The Effects of Exercise Intensity and Initial Timing on Functional Recovery after Sciatic Nerve Crush Injury in Rats

  • PURPOSE: The aim of this study was to investigate the effects of exercise intensity and initial timing on functional recovery following sciatic nerve injury in rats. METHODS: Total of 80 Sprague-Dawley rats was used and randomly divided 6 groups. Under deep anesthesia, the sciatic nerve was nipped by adapted hemostatic tweezers for 30 seconds and the injured nerve was transparent under naked eyes. Acute exercise groups was applied treadmill after sciatic nerve crush injury during 5days with three type intensity. Late exercise groups was also applied treadmill during 5 days with three type intensity after 5 days break. Values of sciatic functional index were measured and analyzed in each group after exercise period. RESULTS: The sciatic functional index values between control groups 1, acute low-intensity group, acute middle-intensity group in acute phase showed statistical significant (p<.05). The sciatic functional index values between control groups 2, late low-intensity group, late middle-intensity group and late high-intensity in late phase showed statistical significant (p<.05). The comparison in acute and late phase, sciatic functional index values of each low-intensity group and each high-intensity group showed statistical significant (p<.05). CONCLUSION: Whether at acute or late phase, treadmill exercise as a therapy obtained beneficial effects of functional recovery and exercise training at low speed is more beneficial effects on the recovery of motor function in acute phase.

Use of the Talk Test for Comparison of Exercise Intensity between Aerobic Activities with Treadmill and Bicycle

  • Purpose: The talk test (TT) is an alternative, self-reported method for prescribing and guiding exercise training in both healthy adults and patients with cardiovascular and pulmonary diseases. This study examined whether the TT is a valid tool for evaluating the exercise intensity during two different types of aerobic activity on a treadmill or stationary bicycle in a healthy population. Methods: A total of ten subjects (six males and four females) who had no medical history related to musculoskeletal, cardiovascular, and pulmonary disorders were enrolled in this study. They were evaluated using the TT, which consisted of three-level of difficulties demanding cardiac loads while performing aerobic activities on a treadmill and bicycle ergometer across two consecutive days in a counterbalanced manner. During the activities, the psychophysiological response markers were collected in terms of the heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents. Results: Statistical analyses revealed a significant difference in the between-subject variance regarding the TT level effect (p<0.05). On the other hand, no significant findings were detected on the between-group variance(p>0.05) and the TT level×group interaction (p>0.05). The independent t-test indicated no significant differences in heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents at any levels of the TT in the two groups. The TT showed a strong correlation with the rating of perceived exertion. Conclusion: This study showed that the TT is a valid and alternative tool for evaluating the aerobic exercise intensity in a healthy population. In addition, differences in the psychophysiological response markers between two aerobic activities, treadmill and bicycle ergometer, were detected in the same exercise intensity scaled with the TT. The TT can be used to evaluate and prescribe the exercise intensity of aerobic activity in cardiovascular and pulmonary physical therapy.

고유수용성촉진법의 목 패턴이 만성 뇌졸중 환자의 균형과 보행 능력에 미치는 영향 (The Effect of Neck Pattern of PNF on Balance and Walking Ability in Patients with Chronic Stroke)

  • Purpose: This study investigated the effects of neck pattern of proprioceptive neuromuscular facilitation (PNF) on balance and walking ability in patients with chronic stroke. Methods: Fourteen participants with chronic stroke were randomly assigned to vestibular rehabilitation and then divided into two groups: the neck pattern group or treadmill group. Each group underwent 20 sessions (20 minutes/day, five days/week, for four weeks). Patients were assessed with the Berg balance scale (BBS) and gait parameters (gait speed, cadence, step length, and double-limb support period) using a GAITRite system. Results: Vestibular rehabilitation for the neck pattern group and the treadmill group showed significant intragroup improvement on the BBS and in terms of gait speed, cadence, step length, and double-limb support period (p < 0.05). Vestibular rehabilitation was more effective for the neck pattern group than for the treadmill group in terms of the BBS (p = 0.00; 95% CI, 1.49-5.94), gait speed (p = 0.01; 95% CI, 0.05-0.16), cadence (p = 0.02; 95% CI, 0.54-4.99), and step length (p = 0.00, 95% CI, 1.55-4.62). Conclusion: This study used the neck pattern of PNF for vestibular rehabilitation in patients with chronic stroke. The results showed significant improvement in the patients' balance and walking ability. Therefore, the neck pattern of PNF for vestibular stimulation may be more effective than treadmill training to improve balance and walking ability in patients with chronic stroke.

