이하선에 발생한 양측성 Warthin씨 종양 치험례 (A Case of Bilateral Warthin's Tumor)

  • Warthin's tumor is a benign neoplasm that most frequently occurs in the parotid gland. Warthin's tumor shows a tendency of bilateral occurrence, but most of bilateral tumors occur at different time. It usually affects older man and grows slowly. Surgical excision is appropriate treatment for patients with Warthin's tumor and rarely recurs after removal. We report a rare case of bilateral Warthin's tumor which detected at the same time. A 54-year-old man had painless, slowly growing masses on the both mandibular angles. The masses were detected ten years ago. CT scan, ultrasonographic evaluation and fine needle biopsy was performed for preoperative diagnostic study. Superficial parotidectomy was done one after the other at ten-day intervals. After excision, histological examination revealed a mixture of ductal epithelium and a lymphoid stroma. The overlying surface of lymphoid tissue was covered with epithelial cells that was abundant of eosinophilic cytoplasm. Both masses were diagnosed as Warthin's tumor. Postoperatively, the patient did well and had no evidence of recurrence of tumor for 2 years follow-up period. Authors experienced a rare case of bilateral Warthin's tumor which detected at the same time. We recommend stepwise superficial parotidectomy at one or two-week intervals as treatment of bilateral Warthin's tumor.

Clinical predictive factors of pathologic tumor response after preoperative chemoradiotherapy in rectal cancer

  • Purpose: The aim of this study was to identify clinical predictive factors for tumor response after preoperative chemoradiotherapy (CRT) in rectal cancer. Materials and Methods: The study involved 51 patients who underwent preoperative CRT followed by surgery between January 2005 and February 2012. Radiotherapy was delivered to the whole pelvis at a dose of 45 Gy in 25 fractions, followed by a boost of 5.4 Gy in 3 fractions to the primary tumor with 5 fractions per week. Three different chemotherapy regimens were used (5-fluorouracil and leucovorin, capecitabine, or tegafur/uracil). Tumor responses to preoperative CRT were assessed in terms of tumor downstaging and pathologic complete response (ypCR). Statistical analyses were performed to identify clinical factors associated with pathologic tumor response. Results: Tumor downstaging was observed in 28 patients (54.9%), whereas ypCR was observed in 6 patients (11.8%). Multivariate analysis found that predictors of downstaging was pretreatment relative lymphocyte count (p = 0.023) and that none of clinical factors was significantly associated with ypCR. Conclusion: Pretreatment relative lymphocyte count (%) has a significant impact on the pathologic tumor response (tumor downstaging) after preoperative CRT for locally advanced rectal cancer. Enhancement of lymphocyte-mediated immune reactions may improve the effect of preoperative CRT for rectal cancer.

$^{99m}Tc$-colloid 섭취를 보인 췌장의 도세포종 : 증례보고 (Radiocolloid Uptake in the Pancreas Islet Cell Tumor: Case Report)

  • Colloid uptake in various hepatic conditions such as focal nodular hyperplasia, regenerating nodules in the cirrhotic liver, hamartoma, hemangioma and rarely hepatoma has been documented. Extrahepatic tumors may show colloid uptake and they include splenic hemangioma, malignant fibrous histiocytoma, breast carcinoma and Kaposi's sarcoma. The mechanism of colloid uptake in those lesions is associated with phagocytic activity in or around the tumors. We report a pancreas islet cell tumor that showed colloid uptake on $^{99m}Tc$-phytate liver scan without histologic evidence of phagocytosis by tumor cells or infiltration of phagocytes in the tumor Microscopically the tumor was highly vascular and showed diffuse hemorrhage throughtout the tumor. We postulated that extravasation of the colloid into the tumor insterstitium caused nonspecific colloid uptake in this tumor. It is expected that hemorrhagic tumor may show nonspecific colloid uptake without phagocytosis in or about the lesion.

광역학적 치료법을 이용한 쥐의 악성종양 괴사 (Mouse Tumor Necrosis Using Photodynamic Therapy)

  • 본 논문은 쥐의 악성종양에 대한 광역학적 치료효과를 조사한 연구이다. 실험방법으로서는 쥐를 대조군과 대상군의 두 그룹으로 나누어 HepG2 and Hela cell line을 주입하여 암조직을 배양하였다. 쥐의 악성종양에 포토포린을 30시간 전에 주입하고 630nm와 650nm의 레이저를 적용하였다. 광역학적 치료후에 쥐의 두 그룹에 대한 악성종양크기, 괴사율, 악성종양 성장률, 악성종양조직의 병리학적 변화를 분석하였다. 실험결과 조직에서 악성종야세포의 괴사를 보였으며, 광조사 시간과 광량에 따라 악성종양 크기가 줄어들고 악성종양의 괴사변화를 나타냈다. 그러나 630nm와 650nm의 파장차이에 대한 악성종양의 변화의 차이는 발견할 수 없었으며 다른 정상조직에서의 손상도 발견되지 않았다.

