Diffusion Weighted MR Imaging of Spinal Cord: Preliminary Report

  • 목적: 척수의 질환에 대한 확산강조영상 소견에 대하여 알아본다. 대상 및 방법: 임상적으로 진단된 척수질환 환자 8명을 (acute transverse myelitis (n=3), spinal dural AV fistula (n=2), multiple sclerosis (n=1), spinal cord tumor (n=2)) 대상으로 하였다. 영상은 Philips Intera 1.5 T system을 이용하였고, multishot spin echo EPI 펄스열 및 EKG gating을 이용하여 영상을 얻었다. 3R-R 간격으로 영상을 얻었으며 TE=72msec, FOV=25cm, 256 matrix, 5mm thickness, b value=400으로 영상매개변수를 정하였다. 척수 중심으로 3개의 절편을 얻었으며 T2 강조 영상과 겉보기확산계수 영상을 같이 구하였다.

Proposal and Development of A Cylinder Type Liquid Variable Compensator for Radiation Therapy

  • In proton and heavy ion radiotherapy, compensators are required to modify the energy of heavy ion, to compensate the local difference of tumor depth. Conventional compensators have to be created, exchanged, and stored for each patient and for each irradiation directions. A Cylinder Type Liquid Variable Compensator is and is under development. Hexagonal cylinders will be arranged in honeycomb structure. In which air and fluid are divided by hexagonal pistons. The position of each piston will be changed in each cylinder for adjusting the thickness of fluid for variable compensator. The location of each hexagonal piston is determined by each controlling cylinder connected to the hexagonal cylinder by inlet pipes of fluid. Each controlling cylinder includes controlling a piston, which is moved mechanically. Each controlling cylinder is to be moved by a motor driven by a computer.

In vivo Visualization of Human White Matter Tract by Diffusion Tensor Imaging Fiber Tractography (DTI-FT)

  • Purpose: To evaluate the white matter fiber connectivity of normal human using diffusion tensor MRI. Method: Normal young healthy volunteers (2 women and 1 man) and 3 brain tumor patients participated in this study. All studies were performed using a 1.5T Philips Gyroscan Intern system. Diffusion weighted imaging was performed using single-shot echo planar imaging, with navigator echo phase correction and SENSE. Diffusion weighting was performed along six independent axes, using diffusion weighting of b=800s/$\textrm{mm}^2$. 128matrix, 23cm FOV, 2.5mm slice thickness were used for Imaging parameters. Data were processed on a Window-2000 PC equipped with IDL and PRIDE (Philips Medical System). Corticospinal tract was traced from mid-pons level via posterior limb of internal capsule. Corpus callosum, cerebellar peduncles and frontal fibers were traced by fiber tractography.

Endoscopic Management of Gastric Subepithelial Tumor (위상피하종양의 내시경적 진단 및 치료)

  • Diagnosis of gastric subepithelial tumors (SETs) is sometimes difficult with conventional endoscopy or tissue sampling with standard biopsy, so non-invasive imaging modalities such as endoscopic ultrasound (EUS) and computed tomography are used to evaluate the characteristics of SETs features (size, location, originating layer, echogenicity, shape). However imaging modalities alone is not able to distinguish among all types of SETs, so histology is the gold standard for obtaining the final diagnosis. For tissue sampling, mucosal cutting biopsy and mucosal incision-assisted biopsy and EUS-guided fine-needle aspiration or biopsy (EUS-FNA or EUS-FNB) is commonly recommended. Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are used for resection of SETs involving the mucosal and superficial submucosal layers, could not treat adequately and safely the SETs involving the deep mucosa and muscularis propria. Submucosal tunneling endoscopic resection (STER) and endoscopic full-thickness resection (EFTR) is used as a therapeutic option for the treatment of SETs with the development of reliable endoscopic closure techniques and tools.

Dose Characteristics of Stereotactic Radiosurgery in High Energy Linear Accelerator Proton Beam (고에너지 선형가속기에 의한 입체방사선수술의 선량특성)

  • Three-dimensional dose calculations based on CT images are fundamental to stereotactic radiosurgery for small intracranial tumor. In our stereotactic radiosurgery program, irradiations have been performed using the 6 MV photon beam of linear accelerator after stereotactic CT investigations of the target center through the beam's-eye view and the coordinates of BRW frame converted to that of radiosurgery. Also we can describe the tumor diameter and the shape in three dimensional configuration. Non-coplanar irradiation technique was developed that it consists of a combination of a moving field with a gantry angle of $140^{\circ}$, and a horizontal couch angle of $200^{\circ}C$ around the isocenter. In this radiosurgery technique, we provide the patient head setup in the base-ring holder and rotate around body axis. The total gantry moving range shows angle of 2520 degrees via two different types of gantry movement in a plane perpendicular to the axis of patient. The 3-D isodose curves overlapped to the tumor contours in screen and analytic dose profiles in calculation area were provided to calculate the thickness of $80\%$ of tumor center dose to $20\%$ of that. Furtheremore we provided the 3-D dose profiles in entire calculation plane. In this experiments, measured isodose curves in phantom irradiation have shown very similiar to that of computer generations.

