The Influence of Subjective Health Status, Post-Traumatic Growth, and Social Support on Successful Aging in Middle-Aged Women (중년여성의 주관적 건강상태, 외상 후 성장, 사회적 지지가 성공적 노화에 미치는 영향)

  • Purpose: This study was done to investigate factors influencing successful aging in middle-aged women. Methods: A convenience sample of 103 middle-aged women was selected from the community. Data were collected using a structured questionnaire and analyzed using descriptive statistics, two-sample t-test, one-way ANOVA, Kruskal Wallis test, Pearson correlations, Spearman correlations and multiple regression analysis with the SPSS/WIN 22.0 program. Results: Results of regression analysis showed that significant factors influencing successful aging were post-traumatic growth and social support. This regression model explained 48% of the variance in successful aging. Conclusion: Findings show that the concept 'post-traumatic growth' is an important factor influencing successful aging in middle-aged women. In addition, social support from friends/co-workers had greater influence on successful aging than social support from family. Thus, we need to consider the positive impact of post-traumatic growth and increase the chances of social participation in a successful aging program for middle-aged women.

A Case of Traumatic Ventral Hernia Repair with a Porcine Dermal Collagen Graft (Permacol) (외상성 복벽 탈장 환자에서 biologic mesh를 이용한 치료)

  • Resection of the bowel is necessary for the repair of a ventral hernia after recovery from trauma in some cases. In such instances, polyester or polypropylene meshcannot be used due to the possibility of infection; we had to use biological mesh instead. We report a case in which a traumatic hernia was repaired with Permacol (Covidien, Norwalk, CT, USA). A 42-year-old male patient had been injured by a factory machine seven months prior to admission. At that time, he had abdominal wall injury and small bowel perforation. His abdominal wall had been a defect after operation. A CT scan of the abdomen showed that the left abdominal wall, which is lateral to left rectus abdominis muscle had only one muscle layer, an external oblique muscle, and that a previous abdominal incision had a defect along the entire incision. During the exploration, 10 cm of small bowel was removed due to firm adhesion to the previous surgical scar. Permacol mesh was applied and fixed with transfascial fixations and tacks by using the intraperitoneal onlay mesh technique. There were no complications after the surgery and the patient was discharged without any problems.

Case Report of Acute Traumatic Rotator Cuff Tear Treatment in Traditional Korean Medicine (외상성 급성 회전근개 파열 환자의 한방치료 1례)

  • Objectives: There is no report on treatment of acute traumatic rotator cuff tear in Traditional Korean Medicine. We reported Traditional Korean Treatment for pain relief and better movement of acute traumatic rotator cuff tear. Methods: Shoulder MRI was used to confirm the diagnosis of tear of rotator cuff. The patient was treated with Traditional Korean Methods (Acupuncture, Herbal medicine, Pharmacopuncture) for 6 months. We evaluated the patient through VAS (Visual Analogue Scale), UCLA shoulder scale, ROM (Range of motion) and Shoulder MRI. Results: After 6 months of treatment, the patient's VAS was decreased whereas UCLA score and Shoulder ROM were increased. Rotator cuff tear was repaired on Shoulder MRI images. Conclusions: In acute traumatic rotator cuff tear, Korean Traditional Treatment is good method for pain relief and better movement.

Protective Effects of Singihwan (腎氣丸) on Traumatic Brain Injury-induced Apoptosis in Rat Hippocampal Dentate Gyrus

  • Backgrounds: Singihwan is used "to strengthen inborn energy" and we suspected a protective effect on brain neuron cells. Objectives: The aim of this study was to evaluate the effects of Singihwan (SGH) on traumatic brain injury-induced delayed apoptosis in rat hippocampal dentate gyrus. Methods: For a surgical induction of traumatic brain injury (TBI), a 5 mm diameter stainless rod was used to make traumatic attack from the surface of the brain used by an impactor. The protective effect of the aqueous extract of SGH against TBI in the rat hippocampal dentate gyrus was investigated by using step-down avoidance task, terminal deoxynucleotidyl transferase (TdT)-mediated dUTP nick end labeling (TUNEL) assay, Bax immunohistochemistry, and 5-bromo-2'-deoxyuridine (BrdU) immunohistochemistry. Results: The aqueous extract of SGH suppressed the TBI-induced increase in apoptosis and cell proliferation in the hippocampal dentate gyrus. Conclusions: It is possible that the aqueous extract of SGH has a neuroprotective effect on TBI-induced neuronal cell death.

Effects of Irrational Parenthood Cognition, Post Traumatic Stress Disorder and Spousal Support on Quality of Life of Infertile Women (난임 여성의 비합리적 부모신념, 외상 후 스트레스 장애, 배우자 지지가 삶의 질에 미치는 영향)

  • Purpose: This study examined degrees of irrational parenthood cognition, post traumatic stress disorder (PTSD), spousal support, and quality of life and investigated factors that influence the quality of life of infertile women. Methods: Research design was a cross sectional correlational survey with a total of 113 female patients receiving treatment for infertility. Data were collected from August 1 to November 30, 2015. The collected data were analyzed using t-test, ANOVA, Pearson's correlation, and multiple regression analysis. Results: The mean score of the quality of life was $59.0{\pm}14.8$. The quality of life was significantly associated with irrational parenthood cognition (r=-.70), post traumatic stress disorder (r=-.65), and spousal support (r=.56). The factors significantly affecting the quality of life in infertile women were irrational parenthood cognition (${\beta}=-.45$), post traumatic stress disorder (${\beta}=-.34$), and spousal support (${\beta}=-.32$). The explained variance by these factors was 70.4%, and the regression model was valid (F=89.81, p<.001). Conclusion: This study may contribute to the development of nursing intervention program to improve the quality of life of infertile women.

