A technique for treating exposed cardiac Pacemaker (피부괴사로 노출된 심장 Pacemaker에 대한 처치 : 증례 보고)

  • The management of the exposed cardiac pacemaker or its lead is a new challenge to the plastic surgeon. This complication is not rare. Coburn et al. [1972] reported less than 5 percent, but Sowton et al. [1974] showed that over a period of 19 years, in a series of 372 patients, the pacing system had to be removed because of local wound breakdown or infection in 10 percent of the cases. The methods used to treat exposure may vary from removal and re-introduction at anterior site to the rotation of local flaps to cover the exposed pacemaker. Recently we have experienced 6 times of migration and recurrent skin ulcerations without pyogenic infection overlying the pacemaker in one patient. We developed a new technique, anchoring the pacemaker to the clavicle by a wire through the hole of clavicle and by creating a pocket under the pectoralis major muscle. Then we would like to emphasize this operating method could be choice of treatment to prevent the migration of pacemaker and the ulceration of skin when complication of implantation of pacemaker is occurred.

  • Pacemaker therapy has been established as a routine procedure in a variety of disturbances of :cardiac rhythm. Cardiac pacing has greatly reduced the mortality rate in patients suffering from bradyarrhythmias complicated by Stokes-Adams attacks. However, in spite of the simplicity of the treatment and reliability of the devices available today, patients and physicians alike are invovled in a multitde of pacemaker specific problems. Clinical experience with permanent transvenous pacing during a 3 year period is presented. A total of 20 pacemaker operations were performed in 13 patients. The complications in our 20 pacemaker operations were headed by skin ulceration of 3 generator pockets and early dislocation of 3 electrodes. In an additional 2 patient, stimulation of diaphragm due to a position of electrode in the coronary sinus was noticed in one and battery failure of pulse generator was detected in the other. Literatures were reviewed.

  • To evaluate clinical pattern and operative outcome of atherosclerotic obstructive disease in the lower extremity, surgical experience in 25 patients between September 1987 and June 1994 was analyzed. There were 24 male patients and 1 female patient. The age of the patients ranged between 36 and 77 years[mean:60.9 years . Disabing intermittent claudication, rest pain, or ischemic gangrene[or ulceration was the operative indication. The patients were classified into three groups according to the level of obstruction: Aortoiliac level[11 cases/44% , femoropopliteal level[6 cases/24% , and whole level[8 cases/32% . Anatomic bypass with prosthetic graft was performed in all cases: aortobifemoral or aortofemoral bypass in 15 procedures, iliofemoral bypass in 5 procedures, femoropopliteal bypass in 13 procedures. The perioperative mortality rate was 12%. Two patients received postoperative major amputation. Twenty two patients were followed-up for 1 to 70 months[mean: 21 months . Late graft obstruction occurred in two patients. The cumulative patency rate at 3 years was 69.5%. The cumulative limb salvage rate at 3 years was 92.0%.

  • Extensive tumoral calcinosis affecting a large joint is uncommon in patients with systemic sclerosis. We report the case of a 52-year-old female patient referred for a growing calcified mass in the shoulder. She was diagnosed with interstitial lung disease and progressive systemic sclerosis. Although the pain and disability associated with the affected joint was not severe, the patient underwent surgical excision because the mass continued to grow and was likely to produce shoulder dysfunction and skin ulceration. The patient appeared well 10 months after surgery with no signs of recurrence. This report highlights the timing and indication of surgical excision in similar cases.

  • The authors have studied histopathologically, based with symptoms, on 51 aged woman who treated in infirmary hospital of dental college of Seoul National University. The results are as follows: 1. Symptoms of patient were swelling of lower left mandible, gingival enlargement, ulceration and severe bleeding. 2. We observed characteristic features on the blood test such as decreased leukocytes with severe decrease of granular leuckocytes and sudden increase of lymphocytes. 3. On the histopathological studies, we observed severe destruction of gingival tissue with few granular leukocytic infiltration, but displaced with massive lymphochtic infiltration.

