The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of maxillary partial edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. Reconstruction of the atrophic maxillary alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be necessary prior to implant placement or simultaneously at the time of implant surgery to provide a restoration with a good long-term prognosis. Regenerative procedures are used for horizontal and vertical ridge augmentation. Many different techniques exist for effective bone augmentation. The approach is largely dependent on the extent of the defect and specific procedures to be performed for the implant reconstruction. It is most appropriate to use an evidenced-based approach when a treatment plan is being developed for bone augmentation cases. The cases presented in this article clinically demonstrate the efficacy of using a autogenous block graft, guided bone regeneration, ridge split, immediated implant placement technique on the atrophic maxillary area.
치주질환이 중증도 이상으로 진행되면 대개 많은 량의 치주조직지지가 상실된다. 일부 치아 주변으로 발생되는 이러한 조직 파괴는 그들의 심한 동요 때문에 일부 치아들은 스스로 탈락되거나 발치가 요구되는 단계까지 진행된다. 이러한 경우에 적절한 치주치료와 유지 그리고 교합평형을 고려한 치주-보철 치료를 포함하는 포괄적 치료계획이 그들의 건강, 기능, 심미 등의 수복을 위해 필요하게 되고, 이러한 치주-보철치료 중의 하나인 크로스-아치 고정성국소의치(cross-arch fixed partial dentures, CAFPDs)가 심하게 감소된 치주조직지지를 갖는 치아들의 안정을 위해 사용될 수 있다. 그러나 불행하게 이러한 보철물의 디자인에 따른 생역학적 개념과 교합양식에 대해 아직 많이 알려져 있지 않은 상태이다. 본 논고에서는 교합성외상 그리고 CAFPDs를 통해서 본 Ante's law의 재조명, 치료 원칙, 교합부여, 장기적 예후 등에 대한 정리를 보여줄 것이며 증례를 통해 그 가능성을 제시해 보고자 한다.
Aggressive revascularization of the ischemic lower extremities in atherosclerotic, occlusive diseases or acute embolic arterial occlusion due to cardiac valvular disease by thromboembolectomy or an arterial bypass operation has been advocated by some authors. We have performed 68 first time vascular operations, including thromboembolectomies on RR patients with ischemic lower extremities, within an 11-year-and-6-month period, from January 1974 to June 1984. We have reviewed and analyzed our vascular operative procedures and post operative results. The patients upon whom thromboembolectomies were performed were 42 males and 13 females ranging from 5 to 72 years of age. The major arterial occlusive sites were common iliac artery in 20 cases, femoral artery in 21 cases, popliteal artery in 8 cases, common iliac artery and femoral artery in 4 cases, and femoral artery and popliteal artery in 3 cases. The underlying causes of arterial occlusive disease were atherosclerosis obliterans in 34 cases; Buerger`s disease in 3 cases; emboli due to cardiac valvular disease in 13 cases; and vascular trauma in 4 cases, including cardiac catheterization in I of those cases. Arterial bypass operations with autogenous or artificial vascular prosthesis were done in 31 cases. Amputations were done on 2 patients carrying out any more vascular operative procedures would have been of no benefit to them. Our bypass operations for ischemic lower extremities were classified as follows: those done between the abdominal aorta and the femoral artery in 17 cases, including those done between the aorta and the bifemoral arteries with a Y graft in four of those cases and long ones done from the axillary to the femoral artery in 4 cases. Five patients died in the hospital following vascular surgery for ischemic lower extremities, the causes of death were not directly related to the vascular reconstructive operative procedures. The leading causes of death were respiratory failure due to metastatic lung carcinoma: renal failure due to complications from atherosclerosis obliterans; sepsis from open, contaminated fractures of the tibia and fibula; and myocardial failures due to open heart surgery in one case and reconstructive surgery of the ascending aorta in another.
