고분자/베타-사이클로덱스트린 포접 화합물로 이루어진 고분자 혼성체 필름의 물성 및 구조에 미치는 게스트 분자의 영향 (Effect of Guest Molecules on Structure and Properties of Polymer/beta-Cyclodextrin Inclusion Compound Hybrid Films)

  • 본 연구에서는 게스트 분자(guest molecule)의 특성이 고분자/베타-사이클로덱스트린(beta-cyclodextrin) 포접화합물(inclusion compound)로 이루어진 고분자 필름의 구조 및 물성에 미치는 영향에 대해서 고찰하고자 한다. 본 연구에서 사용된 게스트 분자는 미백 효과를 지니는 것으로 알려진 3가지로 하이드로퀴논(hydroquinone, HQ), 알부틴(arbutin, AB), 그리고 트랜액사믹 애시드(tranexamic acid, TA)이다. 먼저, 베타-사이클로덱스트린과 게스트 분자 간의 포접화합물의 성공적인 형성과 이를 포함하는 고분자 필름의 제조여부를 라만(Raman) 분광학으로 확인하였다. 포접화합물을 포함하는 고분자 필름의 구조 및 물성은 엑스선 회절법(X-ray diffraction)과 주사열용량법 및 열중량추적법 같은 열분석법으로 고찰하였다. 그 결과, 트랜액사믹 애시드의 영향이 다른 분자의 영향과 비교하여 상당히 상이하였음을 관찰할 수 있었다. 이러한 경향은 간단한 분자 시뮬레이션 기법으로 재검증하였다. 본 연구는 포접화합물을 형성하는 게스트 분자들의 상이한 영향에 대한 체계적인 접근을 통한 실험적 검증의 사례로 향후 관련 연구에 중요한 정보를 제공할 것으로 기대된다.

역행성 견관절 전치환술에서 트라넥삼산의 출혈 및 수혈 감소 효과 (Tranexamic Acid Reduces Postoperative Blood Loss in Reverse Total Shoulder Arthroplasty)

  • 목적: 트라넥삼산(tranexamic acid, TXA)은 하지 수술에서 출혈과 수혈 빈도를 줄이는 것으로 알려졌다. 본 연구에서는 역행성 견관절 전치환술 시에 출혈 및 수혈 요구에 대한 TXA의 효과를 평가하고자 하였다. 대상 및 방법: 2009년 9월부터 2020년 7월까지 역행성 견관절 전치환술을 시행한 환자를 평가하였다. 수술 중 정맥로와 관절강 내 도포를 시행한 군은 TXA 사용군, 시행하지 않은 군은 TXA 미사용군으로 분류하였다. 술 후 출혈량을 나타내는 배액관 배액량과 수혈 여부, 술 후 혈색소와 적혈구 용적률 수치를 분석하였다. 결과: TXA 사용군과 TXA 미사용군 간 연령(72.0±7.0 vs. 71.5±5.8, p=0.656) 및 남, 녀 성별 비율(28:65 vs. 23:61, p=0.689)은 유의한 차이가 없었다. 또한 고혈압, 당뇨병, 두 질환 모두를 가진 환자의 유병률(36:3:13 vs. 32:3:8, p=0.806)은 양 군 간 유의한 차이가 없었다. 수혈 기준에 따라 수혈을 시행한 수혈 환자 수(0 vs. 9, p=0.001)는 TXA 사용군에서 없었으며 TXA 미사용군에서 유의하게 더 많았다. 수술 1일째(98.8±61.2 ml vs. 162.7±98.8 ml, p<0.001) 및 제거 당시 배액관 배액량(73.8±48.4 ml vs. 91.5±54.5 ml, p=0.024)은 둘 다 TXA 사용군에서 유의하게 더 적었다. 수술 전 측정한 혈색소 수치(13.2±1.4 vs. 13.3±1.5, p=0.374)와 적혈구 용적률 수치(39.5±3.9 vs. 39.6±4.7, p=0.946)에서 양 군 간 유의한 차이가 없었다. 수술 1일째(11.7±1.2 g/dl vs. 11.2±1.4 g/dl, p=0.048), 3일째(10.9±1.2 g/dl vs. 10.2±1.2 g/dl, p<0.001), 6일째 혈색소(11.2±1.3 g/dl vs. 10.7±1.3 g/dl, p=0.020)는 TXA 사용군에서 유의하게 높았다. 수술 1일째(35.0%±3.6% vs. 32.5%±3.8%, p=0.001), 3일째(32.3% ±5.0% vs. 29.8%±3.6%, p<0.001), 6일째 적혈구 용적률 수치(33.5%±3.8% vs. 31.5%±3.7%, p<0.001)에서도 TXA 사용군에서 유의하게 높았다. 결론: 역행성 견관절 전치환술 시행 시 정맥로와 관절강 내 국소도포를 이용한 TXA의 사용은 수술 후 수혈의 필요성과 출혈량을 줄일 수 있다.