The Comparison of Health Promotion Behavior, Post Traumatic Growth and Quality of Life according to Stages of Survivorship in Patients with Female Genital Neoplasm (부인암 환자의 생존단계별 건강증진행위, 외상 후 성장 및 삶의 질 비교)

  • Purpose: The purpose of this study was to compare health promotion behavior, post-traumatic growth and quality of life according to the stages of survivorship in patients with female genital neoplasm. Methods: Data were collected from August 1st, 2011 to September 31st 2011 from 142 gynecologic cancer patients who completed treatment or were treated at an out-patient clinic. The instrument were HPLP developed by Walker, Sechrist & Pender, PTGI developed by Tedeschi & Calhoun, and Korean C-QOL. Results: Health promotion behavior scores were significantly higher in the acute survival stage than the extended survival stage. Post-traumatic growth score was higher in the acute survival stage than the extended survival stage. The quality of life scores were higher in the lasting survival stage than the extended survival stage. Conclusion: Gynecological cancer patients in the extended survival stage reported low scores of health promotion behavior, post-traumatic growth and quality of life. Intervention needed to be developed to improve health promotion behavior, post-traumatic growth and quality of life for patients with female genital neoplasm in the extended survival stage.

Arthroscopic Repair of Traumatic Anterior Shoulder Instability with Small Glenoid Bone Defect (관절와에 작은 골결손을 가진 외상성 전방 불안정 견관절의 관절경적 봉합 치료)

  • Purpose: To evaluate the results of arthroscopic repair of traumatic anterior shoulder instability with glenoid bone defect. Materials and Methods: Nineteen patients who had underwent arthroscopic repair for the shoulder with traumatic anterior instability and glenoid bone defect were retrospectively reviewed. Mean age was 24.6 years(range, 20 to 39) and mean follow-up was 23 months(range, 19 to 55). No glenoid bone defect was greater than 7mm in length and 20% of the glenoid. The results were evaluated according to stability, range of motion and function. Results: All patients obtained excellent-good results according to Rowe scoring system. Two patients(10.5%) had instability. The mean loss of external rotation was 15 degrees (range, 0 to 25). Functionally, 17 patients could participate in preinjured work or sports to the same level with or without mild discomfort. The remained 2 patients who had 25 degree loss of external rotation could not play sports. Conclusion: Though arthroscopic repair is a good treatment for traumatic anterior shoulder instability with small glenoid bone defect, it is possible to cause loss of external rotation

  • Traumatic forequarter amputation is an extremely rare and life-threatening injury. It is caused by blunt trauma or tremendous traction force, and the prognosis is very poor because of the involvement of massive associated injury. Traumatic forequarter amputation has been rarely reported in the English language clinical literature, but has never been reported in Korea. We report a case of traumatic forequarter amputation caused by a conveyor belt that was treated with emergency resuscitation and surgery, with an accompanying review of the literature.

  • Objective : Oriental medical treatment may be possible or effective in patients with facial palsy and hearing disturbance caused by traumatic disorder Methods : The authors observed objectively improvement state of patient that treated by acupuncture, herbal medicine Results : 1. A Facial nerve travel long and narrow bone canal in temporal bone. so slow progressive palsy is caused by nerve swelling and impedimental blood circulation in bone canal, if bruise happens. 2. A patient with traumatic facial palsy, acupuncture, herbal medicine(Igigeopung-tang, Boicyangwitang etc.), electro acupuncture are effective to improving symptoms. 3. Despite acupuncture treatment is taken, hearing disturbance is not improved. 4. In general, everyone consider surgical operation first of all, in the case of having traumatic facial palsy. Though this case, the author are thought that can attempt access of Oriental medical treatment without doing surgical operation.

Traumatic Abdominal Wall Hernia (TAWH): Repair by using a Prolen Mesh (복부손상으로 생긴 탈장에 Prolene Mesh를 이용한 수술 1례)

  • Traumatic abdominal wall hernia after blunt abdominal trauma is rare. The prevalence of traumatic abdominal wall hernia in published series is approximately 1%. Recently, by the use of computed tomography has increased the number of occult traumatic abdominal wall hernias (TAWH). A 47-year-old woman presented to the emergency room soon after a traffic accident. She was fully conscious and complained of diffuse, dull, abdominal pain. She had a seat belt on at the time of the accident. Initial computed tomography showed that the lower left abdominal wall had a defect and that a part of the small bowel had herniated through the defect. During the operation, we made an incision at the defect site and confirmed the defect. The defect size was about $15{\times}5cm$. The muscle layers were repaired in layers with absorbable sutures. Prolen mesh was layed down and fixed on the site of the repaired muscle defect. After 6 months, hernia had not recurred, and no weakness of the repaired abdominal wall layers was identified. The patient's postoperative body functions were normal.
