Purpose: Amendment to the Act on Decisions on Life-sustaining Treatment was recently enacted to designate long-term care hospitals as providers of hospice and palliative care. Despite its benefit of providing improved accessibility to end-of-life care, the amendment has raised concerns about its effect on quality of service. This study aimed to use information obtained from an expert group interview and previous studies to compare how cancer patients, family caregivers, physicians, and the general Korean population perceive the potential benefits and risks of this amendment. Methods: We conducted a multicenter cross-sectional study from July to October 2016. The included participants answered a structured questionnaire regarding the extent to which they agree or disagree with the questionnaire items indicating the potential benefits and risks of the amendment. Chi-square tests and univariate and multivariate logistic regression analyses were performed. Results: Compared with the general population, physicians agreed more that long-term care hospitals are currently not adequately equipped to provide quality hospice and palliative care. Family caregivers found improved access to long-term care hospitals more favorable but were more likely to agree that these hospitals might prioritize profits, thereby threatening the philosophy of hospice care, and that families might cease to fulfill filial responsibilities. Compared with the general population, cancer patients were more concerned about the potentially decreased service quality in this setting. Conclusion: Although potential service beneficiaries and providers expected improved accessibility of hospice and palliative care services, they were also concerned whether the system can provide adequate quality of end-of-life care.
Objectives: The purpose of this study was to compare the microshear bond strength (${\mu}$SBS) and bonding interfaces of two-step total-etching and self-etching adhesive systems to three etch types of dentin either the acid etched, laser etched or laser and acid etched. Materials and Methods: The occlusal dentinal surfaces of thirty human molars were used. They were divided into six groups: group 1, 37% $H_3PO_4$ + Single Bond 2 (3M ESPE); group 2, Er:YAG laser (KEY Laser 3, KaVo) + Single Bond 2; group 3, Er:YAG laser + 37% $H_3PO_4$ + Single Bond 2; group 4, Clearfil SE Primer + Bond (Kuraray); group 5, Er:YAG laser + Clearfil SE Bond; group 6, Er:YAG laser + Clearfil SE Primer + Bond. The samples were subjected to ${\mu}$SBS testing 24 hr after bonding. Also scanning microscopic evaluations were made on the resin-dentin interfaces of six specimens. Results: The ${\mu}$SBS of group 2 was significantly lower than that of groups 1 and 3 in Single Bond 2 (p < 0.05). There were significant differences among the uSBS of groups 4, 5, and 6 in Clearfil SE Bond (p < 0.05). Very short and slender resin tags were observed in groups 2 and 5. Long and slender resin tags and lateral branches of tags were observed in groups 3 and 6. Conclusions: Treatment of dentin surface using phosphoric acid or self-etching primer improved the adhesion of Er:YAG lased dentin.
This study was performed to verify the possibility of MTA and calcium sulfate as a pulp capping agent through comparing the dental pulp response in dogs after capping with MTA, calcium sulfate, and calcium hydroxide. 24 teeth of 2 dogs, 8 month old, were used in this study. Under general anesthesia, cervical cavities were prepared and pulp was exposed with sterilized #2 round bur in a high speed handpiece. MTA calcium hydroxide, and calcium sulfate were applied on the exposed pulp. Then the coronal openin,fs were sealed with IRM and light-cured composite. Two months after treatment, the animals were sacrificed. The extracted teeth were fixed in 10% neutral-buffered formalin solution and were decalcified in formic acid-sodium citrate. They were prepared for histological examination in the usual manner. The sections were stained with haematoxylin and eosin. In MTA group, a hard tissue bridges formation and newly formed odontoblasts layer was observed. There was no sign of pulp inflammatory reaction in pulp tissue. In calcium hydroxide group, there was no odontoblast layer below the dentin bridge. In pulpal tissue, chronic inflammatory reaction with variable intensity and extension occurred in all samples. In calcium sulfate group, newly formed odontoblast layer was observed below the bridge. Mild chronic inflammation with a few neutrophil infiltrations was observed on pulp tissue. These results suggest that MTA is more biocompatible on pulp tissue than calcium hydroxide or calcium sulfate.