압력 바이오피드백 제공에 따른 고유수용성신경근촉진법 하지패턴 적용이 뇌졸중 환자의 근력과 보행능력에 미치는 영향 (The Effects of Pressure Biofeedback Units in Lower-Limb PNF Pattern Training on the Strength and Walking Ability of Stroke Patients)

  • Purpose: The purpose of this study was to compare the strength and walking ability of chronic stroke patients following either proprioceptive neuromuscular facilitation (PNF) pattern training with pressure biofeedback units (feedback group) or PNF pattern training without pressure biofeedback units (control group). Methods: Eighteen participants with chronic stroke were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n = 8) and a control group (n = 10). They all received 30 minutes of neurodevelopmental therapy and PNF training for 15 minutes five times a week for three weeks. Muscle strength and spatiotemporal gait parameters were measured. Muscle strength was measured by hand-held dynamometer; gait parameters were measured by the Biodex Gait trainer treadmill system. Results: After the training periods, the feedback group showed a significant improvement in hip abductor muscle strength, hip extensor muscle strength, step length of the unaffected limb, and step time of the affected limb (p<0.05). Conclusion: The results of this study showed that proprioceptive neuromuscular facilitation pattern training with pressure biofeedback units was more effective in improving hip muscle strength and walking ability than the proprioceptive neuromuscular facilitation pattern training without pressure biofeedback units. Therefore, to strengthen hip muscles and improve the walking ability of stroke patients, using pressure biofeedback units to improve trunk stability should be considered.

MPTP 파킨슨 모델의 트레드밀 운동이 알파시누크린 변성과 흑질선조체내 신경성장인자 활성화에 미치는 영향 (Effects of Treadmill Exercise on Alpha-synuclein Mutation and Activated Neurotrophins in Nigrostriatal Region of MPTP-induced Parkinson Models)

  • 목 적 : 규칙적인 운동이 신경보호 효과와 도파민성 신경원의 재구축, 운동기능 향상에 영향을 미친다는 실험실적 연구결과에도 불구하고, 아직까지 파킨슨병 질환자의 트레드밀 운동이 뇌신경 변화에 영향을 미치는지에 대해서는 논란이 되고 있는 상황이다. 더군다나, 증상의 진전이 흑질선조체의 뇌신경 변화에 의한 것인지, 운동에 의한 전반적인 효과인지, 의욕에 영향을 받은 것이지 또한 확실치 않은 상황이다. 이에 본 연구자는 트레드밀 운동이 파킨슨 유발 실험쥐의 뇌신경 변화를 유발하는 것을 밝히고자 본 실험을 수행하였다. 방 법 : 본 실험에서는 파킨슨 모델을 만들기 위해 수컷 C57BL/6 쥐에 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP) 30 mg/kg과 프로베네시드 20 mg/kg을 매 12시간마다 10회 투여(총 5일)하여 파킨슨병을 유발하였다. 이후 운동군을 경사도 $0^{\circ}$, 18 m/min의 속도로, 하루 40분의 트레드밀 운동을 수행하였다. 운동수행의 마지막에는 모든(염류 비교군, 비운동 비교군) 동물의 뇌를 적출하여 신경원성, 신경화학적 변화가 어떤지 비교군, 비운동군과 비교분석하였다. 본 실험에서 Synphilin 단백질은 알파시누크린의 발현 징후로 사용되었다. 흑질과 선조체의 뇌세포를 western blotting에 의해 염색하여 분석하였다. 결 과 : 염류 비교군의 경우 synphilin 단백질의 발현이 발견되지 않았다. 파킨슨 유발을 위한 MPTP(1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine) 투여는 알파시누크린의 응집을 의미하는 synphilin 단백질의 발현이 급증하였다. 하지만, 트레드밀 운동군에서는 synphilin 단백질의 발현이 비운동군에 비해 유의하게 낮았다. 이는 트레드밀 운동이 알파시누크린의 응집도를 낮추는데 영향을 미친다는 것으로 사료된다. 결 론 : 본 연구에는 트레드밀 운동이 파킨슨 모델의 뇌에서 알파시누크린 응집체의 제거를 촉진하고, 병의 진행, 세포사멸을 억제하는 것으로 밝혀졌다.

