Purpose: The success of the bonding between electroformed gold and ceramic is dependent on the surface treatment of the pure gold coping. The purpose of this study was to evaluate the bonding strength between the electroformed gold and ceramic with varying surface treatment. Materials and methods: A total of 32 disks,8 were using conventional ceramometal alloy, 24 were using electroforming technique as recommended by manufacturer, were prepared. 24 electroformed disks were divided 3 groups according to surface treatment, i.e. 50 microns aluminium oxide sandblasting(GES-Sand), gold bonder treatment(GES-Bond) and $Rocatec^{TM}$ system(GES-Rocatec). For control group of conventional alloy 50 microns aluminium oxide treatment was done(V-Supragold). Energy dispersive x-ray analysis and scanning electron microscope image were observed. Using universal testing machine, shear bond strength and bonding failure mode at metal-porcelain interface were measured. Results and Conclusion: The following conclusions were drawn: 1. In the energy dispersive x-ray analysis, the Au was main component in electroformed gold(99.9wt%). After surface treatment, a little amount of $Al_2O_3(2.4wt%)$ were found in GES-Sand, and $SiO_2(4wt%)$ in GES-Bond. In GES-Rocatec, however, a large amount of $SiO_2(17.4wt%)$ were found. 2. In the scanning electron microscopy, similar pattern of surface irregu larities were observed in V-Supragold and GES-Sand. In GES-Bond, surface irregularities were increased and globular ceramic particles were observed. In GES-Rocatec, a large amount of silica particles attached to metal surface with increased surface irregularities were observed. 3. The mean shear bond strength values(MPa) in order were $22.9{\pm}3.7(V-Supragold),\;22.1{\pm}3.8(GES-Bond),\;20.1{\pm}2.8(GES-Rocatec)\;and\;13.0{\pm}1.4(GES-Sand)$. There was no significant difference between V-Supragold, GES-Bond, and GES-Rocatec. (P>0.05) 4. Most bonding failures modes were adhesive type in GES-Sand. However, in V-Supragold, GES-Bond and GES-Rocatec, cohesive and combination failures were commonly observed. From the result, with proper surface treatment method electroformed gold may have enough strength compare to conventional ceramometal alloy.
This study was carried out to investigate the solid wood bending properties of red pine(Pinus densiflora S. et Z.)naturally grown in Korea by boiling treatment The specimens were green and their dimension was $15\times15\times350mm$. The optimum conditions for solid wood bending processing of pine lumber are shown in Table 6 and the minimum solid bending radius was 240 mm in boiling treatment. The effects of knot on the degradation of bending processing properties were very severe and the failures of diagonal and torn grained wood were 20% and 30% respectively.
When the porcelain fused to metal restorations were fractured at the metal interface, various techniques and materials for intraoral porcelain repair have been suggested. The purpose of this study was to investigate the effect of metal surface treatment method and water storage on the shear bond strength of four porcelain repair systems. : Clearfil(Kuraray), All-bond(Bisco), Superbond C & B(Sun Medical), Panavia OP(Kuraray). After the metal surfaces of the specimens were sandblasted by aluminum oxide or roughened by diamond point, they were stored in double deionized water(24 Hr., $37^{\circ}C$) and thermocycling was performed(24 Hr., 1024 cycles), and again half of specimes were stored in water bath(2 Months, $37^{\circ}C$). Mean shear bond strength and mode of failure were recorded. The results of this study were obtained as follows : 1. Differences were observed between the sandblasted and diamond - treated specimens in Clearfil, All-bond, and Superbond. No statistically significant differences were observed in Panavia. 2. The 2-month storage time significantly affected the bond strength of All-bond and Superbond. No statistically significant differences were observed in Clearfil and Panavia. 3. The failures were observed at the interface between opaque resin and the metal in Clearfil and All-bond. 4. The failures were observed at the interface between opaque resin and veneered resin in Panavia. The failures were observed at the interface between opaque resin and veneered resin in Superbond, but 40% of them were fractured at the interface between the metal and opaque resin after 2-month storage time.
