Objectives: The goal of this study was to compare the density of gutta-percha root fillings obturated with the following techniques: cold lateral (CL) compaction, ultrasonic lateral (UL) compaction, and warm vertical (WV) compaction. Materials and Methods: Thirty-three extracted mandibular first molars, with two separate mesial canals in each, were selected. After instrumentation, the canals were stratified into three groups based on canal length and curvature, and underwent obturation with one of the techniques. No sealer was used in order to avoid masking any voids. The teeth were imaged pre- and post-obturation using micro-computed tomography. The reconstructed three-dimensional images were analyzed volumetrically to determine the amount of gutta-percha present in every 2 mm segment of the canal. P values < 0.05 were considered to indicate statistical significance. Results: The overall mean volume fraction of gutta-percha was $68.51{\pm}6.75%$ for CL, $86.56{\pm}5.00%$ for UL, and $88.91{\pm}5.16%$ for WV. Significant differences were found between CL and UL and between CL and WV (p < 0.05), but not between UL and WV (p = 0.526). The gutta-percha density of the roots treated with WV and UL increased towards the coronal aspect, but this trend was not noted in the CL group. Conclusions: WV compaction and UL compaction produced a significantly denser gutta-percha root filling than CL compaction. The density of gutta-percha was observed to increase towards the coronal aspect when the former two techniques were used.
The aim of this study was to evaluate the shaping abilities of four different rotary nickel-titanium instruments with anticurvature motion to prepare root canal at danger zone by measuring the change of dentin thickness in order to have techniques of safe preparation of canals with nickel-titanium files. Mesiobuccal and mesiolingual canals of forty mesial roots of extracted human lower molars were instrumented using the crown-down technique with ProFile, $GT^{TM}$ Rotary file, Quantec file and $ProTaper^{TM}$. In each root, one canal was prepared with a straight up-and-down motion and the other canal was with an anticurvature motion. Canals were instrumented until apical foramens were up to size of 30 by one operator. The muffle system was used to evaluate the root canal preparation. After superimposing the pre- and post-instrumentation canal. change in root dentin thickness was measured at the inner and outer sides of the canal at 1. 3, and 5 mm levels from the furcation. Data were analyzed using two-way ANOVA. Root dentin thickness at danger zone was significantly thinner than that at safe zone at all levels (p < 0.05). There was no significant difference in the change of root dentin thickness between the straight up-and-down and the anticurvature motions at both danger and safe zones in all groups (p > 0.05). ProTaper removed significantly more dentin than other files especially at furcal 3 mm level of danger and safe zones (p < 0.05) Therefore, it was concluded that anticurvature motion with nickel-titanium rotary instruments does not seem to be effective in danger zone of lower molars.
In order to study the cleansing effect of air-driven sonic system, the author prepared root canals on 48 extracted human permanent single rooted teeth using hand instrumentation and aforementioned method, half and half of the cases. In order for comparison of the degree of remnant debris and cleanliness, the cross sections at 3mm and 7mm levels from the apex were stereomicroscopically observed and scored in accordance with predetermined degree. The obtained results were as follows: 1. Air-driven sonic system was more effective in cleansing root canal than hand instrumentation at apical 3mm level. (p < 0.01) 2. Air-driven sonic system was more effective in cleansing root canal than hand instrumentation at apical 7mm level. (p < 0.01) 3. In hand instrumentation, Helisonic file was more effective in cleansing root canal than Rispisonic file. (p < 0.05) In air-driven sonic system, on the other hand, there were no significant difference between the two files.
Maintaining the original canal path during instrumentation is a challenge in narrow curved canals. This study compared the maintenance of the original canal path of curved root canals during instrumentation with two kinds of stainless steel K-files(Brassler USA & Mani Japan), K-flexofiles(Maillefer Swiss) and Ni-Ti files(Brassler USA, Savannah, GA) using circumferential filing technique to # 40 MAF on 60 extracted human molars. Buccal and mesial canals with minimal initial curvature of 20 degrees were used. The maximal initial curvature was 41.5 degrees. Sixty curved canals divided into four groups according to file type(Group 1 : Ni-Ti file, Group 2 : K-flexofile, Group 3: K-file(Brassler), Group 4 : K-file(Mani)). Radiographs of canals were obtained before and after canal shaping. And postoperative radiographs were compared with preoperative radiographs using superimposition method. Data analysis was performed using Covariance analysis and paired-comparison test. The results observed were as follow ; 1. The angle of curvature was better maintained with Ni-Ti file than with stainless steel files. (p<0.01) 2. There was no significant difference in maintaining canal curvature between K-flexofile, Brassler$^{(R)}$ K-file and Mani$^{(R)}$ K-file, although there was some differences in mean values of postoperative canal curvature. 3. Paired-comparison t test revealed significant differences within each of the three stainless steel file types when comparing the mean differences before and after instrumentation, but no significant differences were observed within Ni-Ti file group.