Pretreatment Effect of Running Exercise on HSP70 and DOX-Induced Cardiotoxicity

  • Objective: The purpose of this study was to determine pretreatment effects of moderate-term endurance training before the various dosages (10 and $20{_{mg.kg}}^{-1}$) of DOX on a heat shock protein ($HSP_{70kda}$) and cardiotoxicity in heart tissue. Methods: Forty-eight male rats were randomly assigned to nontraining (NT) and training (T) groups and three subgroups; $DOX{_{10mg.kg}}^{-1}$ and $DOX{_{20mg.kg}}^{-1}$ and saline treatment. The training program included treadmill running between 25-39 min/day and 15-17 m/min, 5 days/wk for 3 wk. Result: DOX administration, in particularly with $20{_{mg.kg}}^{-1}$, caused up-regulation of oxidants and cardiac damage (MDA, CK, CPK-MB and CK/CPK-MB) and down-regulation of cardioprotection ($HSP_{70}$, SOD) markers, as compared to NT+saline group. Pretreatment effect of treadmill running endurance exercise in the presence of DOX with $10{_{mg.kg}}^{-1}$ caused a significant increase in $HSP_{70}$, SOD and a significant decrease in MDA and insignificant decrease in CK, CPK-MB and CK/CPK-MB, in comparison $T+DOX_{10}$ with $NT+DOX_{10}$ group. However, there was no significant difference between $T+DOX{_{10mg.kg}}^{-1}$ and $T+DOX{_{20mg.kg}}^{-1}$ in the aforesaid markers. Conclusion: Dox-induced cardiotoxicity is related to oxidative stress. Our study suggests that pretreatment with endurance exercise may be considered as a potentially useful strategy to improve myocardial tolerance against single dose DOX-induced oxidative damage.

Coffee intake can promote activity of antioxidant enzymes with increasing MDA level and decreasing HDL-cholesterol in physically trained rats

  • This study investigated the effect of coffee intake and exercise on the antioxidative activity and plasma cholesterol profile of physically trained rats while they were exercising. Forty eight rats were under either the control diet with water (C) or control diet with coffee (CF) and at the same time they were given physical training for 4 weeks. In terms of physical training, the rats were exercised on a treadmill for 30 minutes everyday. At the end of 4 weeks, animals in each dietary group were subdivided into 3 groups: before-exercise (BE); during-exercise (DE); after-exercise (AE). Animals in the DE group were exercised on a treadmill for one hour, immediately before being sacrificed. Animals in the AE group were allowed to take a rest for one hour after exercise. TG levels were significantly high in coffee intake group than in control group. Also TG level of AE group was significantly higher than that of BE group. Exercise and coffee-exercise interaction effects were significant in total cholesterol (P = 0.0004, 0.0170). The AE of coffee intake group showed highest total cholesterol levels. HDL-cholesterol was significantly lower in coffee intake group than in control group. Coffee, exercise, and coffee-exercise interaction effects were significant in SOD (P = 0.0001, 0.0001, and 0.0001). The AE and BE of coffee intake group showed higher SOD levels than the other four groups. Catalase activities were significantly higher in coffee intake group than control group. No significant main effect was found in GSH/GSSG. Coffee, exercise, and coffee-exercise interaction effects were significant in MDA levels (P = 0.0464, 0.0016, and 0.0353). The DE and AE of coffee intake group and the DE of control group showed higher MDA levels than the BE of control group. Therefore, coffee intake can promote activities of antioxidant enzyme but it also increases MDA and decreases HDL-cholesterol in physically trained rats.

편마비환자의 균형기능 향상에 관한 문헌적 고찰 (Improvements in Balance of Patients with Hemiplegia A Literature Review)

  • Background: Hemiplegic patients usually have difficulty maintaining balance. Balance training is a major component of there habilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of PLS(Posterior Leaf Spring), FES(Functional Electrical stimulation), treadmill training, and neurodevelopmental treatment on the improvement of balance in patients with hemiplegia. Methods: We looked into published studies from Dankook University’s electronic library databases of RISS4U, KMbase, NCBI, and MEDLIS concerning the effectiveness of any form of intervention leading to improvement of balance. All types of studies relevant to the topic that were published in English during the time period of 1986 to 2005 were included. Results: 1. There were significant differences in standing balance, dynamic activity balance, and gait speed between barefoot subjects and subjects who wore SPAFO and HPAFO(p<.05). 2. The changes in ROM and FRT related to sex, age, height, and weight part of the diagnosis, as well as experience relapse, was of meaningless value. Changes in ROM related to the duration of pain and experiences of falling down were also meaningless. However, FRT showed significant static differences(p<.05). 3. The body-weight-support treadmill training scoring of standing balance, step length, and a timed 10m walking test showed definite improvement. 4. The proprioceptive control approach improved dynamic balance in patients with hemiplegia. Conclusion: Consequently, further study is needed to verify methods when physical therapists are researching balance ability in hemiplegic patient.

