A Basic Study on the Variability of Indoor Spaces for Local Children's Centers - Focused on the Analysis of Design Guideline - (지역아동센터 실내공간 가변방안을 위한 기초연구 - 디자인가이드라인 사례분석을 중심으로 -)

  • Although the government has revised the child welfare act, institutionalized the establishment of local children's centers, and given financial support for them, there are a number of problems with respect to their operation. Specifically, one of main problems is their limited space. Diverse problems that go with the arrangement of limited space and usability have occurred, however there are only a few strategies to use limited space flexibly. This study assumed variability as one of the methods to solve spatial problems. As a basic research for the assumption, Analyzing the variable method and characteristics of domestic and foreign child-care facility guidelines was performed trying to take advantage of as a basic data in order to seek a variable scheme of local children centers' indoor space. For this purpose, study analysed the methods and characteristics of variability centering on aspects related to variability which are environmental design elements required by local children's centers as suggested in previous studies. The results are as follows. First, the variability of space should be planned according to activity and time, and consider the methods of division, storage, and other facilities necessary. Second, methods of variability in space can be classified into creating areas and structures that are variable. Third, as a result of analysing the environmental design elements required by local children's centers, this study discovered the following characteristics: multi-purpose usability through a combination of variable devices, transition through a variable space medium and setting up areas and expansion of indoor variability assuming usability through internal and external connections.

Adaptation Experience among Hemodialysis of Women with End-Stage Renal Disease (여성 말기신부전 환자의 혈액투석 적응경험)

  • Purpose: This study was a qualitative study to explore and understand the adaptation experiences of hemodialysis among women with End-Stage Renal Disease (ESRD) and to develop a substantive theory using the grounded theory method. Methods: Participants were 15 female patients who underwent hemodialysis for ESRD treatment from three general hospitals. The data were collected through in-depth individual interviews. Results: The adaptation experience of participants was emerged as a process of taking care and enduring. There were four adaptation stages as a negative, despair, receptive, and maintenance period in reference to hemodialysis. The causal conditions were a vague expectations of recovery and refusal to undergo hemodialysis. The core phenomenon was that of confinement to dialysis machine. The contextual conditions for this phenomenon were the loss of femininity. They used action/interaction strategies such as transition their life with a focus on hemodialysis, seeking information, and learning how to take care of their body. Through this process, they had a strong will to live or had sustained their life. Conclusion: These results indicate that there is a need for nurses to understand the different steps of adaptation experiences of the given patient population. It is necessary for nurses to support them to lead their life as much normal as possible and improve the adaptation experience of ESRD.

The Legal Base and Validity of Reviewing Medical Expenses in the Health Insurance (건강보험 진료비심사의 법적 근거와 효력)

  • The medical expenses review system in Korea has developed under fee-for-service system with its own unique structure. The importance of reviewing medical expenses has been emphasized, as the size of medical expenditures moving through the health insurance legal context and its weight in the national economy have increased very rapidly. It is, however, analyzed that the feuds and arguments continue among the stakeholders for the lack of laws supporting the medical expenses review system. The medical expenses review is a series of administrative procedures, deciding whether claims from medical care institutions to the insurer are legal and valid or not. It mainly controls the increase of unnecessarily excessive health insurance claim and prevents fraudulent claim and abuse and checks the less use or unsuitable use of medical resources. It also works a function guarantees medical benefits for the appropriate treatment according to the object of health insurance system as a social insurance scheme. The dispute on legal base of the medical expenses review is about the source of law in the medical expenses review. There are the Health Insurance Act and administrative laws as jus scriptum and the guidelines of review as administrative orders. The medical expenses review should reflect various factors, such as the development of medical healthcare technologies, the health expenditures distribution, the financial situation of the health insurance, and the evaluation on the level of appropriate benefits. It is also likely to adapt to the traits of characters of medicine, and trends and transition, Besides it should judge the legality and the validity of medical benefits expenditures by synthesizing these all factors. And the evaluation system of appropriateness of medical benefits was administrative procedure which was consecutive with reviewing the medical expenses system and it was intended to make up for the result of reviewing the medical expenses in more comprehensive levels.

