냉각이 반복된 근수축과 사람의 건 구조에 미치는 영향 (Effects of Cooling on Repeated Muscle Contractions and Tendon Structures in Human)

  • 연구목적: 이 연구는 피부표면 냉각이 생체내의 건구조 점탄성 특성에 미치는 영향을 비교하였다. 방법: 7명의 남성 실험 대상자는 족저굴곡 운동을 10회$\times$10set, 60초 간격으로 최대 수의적 수축을 6초간 각각 실시하였다. 각 측정 전후에 내측 비복근(MG)의 건과 건막의 신장은 초음파 검사법에 의하여 직접 측정되었다(건 장력과 건 신장의 관계로 평가 되었다). 건 횡단면적과 족 관절 모멘트 암은 자기공명영상법(MRI)으로부터 얻어졌다. 건 장력은 관절 모멘트와 건 모멘트 암으로 계산되었다. 또한 스트레스는 횡단면적영역(CSA)을 힘으로 나눔으로써 얻어졌다. 스트레인은 건의 길이로 표준화된 치환으로부터 측정되었다. 결과: 냉각 후에 건 장력은, 비냉각 보다 냉각한 측이 유의하게 높았다. 스티프네스는 비냉각 조건 보다 냉각 조건하에서 높은 유의수준을 나타냈다. 최대의 스트레인과 스트레스는 냉각조건하에서 $7.4{\pm}0.7$$36.4{\pm}1.8$ MPa을 나타냈고, 비냉각 조건하가 $7.8{\pm}8.5$$31.8{\pm}1.1$ MPa (p<0.05)을 나타냈다. 결론: 이 연구의 결과는 피부표면 냉각으로 인해 인간의 근지구력이 건 스티프네스와 탄성률을 증가시키는 것을 시사하는 연구라 하겠다. 피부 표면 냉각으로 인한 근지구력의 개선이 근과 건에 직접적인 영향을 미침을 나타내주고 있다.

몇가지 간담도질환에서 간기능에 따른 Tc-99m EHIDA의 담도영상능 (Tc-99m EHIDA Scintigraphic Demonstrability of Biliary Elements and Liver Function Tests in Hepatobiliary Diseases)

  • In the present communication, the results will be reported on a clinical study of how well scintigraphic visualization of the hepatobiliary elements and several commonly used clinical liver function tests correlate each other in various diseases oft hepatobiliary system. The demonstrability of the biliary tract, gallbladder (GB) and duodenum was rather closely paralleled to serum bilirubin level and less closely to alkaline phosphatase and rather poorly to SGOT and SGPT. The biliary tree could not be visualized scintigraphically when bilirubin exceeded 10.5mg/dl. The usefulness of Tc-99m EHIDA [N-(2,6-diethylacetanilido) iminodiacetic acid, made by Amersham, England] hepatobiliary scintigraphy (Tc EHIDA HBS) in settling diagnostic controversy and ambiguity raised by oral cholecystography, intravenous cholangiography and ultrasonography in many hepatobiliary diseases is well known. The purpose of this investigation was to semiquantitatively evaluate the scintigraphic demonstrability of the hepatobiliary tract, GB and duodenum following intravenous injection of Tc-99m EHIDA in normal subjects and in patients with a disturbed liver function from various hepatobiliary diseases. The hepatobiliary scintigraphy was performed in 10 normal subjects and 39 patients with various hepatobiliary diseases (Table 1) at the Dept. of Radiology, St. Mary's Hospital Catholic Medical College, Seoul, Korea during 2 years period from September 1979. Scintigraphic examination was started at end of 3 minutes after intravenous injection of Tc-99m EHIDA in the amount of $50{\mu}Ci/kg$ and was continued until after 30 minutes at 5 minutes interval. The imaging was usually terminated when the tracer could be seen in the duodenum. Late scintigrams were obatained at 1 1/2, 3, 4 and 6 hours when reeded. Scintigrams were analyzed in terms of promptness and clarity of visualization of the biliary tree, GB and duodenum and demonstrability of these anatomical landmarks was correlated with the values of liver function tests. The demonstrability of the common hepatic duct, common bile duct, GB and duodenum was closely paralleled to the level of serum bilirubin when it is less than 10.5 mg/dl as shown in figure 1. However when the bilirubin exceeded 10.5 mg/dl the time of visualization between protracted reaching a flat curve or plateau around 10.5 mg/dl. The biliary tract could not be visualized when the bilirubin was higher than 10.5 mg/dl. The correlability between scintigraphic demonstration and serum alkaline phosphatase was less strong and between scintigraphic demonstration and SGOT and SGPT was rather poor. The present clinical study confirmed the usefulness and limitation of Tc-99m EHIDA hepatobiliary scintigraphy in visulizing and diagnosing the biliary system and duodenum when radiogrpahy and ultrasonography failed to provide useful informations. Scintigraphy was very helpful in the diagnosis of neonatal hepatitis, biliary atresia, cholecystitis and extrahepatic biliary obstruction. The hepatobiliary system and duodenum were visualized when serum bilirubin level was less than 10.5 mg/dl, SGOT 135 units, SGPT 114 units and alkaline phosphatase 52.2 KAU.

