사이클 페달링 시 페달반력 효율성을 고려한 적정 안장높이 결정방법 (Saddle Height Determination by Effectiveness of Pedal Reaction Force during Cycle Pedaling)

  • The purpose of this study was to compare two saddle height determination methods by the effectiveness of pedal reaction force. Ten male subjects (age: $24.0{\pm}2.4years$, height: $175.1{\pm}5.4cm$, weight: $69.3{\pm}11.1kg$, inseam: $77.8{\pm}4.5cm$) participated in three minutes, 60 rpm cycle pedaling tests with the same load and cadence. Subject's saddle height was determined by $25^{\circ}$ knee flexion angle (K25) when the pedal crank was at the 6 o'clock position (knee angle method) and 97% (T97), 100% (T100), 103% (T103) of trochanter height (trochanteric method). The RF (resultant force), EF (effective force), and IE (index of effectiveness) were compared by measuring 3D motion and 3-axis pedal reaction force data during 4 pedaling phases (phase1: $330^{\circ}-30^{\circ}$, phase2: $30^{\circ}-150^{\circ}$, phase3: $150^{\circ}-210$, phase4: $210^{\circ}-330^{\circ}$). Results showed that there were significant differences in EF at phase1 between T97 and K25, in EF at phase4 between T100 and T103, in IE at total phase between T97 and K25, between T100 and T103, in IE at phase1 & phase2 between T97 and K25. There was higher IE in the K25 than any other saddle heights, which means that K25 was better pedaling effectiveness than the trochanteric method. Therefore it was suggested the saddle height as 103.7% of trochanter height that converted from K25.

  • 본 연구는 성인 뇌졸중 환자의 보행기능을 향상시키기 위해 사용되고 있는 보행보조기의 가장 이상적인 길이를 제시함으로써 뇌졸중 환자만을 위한 전문화된 맞춤형 보행보조기 길이의 사용을 독려하기 위하여 실시하게 되었다. 그 결과, 지팡이 높이에 따른 성인 뇌졸중 환자의 보행균형, 보행분석에서 보행의 속도 증가를 위해서 몸통 굴곡을 통한 무게 중심이 아래쪽에 위치하고, 하지근육의 활성도 증가를 위해서 몸통 신전을 통한 무게 중심이 위쪽으로 이동하여 척추 기립근의 수축으로 뇌졸중 환자의 지팡이 높이에 따른 균형과 보행 변화의 근거를 제시한다.

Periprosthetic Occult Femoral Fracture: An Unknown Side Effect of Press-Fit Fixation in Primary Cementless Total Hip Arthroplasty

  • Purpose: The objectives of this study were to examine the prevalence and risk factors for development of periprosthetic occult femoral fractures during primary cementless total hip arthroplasty (THA) and to assess the clinical consequences of these fractures. Materials and Methods: A total of 199 hips were examined. Periprosthetic occult femoral fractures were defined as fractures not detected intraoperatively and on postoperative radiographs, but only observed on postoperative computed tomography (CT). Clinical, surgical, and radiographic analysis of variables was performed for identification of risk factors for periprosthetic occult femoral fractures. A comparison of stem subsidence, stem alignment, and thigh pain between the occult fracture group and the non-fracture group was also performed. Results: Periprosthetic occult femoral fractures were detected during the operation in 21 (10.6%) of 199 hips. Of eight hips with periprosthetic occult femoral fractures that were detected around the lesser trochanter, concurrent periprosthetic occult femoral fractures located at different levels were detected in six hips (75.0%). Only the female sex showed significant association with an increased risk of periprosthetic occult femoral fractures (odds ratio for males, 0.38; 95% confidence interval, 0.15-1.01; P=0.04). A significant difference in the incidence of thigh pain was observed between the occult fracture group and the non-fracture group (P<0.05). Conclusion: Occurrence of periprosthetic occult femoral fractures is relatively common during primary THA using tapered wedge stems. We recommend CT referral for female patients who report unexplained early postoperative thigh pain or developed periprosthetic intraoperative femoral fractures around the lesser trochanter during primary THA using tapered wedge stems.

Bipolar Hemiarthroplasty in Unstable Intertrochanteric Fractures with an Effective Wiring Technique

  • Purpose: Bipolar hemiarthroplasty has recently been acknowledged as an effective option for treatment of unstable intertrochanteric fracture. Trochanteric fragment nonunion can cause postoperative weakness of the abductor muscle and dislocation; therefore, reduction and fixation of the fragment is essential. The purpose of this study was to perform an evaluation and analysis of the outcomes of bipolar hemiarthroplasty using a useful wiring technique for management of unstable intertrochanteric fractures. Materials and Methods: A total of 217 patients who underwent bipolar hemiarthroplasty using a cementless stem and a wiring technique for management of unstable intertrochanteric femoral fractures (AO/OTA classification 31-A2) at our hospital from January 2017 to December 2020 were included in this study. Evaluation of clinical outcomes was performed using the Harris hip score (HHS) and the ambulatory capacity reported by patients was classified according to Koval stage at six months postoperatively. Evaluation of radiologic outcomes for subsidence, breakage of wiring, and loosening was also performed using plain radiographs at six months postoperatively. Results: Among 217 patients, five patients died during the follow-up period as a result of problems unrelated to the operation. The mean HHS was 75±12 and the mean Koval category before the injury was 2.5±1.8. A broken wire was detected around the greater trochanter and lesser trochanter in 25 patients (11.5%). The mean distance of stem subsidence was 2.2±1.7 mm. Conclusion: Our wiring fixation technique can be regarded as an effective additional surgical option for fixation of trochanteric fracture fragments during performance of bipolar hemiarthroplasty.