1979년 2월부터 1986년 9월까지 서울대학교 병원 치료방사선과에서 치료받은 상악동의 편평상피 암 환자 73예를 분석하였다. 41예에서는 수술과 수술후 또는 수술전 방사선치료를 병용하여 치료하였고 32예에서는 방사선치료만을 주치료로 시행하였다. 전체환자의 5년 생존율은 $40.4\%$였으며 방사선치료만을 주치료로 시행한 집단의 5년 생존율은 $22.1\%$였고 수술과 방사선치료를 함께 시행한 집단의 5년 생존율은 $65.3\%$였다. 치료 후 사망이 확인된 환자 가운데 상세한 치료실패 과정을 알 수 있었던 31예 중 잔류병소를 포함 국소치유실패를 나타냈던 경우가 22예($71.0\%$)였고 경부임파절 재발로 인한 치유실패가 6예($19.3\%$)였으며 인격전이로 인한 치유실패가 3예 ($9.7\%$)에서 나타났다. 그러므로, 상악동 암의 국소치유율을 증가시켜서 생존율을 높이기 위해서는 계획된 수술과 방사선 치료의 병용요법이 필요할 것으로 생각된다.
The purpose of this study was to evaluate the clinical validity of multi-rooted teeth subjected to root-resection treatment. Over a period of 1-7 years, 60 root-resected molars in 59 patients were examined clinically and radiographically. All patients were p0eriodically recalled once or twice a year. Root-resections were due to periodontal, more specifically furcal bone loss or marginal bone loss,in 34 cases and extensive dental carious destruction in 10 cases. In other cases,root-resections resulted from 6 cases of root fracture, 6 cases of periodontal-endodontic combined lesion, and 1 case of endodontic problem. Root-resection was carried out on 26 maxillary molars and 34 mandibular molars. The results are as follows; 1. 14 cases(23.3%) were considered failures. 8 cases(13.3%) of them occurred within the first year(8 cases, 13.3%), 4 cases(6.7%)between 1-3 years, and 2 cases(3.3%) between 4-7 years. 2. 8 cases(13.3%) were considered failures due to periodontal reasons, 3 cases(5%) due to root fracture, 2 cases(3.3%) due to endodontic problem, and 1 case(1.7%) due to prosthetic problem. 3. 37 cases(61.7%) showed up for the recall appointments, and the percentage of failures(13.5%) was lower compared with that of all patients(23.3%). The results of the present study indicate that the prognosis of root-resected teeth is favourable if attention is paid to the selection of proper case and to achieving optimal oral hygiene and periodic check up.
Ease of manipulation, adequate mechanical properties, long years of experience and economical cost are the factors which have established amalgam as the most widely used material for dental restorations. But amalgam restoration may require replacement because of secondary caries, fracture, "fall-out", dimensional change, tarnish or corrosion etc.. These failures of amalgam restorations seem to arise from failures during operations rather than from the inherent shortcomings of the material itself or of the patient's mismanagement. It is anticipated that notonly number of analgam restoration, but failures will be increase after more extensive utilization of the medical insurance which began in 1977. Then authors think that it would be helpful for the development of better treatment in daily dental practice, to know the duration of amalgam restorations and the reasons for their replacement. The data for this survey was compiled from 2, 856 out-patients of the Department of Dentistry, Ewha Woman's University Hospital from January 1975 to December 1977. 260 cases among 1,718 fillings were studied, of which 205 cases both had a single reason for replacement and recognized the date of the previous filling. The results obtained were as follows; 1. Amalgam fillings were 58. 5 percent of all dental restorative materials. Of these, 15. 13 percent of the amalgam restorations had to be replaced. 2. The first reason for replacement of amalgam restorations was secondary caries (56.10%), the second was fracture (23.80%) and the third was "fall-out" (8.78%). 3. Among those amalgms requiring replacement, 52.2 percent had been in place less than 3 years, 70.7 percent within 5 years and 89.8 percent had been in place less than 10 years. Only 10.2 percent had been in place more than 10 years.
