Dental students' clinical confidence in denture treatment after clinical experiences (의치진료 경험 후 치과대학 학생들의 임상진료능력에 대한 자기인식도)

  • Purpose: The purpose of this study is to investigate dental students' self-perception of clinical care ability after a denture treatment experience. Materials and methods: Of the 58 fourth-year students at the dental school in 2019, 50 students completed the questionnaire concerning their denture treatment session, including treatment planning and pre-prosthetic treatment (4 questions), clinical and laboratory procedures (20 questions), and students' opinions on clinical denture education (4 questions). Each question was answered on a five-point scale, of which points four and five, "agree" and "strongly agree" respectively, were classified as positive responses while points one and two, "disagree" and "strongly disagree" respectively, were classified as negative. Results: All the items on clinical and laboratory procedures received a positive response rate of over 60%, and the overall self-perception of the students appeared to be positive. However, the questions on the ability to perform denture treatment independently after graduation received only 48% of positive answer. Those on patient management, explanations of denture precautions, customized tray production, boxing, and work model production were answered with the highest positive response rate whereas those on treatment planning, final impression acquisition, and surveying, were the lowest in each session. Conclusion: The dental students with experience in denture treatment generally have a positive opinion of their clinical care ability, but further education may be necessary to improve their ability to treat their patients independently.

Factors Influencing Self Determination for Withdrawing Life-Sustaining Treatment of the Community Dwelling Elderly (지역사회거주 노인의 연명치료 중단에 대한 자기결정 영향 요인)

  • Purpose: This study was conducted to identify factors that influence on self-determination for withdrawal of life-sustaining treatment in the community dwelling elderly. Methods: This study used a descriptive correlational design. A convenience sample of 201 elderly were recruited from a welfare center located in Seoul, Korea. Data collection was done from September 1 to September 18, 2015. Four survey questionnaires were used: Multidimensional Scale of Perceived Social Support (MSPSS), Life Satisfaction Index-Z (LSI-Z), Attitude on Do Not Resuscitate (DNR), and Self-determination for Withdrawing Life-sustaining Treatment. Data analysis was done by t-test, ANOVA, Pearson's correlation, and hierrarchical multiple regression. Results: Hierarchial multiple regression showed that the factors predicting the level of self-determination for withdrawing life-sustaining treatment were educational level, DNR experience in family, life satisfaction, and DNR attitude. These factors explained 44% of the self-determination for withdrawing life-sustaining treatment in community dwelling elderly. Conclusion: The results of this study suggest developing educational programs aiming at changing elderly's DNR attitude positive ways so that they can enhance self-determination for withdrawing life-sustaining treatment. Further study is needed with more elderly population in extended areas.

  • The completely edentulous patient has few treatment options in conventional dentistry. When implants are considered, treatment plans range from a 2-implant overdenture to a completely implant-supported prosthesis. Fixed prosthesis is often the preferred selection of the edentulous patient. fixed full-arch cert amo-metal restorations can be a predictable implant treatment modality for the edentulous patient. Implant-supported fixed prosthesis has several advantages: predictability, fixedness, retrievability, improved function, lower maintenance of prosthesis, long-term published success. Edentulous patients with a severely resorbed mandible often experience problems with their dentures. Treatment concepts involving two to four implants for the support of an overdenture have been proposed. There seems to be no need to insert more than two endosteal implants to support an overdenture, however, long-term prospective studies are needed to support this notion. Using short endosseous implants and an overdenture in the extremely resorbed mandible is a justified treatment option because of the relative simplicity and low morbidity of this treatment strategy. Implant-supported overdenture has several advantages: Cost, retrievability, hygiene access, profile and contour control, increased retention and stability, implant installed in a predicted region(ant. mandible).

Prosthodontic Treatment of Persons with Disabilities Under Dental Sedation and General Anesthesia (치과진정법과 전신마취를 이용한 장애환자의 보철치료)

  • In most persons with disabilities had poor oral hygiene because of less attention and ability to perform a dental care. So the increased prevalence and severity of dental disease were common oral state. Although most persons with disabilities need a adequate dental treatment, it is often very difficult to treat because of noncooperativity and involuntary muscle movements. Dental treatments under sedation and general anesthesia were make to provide a high-quality dental service because of decreased anxiety and fear associated dental treatment in persons with disabilities. The dental professionals must be able to select and apply the proper sedation methods in agreement with the characteristics of the disabilities, general conditions, sedation experience and capacity of dentist, type and time of dental treatment, equipments of dental clinic, consent of patient's protector. The proshodontic treatment procedures, such as abutment preparation, dental impression taking process, try-in process of prosthesis and adjustment of occlusion, are difficult even for patients without disabilities. Those procedures are more difficult to patients with disability because it's too hard to control breathing and muscle. In this report, we performed prosthetic dentistry procedures to three patients with disabilities under dental sedation and general anesthesia.

The Study on the Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine (SCM) (소양인(少陽人) 망음증(亡陰證)에 관한 고찰 - 갑오본(甲午本)과 신축본(辛丑本)을 중심으로 -)

Objectives: This research was proposed to find out how the recognition on Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine(SCM) evolved from "Donguisusebowon-GabObon(東醫壽世保元甲午本)"(DGO) to "Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)"(DSC). 2. Methods: We compared "Donguisusebowon-GabObon(東醫壽世保元甲午本)"(DGO) with "Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)"(DSC) to know how the symptom, mechanism, treatment principle and treatment methods on Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine(SCM). 3. Results and Conclusions: (1) The way to categorize the Exterior Disease and Interior Disease was different between DSC and DGO. (2) There was only a fundamental mechanism about the Boktongseolsa-syndrome(gastralgia and diarrhea) and no treatment principles and treatment methods about it in DGO. But the Boktongseolsa-syndrome was evolved into Mangeum-syndrome(亡陰證) in DSC with a detailed mechanism and treatment principles of it. (3) The evolution of Mangeum-syndrome(亡陰證) in DSC from Boktongseolsa-syndrome in DGO was attributed to the experience of treatment on Seomeo-syndrome(譫語證).

