진료환경이 환자참여에 미치는 영향: 부정적 감정의 조절효과를 중심으로 (Effect of Treatment Setting of the Medical Services on the Patient Participation : Focusing on Moderating Effect of Negative Emotion)

  • 본 연구는 의료서비스 제공 과정에서의 진료환경이 환자참여에 미치는 영향과 이들 간의 관계에서 부정적 감정의 조절효과를 분석하고자 하였다. 이러한 연구목적 달성을 위하여 국내 C 지역의 병의원 환자 중 진료를 마친 320명을 대상으로 수집한 자료를 SPSS 19.0을 이용하여 단계적 회귀분석을 수행하였다. 실증분석 결과는 다음과 같다. 진료환경의 청결성은 행동적 참여에, 환자의 연령이 높을수록 감정적 참여에, 진료환경의 제3자의 존재는 감정과 정보적 참여에 긍정적 영향을 미치는 것으로 확인하였다. 환자의 부정적 감정은 감정적 참여와 정보적 참여에 부정적 영향을 미쳤고, 진료환경의 시간압박은 환자의 정보적 참여수준에 부정적 영향을 미쳤다. 둘째, 조절효과로는 진료환경의 청결성과 감정적 참여 간에 부정적 감정은 긍정적인 영향을 미쳤다. 마지막으로 이러한 연구결과를 중심으로 의료서비스의 진료환경과 부정적 감정을 고려하여 환자참여를 향상시키기 위한 방법 등 이론적, 실무적 시사점을 제시하였다.

  • We investigated the effect of color depth on polyester fabrics by plasma treatment. In this study, although it have many paper about effects of plasma treatment, we observed interfacial electrokinetic potential of polyester fabrics by plasma treatment and also we investigated relationship between deep coloring agent and plasma treatment to get the effect of color depth on polyester fabrics. The results obtained are as follows, 1. Plasma treatment did not enhanced the effect of color depth of polyester fabrics by plasma treatment independently. 2. In the case of using the deep coloring agent with plasma treatment on polyester fabrics, lightness was more decreased than using the deep coloring agent itself. 3. Plasma treatment could not affect surface shape and tensile strength of treated polyester fabrics.

Toxicity characteristics of sewage treatment effluents and potential contribution of micropollutant residuals

  • Background: A typical sewage treatment plant is designed for organic and nutrient removal from municipal sewage water and not targeted to eliminate micropollutants such as pesticides, pharmaceuticals, and nano-sized metals which become a big concern for sustainable human and ecological system and are mainly discharged from sewage treatment plant. Therefore, despite contaminant removal by wastewater treatment processes, there are still remaining environmental risks by untreated pollutants in STP (sewage treatment plant) effluents. This study performed aquatic toxicity tests of raw wastewater and treated effluents in two sewage treatment plants to evaluate toxicity reduction by wastewater treatment process and analyze concentration of contaminants to reveal potential toxic factors in STP effluents. Methods: Water samples were collected from each treatment steps of two STPs, and acute and chronic toxicity tests were conducted following USEPA (United States Environmental Protection Agency) and OECD (Organization for Economic Cooperation and Development) guidelines. Endpoints were immobility for mortality and reproduction effect for estrogenicity. Results: Acute $EC_{50}s$ (median effective concentration) of influents for Seungki (SK) and Jungnang (JN) STPs are $54.13{\pm}32.64%$ and $30.38{\pm}24.96%$, respectively, and reduced to $96.49{\pm}7.84%$ and 100%. Acute toxicity reduction was clearly correlated with SS (suspended solids) concentration because of filter feeding characteristics of test organisms. Chronic toxicity tests revealed that lethal effect was reduced and low concentration of influents showed higher number of neonates. However, toxicity reduction was not related to nutrient removal. Fecundity effect positively increased in treated wastewater compared to that in raw wastewater, and no significant differences were observed compared to the control group in JN final effluent implying potential effects of estrogenic compounds in the STP effluents. Conclusions: Conventional wastewater treatment process reduced some organics and nutritional compounds from wastewater, and it results in toxicity reduction in lethal effect and positive reproductive effect but not showing correlation. Unknown estrogenic compounds could be a reason causing the increase of brood size. This study suggests that pharmaceutical residues and nanoparticles in STP effluents are one of the major micropollutants and underline as one of estrogenic effect factors.

