Gunshot Injury to the Anterior Arch of Atlas

  • Penetrating injuries to the upper cervical spine resulting from gunshots are rare in South Korea due to restrictions of gun use. Moreover, gunshot wounds to the upper cervical spine without neurological deficits occur infrequently because of the anatomic location and surrounding essential structures. We present an uncommon case involving the surgical removal of a bullet located in the anterior arch of first cervical vertebra (C1) via a transoral approach without neurological complications or subsequent mechanical instability.

  • Branchial cleft cysts are most common neck masses in adults. Most are second branchial cysts, which occur in the neck, anterior to sternocleidomastoid muscle at the mandibular angle. Rarely these cysts may be present in the parapharyngeal space. We report a case of a second branchial cleft cysts in the parapharyngeal space of 32-year-old female, which was misdiagnosed as peritonsillar abscess. It was excised via transoral and transcervical approach.

후두에 발생한 신경초종 1예 (A Case of Neurilemmoma of the Larynx)

  • Neurilemmoma is a relatively rare benign tumor which may be derived from nerve sheath of the peripheral, sympathetic and cranial nerve except the optic nerve and olfactory nerve. And it is a characteristically solitary and well-encapsulated mass. About 25% of neurilemmoma occurs in the head and neck region and the acoustic nerve is most frequently involved, but neurilemmoma of the larynx is rare. Recently, we experienced a case of neurilemmoma occurred in the right aryepiglottic fold of the larynx. The tumor was successfully removed by transoral approach under direct laryngoscope. So we report this case with literature review.

A 30-year neglected foreign body mimicking a maxillary fracture

  • A-50-year-old male patient presented with complaint of tenderness on the left malar area after traffic accident. Our first impression on him was zygomatic fracture, we did radiologic work-up and physical examination on him. But, on the computed tomography scan, there was no fracture or discontinuity on his facial bone. The computed tomography scan was revealed a 4-cm long foreign body in left maxillary sinus with a large amount of fluid collection. After thorough history taking from him, we reveal the patient had a history of trauma 30 years back on the left zygomatic area with a chopstick. The foreign body was removed via transoral approach with the endoscopic assist. There was no complication after operation.

  • Purpose: Anatomical reduction of the fractured condylar process is an important prerequisite for re-establishing function. The authors reported about effectiveness of transoral approach for mandibular subcondyle fracture using trochar device in cases that the fracture line is below the reference line, the perpendicular line of the longitudinal axis of condylar process passing the lowest point of sigmoid notch. As a serial study, we report the open reduction via preauricular approach for mandibular condyle fracture, in cases that the fracture line is above the reference line. Patients and Methods: Sixteen condylar fractures of 15 adult patients were divided two groups and treated by open reduction via preauricular approach (8 cases) or by closed reduction (8 cases). The degree of maximal mouth opening, occlusion, anatomical reduction, condylar resorption and complications were assessed and evaluated for the two groups. Results: The open reduction of condyle via preauricular approach leads to good results without permanent complications. Anatomical reduction of open reduction group and maximal mouth opening range of the closed reduction group is significantly better than the other group. No significant differences were found in the condylar resorption and the occlusion. Conclusion: The preauricular approach was useful to reduce and fix the condylar fragment, in cases that the fracture line is above the reference line.

Clinical and anatomical importance of foramen magnum and craniocervical junction structures in the perspective of surgical approaches

  • This study was conducted to investigate the clinical and anatomical importance of the relevant region from the perspective of surgical approaches by determining the morphometric analysis of the craniocervical junction and foramen magnum (FM) region and determining their distances from important anatomical points. This research was carried out with 59 skulls found at the Anatomy Laboratories of Erciyes and Ankara Medipol University. Metric measurements of FM and condyle, FM shape, condyle-fossa relationship, and pharyngeal tubercle (PT) were made in mm-based dry bone samples of unknown age and sex. The distance between the anterior notches and the FM was 87.01±4.35, the distance between the anterior notches and the PT was 77.70±4.24, the distance between the PT-sphenooccipital junction was 13.23±2.42, and the FM index was 81.86±7.47. The anteroposterior and transverse lengths of FM were determined as 33.80±2.99 and 27.72±2.30, respectively. The morphometric and morphological data available regarding the craniocervical junction showed significant differences between populations. Comprehensive knowledge of this topic will provide a better approach to treat Arnold Chiari Malformation, FM meningiomas, and other posterior cranial fossa lesions. Therefore, we believe that FM and craniocervical junction morphology will be a guide not only for anatomists, but also for radiologists, neurosurgeons, ENT surgeons, and orthopedists.

  • The endoscopic assisted approach for the treatment of condylar fracture is a less invasive alternative treatment modality and is considered to be able to overcome the limited access to the operation field to obtain an accurate reduction and fixation. Six patients with condylar neck and subcondylar fracture underwent the endoscopic assisted open reduction and internal fixation through the transoral approach at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital. The endoscope was inserted through an intraoral incision and the reduction of fracture fragment was performed via a transbuccal approach with two transcutaneous stab incisions. Five patients showed anatomic reduction without any complications. One patient, whose fracture site was fixed with a single plate, showed displacement of fractured condylar segment during the follow up period. No patient had any facial nerve damage.

이상경상돌기증의 치험례 (A CASE OF SURGICAL TREATMENT FOR SYMPTOMATIC ELONGATED STYLOID PROCESS)

  • Styloid process is a slender pointed bone of variable length, which project downward, forward, and slightly medialward from the posteroinferior portion of the tympanic part of the temporal bone. Embryologically, the styloid process is derived from Reichert's cartilage, a structure of second branchial arch origin. Most patients with elongated styloid process remain asymptomatic, but some patients complain pain and sensation of foreign body in the throat, dysphagia, dysphonia, referred otalgia. Recently, we have experienced a case of elongated styloid process in a fifty-nine year old man. He had been suffered from Rt. side sore throat, headache and pharyngeal discomfort during head movement. Through various X-ray examination, Xeroradiography, and palpation of tonsillar fossa, elongated styloid process on Rt. side was confirmed. Under the general anesthesia, it was successfully removed out via transoral approach technique, described by Eagle. The resected styloid process was 2.5cm in it's length and he was freed from the symptoms without further complication.

  • The displacement of an entire tooth into the adjacent anatomical area is an uncommon complication of a tooth extraction. We encountered a 23-year-old woman who had previously undergone surgery under local anesthesia to remove the lower third molar about 12 weeks prior and the upper third molar was extracted 2 days prior to visiting this hospital. Upon admission, she complained of a swallowing discomfort and a mouth opening limitation. Panoramic radiograph and a CT scan revealed a displacement of the entire tooth into the lateral pharyngeal space. The tooth was retrieved via the transoral approach under general anesthesia. The removed tooth had an indentation formed by a dental bur. Therefore, it was concluded that the tooth displaced into the lateral pharyngeal space was the lower third molar. This report describes an unusual case of a third molar that was displaced into the lateral pharyngeal space with a review of the relevant literature.

비구개관낭종 1예 (A Case of Nasopalatine Duct Cyst)

  • Nasopalatine duct cysts are the most common non-odontogenic developmental cyst originating in the incisive canal of maxilla and occuring in approximately 1% of the population. Clinical presentation is mostly asymptomatic in small cysts, but sometimes shows swelling, pain and drainage when it is infected. The definite diagnosis should be based on clinical, radiological and histopathologic findings. Marsupialization of the cystic tissue can be performed, however, complete surgical excision is the the choice of treatment of nasopalatine duct cysts. We report a case of nasopalatine duct cyst occurred in the midline of hard palate treated by complete excision via transoral approach.