Review on Diagnostic Criteria of Neurasthenia : Suggesting Pathway of Culture-bound dieases

  • Objective: Neurasthenia is a disease which consists of increased fatigue or bodily weakness and exhaustion plus pantalgia, dizziness, headache and other symtoms relevant to autonomic nerve dysfunction. There are plenty of studies investigating the history of diagnostic criteria of neurasthenia, which is influenced by diverse cultural(or social) environment. The obejective of this study is to provide review of the previous studys on the changes of neurasthenia diagnoses in the context of local area to find meanings of these transition and improve health care for psychiatric patient. Methods: Literature review was conducted on studies demonstrating diagnostic criteria of neurasthenia with cultural(or social) environment. We investigated the literature reviews or observative studies which described alteration of diagnostic criteria of neurasthenia and assessed its significance. After selecting eligible studies, the authors read the articles and summarized the meaningful contents those were significant in clinical practice. Results: Transformation of Chinese Classification of Mental Disorder(CCMD) integrated with internationally utilized DSM-IV or ICD-10 is controversial about its significance in that it had limited effect on public health care due to the variables of sociocultural context, but primarily differentiated neurasthenia from other disorders. The latter one can be the directing point of the diagnostic criteria of other culture-bound diseases, which is the traits of not outstanding mood(or affect) than other neurotic disorders. Conclusion: As diagnostic criteria of neurasthenia varies, the significance of this variation is controversial, but could be the paragon of other culture-bound diseases.

Data economy in Korea: Cases of finance, real estate, and medical care sectors (한국의 데이터경제 현황 및 평가: 금융, 부동산, 의료 부문을 중심으로)

  • With the recent surge in the share of data-based economic activities, there have been vibrant discussions on the data economy. Yet, few extant works provide a framework for systematically analyzing the transition to the data economy by major industries in Korea. By reviewing the existing literature, we first summarize the main characteristics of the data economy as building platforms, the greater importance of predictive power, and the increased use of new analytics. Next, based on such understanding, we provide a comparative analysis regarding the degree of data-based activities in Korea's financial, real estate, and medical sectors. We find that the speed at which, and the content of the data economy characteristics being realized were different for the different sectors. These findings suggest that differentiated policy approaches by major industrial sectors such as finance, real estate, and medical care are needed to improve economic productivity and increase welfare through the spread of the data economy.

Transition of Occupational Health Nursing Education in Korea (한국 산업간호교육의 변화추세 분석)