Measurement of Tumor Dose Using Optically Stimulated Luminescence Detectors (OSLDs) and Ionization Chambers for Primary and Metastatic Lymph Node Cancers with Head and Neck: Comparison of Beam Spoiler and Bolus (광자극발광선량계와 이온함을 이용한 두경부 원발종양 및 림프절 전이성 종양의 선량 측정: 산란판과 볼루스의 비교)

  • This study conducts cross-comparison through verification of treatment planning of using beam spoiler and bolus, according to the dose variation of different tumor bed and metastatic lymph node cancers, against ionization and optically stimulated luminescence detectors(OSLDs), in head and neck radiotherapy. Verification of treatment planning examined the feasibility of inserting detectors through simulated solid dry water slabs under identical irradiated conditions from treatment planning system to measure beam spoiler and 0.5, 1 cm bolus. In addition, two detectors were cross-compared for verification of treatment planning accuracy and reliability within ${\pm}$2%. The study found that, given a beam spoiler thickness of 0.5 cm and beam spoiler-to-skin distance of 10 cm subjected to optimal dose distribution given for metastatic lymph node cancers, the bolus low-level skin dose was less, and the tumor bed dose reduced slightly. Additionally, two detectors were cross-compared for accuracy within ${\pm}$1%. Accordingly, The use of beam spoiler was determined that reduces skin side effects and can deliver an optimal dose distribution for tumor, and to apply to future clinical studies should be performed.

Kerion Celsi Caused by Trichophyton verrucosum Mimicking a Hypervascular Tumor in a Pediatric Patient: A Case Report (고혈관성 종양으로 오인된 Trichophyton verrucosum에 의한 두피 백선종창 1례)

  • A dermatophyte is a zoonotic infection that causes infection on the skin, hair, and nails and is transmitted through contact. The species of Microsporum, Trichophyton, and Epidermophyton are the major dermatophytes that infect humans. Tinea capitis is a dermatophyte infection in the scalp, and it may progress to kerion celsi, including severe redness, swelling, and pus formation. Kerion celsi is sometimes misdiagnosed as a bacterial infection or a tumor. Trichophyton verrucosum is a zoophilic dermatophyte that mainly causes infection in cattle. It can be spread to dairy farmers or ranchers who have frequent contact with infected cattle. We report a pediatric case who received scalp tumor excision and a split-thickness skin graft for extensive and severe inflammatory scalp tumors that occurred after contact with cattle with ringworm. Finally, the patient was diagnosed with kerion celsi caused by T. verrucosum infection. This was based on the patient's medical history, clinical manifestations, and histopathologic findings.

Verification of skin dose according to the location of tumor in Tomotherapy (토모테라피 시 종양의 위치에 따른 피부선량 검증)

  • Purpose : To verify the skin dose in Tomotherapy-based radiation treatment according to the change in tumor locations, skin dose was measured by using Gafchromic EBT3 film and compared with the planned doses to find out the gap between them. Materials and Methods : In this study, to measure the skin dose, I'm RT Phantom(IBA Dosimetry, Germany) was utilized. After obtaining the 2.5mm CT images, tumor locations and skin dose measuring points were set by using Pinnacle(ver 9.2, Philips Medical System, USA). The tumor location was decided to be 5mm and 10mm away from surface of the phantom and center. Considering the attenuation of a Tomo-couch, we ensured a symmetric placement between the ceiling and floor directions of the phantom. The measuring point of skin doses was set to have 3mm and 5mm thickness from the surface. Measurement was done 3 times. By employing TomoHD(TomoHD treatment system, Tomotherapy Inc., Madison, Wisconsin, USA), we devised Tomotherapy plans, measured 3 times by inserting Gafchromic EBT3 film into the phantom and compared the measurement with the skin dose treatment plans. Results : The skin doses in the upper part of the phantom, when the tumor was located in the center, were found to be 7.53 cGy and 7.25 cGy in 5mm and 3mm respectively. If placed 5mm away from the skin in the ceiling direction, doses were 18.06 cGy and 16.89 cGy; if 10mm away, 20.37 cGy and 18.27 cGy, respectively. The skin doses in the lower part of the phantom, when the tumor was located in the center, recorded 8.82 cGy and 8.29 cGy in 5mm and 3mm, each; if located 5mm away from the lower part skin, 21.69 cGy and 19.78 cGy were respectively recorded; and if 10mm away, 20.48 cGy and 19.57 cGy were recorded. If the tumor was placed in the center, skin doses were found to increase by 3.2~17.1% whereas if the tumor is 5mm away from the ceiling part, the figure decreased to 2.8~9.0%. To the Tomo-couch direction, skin doses showed an average increase of 11% or over, compared to the planned treatment. Conclusion : This study found gaps between planned skin doses and actual doses in the Tomotherapy treatment planning. Especially to the Tomo-cocuh direction, skin doses were found to be larger than the planned doses. Thus, during the treatment of tumors near the Tomo-couch, doses will need to be more accurately calculated and more efforts to verify skin doses will be required as well.

