The Role of Adjuvant Treatment in Patients with High-Grade Meningioma

  • Objective : To investigate the efficacy of adjuvant treatment in patients with high-grade meningioma. Methods : A retrospective analysis was performed for patients with high-grade meningioma, World Health Organization grade 2 or 3, in a single center between 2003 and 2014. The patients were reviewed according to age at diagnosis, sex, the location of meningioma, degree of tumor resection, histological features, and type of adjuvant treatment. These factors were analyzed by Firth logistic regression analyses. Results : Fifty-three patients with high-grade meningioma were enrolled. Thirty-four patients received adjuvant treatment; conventional radiotherapy or radiosurgery. Clinical follow-up ranged from 13-113 months with a median follow-up of 35.5 months. Gross total removal (GTR), Simpson grade 1 or 2, was achieved in 29 patients and, among them, 13 patients received adjuvant treatment. In the other 24 patients with non-GTR, conventional adjuvant radiotherapy and radiosurgery were performed in 11 and 10 patients, respectively. The other 3 patients did not receive any adjuvant treatment. Radiation-related complications did not occur. Of the 53 patients, 19 patients had suffered from recurrence. The recurrence rate in the adjuvant treatment group was 23.5% (8 out of 34). On the other hand, the rate for the non-adjuvant treatment group was 57.9% (11 out of 19) (odds ratio [OR]=0.208, p=0.017). In the GTR group, the recurrence rate was 7.5% (1 out of 13) for patients with adjuvant treatment and 50% (8 out of 16) for patients without adjuvant treatment (OR=0.121, p=0.04). Conclusion : Adjuvant treatment appears to be safe and effective, and could lead to a lower recurrence rate in high-grade meningioma, regardless of the extent of removal. Our results might be used as a reference for making decisions when planning adjuvant treatments for patients with high-grade meningioma after surgery.

Dyeability and Colorfastness of Knitted Fabrics with Natural Dye PinuxTM (Part II) -Improvement of Colorfastness Using Cationizing Agent and Smectite-

  • This study examines the possibilities to improve natural dyestuff's (Pinux$^{TM}$) dyeability and colorfastness for C/R (Cotton/Rayon (40/60)) and W/T (Wool/Tencel (10/90)) knitted fabrics in cationizing and smectiting for pre-treatment, simultaneous-treatment and post-treatment process sequences; as well as various other treatment methods. The sample dyeability showed the strongest K/S value in the order of smectite (S) < cationization + smectite (C+S) < cationization (C); however, the K/S value showed a low level in the simultaneoustreatment method of smectite. Colorfastness to washing improved in the order of C < C+S < S, and after the smectite post-treatment process, C/R improved from Grade 1 to Grade 4 and W/T improved from Grade 1-2 to Grade 4. Colorfastness to perspiration generally improved in the same order as the colorfastness to washing and after the smectite post-treatment process C/R, W/T sample's acidic and alkaline colorfastness to perspiration improved greatly. As for the colorfastness to rubbing, the addition of smectite in the simultaneous-treatment and post-treatment processes resulted in improved wet-colorfastness; however, smectite showed less effect on the colorfastness to light compared to other colorfastness ratings.

Purification of Metallurgical Grade Silicon by Plasma Torch and E-beam Treatment (플라즈마 토치와 전자빔을 이용한 금속급 실리콘 정제)

  • Cost-effective purification methods of silicon were carried out in order to replace the conventional Siemens method for solar grade silicon. Firstly, acid leaching which is a hydrometallurgical process was preceded with grinded silicon powders of metallurgical grade (~99% purity) to remove metallic impurities. Then, plasma treatments were performed with the leached silicon powders of 99.94% purity by argon plasma at 30 kW power under atmospheric pressure. Plasma treatment was specifically efficient for removing Zr, Y, and P but not for Al and B. Another purification step by EB treatment was also studied for the 99.92% silicon lump which resulted in the fast removal of boron and aluminum. That means the two methods are effective alternative tools for removing the doping elements like boron and phosphor.