회전근 개 파열에 대한 수술 전 3차원 초음파 검사의 유용성 (Preoperative Three Dimensional Ultrasonographic Evaluation of the Rotator Cuff Tear)

  • 목적: 본 연구의 목적은 수술 전 회전근 개 파열에 대한 3차원 초음파 검사의 시행은 파열의 실제 크기를 수술 전 확인하는 데 유용함을 보고하고자 하는 것이다. 대상 및 방법: 자기공명영상 촬영상 회전근 개 완전 파열이 있어 수술을 시행한 환자 중 수술 전 3차원 초음파 검사를 시행하여 파열의 크기를 측정하였고, 수술시 회전근 개의 실제 크기를 측정하여 비교하였다. 이 연구에는 수술 시 회전근 개 완전 파열이 확인된 15예(남자가 7명, 여자가 8명)가 포함되었다. 평균 연령은 55.4세($47{\sim}67$)였으며, 우측 견관절은 13예, 좌측 견관절은 2예였다. 결과: 수술 전 초음파 검사로 측정한 파열의 크기와 실제 수술시 측정한 크기의 차이는 가로 길이는 평균 0.7 mm의 오차를 보였고, 세로 길이는 평균 2.0 mm의 오차를 보였다. 결론: 수술 전 3차원 초음파 검사와 실제 수술시 측정한 회전근 개 파열의 가로 및 세로 크기가 거의 일치함을 보여 수술 전 3차원 초음파 검사는 매우 유용한 검사로 사료된다.

Clinical analysis and review of literature on pilomatrixoma in pediatric patients

  • Background: Pilomatrixoma is a benign tumor that originates from the hair follicle matrix. It usually presents as a hard, slow growing, solitary mass that can be easily misdiagnosed as other skin masses. The aim of this study was to clinically analyze a case series of pilomatrixoma in pediatric patients from Korea. Methods: A total of 165 pediatric patients from 2011 to 2018 with a histological diagnosis of pilomatrixoma were included. A retrospective review was performed using the electronic medical records, including patient demographics, number and location of the mass, clinical and imaging presentation, and postoperative outcomes. Results: There were 61 male and 104 female patients with 152 solitary and 13 multiple pilomatrixomas. Among solitary pilomatrixomas, the lesion commonly occurred in the head and neck (84.2%), followed by upper limbs (11.2%), lower limbs (3.3%), and trunk (1.3%). The pilomatrixoma lesion presented as the following types based on our clinical classification: mass (56.02%), pigmentation (25.31%), mixed (12.65%), ulceration (4.82%), and keloid-like (1.2%). Ultrasonography showed a high positive predictive value (95.56%). There were no specific complications observed except for two cases of recurrence. Conclusion: Pilomatrixoma has various clinical feature presentations and commonly occurs in the head and neck. Ultrasonography is a helpful diagnostic tool. Surgical removal of the lesion is the main treatment method with a low recurrence rate.