갈근 부탄올 엑기스가 흰쥐에 유발된 카드뮴 독성에 미치는 영향 (Pharmacological Effects of Puerariae Radix Butanol Extract on Cadmium Toxicity in Rats.)

  • 1) Puerariae Radix butanol ext. (100, 200, 400mg/kg, p.o. single treatment) alone partly showed blood pressure decreasing effect in SHRs and increasing effect of urinary volume in normal rats. 2) Cadmium nitrate (10mg/kg, s.c. single treatment) induced toxicity such as body weight decreasing effect, antidiuretic effect and muscle relaxant effect such as pull-up test, traction test and rota rod test in rats. However, Puerariae Radix butanol ext. (100, 200, 400mg/kg, p.o. single treatment) showed antidotal effects on the above and also in acute toxicity test when coadministered with both of them. 3) Cadmium nitrate (1mg/kg, s.c. 7 days consecutive treatment) did not showed toxicity in body weight change, blood pressure, change, serum biochemical parameters in rats. Puerariae Radix butanol ext. (100, 200, 400mg/kg, p.o. 7 days consecutive treatment) did not also showed any antidotal effects when coadministered with both of them for 7 days.

  • Objectives : The aim of this study was to observe the effect of needle-embedding combined with oriental medical treatment therapy on hemifacial spasm. Methods : 2 patients were treated with needle-embedding therapy with oriental medical treatment. After treatment, we evaluated the effect by Scott's scale. And we checked the satisfaction degree and side effect. Results : After treatment, the grade of spasm classified by Scott's scale was decreased and hemifacial spasm was visibly improved. Conclusions : This study showed that needle-embedding therapy with oriental medical treatment was effective to improve hemifacial spasm.

  • This study was to consider the effect of PUVA treatment on patients with skin disease. The result can be summarized as follows : 1) PUVA treatment may effect directly on lymphocytes to be permeable locally in Skin disease. 2) PUVA treatment may effect on abnormal immune reacton in skin disease. The significance on the effect of PUVA treatment is still unknown. So, we will study continuously for PUVA treatment on patients with skin disease.

  • The purpose of this report is to evaluate effect of Korean medical treatment on idiopathic gonadotropin-dependent precocious puberty (G-DPP) patient received herbal medicine. We administered Aesopjiyoun-tang remedy to idiopathic G-DPP and analyzed the delay effect by hormonal value and radiographs; the height growth effect by measurement of height. After Korean medical treatment, suppression effect to peak-Luteinizing Hormone level (LHL) is 14.39IU/L to 10.9IU/L for 13month, growth effect to height value is 11cm/13month; and change of mean growth velocity (MGV) is 6.08cm/year to 10.06cm/year. The gain in height by treatment is 3.98cm/year. The result suggests Aesopjiyoun-tang can be an effective treatment for G-DPP. Herbal medicine can be used as an alternative treatment in place of the GnRH treatment.

스테인리스강의 가스질화에 미치는 전처리의 영향 (Effect of Pre-Treatment Characteristics on the Gas Nitriding of Stainless Steels)

  • Effect of pre-treatment on the gas nitriding process of austenitic stainless steels has been investigated and the following results were obtained. Minimum pre-treatment time was decreased to 5min with increasing treatment temperature from $200^{\circ}C$ to $600^{\circ}C$. Surface activation effect by the pre-treatment was maintained in the air up to holding time of 64hr, judging from the analysis result of gas nitrided specimens. The Depth of nitrided layer of STS 304 and 316 stainless steels were ranged from $5{\mu}m$ to $90{\mu}m$ at $440^{\circ}C{\sim}600^{\circ}C$. The X-ray diffraction intensity for austenitic stainless steels were increases as nitriding temperature from $440^{\circ}C$ to $600^{\circ}C$.

