Pretreatment prognostic Factors in Early Stage Caricinoma of the Uterine Cervix (초기 자궁 경부암에서 치료전 예후 인자)

  • From March 1979 through December 1986, 124 patients with early stage carcinoma of the uterine cervix received curative radiation therapy. According to FIGO classification, 35 patients were stage IB and 89 were stge II A. In stage IB, five year locoregional control, five year disease free survival, and five year overall survival was $79.0\%$, $76.4\%$ and $81.8\%$, respectively. In stage II A, five year locoregional control, five year disease free survival, and five year overall survival were $78.0\%$, $66.8\%$, and $72.1\%$, respectively. To identify prognostic factors, pretreatment parameters including age, ECOG performance status, number of pregnancies, history of diabetes mellitus and hypertension, histology, size and shape of primary tumor, CT findings and blood parameters were retrospectively analyzed in terms of locoregional control, disease free survival and overall survival using univariate analysis and multivariate analysis. In univariate analysis, tumor size on physicai examination and rectal invasion on CT significantly affected locoregional control, disease free survival and overall survival. Parametrial involvement on CT was a significant prognostic factor on locoregional control and disease free survival. Hemoglobin level affected disease free survival and overall survival. Histology and age were significant prognostic factors on locoregional control. In multivariate analysis excluding CT finding, tumor size on physical examination was a significant factor in terms of locoregioal control and overall survival. Hemoglobin level was significant in terms of disease free survival. In multivariate analysis including CT, histology was a prognostic factor on locoregional control and disease free survival. Hemoglobin level and rectal invasion on CT were significant factors on locoregional control.

Effects of Some Korean Traditional Foods on Gastric Cancer Induced by Carcinogen in Rats (위암을 유도시킨 흰쥐에서 한국 전통 식품이 위암 발생에 미치는 영향)

  • Gastric cancer was inducedd by N-nethyl-N'-nitro-N-nitrosoguanidine(MNNG) in Fisher 344 male rats. Freeze dried typical Koran feeds, aged kimchi, soybean paste and maejoo, and partially scorched barbecued bulgokee were fed to the rat in the diet, which were composing 10% of the total diet. Experimental desing was as follows ; (1) group C : Control, (2) group M : MNNG with control diet, (3) group MK : MNNG with 10% of kimchi, (4) group MS : MNNG with 10% of soybean paste and maejoo, (5) group MB : MNNG with 10% of barbecued bulagokee, (6) group MKSB ; MNNG with 10% of the mixture of kimchi, soybean paste and maejoo, and barbecued bulgokee. Each group was fed with isocaloric diet for 26 weeks. Comparing to control, the growth rate of the experimental group was decreased after administration of MNNG and experimental diet. The mortality rate of group MB was increased by 17% than the control group along with a significant decrease of body weight. The protein efficiency ratio and the food efficiency ratio of group MB were lower than the control. The incidence of gastric cancer in rats fed kimchi and barbecued bulgokee were 73% and 75%, respectively, while that of group M which fed MNNG remained only 56%. On the contrary, soybean paste and maejoo showed an inhibitory effect on the burden of gastric tumor. However, the combination of kimchi, soybean paste and maejoo, and barbecued bulogokee showed a synergistic effect of increasing tumorigenesis in rats. Pathological observations of the rat stomach represent that squamous cell type tumors occupied in most frequencies.