소아 요막관 기형 (Urachal Anomalies in Children)

  • Failure of the urachus to regress completely results in anomalies that may be classified as patent urachus, urachal sinus, urachal cyst and bladder diverticula. The presenting symptoms of children with urachal anomalies are variable and uniform guidelines for diagnosis and treatment are lacking. The purpose of this study was to analyze our experience and develop conclusions regarding the presentation, diagnosis and treatment of urachal anomalies. We retrospectively analyzed the records of 32 patients who were admitted for urachal anomalies from March 1995 to February 2005. The age distribution of these patients at presentation ranged from 1 day to 14 years old (median age 1 month). There were 20 boys and 12 girls. The 32 cases comprised 13 cases of urachal sinus (40.6 %), 10 urachal cyst (31.3 %), and 9 patent urchus (28.1 %). In 30 patients ultrasonography was used for diagnosis and 2 patients with patent urachus were explored without using a diagnostic method. Twenty-three patients were confirmed by ultrasonography alone and 7 patients were examined using additional modalities, namely, computed tomography for 2 patients with an urachal cyst, magnetic resonance imaging for 1 patient with an urachal cyst, and fistulography for 3 patients with an urachal sinus. The presenting symptoms were umbilical discharge (14 patients), umbilical granuloma (8), abdominal pain and fever (3), fever (3), abdominal pain (2), and a low abdominal mass (2). Excision was performed in 29 patients, and 3 patients were conservatively managed. Urachal anomalies in children most frequently presented in neonates, and the most common complaint was umbilical discharge with infection. Urachal anomalies can be diagnosed by a physical examination and an appropriate radiographic test. Ultrasound was the most useful diagnostic method. Complete surgical excision of an urachal anomaly is recommended to avoid recurrence, and the rare development of carcinoma.

유방 양성 종괴 추적 관찰 환자에게서 발견된 관상피내암 증례 보고 (A Case Report of Intraductal Carcinoma Detected in a Patient Undergoing Surveillance for Benign Breast Mass)

  • 유방의 관상피내암(DCIS; ductal carcinoma in situ)은 유방의 상피세포가 악성화되었지만, 아직 정상적인 유관에 한정되어 있는 경우를 지칭하며, 최근 빠르게 증가하고 있다. 본 증례에서 추적관찰 2년 동안은 유방 촬영술 및 초음파 검사상 병변은 BI-RADS Category 3으로 분류되는 작은 mass가 관찰되었지만 낭성 mass 소견을 보여, 전형적인 악성 종양의 소견과는 거리가 있었으나 추적관찰 2년째에 유방초음파상 우측 유방 6시 방향에 불분명한 경계(Lobulated margin)를 보이는 저에코(hypoechoic) 종괴가 약 2.1 × 1.3 cm 크기로 보였으며 조직 검사상 관상피내암으로 진단되었다. 관상피내암은 특징적인 임상소견이 없고 초기에 양성의 임상, 영상 소견을 보일 수 있으므로 정기적인 추적관찰을 시행하는 것이 조기 진단에 중요할 것으로 사료되며, 유방촬영술과 초음파 검사로 관상피내암을 발견하는 것은 유방암 환자 전체의 예후를 좋게 하는 데에 중요하다고 할 수 있겠다. 4년 전 처음 검사 시 증상이 없으며 유방촬영상 정상이고 유방초음파상 악성 징후 없는 낭성병변이 추적관찰 중에 관상피내암의 소견인 비대칭 음영과 미세석회화, 분엽상의 mass가 관찰된 관상피내암의 증례를 보고한다.

Sonographic Diagnosis of Cervical Lymph Node Metastasis in Patients with Thyroid Cancer and Comparison of European and Korean Guidelines for Stratifying the Risk of Malignant Lymph Node

  • Objective: To evaluate the ultrasonography (US) features for diagnosing metastasis in cervical lymph nodes (LNs) in patients with thyroid cancer and compare the US classification of risk of LN metastasis between European and Korean guidelines. Materials and Methods: From January 2014 to December 2018, US-guided fine-needle aspiration was performed on 836 LNs from 714 patients for the preoperative nodal staging of thyroid cancer. The US features of LNs were retrospectively reviewed for the following features: size, presence of hilum, margin, orientation, cystic change, punctate echogenic foci (PEF), large echogenic foci, eccentric cortical thickening, abnormal vascularity, and cortical hyperechogenicity. A multiple logistic regression analysis was performed to identify the independent US features for the diagnosis of metastatic LNs. The diagnostic performance of independent US features was subsequently evaluated. LNs were categorized according to the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) and European Thyroid Association (ETA) guidelines, and the correlation between the two sets of classifications was assessed. Results: Absence of the hilum, presence of cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features of metastatic LNs. Cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity showed high specificity (86.8%-99.6%). The absence of the hilum had the highest sensitivity yet low specificity (66.4%). When LNs were classified according to the ETA guidelines and K-TIRADS, they yielded similar categorizations of malignancy risks and were strongly correlated (Spearman coefficient, 0.9766 [95% confidence interval, 0.973-0.979]). According to the ETA guidelines, 9.8% (82/836) of LNs were classified as "not specified." Conclusion: Absence of hilum, cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features suggestive of metastatic LNs in thyroid cancer. Both K-TIRADS and the ETA guidelines provided similar risk stratification for metastatic LNs with a high correlation; however, the ETA guidelines failed to classify 9.8% of LNs into a specific risk stratum. These results may provide a basis for revising LN classification in future guidelines.

