A Study on the Mental Health of Medical Institution Administration during the Pandemic of Coronavirus Disease 2019 (COVID-19) (코로나바이러스 감염증-19(COVID-19)의 유행 기간 중 의료기관 원무 행정직 정신건강에 대한 연구)

  • Purposes: Evaluate the level of mental health of administrative workers in medical institutions during the COVID-19 epidemic, and check the relationship. Methodology: It targeted 183 administrative workers at medical institutions located in Jeollabuk-do. The data survey method used a self-filled questionnaire, and the data collection period was 30 days from November 16, 2021 to December 16, 2021. A comparative analysis was conducted to find out the correlation between the subjects' biological factors, environmental factors, lifestyle factors, job related factors and depression, Post Traumatic Stress Disorder, and job stress. Findings: As an influencing factor of depression, it was found that the longer the working period, the higher the depression, and the higher the position, job satisfaction, and the total amount of smoking, the lower the depression. Due to the effect on Post Traumatic Stress Disorder, the higher the total amount of smoking, the higher the Post Traumatic Stress Disorder after effects, and the higher the organizational commitment and job satisfaction, the lower the Post Traumatic Stress Disorder aftereffects. It was found that organizational commitment had the highest influence on Post Traumatic Stress Disorder. As an influencing factor on job stress, it was found that the higher the job related factors was job satisfaction and organizational commitment, the lower the job stress. Practical Implications: Variables such as moderate physical activity, alcohol consumption, total smoking, working period, position, job satisfaction, and organizational commitment were identified as factors influencing depression, Post Traumatic Stress Disorder, and job stress during the COVID-19 pandemic.

Effect of Intra-articular Bee Venom Injection on Acute Traumatic Arthritis of Elbow Joint : Two Cases Report (급성 외상성 주관절염에 대한 관절강내 봉약침 치험 2례)

  • Objectives : We got a good effect in two patients not by subcutaneous but by subcutaneous but by intra-articular bee venom injection treatments at traumatic arthritis of elbow joint, so report it. Methods : We used only intra-articular bee venom injection treatments at acute traumatic arthritis of elbow joint. After measured the VAS(visual analogue) and ROM(range of motion) of elbow joint, we bee venom injection effect. Results and Conclusions : The symptoms(swelling, pain) and the functions of elbow joint were quickly improved through intra-articular bee venom injection treatments. One case was improved that Rt. elbow joint flexion was increased from $60^{\circ}$ to $150^{\circ}$, extension was increased from $20^{\circ}$ to $0^{\circ}$. The other case was improved that Lt. elbow joint flexion was increased from $90^{\circ}$ to $150^{\circ}$, extension was increased from $30^{\circ}$ to $0^{\circ}$, and both were decreased in VAS. The results suggest that bee venom intra-articular injection has acute pain reduction, anti-inflammation effect.

A Case Report on the Traumatic Pneumothorax associated with Acupuncture (자침과 관련된 외상성 기흉 1례)

  • Objective : There was no report on traumatic pneumothorax associated with acupuncture in korea yet, although many people guess at its possibility. In order to make known that acupuncture can cause pneumothorax in reality and to prevent more acupuncture-related adverse effect cases in future, this report was made. Methods : Close observation was done on clinical symptoms & radiologic examination of acupuncture-related traumatic pneumothorax patient. Results : During acupuncture therapy on neck stiffness & shoulder pain, a acupuncture needle was inserted too deeply into upper part of thorax unintentionally, patient showed dyspnea, chest pain & discomfort, cough, sweating and mild rupture of right lung in chest radiologic findings, and recovered after 2 weeks of conservative treatment. Conclusion : In order to prevent traumatic pneumothorax during acupuncture therapy, needling into thorax sometimes requires tilted acupuncture needle inserting under consideration of needle width & length. At least 0.40mm width of needle is generally considered appropriate in needling into taut band of thoracic muscles, because too thin and long needle sometimes can be bent and inserted into wrong way, which can cause adverse effects like pneumothorax.

Emergency Nurses' Professional Quality of Life: Compassion Satisfaction, Burnout, and Secondary Traumatic Stress (응급실 간호사의 전문직 삶의 질: 공감만족, 소진, 이차 외상성 스트레스)

  • Purpose: Professional quality of life is the quality one feels in relation to their work as a helper. The purpose of this study was to describe professional quality of life among emergency nurses. Methods: A total of 15 emergency rooms were selected in three cities. Among 263 nurses working at these emergency rooms, 178 nurses consented to participate in this cross-sectional survey. Professional Quality of Life (ProQOL) Scale version 5 was used to measure compassion satisfaction, burnout and secondary traumatic stress. Cluster analysis was used to classify nurses according to professional quality of life. Results: The mean scores (SD) for compassion satisfaction, burnout, and secondary traumatic stress were 32.12 (5.45), 28.27 (4.28), and 28.20 (5.07), respectively. The result of cluster analysis according to standardized score of compassion satisfaction, burnout, and secondary traumatic stress yielded three clusters. Over 50% of participants showed low professional quality of life. Nurses who were included in a cluster representing low professional quality of life were younger, had shorter periods of nursing experience, and perceivedlower social support than other cluster. Conclusion: Education or support programs for emergency nurses are needed to enhance their professional quality of life.