  • The Efficacy of PSB-2061, was investigated in comparision with predinisolone in acetic acid and Picrylsulfonic acid solution (TNBS)-induced rat inflammatory bowel disease (IBD) for 5 days. 5 % TNBS solution were administered with polyethylene (P.E) tube inserted to rats intracolon, which causing colitis to the rats. The TNBS control group (the saline treated colitic rat) exhibited ulceration and inflammation of the distal colon with formation of granuloma and pathologic connections. We checked the inflammatory parameters like rat's weight, food intake quantity change during administration. After 5 days, we sacrificed the rats and checked the colon's length, ulcer and pathologic condition. Oral treatment with PSB-2061 resulted in significant recovery of macroscopic parameters like weight and diet intake change. Especially, PSB-2061 extract had a more potent effect than $mesalazine^{(R)}$ on macroscopic colonic damage score. We can suggest that PSB extract could be a promising drug in the treatment of IBD.

  • Hyperimmunoglobulin E syndrome(HIES) is a primary immunodeficiency characterized by severe recurrent soft-tissue infections, pneumonias, pruritic dermatitis and markedly elevated serum IgE levels. Pneumatocele and empyema develop as result of recurrent pneumonias. It is inherited in an autosomal dominant fashion, with variable expressivity. We experienced the case of a 10 month old girl with hyper IgE syndrome. Patient was admitted for local ulceration and muscle soreness at the site of BCG inoculation. The diagnosis of hyper IgE syndrome was made because she had a coarse facial appearance, pruritic dermatitis, pneumatocele and markedly elevated serum IgE level with a past history of frequent skin abscess and pneumonia.

  • Diabetic polyneuropathy (DPN) is the most common form of diabetic neuropathy, and causes a significant morbidity with an impact on the quality of life in the patients with diabetes. Since DPN frequently induces foot deformity and ulceration, which finally leads to foot amputation, the early detection and treatment is very important for the prevention of a permanent structural change. In the early stage of DPN, the diagnostic methods which can evaluate the function or structure of small nerve fibers should be employed because small nerve fibers are first involved in the course of DPN. However, the nerve conduction study cannot reflect the function of the small nerve fibers, and thus, has a definite limitation in the early diagnosis of DPN. For the early detection of DPN, electrodiagnostic data should be interpreted on a clinical context, along with the careful evaluation of the small nerve fiber functions using the tests such as the analysis of intraepidermal nerve fiber density.

  • Unintentional foreign body ingestion is common in children, and coins are the most common foreign body ingested. Foreign body remaining in the esophagus may be associated with mucosal ulceration or esophageal obstruction and can potentially lead to significant morbidity and even death. Removal of esophageal foreign body is therefore generally recommended. Several methods are utilized including esophagoscopy in operating room, flexible endoscopy in out patient setting, fluoroscopic Foley catheter technique, and advancement using bougienage. We report two cases of esophageal foreign body(paduk stone) which were hard to be removed by usual esophagoscopic removal. Successful removal was accomplished with the aid of Fogarty catheter under the general anesthesia.

  • A two-year-old male peacock (Pavo cristatus) showed acute watery green diarrhea, followed by neurological signs including torticollis and muscular tremor. By the hemagglutination inhibition test for detecting the antibody against the Newcastle disease virus (NDV), the peacock serum inhibited the agglutination of chicken red blood cells. Grossly distinctive hemorrhagic lesions were found in the mucosa of proventiculus and intestine and lung. The spleen revealed multiple variable sized necrotic foci. Histologically, the mucosa of gastrointestinal track had hemorrhagic lesions and some of them underwent ulceration. The spleen exhibited multiple variable sized necrotic foci in which fibrin exudation was marked. Central nervous system had mild non-suppulative menin-goencephalitis consisting of vasculitis, perivascular hemorrhage, gliosis and meningitis. The cells particularly in the cerebellum were degenerative to necrotic. Some of these nerve cells revealed characteristic peripheral chromatolysis. From the present serological and pathological findings, it is suggested that NDV causing death of peacock was velogenic viscerotropic strain. This is the first report of the occurrence of velogenic viscerotropic NDV in an adult peacock in Korea.