부갑상선: 부갑상선 영상에 익숙하지 않은 영상의학과 의사들을 위한 전반적인 검토 (The Parathyroid Gland: An Overall Review of the Hidden Organ for Radiologists)

  • 부갑상선은 부갑상선 호르몬(parathyroid hormone; 이하 PTH)을 생성하여 칼슘 대사를 조절하는 작은 내분비선으로 구성되어 있다. 일반적으로 갑상선 뒤에 4개의 부갑상선이 위치해 있으나 개수 또는 위치는 개인차가 있으며 4개보다 많거나 적은 경우들이 있다. 부갑상선 질환은 부갑상선 기능 장애와 관련이 있으며, 부갑상선 자체의 문제 또는 신장질환으로 인한 비정상적인 혈청 칼슘 수치로 인해 발생할 수 있다. 최근 건강검진이 보편화되면서 우연히 비정상적으로 높은 혈청 칼슘 값이 발견되어 PTH 검사, 초음파, 테크네튬-99m 세스타 미비 부갑상선 스캔, 단일광자방출단층촬영/컴퓨터단층촬영(SPECT/CT), 4차원 컴퓨터단층촬영(4D-CT), 그리고 양전자방출단층촬영/컴퓨터단층촬영(PET/CT) 등의 추가적인 검사가 시행된다. 그러나 부갑상선은 여전히 영상의학과 의사에게 익숙하지 않은 기관이다. 이 종설에서 부갑상선의 해부학, 병태생리, 영상 및 임상 소견에 대해 알아보고자 한다.

Imaging Neuroreceptors in the Living Human Brain

  • For nearly a century it has been known that chemical activity accompanies mental activity, but only recently has it been possible to begin to examine its exact nature. Positron-emitting radioactive tracers have made it possible to study the chemistry of the human mind in health and disease, using chiefly cyclotron-produced radionuclides, carbon-11, fluorine-18 and oxygen-15. It is now well established that measurable increases in regional cerebral blood flow, glucose and oxygen metabolism accompany the mental functions of perception, cognition, emotion and motion. On May 25, 1983 the first imaging of a neuroreceptor in the human brain was accomplished with carbon-11 methyl spiperone, a ligand that binds preferentially to dopamine-2 receptors, 80% of which are located in the caudate nucleus and putamen. Quantitative imaging of serotonin-2, opiate, benzodiazapine and muscarinic cholinergic receptors has subsequently been accomplished. In studies of normal men and women, it has been found that dopamine and serotonin receptor activity decreases dramatically with age, such a decrease being more pronounced in men than in women and greater in the case of dopamine receptors than serotonin-2 receptors. Preliminary studies in patients with neuropsychiatric disorders suggests that dopamine-2 receptor activity is diminished in the caudate nucleus of patients with Huntington's disease. Positron tomography permits quantitative assay of picomolar quantities of neuro-receptors within the living human brain. Studies of patients with Parkinson's disease, Alzheimer's disease, depression, anxiety, schizophrenia, acute and chronic pain states and drug addiction are now in progress. The growth of any scientific field is based on a paradigm or set of ideas that the community of scientists accepts. The unifying principle of nuclear medicine is the tracer principle applied to the study of human disease. Nineteen hundred and sixty-three was a landmark year in which technetium-99m and the Anger camera combined to move the field from its latent stage into a second stage characterized by exponential growth within the framework of the paradigm. The third stage, characterized by gradually declining growth, began in 1973. Faced with competing advances, such as computed tomography and ultrasonography, proponents and participants in the field of nuclear medicine began to search for greener pastures or to pursue narrow sub-specialties. Research became characterized by refinements of existing techniques. In 1983 nuclear medicine experienced what could be a profound change. A new paradigm was born when it was demonstrated that, despite their extremely low chemical concentrations, in the picomolar range, it was possible to image and quantify the distribution of receptors in the human body. Thus, nuclear medicine was able to move beyond physiology into biochemistry and pharmacology. Fundamental to the science of pharmacology is the concept that many drugs and endogenous substances, such as neurotransmitters, react with specific macromolecules that mediate their pharmacologic actions. Such receptors are usually identified in the study of excised tissues, cells or cell membranes, or in autoradiographic studies in animals. The first imaging and quantification of a neuroreceptor in a living human being was performed on May 25, 1983 and reported in the September 23, 1983 issue of SCIENCE. The study involved the development and use of carbon-11 N-methyl spiperone (NMSP), a drug with a high affinity for dopamine receptors. Since then, studies of dopamine and serotonin receptors have been carried out in over 100 normal persons or patients with various neuropsychiatric disorders. Exactly one year later, the first imaging of opitate receptors in a living human being was performed [1].

