The Clinical Efficacy of Yangkyuksanwha-tang on Acute Stroke (급성기 중풍 환자에 대한 양격산화탕의 임상적 효능)

  • Objective : To assess the clinical efficacy of Yangkyuksanwha-tang on acute stroke Methods : We prescribed Yangkyuksanwha-tang to 83 acute stroke patients without thrombolytic treatment. Results : The rate of progressive stroke type was 1.2%, it was remarkably lower than previous reports. 3.6% felt an itching sensation, 3.6% complained headache, dizziness and powerless, 2.4% complained indigestion and diarrhea, 1.2% appeared hematuria and G-I bleeding. Yangkyuksanwha-tang decreased Stroke-Pattern Identification and National Institute of Health Stroke Scale(NIHSS), and increased Modified Barthel Index(MBI). So we could suggest that this medicine have desirable effect to reduce the severity of stroke and improve functional recovery. As to the laboratory findings, all results were within the normal value, which showed no hepatic or renal toxicity. Conclusion : We could suggest that Yangkyuksanwha-tang is a useful medicine which has clinical efficacy for acute stroke, but further investigation for an administration of more than 2 weeks is necessary.

  • Onychomycosis is the most common disease of the nail, with a worldwide prevalence of 5.5%. It causes local pain, paresthesia, and cosmetic problems and decreases the patients' quality of life. Conventional treatments include the administration of topical and systemic agents. However, factors such as subungual hyperkeratosis and biofilm formation may affect the efficacy of these treatments. Moreover, side effects associated with systemic agents are a major concern for patients. Therefore, various novel treatments are being developed; among them, photodynamic therapy (PDT) and Nd:YAG laser are promising, and several studies have demonstrated the efficacy and safety of PDT and laser therapy. Here, we report the efficacy and tolerability of PDT combined with 1064-nm Nd:YAG laser therapy and topical efinaconazole for the treatment of three patients with refractory onychomycosis.

Comparison of the Efficacy of Disinfectants to Control Caseous Lymphadenitis in Korean Black Goat Farms (흑염소의 건락성 림프절염 제어를 위한 소독제 효능 비교)

  • Corynebacterium pseudotuberculosis is the causative agent of caseous lymphadenitis (CLA), a chronic contagious disease in small ruminants. The prevalence of CLA has been reported to be >50% in Korean black goats. CLA is difficult to control due to a lack of efficient vaccines and treatment methods. Effective disinfection of the farm environment may be an alternative strategy for reducing the spread of C. pseudotuberculosis. The objective of this study was to evaluate the efficacy of commercial disinfectants against CLA. The six commercial disinfectants, largely composed of sodium dichloroisocyanurate, sodium hypochlorite, potassium monopersulfate triple salt, quaternary ammonium, citric acid, and copper sulfate, were tested against five different genotypes of C. pseudotuberculosis isolated from goat farms in Korea. Efficacy tests were performed in accordance with the disinfectant efficacy test guidelines recommended by the Animal and Plant Quarantine Agency of Korea with slight modifications. All disinfectants except for copper sulfate exhibited >99.99% killing efficacy under hard water conditions following 30 min of incubation, which is the recommended standard treatment time according to guidelines. The minimum bactericidal treatment time was evaluated by employing treatments for durations of 1, 5, and 15 min. The most effective compounds under hard water conditions were sodium dichloroisocyanurate, potassium monopersulfate triple salt, and sodium hypochlorite, exhibiting >99.99% killing efficacy after 1 min of treatment. In the aqueous solution forms, citric acid and the quaternary ammonium compound were the most effective, but required at least 5 min to kill >99.99% of the bacteria. The current study characterizes the killing efficacy of six commercial disinfectant active compounds against C. pseudotuberculosis. Thus, this study provides essential information regarding the efficacy of the disinfectants used to control CLA in goat farms.