Primary Arthroplasty for Unstable and Failed Intertrochanteric Fractures: Role of Multi-Planar Trochanteric Wiring Technique

  • Purpose: The primary objective of the current study is to demonstrate the trochanteric wiring technique. A secondary objective is to evaluate the clinico-radiological outcomes of use of the wiring technique during primary arthroplasty for treatment of unstable and failed intertrochanteric fractures. Materials and Methods: A prospective study including follow-up of 127 patients with unstable and failed intertrochanteric fractures who underwent primary hip arthroplasty using novel multi-planar trochanteric wiring was conducted. The average follow-up period was 17.8±4.7 months. Clinical assessment was performed using the Harris hip score (HHS). Radiographic evaluation was performed for assessment of union of the trochanter and any mechanical failure. P<0.05 was considered statistically significant. Results: At the latest follow-up, the mean HHS showed significant improvement from 79.9±1.8 (at three months) to 91.6±5.1 (P<0.05). In addition, no significant difference in the HHS was observed between male and female patients (P=0.29) and between fresh and failed intertrochanteric fractures (P=0.08). Union was achieved in all cases of fractured trochanter, except one. Wire breakage was observed in three patients. There were five cases of limb length discrepancy, three cases of lurch, and three cases of wire-related bursitis. There were no cases of dislocation or infection. Radiographs showed stable prosthesis in situ with no evidence of subsidence. Conclusion: Use of the proposed wiring technique was helpful in restoring the abductor level arm and multi-planar stability, which enabled better rehabilitation and resulted in excellent clinical and radiological outcomes with minimal risk of mechanical failure.

Redescriptions of Two Closely Resembling Chrysotoxum Species (Insecta: Diptera: Syrphidae) New to Korea

  • As a result of ongoing systematic study of the genus Chrysotoxum, we have discovered C. ramphostomus and C. tuberculatum for the first time in Korea. These two species closely resemble each other and need to be identified with caution. We here provide detailed redescriptions and illustrations with their diagnostic characters indicated. Chrysotoxum tuberculatum can be readily distinguished from C. ramphostomus by the following characteristics: 1) hind trochanter with small tubercle; 2) compound eyes apparently bare; 3) basal 1/5 of hind femora brownish yellow; and 4) male genitalia asymmetrical in caudal view.

일부 초등학생의 어머니를 대상으로 한 성인 여성의 골밀도에 영향을 미치는 요인에 관한 연구 (A Study on the Factors Affecting Bone Mineral Density in Adult Women - Based on the Mothers of Elementary School Students-)

  • The bone mineral density(vertebrae Ll-4, femoral neck, Ward's triangle and trochanter) of 160 healthy adult women aged 29-45 was measured and general characteristics, anthropomentric measurements, and dietary intake were also studied to determine the main factors affecting the bone mineral density(BMD) of adult women. The nutrient intake of the subjects was adequate to RDA level except energy, Ca, Fe, and vit A. The BMD of vertebrae L24 showed significant positive association with nutrient intake such as animal protein, P, Ca(animal) and Fe(animal). The BMD of the femoral neck correlated significantly with anthropometric measurements such as height, weight, waist circumference, hip circumference, body fat and body mass index. The BMD of both sites(vertebrae L24 and femoral neck) were significantly related to vertebrae L24 was daily Ca intake and that of femoral neck was daily energy expenditure. In conclusion, adequate nutrient intake, especially protein, P, Ca and Fe as well as increases in physical activity were suggested to prevent the loss of bone mineral density in adult women.

지팡이의 높이가 체중 지지분포에 미치는 영향 (Influence of Cane Length on the Weight Distribution)

  • This study investigates the influence of cane length on the weight distribution of the elderly in a standing position. Thirty participants were evaluated using two different cane lengths based on measurements of distal wrist crease to ground (WC cane), and distance of greater trochanter to ground (GT cane). A limloader was used to determine the weight distribution on the subject. It was found that 6.5% of body weight was supported on a GT cane and 7.7% of body weight supported on a WC cane in a standing position. Results suggests that more weight is distributed on a WC cane than a GT cane in the elderly.

십전대보탕 과립제 단일치료로 호전된 2도 욕창환자 치험례 (A Case Study of a Stage Two Pressure Sore Treated with Sipjeondaebo-tang Extract)

  • Objective: To evaluate the effects of Sipjeondaebo-tang (十全大補湯) extract on a stage two pressure sore. Methods: Sipjeondaebo-tang extract (5 g) was given three times a day to a treat the pressure sore. Results: The pressure sore in the sacrococcygeal region was improved. A second pressure sore ofthe right greater trochanter region, which emerged during the study, was also improved. Conclusions: Sipjeondaebo-tang extract may represent an effective treatment for pressure sores, but further studies are needed.

한국산 유령거미속(거미목: 유령거미과)의 1신종 (A New Species of Pholcus (Araneae: Pholcidae) from Korea)

  • 약유령거미과, 유령거미속에 속하는 신종, 새재유령거미 (신칭) (Pholcus joreongensis sp. nov,)를 그림과 함께 기재한다. 이 종은 중국의 고유종인 P.ciavimaculatus와 수컷 더듬이기관의 구돌기(uncus)와 삽입기의 모양에서 많이 닮았으나, 구돌기의 크기, 삽입기의 길이, 후측면부배엽 (procursus)과 전절돌기의 모양 그리고 셋째와 넷째 다리의 길이에서 잘 구별이 된다.