소아에서의 첫 번째 급성 신우신염에 따른 임상 소견 및 방사선학적 검사의 진단적 유용성 (The Diagnostic Value of Clinical and Radiologic Findings in Children after the First Episode of Acute Pyelonephritis)

  • 목 적 : 본 연구에서는 급성 신우신염으로 진단 받은 환아에서 여러 임상 증상 및 검사 결과와 방사선학적 검사에서 확인된 신실질 손상의 관계를 알아보기 위하여 DMSA 신 스캔, 신장 초음파, IVP, VCUG 등의 방사선학적 검사의 유용성과 방사선학적 검사의 결과에 따른 환아들의 임상 증상 및 검사 소견을 비교 분석하였다. 방 법 : 1998년 1월부터 2002년 12월까지 열성요로 감염으로 처음 진단되어 입원 치료받았던 환아 115명을 대상으로 연구를 실시하였다. 이들은 DMSA 신 스캔, 신장 초음파, IVP, VCUG를 포함한 방사선학적 검사를 시행하였으며 각 검사의 양성률을 통하여 그 유용성을 비교하였다. 또한, 환아들의 연령, 성별, 발열 기간, 원인 균, 혈액학적 소견(백혈구 수, CRP, ESR)을 방사선학적 검사 결과와 비교 분석하였다. 연령에 따라 1군(1세 미만)과 2군(1세 이상)으로 분류하였으며, 각 군간의 검사 양성률 비교를 위해 chi-square test를 이용하여 통계 분석하였다. 결 과 : 방사선학적 검사 중 DMSA 신 스캔이 가장 높은 양성률을 보였다(DMSA 신 스캔 46$\%$, 신장 초음파 32$\%$, IVP 28$\%$, VCUG 22$\%$, P<0.05). 환아의 발열 기간, 백혈구 수, 원인균은 방사선학적 검사 결과와 관련이 없었으나 CRP, ESR이 DMSA 신 스캔의 양성률과 밀접한 관계를 보였다. 연령별로 비교했을 때 1군에 비해 2군에서 DMSA 신 스캔 양성률이 증가하였으며 (1군 40$\%$, 2군 79$\%$, P<0.05), VCUG 역시 2군에서 녹은 양성률을 보였다(1군 16$\%$, 2군 50$\%$, P<0.05). 그러나 신장 초음파와 IVP는 각 군간 유의한 차이를 보이지 않았다. 성별로 비교했을 때 남아에 비하여 여아에서 DMSA 신 스캔 및 VCUG의 양성률이 높았으나(DMSA; 남아 39$\%$, 여아 67$\%$, VCUG; 남아 15$\%$, 여아 43$\%$, P<0.05) 신장 초음파와 IVP는 성별에 따른 유의한 차이를 보이지 않았다. 결 론 : 급성 요로 감염에 이환된 환아는 신손상 여부의 확인을 위하여 DMSA 신 스캔을 시행하는 것이 가장 도움이 되며, DMSA 신 스캔과 VCUG의 이상은 여아와 2세 이상에서 양성률이 높으므로, 이와 같은 경우에는 이 두 가지 검사를 반드시 시행하여야 할 것으로 사료된다.
