Ultrasonographic evaluation of rotator cuff leison

  • We investigated the reliablity of the ultrasonography for diagnosis of rotatar cuff taers. We prepared histological sections of forty shoulder joints In match the ultrasound planes, and conducted a comparative study of tke two sets of images. hi cases of rotator cuff tear, hyperechogenic and cartilaginfication of the tear site were detected as hyperechogenic foci. On the countrary, the regions in which tendon fibers were decreased or absent showed sonolucent areas. We caluculated the echo level of rotator cuff objectively. The area of abnormal echo level and the other regions located in the same depth of the rotator cuff were defined by ultrasonic equipment. The mein difference in these two echo levels was calculated. The mean difference in patients with rotator cuff tears was larger than that in normal cases. These results indicate that ultrasonography is a useful procedure in many patients with rotator cuff lesions.

  • Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.

  • Ulnar neuropathy at the elbow (UNE) may seem easy to diagnose when the characteristic clinical manifestations are present, and electrodiagnostic studies have high sensitivity, although they are non-localizing in some cases and unable to reveal structural lesions. Ultrasonography is noninvasive and able to find the exact location of the lesion and visualize perineural structures. We present two cases of UNE in which we found hypoechoic mass lesions near medial epicondyle with ultrasonography and discuss its usefulness in diagnosis of UNE.

  • Ultrasonography of benign prostatic hypertrophy(BPH) has been used a lot to determine the size of the prostate with a biopsy. In this study, we measured the size of the prostate in a proportion of the transition zone and the peripheral zone quantitatively, we propose a method that can be diagnosed with BPH automatically ultrasound image.

  • Malignant peripheral nerve sheath tumors are rare sarcomas of the heart. Herein, we report the case of a 24-year-old man who complained of dyspnea, cough, and upper left back pain. He was found to have multiple primary heart tumors obstructing the right superior pulmonary vein in the left atrium, which were diagnosed as malignant peripheral nerve sheath tumors. The patient underwent successful resection of the tumors and immunohistochemistry was utilized for diagnosis.

  • Some canine typical Sertoli cell tumors (SCT) induce signs of hyperestrinism. However, whether variant lipid-rich SCTs have signs of hyperestrinism remains largely unknown. In the present study, an 11-year-old male Pekingese dog showed significantly elevated serum estrogen and characteristic signs of hyperestrinism such as gynecomastia and alopecia. Cytological, radiological and ultrasound examinations found testicular mass, prostatitis with squamous metaplasia, and cystitis. Pathologically, the tumor lesions consisted mainly of lipid-rich tumor cells with signet-ring appearance, which were immunohistochemically positive for vimentin and anti-$M{\ddot{u}}llerian$ hormone. Based on the findings, a diagnosis of lipid-rich Sertoli cell tumor was established. In conclusion, a canine lipid-rich SCT induced signs of hyperestrinism and caused prostatitis via squamous metaplasia due to its excessive secretion of estrogen.

  • An intraneural ganglion in the peripheral nerve and the resulting ulnar neuropathy at the elbow are uncommon and may show various symptoms ranging from local pain to motor and sensory impairment. We report a case of a 76-year-old man who was diagnosed with ulnar neuropathy caused by an intraneural ganglion derived from the elbow. We also discuss the pathophysiology, treatment, prognosis, and diagnostic value of ultrasonography in neuropathy caused by a ganglion.

Cutaneous metastasis of mammary gland tumor in a dog: a case report

  • An 8-year-old spayed female, mixed dog presented multiple, bloody exudative skin lesions on the bilateral flank which spread 1 week after mastectomy for treatment of mammary gland tumor (MGT). Multiple, ill-marginated, irregular, and heterogeneously thickened cutaneous and subcutaneous lesions and enlarged lymph nodes were identified in ultrasound and computed tomography. Histopathological examination confirmed adenocarcinoma with lymphatic invasion presumed to be metastatic MGT. Clinical signs improved after chemotherapy but died after 1 month. This study suggests that cutaneous metastasis be considered for differential diagnosis of cutaneous lesions in dogs with a history of MGT, although skin metastasis from MGT is rare.

  • The Korean Thyroid Imaging Reporting and Data System (K-TIRADS) is an ultrasound-based risk stratification system for thyroid nodules that has been widely applied for the diagnosis and management of thyroid nodules since 2016. This review article provides an overview of the use of the K-TIRADS compared with other risk stratification systems. Moreover, this review describes the challenges in the clinical application of the K-TIRADS, as well as future development directions toward the personalized management of patients with thyroid nodules.

  • Sparganosis is a parasitic infection caused by the sparganum, the plercercoid of the genus Spirometra. The preoperative diagnosis of breast sparganosis is difficult in most cases because it is a rare parasitic infection less than 2% of all cases. We report a 62-year-old woman case of breast sparganosis that were confirmed by surgical removal of worms from the right breast. The radiologic images of the patient also revealed characteristic features of breast sparganosis. The patient described the migrating palpable breast mass, which strongly suggested the possibility of breast sparganosis. The treatment of choice and confirmative diagnosis for sparganosis are complete surgical extraction of the sparganum irrespective of infected site. Inspection of the mass site with detailed medical history and radiological examinations are important for preoperative diagnosis of sparganosis patients.