Improving visualization in shoulder arthroscopy

  • Arthroscopic shoulder procedures are one of the most common procedures used to restore function through minimally invasive techniques. With the demand for shoulder arthroscopic procedures comes the need for safe, effective, and efficient surgery that maximizes patient outcomes while minimizing complications. Many variables contribute to visualization in shoulder arthroscopy including vascular anatomy, blood pressure control, arthroscopic pump systems, turbulence control, epinephrine, and tranexamic acid. Furthermore, patient positioning can have a dramatic effect on visualization with both the beach chair position and lateral decubitus positioning having various strengths and weaknesses depending on the intended procedure being performed. The purpose of this review is to examine the benefits and complications reported in the literature for improving visualization in shoulder arthroscopy.

Change of Management Results in Good-grade Aneurysm Patients

  • Objective : The present study attempts to address the change of management results over time during the past 13 years in good-grade patients with intracranial aneurysm. Methods : Six hundred twenty five [Hunt-Hess grade I to III] out of 826 patients with ruptured intracranial aneurysms operated by the same operator within 3 days after the attack from 1990 to 2002 were selected. Since 1998, endovascular aneurysmal occlusion was done in selected cases of 21 patients. The change of management results over time, including rebleeding rate, delayed ischemic neurologic deficit[DIND] as a cause of morbidity and mortality and surgical outcome were examined. Results : The ratio of poor-grade patients in all patients tended to decrease over the years. The early rebleeding rate declined from 5.0% to 1.2% with the use of tranexamic acid and computed tomography angiogram DIND as a cause of mortality and morbidity has decreased from 12.5% in 1990 to approximately 0% currently. Surgical outcome began to improve significantly in 1994 [poor outcome : 25% in 1990, 12.2% in 1994,6.8% in 2002]. Conclusion : These results suggest that the advances in care and increased experience of the operator significantly affect the change of overall outcome, and early detection of the aneurysm is needed for reducing the ratio of poor-grade patients.

Treatment of Refractory Melasma with Microwave-generated, Atmospheric-pressure, Non-thermal Nitrogen Plasma

  • Periorbital melasma is often refractory to treatment and highly associated with rebound hyperpigmentation or mottled hypopigmentation after laser treatment in Asian patients. In this report, we describe 2 patients with cluster-1 periorbital melasma and 1 patient with cluster-2 periorbital melasma who experienced remarkable clinical improvements after microwave-generated, atmospheric-pressure, non-thermal nitrogen plasma treatments. All patients exhibited limited clinical responses after combination treatments with topical bleaching agents, systemic oral tranexamic acid, and low-fluenced Q-switched neodymium (Nd):yttrium-aluminum-garnet (YAG) lasers. Low-energy nitrogen plasma treatment at 0.75 J elicited remarkable clinical improvement in the periorbital melasma lesions without post-laser therapy rebound hyperpigmentation and mottled hypopigmentation. We deemed that a single pass of nitrogen plasma treatment at 0.75 J induces mild microscopic thermal tissue coagulation and modification within the epidermis while preserving the integrity of the basement membrane in patients with periorbital melasma. Accordingly, nitrogen plasma-induced dermal tissue regeneration could play a role in the treatment of melasma lesions.