Gastrointestinal Adverse Effects in Advanced Colorectal Carcinoma Patients Treated with Different Schedules of FOLFOX

  • Background: To assess the frequency and severity of gastrointestinal adverse effects in advanced colorectal carcinoma patients treated with four different schedules of FOLFOX. Materials and Methods: Patients (median age 61 years) who underwent surgery were included in the study. All had measureable disease at CT scan, ultrasonography or clinical examination. Toxicity was graded on a scale of 1-5 according to the general grade definition of CTC v2.0. The severity of adverse effects (Grade 3 and 4) assessed in each treatment arm was compared. Results: Differences between the incidence rates of 3 and 4 toxicity and all grades of toxicity for all parameters in GI toxicity were very highly significant (p<0.001). Severe gastrointestinal symptoms of toxicity were noted with FOLFOX7 (oxaliplatin $130mg/m^2$). Grade 3 diarrhea was reported in 25% patients and grade 4 diarrhea in 4% in the FOLFOX7 treatment arm. Grade 2 vomiting was very frequently reported in the FOLFOX4 treatment arm (oxaliplatin $85mg/m^2$). Grade 2 stomatitis was reported in 42% patients treated with mFOLFOX6 (oxaliplatin $100mg/m^2$). Differences in the incidence rate of nausea, diarrhea and stomatitis among all treatment arms of FOLFOX were significant (p<0.05). Conclusions: Severe diarrhea is associated with FOLFOX7 treatment. No grade 3 or 4 GI toxicity was reported in patients of the mFOLFOX6 arm.

Effect of Oriental Medicine Treatment on Inpatient with Thoracolumbar Compression Fracture (흉요추 압박골절로 입원한 환자에 대한 한의학적 치료 효과)

  • Objectives : This Study was to investigate the tendency of inpatient with Thoracolumbar Compression Fracture. Also we investigated effectiveness of Pharmacopuncture and Warming Acupuncture on Thoracolumbar Compression Fracture. Methods : We investigated 48 cases of patients with thoracolumbar compression fracture. We distributed inpatint with age, sex, number of injured vertebra, level of injured vertebra, admission duration, grade of clinical symptom, result of treatment and applied treatments. And we calculated statistically signification with this data. Results : 1. Female is prominent in distribution of sex, the average age bracket is 70's, and the largest group was accidental fall in the distribution of causes. 2. In regard to level of injury, T12 was found to be most prominent, followed in turn by L2, L1, T12. In regard to grade of clinical symptoms, Grade III was most Prominent, followed in turn by Grade IV, Grade II. In regard to grade of result of treatment, Good was most prominent. 3. There was not statistically significantly correlation about admission duration and result of treatment with number of injured vertebra, grade of clinical symptom, applied treatments. Conclusions : Most inpatients with thoracolumbar compression fracture was cured by oriental medicine treatment.

Treatment Results in Patients with Salivary Gland Malignancy (타액선 악성 종양 환자에 대한 치료성적)

  • Objectives: Primary malignant tumors in the salivary glands are relatively rare. Because of the rarity and the different histopathologic patterns, it is difficult to establish a uniform treatment strategy. The prime treatment of salivary gland malignancy is the surgery, but the role of radiotherapy has been under debate. Radiation therapy combined with conservative surgical procedures may be as successful and perhaps more rational treatment than radical surgery alone. The aim of this study is to evaluate clinical pattern, incidence, treatment modality and outcome of the salivary gland maligancy. Materials and Methods: The medical records of 32 patients with malignant neoplasm of salivary gland who treated at the Keimyung university Dongsan hospital were analyzed retrospectively. Results: The overall 5 year survival rate was 77.9% stage I : 100%, stage II : 75%, stage III : 66.7%, stage IV : 55.6%). The 5 year survival rate according to tumor grade was 100% in low grade malignancy, 71.8% in high grade malignancy. The 5 year survival rate according to treatment modalities was as follows: Surgery only group was 83.3%, combined treatment group with surgery and posoperative radiation was 74.6%. Conclusion: The factors affecting prognosis is variable, but the stage at the time of diagnosis, site of lesion, tumor grade, histologic subtype were important factors. Surgery was the prime treatment tool and postoperative radiotherapy was also imperative in higher stage patient, high grade tumor, or patients with positive surgical margin.

Clinical Comparison Studies on Bell's Palsy Patients by Existence of Postauricural Pain (이후통과 Bell's palsy의 예후와의 상관성 연구)

  • Objectives : This study was designed to evaluate the influence of postauricular pain on Bell's palsy patients. Methods : We investigated 71 cases of patients with Bell's palsy and classified them as existence of Postauricural pain, 71 patients were sequentially interviewed and examined. We evaluated the treatment effect of each group by using Gross Grading System of House-Brackmann(H-B grade) before treatment and after final treatment and we researched differences of sequelae of Bell's palsy, period of treatment, changing point -period from onset of Bell's palsy to the day which the change begins to be seen at the face- and improvement -period which Bell's palsy is improved from onset to H-B gradeII. Results : 1. In age, sex, lesion, duration of disease, we found that two groups have no significant differences. 2. In improvement and period of treatment, we found that two groups have significant differences. In changing-point, we found that two groups had the difference of the average, but they were not statistically significant. 3. As a result of evaluation by using H-B grade, treatment score after final treatment was marked higher than that before treatment within each group. 4. After final treatment, Non-postauricular pain group had significant difference(result) on H-B grade compared with Postauricular pain group. 5. In frequency of sequelae symptoms of Bell's palsy, Postauricular pain group had more higher compared with Non-postauricular pain group. Conclusion : These results suggested that Non postauricular pain group should be get better than Postauricular pain group.