A Case Study of Four Hemorrhoid Patients Treated by Korean Medical Treatment (한의치료로 호전된 치핵 환자 증례보고 4례)

  • Objectives: This study was designed to evaluate the effect of Korean medicine treatment on four cases involving hemorrhoid patients. Methods: Four patients suffering from hemorrhoids were treated with Korean medicine treatment, such as herbal medicine, and acupuncture. Patients were diagnosed using pictures of the anus, which were used to compare before and after treatment and to evaluate treatment efficacy. Results: After treatment, the symptoms of pain and other complaints conspicuously improved. Conclusions: As indicated by before and after treatment pictures of the anus and decreased symptoms, Korean medicine treatment is effective in treating hemorrhoids.

Trans-arterial Chemo-Embolization in Treating Elderly Patients with Hepatocellular Carcinoma

  • Background: This analysis was conducted to evaluate the efficacy and safety of Trans-arterial Chemo-Embolization (TACE) in treating Elderly patients with Hepatocellular Carcinoma (EHPC). Methods: Clinical studies evaluating the efficacy and safety of TACE on response and safety for patients with EHPC were identified by using a predefined search strategy. Pooled response rate of treatment were calculated. Results: In TACE based regimen, clinical studies which including patients with EHPC were considered eligible for the evaluation of response. And, in these TACE based treatments, pooled analysis suggested that, in all 288 patients whose response could be assessed, the pooled reponse rate was 29.5%(85/288) in TACE based treatment. The most commonly encountered TACE-related morbidity was liver function impairment. No grade III or IV renal or liver toxicity were observed. No treatment related death occurred in EHPC patients with TACE based treatments. Conclusion: This evidence based analysis suggests that TACE based treatments are associated with mild response rate and accepted toxicities for treating patients with EHPC.

Thalidomide Combined with Chemotherapy in Treating Patients with Advanced lung Cancer

  • Objectives: To evaluate efficacy and toxicity in patients with advanced lung cancer, including non-small cell and small cell variants (NSCLC and SCLC), treated with thalidomide plus chemotherapy. Methods: Fourteen patients with advanced lung cancer were scheduled to receive chemotherapy combined with thalidomide. All patients in this study received thalidomide (100 mg orally per night before sleeping, produced by Changzhou Pharmaceutical Factory Co.Ltd) after the start of chemotherapy for at least 14 days. Chemotherapy was administered according to the condition of patients. After at least 14 days of treatment, efficacy and toxicity were evaluated. Results: There were 6 female and 8 male patients with advanced lung cancer recruited into this study, including 2 with SCLC and 12 with NSCLC. The median age was 56.7 (44-65) years. Progressive disease was observed in 12 patients (12/14), and stable disease in 2 (2/14). Grade 1 to 2 myelosuppression was observed in 4/14 patients, and Grade 1 to 2 elevation of hepatic enzymes was recorded in 5/14 patients. Adverse effects on the gastrointestinal tract were documented in 2/14 patients, all beingGrade 1. No Grade 3-4 toxicity was recorded. No treatment related deaths occurred. Conclusions: Our results demonstrate that thalidomide combined with chemotherapy is mildly effective and safe for treating patients with advanced lung cancer. However, further evaluation of this combination is warranted.

Efficacy of Chamomile in the Treatment of Premenstrual Syndrome: A Systematic Review

  • Premenstrual syndrome (PMS) encompasses a vast array of physical and psychological symptoms. Of the herbal supplements mentioned for remedy PMS symptoms, chamomile used as an effective herbal medicine. The overall purpose of this review was to determine the efficacy of chamomile on the treatment PMS. An extensive research review using Web of Science, the Cochrane Controlled Trials Register database, PubMed, Chinese Biomedical Database (CBM), CINAHL, China National Knowledge Infrastructure (CNKI), Psych INFO, Social Science Research Network, SID, Google Scholar, Iran Doc, Magiran and Iran Medex. Eligible studies were identified from English and Persian databases, published between 1990 and 2019. Studies were screened independently by two researchers who performed the data extraction. Of Twenty-seven studies identified, Eight RCTs met our inclusion criteria. Chamomile has been used to treat PMS relief because of therapeutic properties such as anti-inflammatory effects (Chamazulene and α-Bisabolol); anti-spasmodic effects (Apigenin, Quercetin, and Luteolin, Metoxicomarin, Matrisin, and Phytoestrogens); anti-anxiety effects (Glycine, Flavonoid). The results of this review show that Chamomile is effective for the treatment of PMS. Based on these results, we believe that Chamomile can be used as good herbal medicine to treat in women with PMS.