Relationship between depressive symptoms and unmet dental treatment according to gender of the elderly in Korea: 7th National Health and Nutrition Survey (우리나라 노인의 성별에 따른 우울 증상과 미충족 치과 치료의 관련성: 제7기 국민건강영양조사 자료 활용)

  • Objectives: This study aimed to study the association between unmet dental treatment and depression in the dental area of the elderly. Methods: The data was from the 7th period of the National Health and Nutrition Examination Survey. Multiple logistic regression analysis was performed to evaluate the relationship between depression and unmet dental treatment when confounding factors such as income quintile and smoking were considered. Statistical software, SAS 9.4 version was used. Results: After correcting all confounding factors, the analysis showed that the experience of unmet dental treatment was 2.73 times more likely among depressed men and 2.52 times more likely among depressed women (p<0.05). Conclusions: The results of this study suggested that we should consider that depression in the elderly can affect unmet dental treatment regardless of gender.

  • Aus-tempering heat treatment is suitable for thin and small-sized in precision parts. However, the heat treatment process relies on the experience and skill of the operator, making it challenging to produce precision parts due to the cold forging process. The aims of this study is to explore suitable machine learning models using data from the aus-tempering heat treatment process and analyze the factors that significantly impact the mechanic properties (e.g. hardness). As a result, the study analyzed, from a machine learning perspective, how hardness prediction varies based on the quenching temperature, carbon (C), and copper (Cu) contents.

  • Identifying clinical pathways for disease diagnosis and treatment process recommendations are seriously decision-intensive tasks for health care practitioners. It requires them to rely on their expertise and experience to analyze various categories of health parameters from a health record to arrive at a decision in order to provide an accurate diagnosis and treatment recommendations to the end user (patient). Technological adaptation in the area of medical diagnosis using AI is dispensable; using expert systems to assist health care practitioners in decision-making is becoming increasingly popular. Our work architects a novel knowledge-based recommender system model, an expert system that can bring adaptability and transparency in usage, provide in-depth analysis of a patient's medical record, and prescribe diagnostic results and treatment process recommendations to them. The proposed system uses a set of parallel discrete fuzzy rule-based classifier systems, with each of them providing recommended sub-outcomes of discrete medical conditions. A novel knowledge-based combiner unit extracts significant relationships between the sub-outcomes of discrete fuzzy rule-based classifier systems to provide holistic outcomes and solutions for clinical decision support. The work establishes a model to address disease diagnosis and treatment recommendations for primary lung disease issues. In this paper, we provide some samples to demonstrate the usage of the system, and the results from the system show excellent correlation with expert assessments.

Experience of Patients with Cancer Participating in a Clinical Trial for the Development of a New Drug (암환자의 신약개발 임상시험 참여 경험)

  • Purpose: The purpose of this study was to describe the lived experience of the patients with cancer participating in a clinical trial for the development of a new drug. Method: This study was based on a phenomenological approach. The eight patients participating in a clinical trial were selected as the participations of this study. The data were obtained through in-depth interviews from the participants and were analyzed using the Giorgi method. Results: Essential experiences of the patients with cancer under a clinical trial consisted of anticipating recovery of physical health and a social role, passing the strict criteria of a clinical trial, diminishing economic burden, satisfaction with special treatment receiving, social contribution, concerns about side effects and withdrawal from the clinical trial, conflicts as a participant, pain, limited administration of other treatments, regret for giving up other treatments, strict compliance with instructions, prevention of side effects and maintaining desirable life-style. Integrated units of meaning of these components were hope, good luck, a sense of satisfaction, fear, distress, and the will of self-control. Conclusion: The most essential meaning of the cancer patients participating in a clinical trial was hope. Hope was found to be a primary factor reinforcing the will of self-management. The results of this study can be of great help to the research nurses to understand the lived experience of the patients with cancer and to plan an effective nursing intervention for the patients.

  • The questionaire survey in this study was obtained from 1256 upper elementary children in a certain elementary school in In-Cheon city from May 1 to May 31 1993. The purpose of this study was to promote an understanding on the Actual condition of oral health management and to plan a desirable solution for oral health program by surveying the current status of oarl health management. The survey was carried out by using the questionaire after being modified and complemented which was used for the survey of the study on the actual condition of oral health management by Sin Seon Hang (1989). The major results are summarized as follows: 1. General tooth brushing of respondents. 1) As for frequency, 2 times it day is highest. 2) After breakfast meal and below 3 minutes is high. 3) Respondents brushed only teeth with the direction of up and down, left and right side. 4) The period of using for one tooth brush is from 4 months to 6 months. 2. The degree of recognition on oral health knowledge is low. 3. 1) The degree of dental examine's experience with scaling experience are 26.83%. 2) Respondents who experience dental treatment are 57.88%. 3) In oral disease, dental caries is highest. 4) Great difficulty of going to the dentists' is psychological thread and next is lack of time.