Purpose: The purpose of this study was comparing the efficacy of passive irrigation (PI) and passive ultrasonic irrigation (PUI) for ability to remove debriment of canals. Materials and Methods: Mandibular premolars were decoronated and standardized to 16 mm length. After root canal enlargement and half separating longitudinally, standardized groove of 4 mm length, 0.2 mm width and 0.5 mm depth were formed on the dentin wall of one half. Three depressions in the canal wall of the opposite half, 0.3 mm in diameter and 0.5 mm in depth, were formed. After each groove and depression was filled with dentin debris, two sections of each half were reassembled using impression putty material. In group 1 the canals were irrigated with 2.5% NaOCl by PI. In group 2 the canals were irrigated with 2.5% NaOCl by PUI. Before and after root canal irrigation, the root canal wall of the section was taken with a microscope and a digital camera as images. The amount of dentin debris remaining in grooves and depressions was assessed using a scoring system. Results: There was no significant difference between PI and PUI except for the middle 1/3 of the root canal (P = 0.004). Conclusion: At the middle 1/3 of the root canal, PUI removed more dentine debris than PI. But the removal efficiency of dentin debris is not significantly different between the PUI and PI at the apical area of root canal in mandibular premolars.
The purpose of this study was to spectrophotometrically investigate the sealing effect of the ultrasonic canal obturation with softened gutta-percha utilizing an endosonic plugger by means of ultrasonic vibrations and heat. The 120 extracted human central and lateral incisors with single root were randomly selected, and the root canals were instrumented up to size #60 file by conventional method. The prepared canals were obtruated with gutta-percha by lateral condensation method, McSpadden technique and ultrasonic condensation method, with or without sealer. All specimens were immersed in 2% methylene blue in an incubator at $37^{\circ}C$ for 10 days. The teeth were then dissolved in 5ml of 60% nitric acid solution and the dye present within the root canal system was returned to solution. The leakage of dye was quantitatively measured via spectrophotometric method. The obtained data statistically evaluated usint two-way ANOVA and Student's t-test. The results were as follows : No statistically significant difference in leakage was observed between the lateral condensation method and ultrasonic condensation method, with and without sealer. When sealer was used or not, McSpadden technique showed significantly greater leakage than lateral or ultrasonic condensation method. Statistical analysis of the data indicated that the canals obturated in conjunction with sealer demonstrated less dye leakage than the canals obturated without sealer(p<0.01), except McSpadden technique. The ultrasonic condensation method appeared comparable sealing ability to the lateral condensation method.
The purpose of this study was to evaluate the sealing ability of five obturation methods in conjunction with sealer. Fifty extracted upper and lower anterior teeth were selected and the access cavities were prepared lingually with a round bur. The working length was determined with a #15K file, and the root canals were instrumented with a #40K file 1mm short of the apical foramen. The apical third of root canal was flared by step-back technique and the coronal two thirds of root canal using #2 - 3 Gates Glidden drills. And then, the teeth were randomly assigned to five groups of 10 teeth each. A thin coat of Tubliseal was placed into the canal into the canal using the # 35 reamer and the canals were filled by lateral condensation, vertical condensation, ultrasonic activated lateral condensation, ULTRAFIL injecting, and McSpadden compaction methods. All teeth were stored in 100 % relative humidity at 31c for 48 hours. The roots were suspended in 2 % methylene blue solution for 48 hours, left to dry for 24 hours and then, half side of root was removed longitudinally using the fissure bur. A Boley gauge wes used to measure the distance, to the nearest 0.1mm, from the apical foramen to the most coronal level of dye penetration. The measurement of dye penetration was statistically compared by Duncan's multiple range test The results were as follows 1. Ultrasonic group showed the best sealing ability among the all experimental groups, but there were statistically no significant difference in the sealing ability between ultrasonic group and vertical condensation group. 2. There were no statistically significant difference in the sealing ability among the lateral, vertical, and ultrasonic groups. 3. McSpadden group showed the worst sealing ability among the all exprimental groups.