  • In December 1990, Occupational Safety and Health Law was amended to reinforce employer's responsibilities on employees' health and safety. Among the amended law it was important to expand the role of an occupational health nurse to the role of an occupational health manager. An occupational health manager should take charge of coordinating periodic health examination and environmental hazard evaluation, providing primary care, monitoring employees' health status, giving the workplace walk-through, selecting safe protection equipment, providing health information, counseling and health education, independently. This position of occupational health nurse is equivalent to the role of doctors or occupational hygienists. In 1991, government made a master plan to prevent occupational disease and injury. Under the plan, Korea Industrial Nursing Association (KINA) was established in 1994 with the purpose of improving health services and upgrading career opportunities for members. Therefore, this study was designed to analyze the transition of occupational health nursing education with the changes of law and policy in Korea between 1991 and 1996. In details, it was to analyze the rate of school providing occupational health nursing practice based lecture, lecture hours, lecture contents in undergraduate curriculum, program contents of graduate school, kinds of continuing education, etc. For this purpose, we conducted survey two times. In February 1991, baseline study was conducted with all nursing programs in Korea (19 BSN programs and 43 nursing departments of junior college). From April to May in 1996, the second survey was conducted with all nursing programs (38 BSN programs and 69 junior colleges). The first response rate was 66.1% and the second was 40.6%. Structured questionnaires were mailed to the deans or the community health nursing faculties. In the case of graduate school, telephone survey was conducted with 10 school of public health or environmental health area. Data from the yearbook of Industrial Safety Training Institute (ISTI), the history of Korea Industrial Health Association, and the journals of KINA were also included in the analysis. As the results, we found that there were remarkable improvement in undergraduate and graduate programs, obligatory as well as voluntary continuing education in terms of occupational health nursing expertise between 1991 and 1996. 1) The number of school providing occupational health nursing practice-based lecture was increased with the rate from 7.3% to 25.6%. The rate of school giving over 15 class-hours was increased from 33.3% to 46.6%. 2) Content areas were composed of introduction of occupational health, occupational epidemiology, industrial hygiene, occupational disease and injury, law and policy, health education, concept of occupational health nursing, role of occupational health nurse, occupational health nursing process, etc. Of content areas, occupational health nursing process was more emphasized with the increased rate from 43.9% to 88.4%. 3) In the case of graduate school, occupational health programs were increased from 4 to 10. One of them has developed occupational health nursing program as an independent course since 1991. 4) The law increased educational hours from 28 hours to 36 hours for introductory course at the time of appointment, and from 14 hours to 24 hours every 2 years for continuing education. Course contents were Occupational safety and health law, introduction of occupational health, health education methodology, planning and evaluation, periodic health exam, occupational disease care, primary care, emergency care, management, industrial environment evaluation, etc. In 1996, Korea Industrial Nursing Association has begun to provide continuing education after Industrial Safety Training Institute. 5) Various educational programs in voluntary base were developed such as monthly seminar, CE articles, annual academic symposium, etc. It was shown that changes of law and policy led rapid growth of occupational health nursing education in various levels. From this trend, it is expected that occupational health nurse expertise be continuously to be enhanced in Korea. Legal and political supports should proceed for the development of occupational health nursing in early stage.

Effects of a Full Body Massage on Uterine Contraction, Length of Labor, Type of Delivery, and Drug Intervention for Primipara during Labor (분만 중 전신마사지가 초산부의 자궁수축, 분만소요시간, 분만형태 및 약물사용에 미치는 효과)

  • This study was designed to test the effects of a full body massage on uterine contraction, length of labor, type of delivery, and drug intervention for primipara during labor. Data were collected using a quasi-experiment method (nonequivalent control group, pre-post test design) from November 1, 2001 to July 31, 2002. The subjects of this experiment consisted of 28 women in the experimental group and 29 in the control group, out of 57 primipara hospitalized at the U OB & GYN hospital in Inchon. The experimental group was given a 20 minute full body massage for each of the three delivery phases (latent, active, and transition). The control group was given conventional delivery care. Three (3) parameters were analyzed in this experiment. (1) The interval, duration, and strength of uterine contraction, using an electric tocodynamometer (2) The elapsed time for stage 1 and stage 2 labor. (3) The types of deliveries and drug interventions, using postpartum medical records The data collected were analyzed using the repeated measures analysis of variance (ANOVA), t-test, and $x^2$ test of the SPSS program. The results of the experiment are as follows: 1) Uterine contraction interval was significantly reduced (F=3.210, p=.050). Duration of uterine contraction showed significant increase only during the transition phase (t=-2.319, p=.023). Strength of uterine contraction showed no significant difference. 2) Total length of labor was significantly shortened (t=-5.245, p=.000). The length of 1st stage labor was significantly shortened (t=-5.164, p=.000), with latent phase showing (t=-4.709, p=.000), active phase (t=-2.973, p=.005), and transition phase (t=-2.031, p=.047). The length of 2nd stage labor showed no significant difference. 3) The number of natural deliveries were significantly increased ($x^2$=13.127, p=.004). 4) The number of drug interventions were significantly fewer ($x^2$= 4.493, p=.034). In conclusion, this study shows that a full body massage has a significantly positive effect on uterine contraction interval, length of labor, type of delivery, and drug intervention. Therefore, this study suggests that a full body massage be used clinically to help primipara during labor.