  • 위장관이외의 장소에서 위장관 간질 종양(Gastrointestinal stromal tumor, GIST)이 발생하는 경우는 매우 드물기 때문에 일차적 병소가 아닌 전이 병소로 진단되는 경향이 있다. 이에 본 저자들은 25세 남자에서 좌측 둔부 중둔근, 소둔근에서 발생한 위장관 간질 종양이 복부의 일차 병소없이 인접한 장골 및 천골을 침범한 1례를 경험하였다. 이에 위장관외 간질종양(Extragastrointestinal stromal tumor, EGIST)이라는 진단명과 함께 보고하고자 한다.

LKB1/STK11 Tumor Suppressor Reduces Angiogenesis by Directly Interacting with VEGFR2 in Tumorigenesis

  • Cervical tumors represent a prevalent form of cancer affecting women worldwide; current treatment options involve surgery, radiotherapy, and chemotherapy. Angiogenesis, the process of new blood vessel formation, is a crucial factor in cervical tumor growth. The molecular mechanisms underlying the effects of the liver kinase B1 (LKB1/STK11) tumor suppressor protein on tumor angiogenesis have not been elucidated. Therefore, we investigated the role of LKB1 in cervical tumor angiogenesis both in vitro and in vivo in this study. Our results demonstrated that LKB1 inhibited cervical tumor angiogenesis by suppressing the expression of angiogenesis-related factors such as vascular endothelial growth factor (VEGF) and hypoxia inducible factor-1α. LKB1 directly affected both carcinoma and vascular endothelial cells, resulting in a significant reduction in tumor growth and angiogenesis. Furthermore, LKB1 was found to bind to VEGF receptor 2 (VEGFR-2) and target the VEGFR-2-mediated protein kinase B/mechanistic target of rapamycin signaling pathway in endothelial cells, thereby reducing cervical tumor growth and angiogenesis. Our study provides new insights into the molecular mechanisms underlying the anti-tumor and anti-angiogenic effects of LKB1 in cervical cancer. These findings will help develop new therapeutic strategies for cervical cancer.

Dumbbell 신경종양 -1례 보고- (Dumbbell Neurogenic Tumor)

  • Dumbbell neurogenic tumor is a rare disease, arising about 10 % of mediastinal neurogenic tumor. We report a 31 year old man who was suffered from paresthesia and weakness of lower extremities for about 8 months. Chest x-rays revealed a dumbbell shaped mass shadow in left lower posterior mediastinum. MRI demonstrated a well defined dumbbell shaped mass in the mediastinum of T7 level, including spinal cord compression by the extended tumor into the spinal canal and enlarged intervertebral foramen. The patient underwent one-stage combined resection of the tumor through the thoracotomy and laminectomy, simultaneously. The tumor was confirmed as neurilemmoma. The postoperative course was smooth and uneventful.

종격동 사구종양;1례보고 (Mediastinal Glomus Tumor; A Case Report)

  • The glomus tumor is a distinctive benign neoplasm, the cells of which resemble the modified smooth muscle cells of the normal glomus body. This tumor occurs most frequently in the extremities but may find elsewhere in the body. Only one case of mediastinal glomus tumor has been reported in the world. Recently we experienced a case of mediastinal glomus tumor managed with surgical removal and followed up without any significant complications for 17 months.

  • Single cell transcriptome analysis is a powerful tool for defining cell types or sub-populations within a heterogeneous bulk population. Tumor-associated microenvironment is a complex ecosystem consisting of numerous cell types that support tumor growth, angiogenesis, immune evasion, and metastasis. With the success of checkpoint inhibitors targeting the immune cell compartment, tumor microenvironment is emerging as a potential anti-cancer target, and understanding it has become an imminent subject in cancer biology.

Anti-tumor Activity of Acanthospermum hispidum DC on Dalton Ascites Lymphoma in Mice

  • The present study investigated the anti-tumor activity of ethyl acetate extract of Acanthospermum hispidum DC against daltons ascites lymphoma in mice. The extract was prepared by cold maceration with ethyl acetate for 3 - 7 days and evaporated in vacuum to dry. (Yield : 14.2 g, 1.42% w/w). The extract was fractionated by column chromatography by using gradient elution technique and the diterpenes fraction isolated (0.649 g). Both extract and the fraction were administered as oral suspension with tween 20 in water to tumor bearing mice (DAL) and changes in dead cell count, histopathology of tumor cells, hematological parameters and median survival time (MST) were examined and compared with that of tumor control or 5-Fluorouracil (5-FU). The results indicate that both ethyl acetate extract and fraction possess anti-tumor activity. The study suggests that Acanthospermum hispidum DC seems promising as a source of diterpenes for potential anti-tumor activity.