The purpose of this study is to evaluate the two warm gutta-percha filling techniques by measuring the weight changes of resin blocks before and after canal filling in ribbon shaped canal. Simulated ribbon shaped root canals in 30 transparent resin blocks were instrumented to #40 using .06 taper Profile. 15 resin blocks were obturated with gutta-percha using cold lateral condensation. Warm lateral condensation using the Endotec II was then accomplished on the same 15 blocks. Another 15 resin blocks were obturated using the System B. All canals were obturated without sealer. The resin blocks were weighed after canal preparation and after each subsequent obturation, and then weight changes of the resin blocks were calculated The results were as follows. 1. Warm lateral condensation using Endotec II and continuous wave of condensation using System B produced a denser obturation of gutta-percha compared with conventional cold lateral condensation (p<0.01). 2. There was no significant difference between warm lateral condensation and continuous wave condensation. In conclusion, the warm gutta-percha condensation techniques like warm lateral condensation and continuous wave condensation can be expected to bring favorable canal obturation results in ribbon shaped canals.
The purpose of this study was to evaluate Ultrasonic devices in root canal enlargement, about the effects on the canal shape and on the cutting ability beyond the curvature in curved canals. 180 resin blocks with $40^{\circ}$ curvature in apical third and 16mm long canal were made of epoxy resin and smooth broaches. These blocks were devided into six groups. According to the devices (ENAC$^{(R)}$, HARMOSONIC$^{(R)}$, Sonic Air MM 3000$^{(R)}$) and files (Zipperer file, H-file, Flexofile, K-file, Sharper file), five groups were instrumented one minute with # 15 files, then the enlarged size was measured. And # 20 files were used again in the same groups, then the enlarged size was measured. In control group, the time which was taken to enlarge the canal from # 15 to # 20 by hand technique was measured. The data was analyzed statistically. Then the enlarged shapes were evaluated in six groups with the stereomicroscope and recorded in ideal and non-ideal canal shape to compare the effects of ultrasonic devices on the canal shape. Only the ideal shaped canals were used in the study whether the cutting ability beyond the curvature in curved canals was, or not. The files with whole flutes, no flutes, and flutes in apical 5mm only were used. The weight differences of pre-and post-instrumentation by Sonic Air MM 3000$^{(R)}$ for two minutes were compared. The results were as follow: 1. Intracanal instrumentation for 1 minute with ultrasonic devices using # 15 and # 20 file in curved root canal of the epoxy resin block can not reach to the next file size. 2. Sonic Air MM 3000$^{(R)}$ shows higher cutting ability than the other two devices (p=0.001), however the percentage of non-ideal canal shape was the highest. 3. Two ultrasonic devices except Sonic Air MM 3000 considered normal in ideal canal shaping ability. 4. little cutting ability was shown beyond the curvature of curved canals.
The purpose of this study was to investigate the effect of moisture on apical sealing properties of root canal. Fifty five single rooted human teeth were selected from maxillary and mandibular teeth. After removing crown portion at the cemento-enamel junction, all teeth were routinely prepared with step-back method. And then, the canals were dried with paper point and the teeth were randomly divided into 3 groups of 15 teeth each, and remaining 10 teeth were used as positive and negative control teeth : Group 1 were irrigated with 1ml of 95% alcohol and dried with air and paper point. Group 2 and 3 were intentionally contaminated with 0.05ml of 3.5% NaOCl or saliva, respectively. All the teeth were obturated with sealapex and gutta percha cone by lateral condensation technique, and covered with two coat of nail varnish after 48 hours of obturation. The teeth were immersed in india ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows : 1. All experimental groups showed varying degrees of dye penetration, and the mean degree of dye penetration was 0.1mm to 0.7mm. 2. Saliva contamination group(group 3) showed the highest amount of dye penetration, followed by NaOCl contamination group, then alcohol dried group, but there was no significant difference among three experimental groups. * This results suggest that there was no significant differences of apical leakage after canal obturation between alcohol dried canal and moisture present canals and the use of alcohol instead of paper point is unnecessary to dry the canals prior to canal filling. But other factors such as bacterial contamination and sealer discoloration by moisture must be considered in application of this results to clinical practice.
