all sonicated samples due to the difference of failure mechanism between the tensile test and the impact test.

Use of ultrasound Doppler to determine tooth vitality in a discolored tooth after traumatic injury: its prospects and limitations

  • When a tooth shows discoloration and does not respond to the cold test or electric pulp test (EPT) after a traumatic injury, its diagnosis can be even more difficult due to the lack of proper diagnostic methods to evaluate its vitality. In these case reports, we hope to demonstrate that ultrasound Doppler might be successfully used to evaluate the vitality of the tooth after trauma, and help reduce unnecessary endodontic treatments. In all three of the present cases, the teeth were discolored after traumatic injuries and showed negative responses to the cold test and EPT. However, they showed distinctive vital reactions in the ultrasound Doppler test during the whole observation period. In the first case, the tooth color returned to normal, and the tooth showed a positive response to the cold test and EPT at 10 wk after the injury. In the second case, the tooth color had returned to its normal shade at 10 wk after the traumatic injury but remained insensitive to the cold test and EPT. In the third case, the discoloration was successfully treated with vital tooth bleaching.

Usefulness of Ultrasound in Treatment of Patella Fracture (슬개골 골절 치료에서 초음파의 유용성)

  • Purpose: The purpose of this study is to evaluate the clinical usefulness of ultrasound in treatment of patella fracture. Material and Methods: Twenty cases of twenty patients with patella fracture radiographically from March, 2006 to September, 2009 were involved in this study, there were 8 males and 12 females, there were 13 right knees, 7 left knees, the average age was 43 years (range, 14~72 years). We decided the methods of treatment by using the ultrasound in conjunction with Drayer's test that examining the intactness of patella retinaculum. There were 10 transverse, 8 vertical and 2 comminuted fractures, we examined the patella retinaculum by using the ultrasound and performed Drayer's test. Results: In 12 cases of intact retinaculum, Drayer's test was negative and we allowed patients ambulation without surgical fixation especially. In 4 cases of completely ruptured retinaculum, Drayer's test was positive and we treated the patella fractures by the use of ORIF (open reduction and internal fixation) method, in 2 cases of partially ruptured retinaculum, Drayer's test was negative but displacement of fractures were more than 2 mm, we treated the patella fractures by the use of CRIF (closed reduction and internal fixation) method and obtained clinically satisfactory results. Conclusion: In conjunction with radiographs and Drayer's test, an ultrasound would be one of the useful diagnostic modality for determining treatment method of the patella fracture.

Effect of Ultrasound on the Mechanical Properties of Electrodeposited Ni-SiC Nano Composite

  • Ni-SiC nano composite coatings were fabricated using electrodeposition technique with the aid of ultrasound. The properties of the nano composite were investigated by using SEM, XRD, Wear test and Vicker's microhardness test. The results demonstrated that the microhardness of composite coatings under ultrasonic condition was improved significantly as compared to conventional electrodeposition techniques without ultrasound. The nano particles were found to be distributed homogeneously with reduced agglomeration. The synergistic combination of superior wear resistance and improved microhardness was found in ultrasonicated conditions to the Ni-SiC nano composite coatings.

Design of Gastrointestinal Diagnosis System based on Ultrasonic Response Characteristics (초음파 응답특성 분석에 의한 위장 경화 진단시스템의 설계)

  • Functional gastrointestinal disorders affect millions of people of all age regardless of race and sex. There are, however, rare diagnostic methods for the functional gastrointestinal disorders because functional disorders show no evidence of organic and physical causes. Our research group identified recently that the gastrointestinal tract well in the patients with the functional gastrointestinal disorders becomes more rigid than healthy people when palpating the abdominal regions overlaying the gastrointestinal tract. The aim is, therefore, to develop a diagnostic method for the functional gastrointestinal disorders based on quantitative measurement of the rigidity of the gastrointestinal tract well using ultrasound technique. For this purpose, a preliminary ultrasound diagnostic system was developed and verified through phantom tests. The system consisted of transmitter, ultrasonic transducer, receiver, TGC, and CPLD, and verified via a phantom test. For the phantom test, ten soft-tissue specimens were harvested from porcine. Five of them were then treated chemically to mimic a rigid condition of gastrointestinal tract well, which was induced by functional gastrointestinal disorders. Additionally, the specimens were tested mechanically to identify if the mimic was reasonable. The customized ultrasound system was finally verified through application to human subjects with/without functional gastrointestinal disorders(Normal and Patient Groups). It was identified from the mechanical test that the chemically treated specimens were more rigid than normalspecimen. This finding was favorably compared with the result obtained from the phantom test. The phantom test also showed that ultrasound system well described the specimen geometric characteristics and detected an alteration in the specimens. The maximum amplitude of the ultrasonic reflective signal in the rigid specimens $(0.2{\pm}0.1Vp-p)$ at the interface between the fat and muscle layers was explicitly higher than that in the normal specimens $(0.1{\pm}0.0Vp-p)$ (p<0.05). Clinical tests using our customized ultrasound system for human subject showed that the maximum amplitudes of the ultrasonic reflective signals nea. to the gastrointestinal tract well for the patient group$(2.6{\pm}0.3Vp-p)$ were generally higher than those in normal group$(0.1{\pm}0.2Vp-p)$ (p<0.05). These results suggest that newly designed diagnostic system based on ultrasound technique may diagnose enough the functional gastrointestinal disorders.