  • Objective : Oriental medical treatment may be possibility or effect in patients with facial nerve paralysis caused by traumatic temporal bone fracture. Methods : The authors compared objectively improvement state of patient that operated acupuncture, herbal medicine and herbal acupuncture. Results : 1. A Facial nerve travel long and pass by narrow bone canal in temporal bone. so slow progressive paralysis is caused by nerve swelling and impedimental blood circulation in bone canal, if bruise happens. 2. At patients with traumatic facial nerve paralysis, acupuncture(LI TE centered operation), herbal medicine(理氣祛風散, 加味補益湯加滅 etc..) and herbal acupucture(SY-消炎, Hominis Placenta-紫河車, JGH-中氣下陷) are effective to improving symptoms. 3. In general, everyone consider surgical operation first of all, in the case of having traumatic facial nerve paralysis. Through this case, the authors are thought that can attempt access of Oriental medical treatment without doing surgical operation.

The Influence of Workplace Violence on Anger and Post Traumatic Stress Disorder among Nurses (간호사의 직장폭력 경험에 따른 분노 및 외상 후 스트레스 장애)

  • Purpose: This study was done to identify the influence of workplace violence on anger and post traumatic stress disorder among nurses. Methods: The research design for this study was a descriptive survey design using a random sampling. Data collection was done using self-questionnaire with 477 nurses. The collected data were analyzed by descriptive statistics, ${\chi}^2$-test and logistics regression. Results: The incidences of total violence, verbal, physical, and sexual violence were 31.2%, 28.7%, 6.3%, 3.6% of the nurses, respectively. Anger was significantly associated with verbal violence (OR: 2.34, CI: 1.40~3.91) and physical violence (OR:4.85 CI: 1.67~14.13). Post traumatic stress disorder was significantly associated with verbal violence (OR: 15.99, CI: 9.58~26.69) and physical violence (OR: 5.37, CI: 1.66~17.40). Conclusion: To promote psychological health in nurses, there is a need to develop prevention programs to decrease workplace violence and to develop programs supporting psychological aspects of verbal violence that nurses experience.

  • The incidence of the Attention Deficit Hyperactivity Disorder secondary to the traumatic brain injury, such as traffic accidents, is increasing;the variety of the treatment modality is also increasing. This case was studied to see if OROS Methylphenidate(Concerta), which is one of the most commonly used medication in Attention Deficit Hyperactivity Disorder patients, not only improves the patient's attention, but also their impulsivity, hyperactivity and aggression. According to the case result, the medication showed an improvement of the impulsivity, aggression, and attention in the secondary Attention Deficit Hyperactivity Disorder patients after the traumatic brain injury.

  • Eleven cases of traumatic diaphragmatic injuries were treated at the department of Surgery from Jan. 1972 to Nov. 1976 were reviewed in this study. 1. Sex ratio of the observed patients was 10 : 1 showing definitely high incidence male patients. 2. The age distribution in this series ranged from 4 to 49 years of age, and average age was 26 years age. 3. Seven cases were received blunt injury on lower chest and upper abdomen, three cases were stabbed by knife and gunshot injury case was on case 4. Six cases were diagnosed and treated early post-traumatic stage within 24 hours, three cases were repaired within I0 days, and the last case was repaired after 5 years post-trauma. 5. The operative mortality was 10% (1/10), the cause of death was liver failure, after right lobectomy of the liver for traumatic liver rupture.

  • This case report included 4 cases of traumatic diaphragmatic hernias, 3 cases of non-traumatic diaphragmatic hernias, and 3 cases of eventration of the diaphragm. Among the traumatic hernias, one case was in immediate phase of traumatic diaphragmatic rupture by traffic accident, 2 cases were in intermediate phase with chronic respiratory or vague gastrointestinal symptoms after traffic accident, and the other was developed after an operation, decortication for a chronic empyema with severe pleural calcifications, damaging the diaphragm. Three cases of nontraumatic diaphragmatic hernia were presented, including 2 cases of probable Bochadlek's hernia (Parents refused operation) and a case of Morgagni's hernia with severe gastrointestinal symptoms. And three cases of eventration of the diaphragm with symptoms were also reported. Results of all treated cases were excellent.

Traumatic Intracerebellar Hematomas (외상성 소뇌내 혈종들)

  • Objective: We report six patients with traumatic intracerebellar hematomas between 1997 and 2003 at our hospitals. Methods: Each data about patients' clinicoradiologic findings, management, and outcomes, which were retrospectively reviewed. Results: All patients had skull fracture on occiput and five patients with large hematomas(three cm or greater) were operated on. In the results of surgery, three patients were good outcome but two patients were fatal due to compression of brain stem. One patient with small hematoma (1.5cm) was treated conservatively and recovered. Conclusion: In our cases, the clinical course and prognosis of traumatic intracerebellar hematoma were grave. The results of this study support that early diagnosis based on strict observation in patients with occipital fracture will lead to best results.