악안면부의 결손은 선천적으로 결손을 가지고 있거나 외상이나 수술적인 절제 등에 의한 후천적 원인에 의해 발생할 수 있다. 특히 구강 내 결손을 가진 환자 중 상악의 결손과 연관되어 있는 경우가 높은 비중을 차지하고 있으며 보철적 치료의 필요성이 높다. 하악의 부분적 결손을 가질 경우 기능적 회복에 상당한 한계점을 보이나 양측 턱관절이 정상적으로 남아 있을 경우 국소의치를 이용한 악안면 보철물을 제작함으로써 양호한 결과를 기대할 수 있다. 본 증례의 환자는 58세 남자 환자로서 안면부 총상으로 인한 구개부 및 좌측 하악 구치부 결손을 가진 분으로 obturator와 하악 RPD 재제작을 위해 내원하였다. 환자의 상악 결손 범위는 Aramany 분류법 Class IV에 해당되는 상태이며, 하악은 Cantor와 Curtis 분류법 Type V resection 상태이다. 상, 하악의 우측 구치부가 잔존하였으나, 수술 후 악골 변형으로 인해 서로 교합되지 않았으며 치주상태 불량으로 인한 동요도도 존재하는 상태였다. Obturator의 유지를 위해 잔존 지대치를 최대한 활용하였고 안정적인 교합이 형성되도록 치아를 배열하였다. 하악 RPD는 하악 절제술로 변형된 연조직에 적합하도록 제한된 범위를 피개하는 RPD를 제작하였다. 이에 환자분의 저작, 연하, 발음 기능이 많이 개선되었으며 만족할 만한 결과를 보여주어 본 증례를 발표하는 바이다.

Objectives: Mandibular fractures vary significantly with respect to epidemiological and demographic parameters among populations. To date, no study has evaluated these aspects of mandibular fractures in Nuh, Mewat, Haryana, India. To retrospectively analyze the incidence, age and sex distributions, etiology, anatomic distribution, occlusal status, treatment modality provided, and their correlation in patients who suffered isolated mandibular fractures. Materials and Methods: The records of maxillofacial injury patients who reported to the Department of Dentistry, SHKM Government Medical College from January 2013 to December 2019, were retrieved from our database, and necessary information was collected. The data collected were analyzed statistically using IBM SPSS ver. 21. Results: Totals of 146 patients and 211 fractures were analyzed. There were 127 males and 19 females with an age range of 3-70 years (mean age, 26 years). Road traffic accident (RTA) was the most common cause of fracture (64.4%), followed by fall (19.9%), assault (15.1%), and sports injury (0.7%). Of all patients, 42.5% had bilateral fractures, 31.5% had left side fracture, 21.2% had right side fracture, 3.4% sustained midline symphyseal fracture, and 1.4% had symphyseal fracture along one side of the mandible. Site distribution was as follows: parasymphysis (34.6%), angle (23.7%), condyle (20.4%), body (12.8%), symphysis (4.3%), ramus 2.4%, and dentoalveolar 1.9%. The most common facture combination was angle with parasymphysis (17.8%). Occlusion was disrupted in 69.2% patients. Closed reduction was the predominant treatment modality. Conclusion: The data obtained from retrospective analyses of maxillofacial trauma increase the understanding of variables and their outcomes among populations. The results of the present study are comparable to those of the literature in some aspects and different in others.
Background: The aim of this study was to investigate the effects of using a systematic approach to tracheostomy care by a clinical nurse specialist and surgical intensivists for patients with a tracheostomy who were transferred from the surgical intensive care unit (SICU) to the general ward. Methods: In this retrospective study, subjects were limited to SICU patients with a tracheostomy who were transferred to the general ward. The study period was divided into a preintervention period (January 1, 2007 to December 31, 2010) and a postintervention period (January 1, 2011 to December 31, 2014), and electronic medical records were used to analyze and compare patient characteristics, clinical outcomes, and readmission to the SICU. Results: The analysis included 44 patients in the preintervention group and 96 patients in the postintervention group. Decannulation time ($26.7{\pm}25.1$ vs. $12.1{\pm}16.0days$, P=0.003), length of stay in the general ward ($70.6{\pm}89.1$ vs. $40.5{\pm}42.2days$, P=0.008), length of total hospital stay ($107.5{\pm}95.6$ vs. $74.7{\pm}51.2days$, P=0.009), and readmission rate of SICU decreased due to T-cannula occlusion (58.8% vs. 5.9%, P=0.010). Conclusions: Using a systematic approach to tracheostomy care in the general ward led to reduction in decannulation time through professional management, which resulted in a shorter hospital stay. It also lowered SICU readmission by solving problems related to direct T-cannula.