  • Background and Objectives : Anaplastic thyroid carcinoma(ATC) is a rare but highly aggressive thyroid malignancy that is associated with an extremely poor survival despite the best multidisciplinary care. BRAF(V600E) mutation is detected in about a quarter of ATC, but unlike its high treatment response to selective BRAF inhibitor (PLX4032) in metastatic melanoma, the treatment response of ATC is reported to be low. The purpose of this study is to investigate the innate resistance mechanism responsible for this low treatment response to BRAF inhibitor and its effect on epithelial-mesenchymal transition(EMT). Materials and Methods : Two ATP cell lines, 8505C and FRO were selected and treated with PLX4032 and its drug sensitivity and effects on cell migration and EMT were examined and compared. Further investigation on the changes in signals responsible for the different treatment response to PLX4032 was carried out and the same experiment was performed on both orthotopic and ectopic xenograft mouse models. Results : FRO cell line was more sensitive to PLX4032 treatment compared to 8505C cell line. The resistance to BRAF inhibition in 8505C was due to increased expression of EGFR. Effective inhibition of both EGFR and p-AKT was achieved after dual treatment with BRAF inhibitor(PLX4032) and EGFR inhibitor(Erlotinib). Similar results were confirmed on in vivo study. Conclusion : EGFR-mediated reactivation of the PI3K/AKT pathway and MAPK pathway contributes to the relative insensitivity of BRAF(V600E) mutant ATC cells to PLX4032. Dual inhibition of BRAF and EGFR leads to sustained treatment response including cell invasiveness.

A Study on Factors Related to of Clonorchiasis Infection among the Inhabitants in the Nakdong Riverside Area (낙동강 유역 주민들의 간흡충 감염에 영향을 주는 개인요인 및 지역요인 분석)

  • Purpose: This study investigated the factors of C. sinensis infection among the residents in vicinity of the Nakdong River and the infection status of fishes in the stream with C. sinensis maetacercaria. Methods: Selected five regions of the Nakdong River and collected freshwater fishes from the stream to examine their metacercarial infections; and studied the status and causes of C. sinensis infection in 552 residents living within 5 kilometers from the river. Results: Among the 552 residents studied, 13.8% were infected with C. sinensis, and among 379 freshwater fishes of 20 species collected from the five regions, 41 fishes of 8 species were infected with C. sinensis metacercaria. Conclusion: In order to prevent human C. sinensis infection and transition to chronic liver ions, we need to develop and apply good programs such as continuous health education, personal hygiene improvement, active medical treatment for those infected with C. sinensis, and residents’ voluntarily stop of eating freshwater fishes in the raw state.

Essential Logical Model Approach in Analysis and Design for Patient Management and Accounting System : A Case Study (본질적 논리모형에 근거한 원무관리시스템의 분석과 설계)

  • In developing total hospital information system, large amount of time and expense are to be spent while its results are likely to lead itself to end-users' dissatisfaction. Some of the main complaints on the part of end-users come from insufficient consideration of end-users environment as well as inappropriate representation of their requirement in the system alalysis and design. This papre addresses some advantages of Essential Logical Modeling Process for better analysis and design, explaining by example the developmental process of the Patent Management and Accounting System for a tertiary care hospital. In the case, the Essential Model, suggested by McMenamin and Palmer, proved to be an effective tool for clear separation of analysis and design phase and for better communication among system developers and with end-users. The modeling process itself contributed to better program modularity as well, shown in a Structured Chart. Difficulties in learning how to identify' essential activities' for the modeling practice were experienced in the beginnins stage, which were, however, overcome by elaborating some heuristic guideling and by rdferring to necessary tools including State Transition Diagram, Control Flow Diagram, and so many. While full evaluation of the Essential Model usag remains to wait till the completion of the case project, its strengt in making clear distinction between analysis and design phase was enough to be attractive to system analysts. The model concepts are open to many further application fields, particularly such areas as business re engineering, process remodeling, office automation, and organizational restructuring.