Medical nutrition therapy (MNT) is considered a keystone of medical treatment of chronic diseases. However, only few studies have evaluated medical and economical outcome of MNT. The study was performed on the patient with type 2 diabetes mellitus to evaluate the effect of clinical and cost-effective outcomes of MNT. Subjects from two general hospitals were randomly assigned to two different groups; One receiving basic nutritional education (BE) (n = 35), and the other receiving intensive nutritional education (IE) (n = 32) for a 6-month clinical trial. The group which received BE had a single visit with a dietitian, while the other group which received IE had an initial visit with a dietitian addition to two visits during the first 4 weeks of the study periods. Anthropometric parameters, blood components, and dietary intake were measures at the beginning of study period and after 6 month. Cost-effective analysis included direct labor costs, educational materials and medication cost difference during 6 months. After 6 month, subjects from IE group showed significant reduction of body weight (p <0.05) and systolic blood pressure (p <0.05), whereas BE group did not show any significant changes. Result from biochemical indices showed glycated hemoglobin concentration was significantly reduced by 0.7% (p <0.05) only in the IE group. The ratio of energy intake to prescribed energy intake decreased significantly in both groups (p <0.05). Mean time taken for a dietitian to educate the subject was 67.9 ${\pm}$ 9.3 min/person for BE group, while 96.4 ${\pm}$ 12.2 min/person for IE group. Mean number of educational materials was 1.9 ${\pm}$ 0.7/person for BE group and 2.5 ${\pm}$ 0.7/person for IE group. Change in glycated hemoglobin level along the 6 month period of study can be achieved with an investment of \88,510/% by implementing BE and \53,691/% by implementing IE. Considering the net cost-effect of blood glucose control and HbA Ic, IE which provides MNT by dietitian had a cost efficiency advantage than that of BE. According to this study, MNT provided by dietitian had a significant improvements in medical and clinical outcomes compared to that of BE intervention. Therefore, MNT protocol should be performed by systemic intensive nutrition care by dietitian in clinical setting to achieve good therapeutic results of DM with lower cost.
We have studied quality changes of fully ripe fruit of Korean native pumpkin 'Yangsan' regarding the following parameters: pH, sugar content, weight, water content, contents of crude protein and amino acids during 60 days storage at room temperature. As the results, there was no changes in sugar contents according to the storage period, but pH was changing to a little acidic direction with slight decrease in weight and water content. Contents of total crude proteins and comprising amino acids were increased during the storage period. The main contents of amino acids of the Korean native pumpkin, Yangsan, were glutamic acid (15.5%), aspartic acid (10.1%), lysine (8.7%), valine (7.5%), leucine (7.1%) and alanine (6.6%), which were not highly influenced during storage period. Additionally we have investigated the content of free amino acids and color changes during processing of Yangsan under high temperature at $121^{\circ}C$ and high pressure at $1\;kg/cm^2$. In fully ripe fruits, a total of 29 kinds of free amino acids were detected including 8 kinds of essential amino acids (histidine, isoleucine, leucine, lysine, phenylalanine, methionine, threonine and valine). More than 35% of total free amino acids were aspartic acid (20.3%) and asparagine (15.4%); ornithine, citrullin, and arginine, which are related to Ornithine cycle, were also detected in fully ripe fruits. But when treated with high temperature and high pressure, glutamic acid and arginine were decreased rapidly whereas ammnonium chloride was relatively increased. Moreover "b" value as yellow color indicator was decreased from 17.45 to 9.14 while treated for 60 minutes with high temperature and pressure, caused by the degradation of ${\beta}-carotene$ and other yellowish pigments in Yangsan.
This study was devised to investigate the effects of flavonoids on carbon tetrachloride toxicity of rats. Through the experiments, the contents of total cholesterol, transaminase(GOT, GPT) and alkaline phosphatase(Alk. P) activities in the liver and serum were determined. The results obtained are summarized as follows; (1) The liver cholesterol concentration of flavonoid injected groups were remarkably increased up to 45% at 2 days after $CCl_4$ injection and then the values were almost recovered to the level of control group at 4 days after $CCl_4$ injection. The increase of serum cholesterol content was inhibited by the flavonoids after $CCl_4$ injection, but the result was not significant. (2) Flavonoids injection was found to have the ability to decrease the elevated serum GOT, Alk. P activities resulting from injection of $CCl_4$ and induce rapid recovery from such an elevated level and the extent of such a decreasing action was greatest in hesperidin injection group. The liver microsomal Alk. P and GOT activities were not affected by the treatment of flavonoids. (3) The elevated serum and liver microsomal GPT activities induced by $CCl_4$ injection were inhibited by the injection of flavonoids.
This study was carried out to investigate the effects of the level of cracked whole barley on daily feed intake, daily body weight gain, carcass characteristics and meat composition of finishing Hanwoo steers (feeding from 24 months to 30 months of age). The dietary treatments were consisted of five types (C; normal concentrate as a basal diet, T1; 10% addition of cracked whole barley, T2; 20% addition of cracked whole barley, T3; 30% addition of cracked whole barley, T4; 40% addition of cracked whole barley). A total 30 Hanwoo steers (588.6${\pm}$11.8kg) were allocated to 5 feeding groups. The daily feed intake and daily body weight gain were high in the order of T2 > T3 >T4 > T1 > C. The back fat thickness and longissmus muscle area were highest in C and T1, respectively (P<0.05) than other treatments. The meat yield index decreased with increased back fat thickness. The marbling score and meat quality were highest in T1 (P<0.01, 0.05, respectively), but maturity, fat color and meat color were not significantly different among treatments. The crude fat was highest in T1 (17.59%), while in T4 (7.47%) it was lowest (P<0.05). The crude protein and crude ash were not significantly different among treatments. The energy value of cracked whole barley treatments (T1, T2, T3, T4) was higher than C (P<0.05). The contents of Ca, Cu, K, Mg, Mo, Na and Zn were higher in C than the other treatments (P<0.01), but Co was higher in T2 (P<0.01). The CIE $L^*$ value of whole cracked barley treatments (T1, T2, T3, T4) was higher than C, however there were not differences among the treatments. The CIE $a^*$ value was highest in T1(P<0.05) than others. The CIE $b^*$ value was highest in C and it was decreased with increased feeding of cracked whole barley. Based on the above results, T1 treatment compared to other treatments improved the carcass quality parameters like loin muscle area, marbling score, meat quality, and CIE $L^*$ value.