Human Lung Cancer Cell Xenografts Implanted under the Capsule of Kidney, Spleen and Liver (폐암 세포주를 사용한 신, 비장 및 간 피막하 분식법의 비교)

  • Bakground : Complete resection by the surgery has been selected as the treatment of choice in lung cancer patients, but in cases of recurrence after excision or inoperable cases, the importance of anticancer chemotherapy has been emphasized. If one can select a set of the sensitive chemotherapeutic agents before anticancer chemotherapy, it will give more favourable results. Subrenal capsular assay has been recognized as a useful in-vivo chemosensitivity test of thoracic and abdominal tumors and it can be done in a short time for a rapid interpretation of tumor responsiveness to anticancer chemotherapeutic drugs. It has been reported that various kinds of cancer cells can be implantable to the kidney, but so far there is no comparative study of xenogeneic cell implantation on liver, spleen and kidney. The author implanted the human lung cancer cells under the capsule of S.D rat's liver, spleen and kidney respectively and compared the pattern of growth and histology. Material and Method: After incubation of human lung cancer cell line (SW-900 G IV) in RPMI 1640 (Leibovitz L-15 medium) culture media, 3${\times}$3${\times}$3 mm size fibrin clots which contain 108 cancer cells were made. Thereafter the fibrin clots were implanted at subcapsule area of liver, spleen and kidney of S.D. female rat. For immune suppression, cyclosporin-A (80 mg/Kg) was injected subcutaneously daily from post-implantation first day to sixth day. The body weight was measured at pre and post implantation periods. The growth pattern and the size of tumor mass were observed and the pathologic examination and serum tumor marker tests were performed. Result: Body weight increased in both of control and experimental groups. Serum Cyfra 21-1 was not detected. Serum levels of CEA and NSE revealed no significant change. The SCC-Ag increased significantly in implanted group. The growth rate of human lung cancer cells which was implanted on spleen was higher than on liver or kidney. The surface area, thickness, and volume of tumor mass were predominant at spleen. The success rates of implantation were 80% on kidney, 76.7% on spleen and 43.3% on liver. Pathologic examination of implanted tumors showed characteristic findings according to different organs. Tumors that were implanted on kidney grew in a round shape, small and regular pattern. In the spleen, tumors grew well and microscopic neovascularization and tumor thrombi were also found, but the growth pattern was irregular representing frequent daughter mass. Human lung cancer cells that were implanted in the liver, invaded to the liver parenchyme, and had low success rate of implantation. Microscopically, coagulation necrosis and myxoid fibrous lesion were observed. Conclusion: The success rate of implantation was highest in the kidney. And the mass revealed regular growth that could be measured easily. The SCC-Ag was presented earlier than CEA or Cyfra21-1. The Cyfra21-1 was not detected at early time after implantation. The best model for tumor implantation experiment for chemosensitivity test was subrenal capsular analysis than liver and spleen and the useful serum tumor marker in early period of implantation was the SCC-Ag.

Effects of Parsley Extract on Skin Anti-aging and Anti-irritation (파슬리추출물의 피부 노화 방지와 자극 완화에 대한 효과)

  • In order to investigate the beneficial effects of parsely (Petroselinurn sativum) extract on skin, we measured the synthesis of total collagen and type I procollagen in cultured normal human fibroblast (NHF), the synthesis of prostaglandin E$_2$(PCE$_2$), interleukin 1 ${\alpha}$(IL -1 ${\alpha}$) and tumor necrosis factor ${\alpha}$ (TNF ${\alpha}$) in HaCaT cell and we also measured dermal thickness and density in hairless mouse (Female albino hairless mice, Skh:hr-1). As the results, the synthesis of total collagen and type I procollagen were increased 23% and 18% respectively, after 1 $\mu\textrm{g}$/mL parsley extract treatment. The producions of PGE$_2$ induced by UVB irradiation were decreased 60% after 1 $\mu\textrm{g}$/mL parsley extract treatment. The treatment with 1 $\mu\textrm{g}$/mL parsley extract also decreased the synthesis of IL -1 ${\alpha}$ and TNF ${\alpha}$ induced by 10 uM RA, 100 $\mu\textrm{g}$/mL SLS and 30 mJ/$\textrm{cm}^2$ UVB irradiation, After 4 days treatment with 1% parsley extract, the dermal thickness of hairless mouse was increased 1.5 times and the density of dermis was tighter than control. These results indicate that parsley extract have anti-aging and anti-irritation effects on skin.