배경: Fogarty 카테터는 원위부 동맥 색전을 제거하기 위하여 고안되었으며 1960년대 이래로 급성혈전색전의 치료에 획기적인 치료법이 되었다. 그러나 지난 30년동안 동맥폐쇄의 주원인이 심장에서 비롯하는 색전으로부터 죽상동맥경화증으로 변화하였으며 이런 맥락에서 단순 Fogarty 혈전색전제거술이 여전히 효과가 있는지 의문점이 생긴다. 대상 및 방법: 1990년 3월부터 2008년 8월까지 본원에서 Fogarty 혈전제거술을 시행한 156명을 대상으로 하였다. 환자를 혈전제거술만 시행한 79명(제 1 군)과 혈관우회수술을 같이 시행한 77명(제 2 군)으로 나누어 증상의 기간, 혈전의 원인, 응급실 내원 여부, 한방치료 및 오진경험, 동반질환, 주발생부위, 사망원인 등을 후향적으로 T 검증, 교차분석, 카이제곱 및 Kaplan-Meier을 이용하여 비교분석 비교하였다. 결과: 두군 모두 환자의 나이는 평균 64$\pm$10세로 비슷하였다. 제1군과 2군의 다리 통증 기간은 평균 12$\pm$4일 vs 71$\pm$14일(p=0.001), 응급실 내원한 경우는 50명(63%) vs 18명(23%) (p=0.005), 디스크로 오인하여 치료를 받거나 침을 맞은 경우가 20명(25%) vs 30명(39%), 내원전 항응고제 치료 받은 경우는 22명(28%) vs 11명(14%), 혈전원인은 심장질환 24명(30%) vs 6명(8%) (p=0.001), 동맥경화증 46예(58%) vs 67명(87%) (p=0.001), 외상 9명(11%) vs 6명(8%)이었다. 동반질환으로는 뇌졸증, 고혈압 당뇨가 주를 이루었으며(22$\sim$37%), 막힌 부위는 대부분 장골 및 대퇴동맥이었다 우회수술은 58명(75%)에서 대퇴-대퇴 및 대퇴-슬와동맥간 우회수술을 시행했다. 내막절제술은 각각 7명(9%) vs 18명(23%)에서 동반시술이 이루어졌다(p=0.012). 수술의 성공율은 27명(34%) vs 40명(52%) (p=0.019), 다시 막힌 경우는 37명(47%) vs 20명(26%) (p=0.000), 하지절단 4명(5%) vs 12명(16%) (p=0.012), 사망 10명(13%) vs 3명(4%) (p=0.044)으로 의의있는 차이를 보였다. 결론: 최근 급성동맥폐쇄증의 원인이 류마티스 심장질환에서 동맥경화성 질환으로 변화함으로서 단순한 Fogarty 혈전색전 제거술의 효과가 줄어들고 있어 이러한 단순 시술 대신에 부가적인 우회수술법이 고려되어야 할 것이다.