All-ceramic prostheses are widely used to fulfill the high esthetic demand. However, bonding failure between zirconia and porcelain is one of the all-ceramic prostheses failures. In order to improve clinical sucess of all-ceramic prostheses, laboratory or in-office surface conditioning techniques on zirconia have been studied.
A total of 47 patients with a diagnosis of nasopharyngeal carcinoma was treated in Department of Therapeutic Radiology, Seoul National University Hospital during last 4 years. Of the 47 patients, 23(49%) had undifferentiated carcinoma, 20(43%) had squamous cell carcinoma, while 4(8%) had lymphoepithelioma. Most of the patients(71%) has Stage IV disease, cervical lymph node metastases were found in 36(77%) and distant metastasis was found in 1 at the time of diagnosis. Complete response rate after radiotherapy for 47 patients of nasopharyngeal carcinoma was 85.1%. The overall actuarial 3 year survival rates was 0.718 and the disease free actuarial 3 year survival rates was 0.468. Nodal involvement and symptom duration were statistically significiant influencing factors for actuarial survival rate. Treatment failures were found in 20 patients (42.6%), local recurrence only in 6(30%), local and neck recurrence in 3(15%), local recurrence with metastasis in 4(20%) and distant metastasis only in 7(35%). Local failures were more frequent in the patients with cranial nerve symptoms (P=0.032). Distant metastases were more frequent with T4 lesions (P=0.047), and with nodal involvement (P<0.01). Retreatment after the tumor recurrence was chemotherapy and/or radiotherapy, two pationts refreated for local recurrence were alive without evidence of disease for more than 19 and 44 months after retreatment.
Implant material, implant design, surface quality, status of the bone, surgical technique, and implant loading conditions were regarded as prerequisites for osseointegration which is a prime condition for implant success. The aim of this review paper was to investigate the survival rate of single implants in relation to the prerequisites for osseointegration. Fifty-eight papers reporting survival rates of single implants were selected by use of the 'PubMed' and hand searching. The survival rate of single implants were assessed with reference to factors influencing osseointegration. The results showed that single implants in general showed a high survival rate except a few failures in certain extreme conditions and early stages. Those failures and complications such as screw loosening and esthetic problem were almost solved with the development of implant components and surgical techniques and a better understanding of biology around a single implant. Single-tooth implant-replacement is now considered as a reliable and predictable treatment option for a single missing tooth and its application seems to expand to compromised situations which were previously thought to be impossible for single implant therapy.
Objective: The purpose of this study was to evaluate the effects of self-ligating brackets (SBs) and other factors that influence orthodontic treatment outcomes. Methods: This two-armed cohort study included consecutively treated patients in a private practice. The patients were asked to choose between SBs and conventional brackets (CBs); if any patient did not have a preference, he or she was randomly allocated to the CB or SB group. All patients were treated using an identical archwire sequence. Evaluated parameters were as follows: treatment duration, number of bracket failures, poor oral hygiene, poor elastic wear, extraction, use of orthodontic mini-implants (OMI), OMI failure, American Board of Orthodontics (ABO) Discrepancy Index (DI), arch length discrepancy, and ABO Cast-Radiograph Evaluation (CRE) score. Stepwise regression analysis was performed to generate the equation for prediction of the CRE. Results: The final sample comprised 134 patients with an average age of 22.73 years. The average DI, CRE, and treatment duration were 21.81, 14.25, and 28.63 months, respectively. Analysis of covariance showed a significant difference in CRE between the CB and SB groups after adjusting for the effects of confounding variables. Stepwise regression analysis using four variables, namely extraction, SB use, poor elastic wear, and additional appliance use, could explain only 25.2% of the variance in the CRE. Conclusions: Although the CRE was significantly better for CBs than for SBs, the clinical significance of this result seems to be limited. Extraction, SB use, poor elastic wear, and additional appliance use may have significant effects on treatment outcomes.