조기 뇌동맥류 수술전에 항섬유소융해제 치료의 이점 (Benefits of Antifibrinolytic Therapy before Early Aneurysm Surgery)

  • Objective : Antifibrinolytic treatment after aneurysmal subarachnoid hemorrhage has been shown to have no significant effect on outcome since a reduction in the rate of rebleeding was offset by an increase in the incidence of hydrocephalus and ischemic events. As the results of early aneurysm surgery and a change of strategy in the intensive medical treatment, outcome in patients with cerebral ischemia has been improved. On the other hand, rebleeding still remains as a major cause of death. A short course of tranexamic acid(TA) was tried to study its efficacy and safety in reducing the incidence of rebleeding before aneurysm surgery. Methods : A total of 507 patients with ruptured cerebral aneurysm operated within 3 days after the attack from 1990 to 1999 were included in this study. Group A consisted of 302 consecutive patients treated from 1990 through 1995 served as control. Two hundred-five patients in group B were treated with TA from 1996 through 1999. Both groups were evaluated for comparability of demographic and clinical variables including age, Hunt-Hess grade, Fisher grade, aneurysm location, hypertension, day of surgery, and initial hydrocephalus. The relationships of TA with rebleeding, ischemia, and chronic hydrocephalus were also studied. Results : There was no significant difference in patient demographics and clinical characteristics between group A and group B. Sixteen patients(5.3%) suffered a recurrent hemorrhage in group A and three(1.5%) in group B(p<0.05). Chronic hydrocephalus requiring a shunt was found in a significantly greater proportion in group B than in group A(p<0.05). The incidence of cerebral ischemia was not elevated in group B compared with group A. Conclusion : Considering the fact that the reduction of fatal rebleeding outweighed the increased incidence of hydrocephalus, the authors believe that a short course of TA is beneficial in diminishing the risk of rebleeding prior to early surgical intervention.

Spontaneous Resolution of Chronic Subdural Hematoma : Close Observation as a Treatment Strategy

  • Objective : Chronic subdural hematoma (cSDH) is common condition in neurosurgical field. It is difficult to select the treatment modality between the surgical method and the conservative method when patients have no or mild symptoms. The purpose of this study is to provide a suggestion that the patients could be cured with conservative treatment modality. Methods : We enrolled 16 patients who had received conservative treatment for cSDH without special medications which could affect hematoma resolution such as mannitol, steroids, tranexamic acid and angiotensin converting enzyme inhibitors. The patients were classified according to the Markwalder's Grading Scale. Results : Among these 16 patients, 13 (81.3%) patients showed spontaneously resolved cSDH and 3 (18.7%) patients received surgery due to symptom aggravation and growing hematoma. They were categorized into two groups based on whether they were cured with conservative treatment or not. The first group was the spontaneous resolution group. The second group was the progression-surgery group. The mean hematoma volume in the spontaneous resolution group was 43.1 mL. The mean degree of midline shift in the spontaneous resolution group was 5.3 mm. The mean hematoma volume in the progression-surgery group was 62.0 mL. The mean degree of midline shift in the second group was 6 mm. Conclusion : We suggest that the treatment modality should be determined according to the patient's symptoms and clinical condition and close observation could be performed in patients who do not have any symptoms or in patients who have mild to moderate headache without neurological deterioration.

When do we need more than local compression to control intraoral haemorrhage?

