복합적인 몸통 자세의 심물리학적 불편도 평가 (Psychophysical Discomfort Evaluation of Complex Trunk Postures)

  • Low back disorders (LBDs) are one of the most common and costly work-related musculoskeletal disorders. One of the major possible risk factors of LBDs is to work with static and awkward trunk postures, especially in a complex trunk posture involving flexion, twisting and lateral bending simultaneously. This study is to examine the effect of complex trunk postures on the postural stresses using a psychophysical method. Twelve healthy male students participated in an experiment, in which 29 different trunk postures were evaluated using the magnitude estimation method. The results showed that subjective discomfort significantly increased as the levels of trunk flexion, lateral bending and rotation increased. Significant interaction effects were found between rotation and lateral bending or flexion when the severe lateral bending or rotation were assumed, indicating that simultaneous occurrence of trunk flexion, lateral bending and rotation increases discomfort ratings synergistically. A postural workload evaluation scheme of trunk postures was proposed based on the angular deviation levels from the neutral position. Each trunk posture was assigned numerical stress index depending upon its discomfort rating, which was defined as the ratio of discomfort of a posture to that of its neutral posture. Four qualitative action categories for the stress index were also provided in order to enable practitioners to apply corrective actions appropriately. The proposed scheme is expected to be applied to several field areas for evaluating trunk postural stresses.

정상 성인에서 체간 굴근, 신근의 근력과 요추 전만각의 상관관계에 관한 연구 (A Study on Correlation between Power of Trunk Flexors, Extensors and Lumbar Lordotic Angle in Normal Adults)

  • Objectives : The purpose of this study was to investigate the correlation between lumbar lordotic angle and the power of trunk flexors, extensors in normal adults Methods : 34 normal participants participated in this study. Their lumbar lordotic angle(L1-S1 Cobb's angle and L1-L5 cobb's angle) was measured by x-ray taken on lateral direction, erect cross-arm position. And muscle power of trunk flexors and extensors of each participant measured using Cybex HUMAC NORM. Results : 1. The average of L1-S1 Cobb's angle was $47.21{\pm}8.88^{\circ}$ and the average of L1-L5 Cobb's angle was $36.32{\pm}9.62^{\circ}$(Table IV). 2. The average ratio of trunk flexors/extensors was $6.44{\pm}19.31%$(Table V). The average power of the trunk flexors was $165.18{\pm}55.05$(Newton-Meter/kg), and the power of trunk extensors was $257.18{\pm}85.53$ (Newton-Meter/kg)(Table VI). 3. Lumbar lordotic angle has no relation to the ratio of trunk flexors/extensors(Table VII, Fig. 4). 4. Lumbar lordotic angle has no relation to both the power of the trunk flexors and extensors(Table VIII, Fig. 5, Fig. 6). Conclusions : These results suggest that the lumbar lordotic angle measured by radiograph could not evaluate the power and ratio of trunk flexors, extensors.

Hook-lying 자세에서 한쪽 다리 들기시 지지면의 안정성에 따른 복사근의 근활성도 비교 (Comparison of Abdominal Oblique Muscle Activity During Leg Raising in Hook-lying Position According to Surface Conditions)

  • To improve trunk stability, various exercise protocols were introduced into the clinical field. Trunk and lumbar stability exercises on unstable surfaces are especially recommended to improve lumbar stability. The purpose of this study was to compare abdominal oblique muscle activity during leg raising in hook-lying position among 3 different type of surface conditions (on floor (F), vestibular board (VB), and foam roll (FR)). Sixteen able-bodied volunteers, who had no medical history of lower extremity or lumbar spine disease, were recruited for this study. Surface electromyography (EMG) activity was recorded from the internal and external oblique muscles of both sides. The normalized EMG activity was compared using a one-way repeated ANOVA. The results showed that the EMG activities of the internal oblique and external oblique of the lifted leg side during straight leg raising significantly increased under the FR condition when compared to the F condition. There was no significant difference of the EMG activity in abdominal oblique muscles between the VB and the FR conditions. The EMG activity of the internal oblique of supported leg side during the straight leg raising was significantly greater under the FR condition than the VB and F conditions (p<.05). The composition ratio of EMG activity of internal oblique muscles during straight leg raising was significantly increased under the FR condition. Therefore, straight leg raising exercise on foam roll in hook lying position could be beneficial to improve trunk and lumbar stability.