Virtual Reality Community Gait Training Using a 360° Image Improves Gait Ability in Chronic Stroke Patients

  • Purpose: Gait and cognitive impairment in stroke patients exacerbate fall risk and mobility difficulties during multi-task walking. Virtual reality can provide interesting and challenging training in a community setting. This study evaluated the effect of community-based virtual reality gait training (VRGT) using a 360-degree image on the gait ability of chronic stroke patients. Methods: Forty-five chronic stroke patients who were admitted to a rehabilitation hospital participated in this study. Patients meeting the selection criteria were randomly divided into a VRGT group (n=23) and a control group (n=22). Both these groups received general rehabilitation. The VRGT group was evaluated using a 360-degree image that was recorded for 50 minutes a day, 5 days per week for a total of 6 weeks after their training. The control group received general treadmill training for the same amount of time as that of the VRGT group. The improvement in the spatiotemporal parameters of gait was evaluated using a gait analyzer system before and after training. Results: The spatiotemporal gait parameters showed significant improvements in both groups compare with the baseline measurements (p<0.05), and the VRGT group showed more improvement than the control group (p<0.05). Conclusion: Community-based VRGT has been shown to improve the walking ability of chronic stroke patients and is expected to be used in rehabilitation of stroke patients in the future.

고지방식이를 섭취하는 흰쥐에서 가르시니아캄보지아 껍질추출물, 대두펩타이드 및 L-카르니틴 조성물 섭취와 규칙적인 트레드밀운동이 체중감량에 미치는 영향 (Effect of Treadmill Exercise Training and Dietary Intake of Garcinia Cambogia Extract, Soypeptide and L-Carnitine Mixture on Body Weight Reduction in Rats Fed High-Fat Diet)

  • This study was undertaken to examine effects of dietary intake of garcinia cambogia extract, soy peptide and L-carnitine mixture on body weight gain and obesity-related bio-markers in rats fed high-fat diet for 9 weeks with or without regular treadmill exercise. Forty 5-week-old male Sprague-Dawley rats were randomly divided into four groups; sedentary control group (SC), exercised control group (EC), sedentary formula-fed group (SF), and exercised formula-fed group (EF). The SC and EC rats were fed high-fat control diet (fat comprises$40\%$ of total caloris), and SF and EF rats were fed high-fat formula (composed of garcinia cambogia, soy peptide and L-carnitine) supplemented diet. Statistical analyses by two-way ANOVA indicated that the regular treadmill exercise significantly lowered cumulative body weight gain, total visceral fat mass, and epididymal, perirenal and retroperitoneal fat pad weights, and serum concentrations of total cholesterol and LDL + VLDL cholesterol, insulin, c-peptide and leptin. Feeding the formula also resulted in significant reductions in cumulative body weight gain and visceral fat pad weights, along with other related parameters including serum total and LDL + VLDL cholesterol levels, and hepatic enzyme activities involved in fatty acid synthesis. Statistical analyses by one-way ANOVA revealed that the formula consumption significantly improved body weight gain ($18\%$ reduction), total visceral fat weight ($20\%$ reductions), and serum total ($43\%$ reduction) and LDL + VLDL cholesterol ($54\%$ reduction) levels, as well as serum levels of insulin ($49\%$ reduction), and c-peptide ($41\%$ reduction) in sedentary rats, but failed to exhibit significant reductions in these indices in animals under treadmill exercise program. Taken together, these results suggest that the treadmill exercise per n exhibited significant improvements in body fat reduction and other related bio-markers, and so the formula consumption did not achieve a further significant reductions in these bio-markers in exercised rats. Nevertheless, animals fed the formula with regular exercise showed the most efficient weight reduction compared to other groups either fed formula without exercise or received regular exercise without dietary supplementation.