이갈이는 이를 갈거나 악무는 것을 포함하는 부기능적인 행위를 말하며 광범위하게는 주간 및 야간에 나타나는 것을 모두 포함한다. 이갈이의 원인으로는 기본적으로 말초성 요인과 중추성 요인으로 나누어 살펴볼 수 있는데 현재까지의 연구 결과를 종합하여 볼 때 중추성 요인이 주로 작용하는 것으로 알려져 있다. 이러한 이갈이는 교모, 치경부 미세파절, 교근 비대, 교근 및 측두근의 통증, 턱관절의 통증 및 움직임의 제한, 치아 및 수복물 파절, 치수염, 외상성 교합 등을 유발 할 수 있고 특히 이악물기의 경우 협점막 압흔이나 협점막 백선 또는, 혀의 측면에 압흔 등을 유발한다. 이갈이의 정확한 진단을 위해 구강내 장치, 근전도, 수면다원검사 등을 이용하며 미국수면장애학회의 경우 이갈이의 임상 진단 기준을 제시하고 있다. 그러나 아직까지 이갈이의 명확한 원인이 밝혀져 있지 않기 때문에 치료법의 선택은 신중하여야 한다. 이갈이가 중등도 이상이고 임상 증상이나 징후를 유발하는 경우에 이갈이를 관리하기 위한 방법으로는 위험 요인의 조절, 구강내 장치, 보툴리눔 독소 주사, 약물 치료, 바이오피드백 등이 있으며 구강내 장치 요법이 현재로서 가장 합리적인 방법이다. 이갈이 치료와 교근비대에 대한 심미적인 개선을 같이 원하는 환자에서는 보툴리눔 독소 주사 요법이 이용될 수 있다.
The purpose of this study was to evaluate the incidence and radiographic features of 677 cases of hypercementosis by means of the analysis of full mouth periapical radiograms in 4,236 persons visited the Department of Oral Radiology, School of Dentistry, Kyung Hee University during January 1984 to December 1989. The obtained results were as follows: 1. The incidence of hypercementosis was revealed to be 8.2% in total examined persons, and there was a higher incidence in females (9.4%) than in males (7.1%). 2. The hypercementosis was most frequently occurred in the 6th decades (29.2%), and the incidence was increased by advancing age until 6th decades. 3. There was a higher incidence in the maxilla (59.5%) than in the mandible (40.5%), and maxillary second premolar (18.5%) was the most frequently involved tooth. The maxillary first premolar (11.7%) was next in order to frequency followed by maxillary canine (10.0%) and mandibular first and second premolars (9.6%). 4. In the etiologic factors, 35.0% were inflammation, 31.2% were elongation, 6.2% were trauma from occlusion, 0.1 % were uneruption, and 27.5% were unknown. 5. In the degree of cementum apposition, 35.3% were Type Ⅰ, 29.5% were Type Ⅱ, and 35.2% were Type Ⅲ. 6. In the status of cementum apposition, 3.2% were mesial side, 8.9% were distal side, and 87.9% were mesial & distal side. 7. In the identification of radiographic density between normal cementum and excessive cementum, 12.7% could be identified, and 87.3% could be unidentified.
Except for special situations, it is generally agreed that best results in the treatment of facial fractures is expected if reduction is done within the first 2 or 3 weeks after injury. We reduced facial bone fractures at 4 to 7 weeks after trauma. A 44-year-old female patient underwent open reduction for her right zygomaticomaxillary complex fracture at 7 weeks after injury. A 59-year-old female patient underwent surgery for the right mandible body and left parasymphysis fractures at 4 weeks after injury. Using traditional approaches, granulation tissue and callus were removed from the fracture sites, and malunited fracture lines were separated by a small osteotome. We reduced the displaced fractured zygoma and mandible to their normal anatomical positions and fixed them using titanium plates. No complications such as asymmetry, malunion, malocclusion, or trismus were seen. Unfavorable asymmetric facial contours were corrected, and we obtained good occlusion with favorable bony alignment. The functional and aesthetic outcomes were satisfactory. Through removal the callus and limited osteotomy, a successful approach to the previously fractured line was possible, and an exact correction with symmetry was obtained. This method can be a good option for obtaining good mobility and clinical results in treating delayed facial bone fractures.