엎드린 교각운동 자세에서 견갑골 전인 운동 시 익상 유무에 따른 다리 들기 차이가 체간근의 근 활성도에 미치는 영향 (The Effect of Legs Difference on The Trunk Muscle Activities With and Without Winging Scapular During Scapular Protraction in Prone-Bridge Position)

  • Background: The purpose of this study was to investigate the effect of leg lift difference on serratus and upper trapezius when exercising in a scapula in a prone position, a typical waist stabilization exercise for subjects with a winged scapula. Method: Twenty normal adults and 20 subjects with winged scapula participated in the experiment. The surface EMG recordings were obtained from external oblique muscle and internal oblique muscle during scapula protraction exercise. The presence or absence of winging of the shoulder bone was measured using an electronic digital caliper for the distance the medial border of the scapula is lifted to the rear. In prostrate pier movement posture in both groups, both legs supporting, dominant leg lifting, and non-dominant leg lifting including the scapula protraction were conducted respectively. Results: In the results of comparison between the two groups, the dominant external oblique muscle and the non-dominant internal oblique muscle tended to increase according to the difference of the leg lifting of normal people. In the winged scapula group, internal oblique muscle showed increased muscle activity more than external oblique muscle. Conclusion: It was most effective to exercise with lifting the same position leg for strengthening the same external oblique muscle, and the opposite internal oblique muscle. Also, it is effective to exercise in prone pier movement posture for trunk stability. In addition, internal oblique muscle shows increased muscle activity in subjects with winged scapula. Therefore, appropriate adjustment of external oblique muscle and internal oblique muscle may have a positive effect on scapula dysfunction for trunk stability.

플렛타이 인력물자취급서 몸통 비틀기에 따른 신체자세 동요에 대한 연구 (Effects of Trunk Twist on Postural Sway During Manually Handling Flat Ties)

  • We investigated the effects of trunk twist on postural stability during manually handling flat ties. Ten male subjects participated in this study. While handling 5kgf and 10kgf bundles of flat ties respectively, their centers of pressure (COPs) were measured under two levels of body position (twisted and fixed), two levels of direction (left and right), and three levels of object position ($30^{\circ}$, $45^{\circ}$, and $60^{\circ}$). Subjects' postural stability was quantified by calculating the sway length. Results showed that the effect of different object position was significant on postural sway length in subject's medio-lateral axis. Post-hoc multiple comparions revealed that, under the 5kgf load condition, the sway length was increased significantly as the object position increased to $45^{\circ}$. Under the 10kgf load condition, however, the sway length was increased significantly at the object position of $60^{\circ}$. Actual or potential applications of this research include guidelines for the design of working posture evaluation techniques.

Effect of Paretic and Non-paretic Side Spine Taping on Balance Ability in Patients with Stroke

  • Background: A number of researchers have attempted to improve the balance of stroke patients, however there is still a question as to whether taping is effective in increasing balance. Objective: To determine the effect of paretic and non-paretic side taping on the balance ability in patients with stroke. Design: A single-blind randomized controlled trial Methods: This randomized single-blind controlled clinical trial with a repeated measures study included 45 subjects who were randomly assigned to paretic side taping groups (n=15), non-paretic side taping groups (n=15), and trunk exercise groups (n=15). Trunk exercise and paretic side taping groups had taping on the paralyzed erector spinae, while the non-paretic side taping group had taping on the non-paralyzed erector spinae. Trunk exercises were performed for 30 minutes to promote core muscles. The balance ability measured the center of pressure movement (paretic side, non-paretic side,forward, backward, limit of stability) in the sitting position. All measurements were evaluated using BioRescue. Results: All three groups showed significant increase in all variables after 4 weeks. The paretic and non-paretic side taping groups had a significant increase in all variables after 30 min of attachment. However, there was no significant difference among the three groups. Conclusions: Paralysis and non-paralysis taping improved the balance ability of patients with stroke in an immediate effect of 30 min. However, after 4 weeks of intervention, taping with trunk exercise did not differ from single trunk exercise. In future studies, various analyses need to be conducted through more diverse evaluations.