A Clinical Report on 35 Inpatients with Facial Paralysis Effects of Early Oriental Medicine Treatment on House-Brackmann Grade in Bell's Palsy (벨마비 입원환자 35례에 대한 임상보고 조기 한방치료가 벨마비에서 House-Brackmann grade에 미치는 영향을 중심으로)

  • Objective : This study was designed to evaluate the influence of time from onset of Bell's palsy until commenced Oriental medicine therapy on Bell's palsy. Methods : We investigated 35 cases of patients with peripheral facial palsy who visited Wonkwang University Oriental Hospital from 10th, December 2010 to 31st August, 2011 and classified them as time from onset of Bell's palsy until commenced Oriental medicine therapy. Time from onset of Bell's palsy until commenced Oriental medicine therapy of Group A was within 3 days. Group B was from 4 days to 8 days. We evaluate the treatment effect of each group by House-Brackmann grade before treatment and 1,3 week treatment. Results : 1. There was a statistical difference in period from onset of Bell's palsy until commenced Oriental medicine therapy between Group A and Group B. 2. There was a statistical difference after 1 week treatment in Group B. 3. There was a statistical difference after 3 weeks treatment in both Group A and B.3. After 3 week treatment, there was statistical significant difference in decreased value of House-Brackmann grade between Group A and Group B. Group A showed more recovery compared with Group B. Conclusions : 1. These results suggest that early oriental medicine treatment could be more effective in recovery of Bell's palsy. 2. Further studies will be required to identify the beneficial effect of early oriental medicine treatment in bell's palsy.

Effects of Jungsongouhyul Herbal Acupuncture(JSO) Multi-treatment for Whiplash Injury by Traffic Accident (중성어혈약침(中性瘀血藥鍼) 복합치료(複合治療)가 교통사고로 인한 경추 편타손상에 미치는 영향)

  • Objectives : To examine the effects of JSO multi-treatment for whiplash injury patients by traffic accident. Interventions : Nonrandomized, Nonblinded compartson of the JSO+Acupuncture and Acupuncture Treatment. Main Outcome Measures : Subjective evaluation was followed by Clinical Grade and VAS after 5 times treatments. The SPSS 10.0 for windows was used to analyze the date and the Wilcoxon signed rank test and Mann-Whitney U-testwere used to verify the results. Results : The following results were obtained ; 1. Clinical Grade of the JSO+Acupuncture treatment was centralized to Gr II(50%) before treatment. After 5 times treatments, it was centralized to Gr I(58.3%). Also Clinical Grade was significantly changed from GR ll to Gr I(p<0.0002). 2. Clinical Grade of the Acupuncture treatment was centralize(d) to Gr II(54.2%) before treament. After 5 times treament, it was tend to cenualize to Gr I(50%). But Clinical Grade was still remained at Gr II(p<0.001) 3. VAS of change between JSO+Acupuncture and Acupuncture treatment, Vas of JSO+Acupuncture was significantly decreased (p<0.003). C onclusion : It is suggested that JSO multi-treatment has development-effectiveness on whiplash injury patients by traffic accident.

A Study of Annealing Heat-treatment for Ti(Grade 2) by Electrochemical Methods (전기화학적 방법을 이용한 Ti(Grade 2)재의 최적 어닐링 열처리에 대한 연구)

  • In this paper, the annealing heat treatments for the best corrosion resistant of Ti(Grade 2) were studied in a 3.5% NaCl solution by electrochemical methods. The annealing heat treatments were accomplished at 650, 700 and $750^{\circ}C$ with different time of 30min., 1hour and 2 hours in a vacuum condition. The obtained results are: 1) in the case of solution heat treated $930^{\circ}C$ for 2 hours in a vacuum and air, the corrosion potentials were -348.7 and -567. 1mV, and current densities 2.32 and $22.62\mu\textrm{A}$, respectively, 2) as increase both annealing heat treatment temperature 650, 700, $750^{\circ}C$ and time 30min., 1 hour, 2 hours, the corrosion potential were decreased, whereas corrosion current density increased, 3) in the case of cyclic polarization, the measured charges were increased as increasing solution heat treatment temperature and time, 4) on the bases of corrosion potential, current density and charge, the best annealing temperature and time were measured as $700^{\circ}C$ and 30min. for Ti(Grade 2) material.