  • Objective : This study was undertaken to determine which compound of Bee Venom for herb-acupuncture has cytotoxicity on mouse mast cell line. Methods : We compared crude bee venom and its compounds such as melittin, mast cell degranulating peptide (MCD peptide), apamin with control groups on cytotoxicity by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Results & Conclusion : 1. Crude bee venom showed significant cytotoxic effect(p<0.01) in 1 hour treatment with $1{\mu}g/m{\ell}$ in comparison with control group in 1 hour treatment with low concentration of $10-4{\mu}g/m{\ell}$, $10-3{\mu}g/m{\ell}$, $10-2{\mu}g/m{\ell}$, $10-1{\mu}g/m{\ell}$ and $1{\mu}g/m{\ell}$, but it showed no significant cytotoxic effect in 6 hours treatment. 2. Melittin group showed no significant cytotoxic effect in comparison with control group in 1 and 6 hours treatment with low concentration of $10-4{\mu}g/m{\ell}$, $10-3{\mu}g/m{\ell}$, $10-2{\mu}g/m{\ell}$, $10-1{\mu}g/m{\ell}$ and $1{\mu}g/m{\ell}$. 3. MCD peptide and Apamin group showed no significant cytotoxic effect in comparison with control group in 1 and 6 hours treatment with low concentration of $10-4{\mu}g/m{\ell}$ $10-3{\mu}g/m{\ell}$, $10-2{\mu}g/m{\ell}$, $10-1{\mu}g/m{\ell}$ and $1{\mu}g/m{\ell}$. 4. Crude bee venom showed significant cytotoxic effect(p<0.01) in 1 and 6 hours treatment in comparison with control group in 1 and 6 hours treatment with high concentration of $10{\mu}g/m{\ell}$, $20{\mu}g/m{\ell}$ and $102{\mu}g/m{\ell}$. 5. Melittin group showed significant cytotoxic effect(p<0.01) in 1 hour treatment in comparison with control group in 1 hour treatment with high concentration of $10{\mu}g/m{\ell}$ but it showed no significant cytotoxic effect in 6 hours treatment. 6. Crude bee venom and its compounds have more cytotoxic effect in 1 hour treatment than in 6 hours treatment. It means cytotoxicity tends to decrease according to the treatment time.

Apoptotic effect of $IP_6$ was not enhanced by co-treatment with myo-inositol in prostate carcinoma PC3 cells

  • Inositol hexaphosphate ($IP_6$) is a major constituent of most cereals, legumes, nuts, oil seeds and soybean. Previous studies reported the anticancer effect of $IP_6$ and suggested that co-treatment of $IP_6$ with inositol may enhance anticancer effect of $IP_6$. Although the anticancer effect of $IP_6$ has been intensively studied, the combinational effect of $IP_6$ and inositol and involved mechanisms are not well understood so far. In the present study, we investigated the effect of $IP_6$ and myo-inositol (MI) on cell cycle regulation and apoptosis using PC3 prostate cancer cell lines. When cell, were co-treated with $IP_6$ and MI, the extent of cell growth inhibition was significantly increased than that by $IP_6$ alone. To identify the effect of $IP_6$ and MI on apoptosis, the activity of caspase-3 was measured. The caspase-3 activity was significantly increased when cells were treated with either $IP_6$ alone or both $IP_6$ and MI, with no significant enhancement by co-treatment. To investigate the effect of $IP_6$ and MI of cell cycle arrest, we measured p21 mRNA expression in PC3 cells and observed significant increase in p21 mRNA by $IP_6$. But synergistic regulation by co-treatment with $IP_6$ and MI was not observed. In addition, there was no significant effect by co-treatment compared to $IP_6$ treatment on the regulation of cell cycle progression although $IP_6$ significantly changed cell cycle distribution in the presence of MI or not. Therefore, these findings support that $IP_6$ has anticancer function by induction of apoptosis and regulation of cell cycle. However, synergistic effect by MI on cell cycle regulation and apoptosis was not observed in PC3 prostate cancer cells.