The Effect of Robo-horseback Riding Exercise on Trunk Muscle Activity Ratios in Patients with Low Back Pain

  • Purpose: We investigated to identify the effect of robo-horseback riding exercise (RHRE) on trunk muscle activity ratios for patients with low back pain. Methods: Twelve patients with low back pain and twelve healthy adults were recruited for this study. Subjects performed the RHRE with the neutral spine position. The amplitude of electromyography activity was recorded from the selected trunk muscles (internal oblique [IO], rectus abdominis [RA], multifidus [MF], the thoracic part of the iliocostalis lumborum [ICLT]). The ratios of the relative local muscle activity to the global muscle activity in abdominal and lumbar parts were calculated. Results: There were significant differences in the change values of the IO/RA and the MF/ICLT between low back pain (LBP) patients and healthy adults. The IO/RA ratio and the MF/ICLT ratio showed significant increase in LBP patients after training. Conclusion: The RHRE improved the trunk muscle activity ratio in patients with low back pain. The information presented here is important for investigators who use lumbar stabilization exercises as a rehabilitation exercise.

Short-term Effects of Lumbar Rotation Mobilization on the Single-Leg Standing Balance Ability in Healthy People: A Pilot Study

  • The purpose of this study was to investigate the short-term effects of lumbar rotational mobilization under the single-leg standing (SLS) position. Fifteen healthy individuals were recruited, and randomized to agroup of trunk rotational exercise (TRE) and lumbar rotational mobilization (LRM). Trunk twist rotational exercise was performed to the TRE group, and mobilization was applied to the LRM group on the lumbar spine. Velocity of the center of pressure (VCOP) and center of pressure (COP) for each participant were measured through SLS. COPs were not significantly increased or decreased after treatment in both groups. VCOPs also did not change considerably except on the right side when the eye was closed. There was no significant difference between COPs and VCOPs in two groups. This study suggests that trunk rotation exercise and lumbar rotation mobilization would have similar effects on balance ability.

  • Purpose: The purpose of this study was to investigate the effects of forward-and-backward shift trunk exercise using a proprioceptive neuromuscular facilitation (PNF) diagonal pattern in a closed kinematic chain exercise on the upper limb function and activity of daily living (ADL) in a stroke patient. Methods: One subject participated in this study. The study used a reversal A-B-A' design, where A and A' were the baseline period (no intervention), and B was the intervention period. The intervention was a forward-and-backward trunk shift exercise, using a PNF diagonal pattern on both a stand-on-hand position and a quadruped position of closed kinematic chain exercises, for 20 min per day for 2 weeks. The range of motion (ROM) of the shoulder joint was measured and a Fugl-Meyer assessment of upper extremity (FMA-UE) and a functional independence measure (FIM) were performed to measure upper limb function and activity of daily living (ADL). Results: ROMs of shoulder joint (flexion, extension, abduction, and external rotation) increased in the intervention phase. The FMA-UE score increased (from 28 to 36) in the intervention phase. The FIM score increased (from 20 to 25) in the intervention phase. These increases were maintained after intervention (Baseline II). Conclusion: These results suggest that forward-and-backward shift trunk exercises using a PNF diagonal pattern in a closed kinematic chain exercise have a positive effect on stroke patients' upper limb function and ADL ability.

대퇴근막장근의 길이가 한발서기 시 골반의 회전에 미치는 영향 (The Effect of Tensor Fasciea Latae Length on the Rotation of Pelvic during One Leg Stance)

  • Purpose : The purpose of this study was to investigate the effect of tensor fasciae latae length on the rotation of pelvis during one leg stance. Methods : 41 healthy adults participated in this study. The movement of the pelvis and trunk was measured using 3-dimensional motion analyzer, during one leg stance. The movement of the pelvis and trunk was collected lateral shift, rotation, side bending, and flexion-extension. Tensor fasciae latae length of subjects was measured in sidelying positon with neutral position of hip joint and flexion $90^{\circ}$ of knee. Also, the range of motion of hip exteral and interal rotaion were measured in prone position wih lexion $90^{\circ}$ of knee. The subjects were separated 2 groups that more pelvic rotation group(n=15) and less pelvic rotation group(n=15) according to the degree of pelvic rotation. Results : The more pelvic rotation group was showed significantly higher in the ROM of hip external rotation than less pelvic rotation group(p<0.05). The difference of tensor fasciae latae length not showed significant difference between groups. During one leg stance, The movement of the shifting and flexion-extension of trunk and pelvis were not showed significant difference. But the side bending and the rotation of pelvis and trunk showed significant difference between groups. Conclusion : The difference of tensor fasciae latae length not showed significantly in more pelvic rotation group and less pelvic rotation group. But, this study suggests that the pelvis instability brings the instability of the trunk during one leg stance.