Effects of Taraxaci Herba on Surgically Induced Endometriosis in Rats (포공영(浦公英) 추출물이 자궁내막증 유발 백서(白鼠)에 미치는 영향)

  • Purpose : Taraxaci Herba is a herb with an effect on extravasated blood and is widely used in gynecology. This study examined the effects of Taraxaci Herba on endometriosis. Methods : Rats with surgically induced endometriosis were given an oral dose of Taraxaci Herba for 40 days. The size of the ectopic uterine implants at the serosal wall and the concentration of progesterone, estradiol, tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) and interleukin(IL)-2, 4, 6, and 10 in the blood were examined and compared with the control group. Results : The size of the ectopic uterine implants in the treated group was much smaller than that in the control group. The estradiol concentration was significantly lower in the experimental group than in the control group. The IL-10 level was higher and the TNF-${\alpha}$, IL-2 and IL-4 concentration were lower in the experimental group than in the control group and there was statistically significant difference. There was no significant difference in the IL-6 level between the experomental and the control group. Conclusion : These results indicate that Taraxaci Herba reduces the size of ectopic uterine implants at the serosal wall and inhibits the growth of ectopic uterine implants. This suggests that Taraxaci Herba is an effective treatment for endometriosis.

Effects of Retention Enema Therapy Gamihongdeung-tang on Surgically Induced Endometriosis in Rats (가미홍등탕(加味紅藤湯) 직장 내 투여가 자궁내막증을 유발한 흰쥐에 미치는 영향)

  • Purpose : To observe the effect of retention enema therapy Gamihongdeung-tang on experimentally induced endometriosis. Methods : Rats with surgically induced endometriosis were administered in Gamihongdeung-tang rectally. After treatment for 40 days, the serum concentrations of progesterone, estradiol, tumor necrosis factor-${\alpha}$(TNF-${\alpha}$), interleukin(IL)-2, IL-4, IL-6 and IL-10 were measured. Results : The size of the ectopic uterine implants in the treated group was much smaller than that in the control group. The concentration of estradiol was significantly lower in the experimental group than in the control group. The IL-4, IL-6 levels were significantly lower in the experimental groups than the control groups. TNF-${\alpha}$, IL-2 levels were lower and IL-10 level was higher in the experimental groups than in the control groups but the difference was not significent. The progesterone level was similar in the experimental and control group. Conclusion : These results indicate that Retention enema therapy with Gamihongdeung-tang reduces the size of ectopic uterine implants at the serosal wall and inhibits the growth of ectopic uterine implants. This suggests that retention enema therapy with Gamihongdeung-tang is an effective treatment for endometriosis.

Effect of Salvia miltiorrhiza Extract in Urethane-induced Lung Tumorigenesis in A/J Mice (단삼(丹蔘) 추출액이 Urethan으로 유발된 생쥐의 폐암에 미치는 영향)

  • Objectives: The aim of this study was to determine the effects of Salvia miltiorrhiza (SM) extract in urethane-induced lung tumorigenesis in A/J mice. Methods : We examined change of body weight, histological, apoptosis, immunohistochemical and gene expression of cyclooxygenase (COX-2) in lung tumors. Mice were divided into 3 groups: normal, saline, and experimental group administered SM extract after injection with urethane. Results : Histological observation showed shrunken alveoli in the control group, but recovered from damage in the SM extract administered group. The COX-2 positive materials were observed in the smooth muscle of terminal bronchiole and alveoli from the control group, but these positive materials decreased in the SM extract treatment group. The results of electron microscopical observation, dilated capillary and degenerated endothelia were observed in the control group. The apoptotic nuclei increased more in the control group compared with the normal and SM extract administered groups. Serial sections of the whole lung showed solid and papillary tumors. The size and number of tumors decreased in the SM groups compared with the control groups. Conclusions : These results suggest the possibility that SM may exert an anti-tumor effect on urethane-induced lung tumorigenesis.