This study was conducted to investigate the effect of feeding colored barley (CB) and whole crop barley (WCB) on performance, egg quality and blood composition of laying hens. A total of 280, 35 weeks old Brown Nick laying hens were allocated into the individual cage for the period of four weeks. Experimental diets contained 2,750 kcal/kg ME and 16% CP, respectively. The treatments consisted of 5, 10, 15% CB and WCB as feed ingredients with control and there were five replications in each treatment. Higher egg production was noticed at 10% level of CB and 5% level of WCB in the diets, respectively. Feed intake was higher in WCB and CB treatments compared to those of the control (P<0.05), but the feed conversion was not different. Yolk color tended to increase both in WCB and CB groups. Haugh unit seemed to increase WCB treatments. When the CB and WCB were fed at 15% level, blood total proteins, albumin, total cholesterol and triglyceride contents were decreased than that of the control birds, but total cholesterol and HDL-cholesterol tended to be higher with the increasing level of WCB. As results, no significant differences were observed in performance and egg quality with different levels of dietary WCB and CB. Thus, CB and WCB can be substituted as a feed ingredient up to 15% level in the laying hens' diet. However, further studies are required by feeding more than 15% levels of CB and WCB in the diet of laying hens.
This study was carried out to investigate the effects of the feeding level of cracked whole barley (CWB) on fatty acid composition, free amino acid content, organic acid content, pH, HDL cholesterol, total phenol and DPPH (1,1-diphenyl-2-picrylhydrazyl) radical scavenging activity of finishing Hanwoo steers. The dietary treatments were consisted of five types (C: base feed + 0% CWB, T1: base feed + 10% CWB, T2: base feed + 20% CWB, T3: base feed + 30% CWB, T4: base feed + 40% CWB). A total 30 Hanwoo steers(588.6${\pm}$11.8kg) were allocated into 5 feeding groups, and a total of thirty Hanwoo steers raised from 24 to 30 months. Linoleic acid of fatty acid composition was significantly higher for cracked CWB treatments (T1, T2, T3, and T4) than C, and T3 of CWB treatments was the highest than the other treatments (P<0.01). SFA, USFA, MUFA and USFA/SFA did not differ among the treatments. On the contrary, PUFA was significantly higher for CWB treatments(T1, T2, T3, and T4) than C, and T3 was the highest (P<0.05). EAA was higher in the order of T1 > T2 > C > T3 > T4 (P<0.05). NEAA was higher in the order of T2 >T3 >T1 >C >T4, although a statistical significance was not detected. The total organic acid content was the highest for T3 (20.15 mg/100g) and the lowest for T2 (13.19mg/100g). pH and total phenol of all treatments were did not differ. HDL cholesterol was higher in the order of T1 > C > T4 > T2 > T3 (P<0.01). DPPH radical scavenging activity was in order of T1 > T2 >T4 > C > T3 (P<0.01). Based on the above results, T1 treatment compared to other treatments have been shown to improve EAA, DHL, and DPPH radical scavenging activity.

The objective of this study was to investigative the effects of retinol supplement to IVM and/or IVC medium on maturation, fertilization and development of pig oocytes. North Carolina State University (NCSU) 23 medium containing porcine follicular fluid (pFF) was used as base medium. Each 1 uM, 5 uM and 10 uM concentration of retinol was supplemented to IVM and /or IVC medium. When the retinol was supplemented to maturation medium, the maturation rates were not different (p>0.05) among treatment groups ($66.7{\pm}6.0{\sim}69.2{\pm}5.3%$), but the developmental rate to blastocyst stage was higher (p<0.05) in $5{\mu}M$ group ($20.4{\pm}2.6%$) than in 0 uM ($13.6{\pm}2.1%$) and 10 uM groups ($9.7{\pm}1.7%$). Moreover, total cell number was significantly greater (p<0.05) in the 5 uM group ($37.0{\pm}1.6$) than in the other groups ($29.8{\pm}1.0{\sim}33.2{\pm}1.0$). Retinol supplement to maturation medium did not significantly affect the rates of fertilization and polyspermy (p>0.05). When the retinol was supplemented to culture medium or both maturation and culture medium, the rates of cleavage, and develop to morula and blastocyst stage were not affected, while those of 10 uM group were significantly decreased (p<0.05). These results indicate that 5 uM retinol supplement in maturation medium significantly stimulates embryo development, also improves the total cell number of blastocyst stage in pig.