Anthelminthic efficacy of combined preparation of pyrantel pamoate and oxantel pamoate on human intestinal nematodes (Pyrantel pamoate 및 Oxantel pamoate 합제(合劑)의 장내선충류(腸內線蟲類)에 대(對)한 구충효과(驅蟲效果))

  • A combined syrup preparation of pyrantel pamoate and oxantel pamoate containing 50mg/ml of respective anthelminthics was evaluated on the efficacy to treat the human intestinal nematodes. Total 24 cases were treated with oral administration of above preparation 10 mg/kg body weight by single dose. The results were summarized as follows: 1. Out of 21 cases of Ascaris lumbricoides infection, 19 (90.5%) turned to be egg negative on 21 days after treatment and egg-reduction rate was 99.7%. 2. 17 cases out of 19 cases infected with Trichocephalus trichiurus were egg negative after treatment (84.2%) and egg reduction rate was 60.6%. 3. As for Ancylostoma duodenale infection, egg negative conversion rate was 88.9% and egg reduction rate was 98.1% by single oral administration among 9 infected cases. 4. In Enterobius vermicularis infection, all of 17 cases gave negative anal swab on 21th day after the treatment. From the above results, it was assumed that the anthelminthic activities of pyrantel pamoate and oxantel pamoate were not potentiated by the mixture of two kinds of drugs but showed simple additive effect.

Effectiveness of acupuncture as an adjunctive therapy for lung cancer: A systematic review and meta-analysis

  • Objectives : Lung cancer is one of the most common cancer and the leading cause of cancer deaths worldwide. This study aimed to evaluate the role of acupuncture as an adjunctive therapy for lung cancer. Methods : We conducted a systematic review and meta-analysis on the role of acupuncture therapy in lung cancer treatment by electronic and manual searching in ninedatabases, including PubMed, Cochrane library, Embase, Korean databases, and Chinese medical databases. Results : A total of 21 trials were included in the meta-analysis. The study results showed that acupuncture therapy had significant efficacy in immuneregulation, including CD3 andCD4. Further analysis revealed that acupuncture therapy significant improvements in quality of life, including Karnofsky performance status(KPS)score, functional assessment of cancer therapy-lung cancer subscale (FACT-L) and European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30). In addition, the pooled studies also showed that acupuncture therapy reduced cancer pain and chemotherapy-induced nausea and vomiting. Conclusions : Our study provides moderate evidence of the efficacy of the acupuncture therapy in the treatment of lung cancer.

Modalities in managing postherpetic neuralgia

  • Postherpetic neuralgia (PHN) is the most troublesome side effect of Herpes Zoster (HZ), which mainly affects the elderly and immunocompromised populations. Despite the current advancement of treatments, PHN persists in many individuals influencing their daily activities and reducing their quality of life. Anticonvulsants, antidepressants, topical therapies including lidocaine and capsaicin, and opioids, are the most widely used therapies for the treatment of PHN. These medications come with their adverse effects, so they should be used carefully with the elderly or with patients with significant comorbidities. Other measures like botulinum toxin, nerve blocks, spinal cord stimulation, and radiofrequency have also contributed significantly to the management of PHN. However, the efficacy, safety, and tolerability of these invasive methods need to be carefully monitored when administering them. Early diagnosis and early initiation of treatment can reduce the burden associated with PHN. The zoster vaccine has effectively reduced the incidence of HZ and PHN. In this article, we discuss the treatment options available for the management of PHN, mainly focusing on the efficacy and safety of different therapeutic modalities.