Negative impact of pretreatment anemia on local control after neoadjuvant chemoradiotherapy and surgery for rectal cancer

  • Purpose: Although anemia is considered to be a contributor to intra-tumoral hypoxia and tumor resistance to ionizing radiation in cancer patients, the impact of pretreatment anemia on local control after neoadjuvant concurrent chemoradiotherapy (NACRT) and surgery for rectal cancer remains unclear. Materials and Methods: We reviewed the records of 247 patients with locally advanced rectal cancer who were treated with NACRT followed by curative-intent surgery. Results: The patients with anemia before NACRT (36.0%, 89/247) achieved less pathologic complete response (pCR) than those without anemia (p = 0.012). The patients with pretreatment anemia had worse 3-year local control than those without pretreatment anemia (86.0% vs. 95.7%, p = 0.005). Multivariate analysis showed that pretreatment anemia (p = 0.035), pathologic tumor and nodal stage (p = 0.020 and 0.032, respectively) were independently significant factors for local control. Conclusion: Pretreatment anemia had negative impacts on pCR and local control among patients who underwent NACRT and surgery for rectal cancer. Strategies maintaining hemoglobin level within normal range could potentially be used to improve local control in rectal cancer patients.

The Effect of Antioxidants on Tumor Necrosis Factor Cytotoxicity (종양괴사인자 세포독성에서 항산화제의 효과)

  • Background : Tumor necrosis factor(TNF) has been considered as an important candidate for cancer gene therapy based on its potent anti-tumor activity. The mechanisms of TNF cytotoxicity are not clearly understood and some has reported that reactive oxygen species(ROS) might be associated with them, but there is controversy about antioxidant effect on TNF cytotoxicity. This study was designed to compare the TNF cytotoxicity after antioxidant pretreatment with that of control to evaluate the role of ROS in the mechanism of TNF cytotoxicity. Method : We compared the TNF cytotoxicies to WEHI164(murine fibrosarcoma cell line) and ME180(human cervix cancer cell line) after antioxidant pretreatment with those of control by MIT(dimethylthiazolyl-diphenyltetrazolium bromide) assay. Results : In the control group, the TNF cytotoxicities were $92.2{\pm}2.8%$(WEHI164) and $59.9{\pm}7.0%$(MEl80). In the TMTU(tetramethyl thiourea) pretreatment group, those were $91.4{\pm}3.7%$ and $74.6{\pm}7.0%$. In the PMZ(promethazine) pretreatment group, those were $90.2{\pm}2.5%$ and $62.5{\pm}5.7%$. In the BHT(butylated hydroxytoluene) pretreatment group, those were $93.2{\pm}1.3%$ and $66.3{\pm}6.1%$. So there was no reduction in TNF cytotoxicity after antioxidants pretreatment. Conclusion : The ROS may not have major role in the mechanisms of TNF cytotoxicity.

Association between Tumor Necrosis Factor-$\alpha$ Gene Polymorphism and Bronchopulmonary Dysplasia in Preterm Infants (조산아 기관지폐이형성증과 Tumor Necrosis Factor-$\alpha$ 유전자 다형성과의 연관성)

  • Purpose: Several factors including prolonged inflammatory response are thought to contribute to the pathogenesis of bronchopulmonary dysplasia (BPD). The clinical findings can be explained by an increased production of proinflammatory cytokines such as tumor necrosis factor alpha (TNF-$\alpha$ ). We investigated the relationship between susceptibility to BPD and TNF-$\alpha$ promoter polymorphisms to identify genetic factors of the disease. Methods: Thirty-eight preterm infants who had developed BPD and 55 controlled infants with a birth weight <1,500 g were analyzed for TNF-$\alpha$ genotypes. The alleles of five promoter sites (-1031/-863/-857/-308/-238) of TNF-$\alpha$ gene were determined using $Taqman^{(R)}$-based allelic discrimination assays. Results: Gestational age ($27^{+5}{\pm}2^{+0}$ wk vs. $29^{+2}{\pm}1^{+4}$ wk, P<0.0001) and birth weight (990${\pm}$270 g vs. 1,220${\pm}$230 g, P<0.0001) were lower in the BPD group compared to the control group. The incidence of respiratory distress syndrome (71.1% vs. 49.1%, P=0.035) and patent ductus arteriosus (71.1% vs. 50.9%, P=0.052) was higher in the BPD group compared to the control group. The frequencies of the alleles and genotypes of five promoter sites (-1031/-863/-857/-308/-238) of TNF-$\alpha$ gene did not show differences between the BPD group and the control group. Conclusion: TNF-$\alpha$ promoter polymorphisms are not associated with susceptibility to BPD in Korean preterm infants.

Expression of Matrix Metalloproteinases-9 and Stromelysin-3 in Peripheral Blood in Patients with Lung Cancer (폐암 환자의 말초혈액에서 Matrix metalloproteinase-9 및 Stromelysin-3의 발현)

  • Background: Matrix metalloproteinases(MMP) are essential enzymes for tumor invasion and metastasis. Among the MMP family, elevated MMP-9 and stromelysin-3(STR-3) expression have been reported to be poor prognostic factors in lung cancer patients. To evaluate the possibility of a molecular diagnosis of lung cancer using peripheral blood, the mRNA expression level of MMP-9 and STR-3 was measured using a reverse transcriptase-polymerase chain reaction (RT-PCR) in patients with lung cancer. Methods : Ninety six patients(44 patients with lung cancer, 19 pulmonary infection, and 33 control) were included. To detect MMP-9 and STR-3 mRNA expression, RT-PCR was performed in peripheral blood mononuclear cells. ELISA was also used to measure the serum level of MMP-9. Results : MMP-9 was expressed more frequently in patients with a pulmonary infection(18/19, 94.7%) compared to lung cancer patients(26/44, 59.1%) or the controls (23/33, 69.7%) (p=0.018). On the other hand, STR-3 expression was observed more frequently in patients with lung cancer(37/44, 84.1%) compared to the lung infection patients(8/19, 42.1%) or control(20/33, 60.6%) (p=0.003). Among the lung cancer patients, MMP-9 was expressed more frequently when a tumor invaded the lymph nodes(17/24, 70.8%) compared to when a tumor did not(3/13, 23.1%) (p=0.005). The MMP-9 and STR-3 expression levels had no relationship with age, sex, tumor size, distant metastasis, or tumor histology. The serum MMP-9 concentration was not higher in lung cancer patients compared to patients with a pulmonary infection or the control subjects. Conclusion : STR-3 may be used as a diagnostic marker in the peripheral blood of lung cancer patients using RT-PCR. Further studies to evaluate the clinical significance of elevated STR-3 expression in lung cancer patients is recommended.

Patterns of failure and prognostic factors in resected extrahepatic bile duct cancer: implication for adjuvant radiotherapy

  • Purpose: To find the applicability of adjuvant radiotherapy for extrahepatic bile duct cancer (EBDC), we analyzed the pattern of failure and evaluate prognostic factors of locoregional failure after curative resection without adjuvant treatment. Materials and Methods: In 97 patients with resected EBDC, the location of tumor was classified as proximal (n = 26) and distal (n = 71), using the junction of the cystic duct and common hepatic duct as the dividing point. Locoregional failure sites were categorized as follows: the hepatoduodenal ligament and tumor bed, the celiac artery and superior mesenteric artery, and other sites. Results: The median follow-up time was 29 months for surviving patients. Three-year locoregional progression-free survival, progression-free survival, and overall survival rates were 50%, 42%, and 52%, respectively. Regarding initial failures, 79% and 81% were locoregional failures in proximal and distal EBDC patients, respectively. The most common site was the hepatoduodenal ligament and tumor bed. In the multivariate analysis, perineural invasion was associated with poor locoregional progression-free survival (p = 0.023) and progression-free survival (p = 0.012); and elevated postoperative CA19-9 (${\geq}37U/mL$) did with poor locoregional progression-free survival (p = 0.002), progression-free survival (p < 0.001) and overall survival (p < 0.001). Conclusion: Both proximal and distal EBDC showed remarkable proportion of locoregional failure. Perineural invasion and elevated postoperative CA19-9 were risk factors of locoregional failure. In these patients with high risk of locoregional failure, adjuvant radiotherapy could be considered to improve locoregional control.