  • Objectives: The aims of this study were to determine the effectiveness of local compression in patients presenting to the emergency room with intraoral bleeding and to identify when complex haemostatic measures may be required. Materials and Methods: Five hundred forty patients who had experienced intraoral haemorrhage were retrospectively reviewed. The outcome variable was the haemostasis method used, i.e., simple (local compression with gauze) or complex (an alternative method after local compression has failed). Predictor variables were sex, age, American Society of Anesthesiologists (ASA) class, hepatic cirrhosis, bleeding disorder, use of antithrombotic agents, and site/cause of haemorrhage. Results: The mean patient age was 48.9±23.9 years, 53.5% were male, 42.8% were ASA class II or higher, and 23.7% were taking antithrombotic agents. Local compression was used most often (68.1%), followed by local haemostatic agents, sutures, systemic tranexamic acid or blood products, and electrocautery. The most common site of bleeding was the gingiva (91.7%), and the most common cause was tooth extraction (45.7%). Risk factors for needing a complex haemostasis method were use of antithrombotic agents (odds ratio 2.047, P=0.009) and minor oral surgery (excluding extraction and implant procedures; odds ratio 6.081, P=0.001). Conclusion: A haemostasis method other than local compression may be needed in patients taking antithrombotic agents or having undergone minor oral surgery.

Outcomes and physiologic responses associated with ketamine administration after traumatic brain injury in the United States and Canada: a retrospective analysis

  • Purpose: Ketamine has historically been contraindicated in traumatic brain injury (TBI) due to concern for raising intracranial pressure. However, it is increasingly being used in TBI due to the favorable respiratory and hemodynamic properties. To date, no studies have evaluated whether ketamine administered in subjects with TBI is associated with patient survival or disability. Methods: We performed a retrospective analysis of data from the multicenter Prehospital Tranexamic Acid Use for Traumatic Brain Injury trial, comparing ketamine-exposed and ketamine-unexposed TBI subjects to determine whether an association exists between ketamine administration and mortality, as well as secondary outcome measures. Results: We analyzed 841 eligible subjects from the original study, of which 131 (15.5%) received ketamine. Ketamine-exposed subjects were younger (37.3±16.9 years vs. 42.0±18.6 years, P=0.037), had a worse initial Glasgow Coma Scale score (7±3 vs. 8±4, P=0.003), and were more likely to be intubated than ketamine-unexposed subjects (88.5% vs. 44.2%, P<0.001). Overall, there was no difference in mortality (12.2% vs. 15.5%, P=0.391) or disability measures between groups. Ketamine-exposed subjects had significantly fewer instances of elevated intracranial pressure (ICP) compared to ketamine-unexposed subjects (56.3% vs. 82.3%, P=0.048). In the very rare outcomes of cardiac events and seizure activity, seizure activity was statistically more likely in ketamine-exposed subjects (3.1% vs. 1.0%, P=0.010). In the intracranial hemorrhage subgroup, cardiac events were more likely in ketamine-exposed subjects (2.3% vs. 0.2%, P=0.025). Ketamine exposure was associated with a smaller increase in TBI protein biomarker concentrations. Conclusions: Ketamine administration was not associated with worse survival or disability despite being administered to more severely injured subjects. Ketamine exposure was associated with reduced elevations of ICP, more instances of seizure activity, and lower concentrations of TBI protein biomarkers.

Clinical Practice Guideline for the Treatment of Traumatic Shock Patients from the Korean Society of Traumatology

  • Purpose: Despite recent developments in the management of trauma patients in South Korea, a standardized system and guideline for trauma treatment are absent. Methods: Five guidelines were assessed using the Appraisal of Guidelines for Research and Evaluation II instrument. Results: Restrictive volume replacement must be used for patients experiencing shock from trauma until hemostasis is achieved (1B). The target systolic pressure for fluid resuscitation should be 80-90 mmHg in hypovolemic shock patients (1C). For patients with head trauma, the target pressure for fluid resuscitation should be 100-110 mmHg (2C). Isotonic crystalloid fluid is recommended for initially treating traumatic hypovolemic shock patients (1A). Hypothermia should be prevented in patients with severe trauma, and if hypothermia occurs, the body temperature should be increased without delay (1B). Acidemia must be corrected with an appropriate means of treatment for hypovolemic trauma patients (1B). When a large amount of transfusion is required for trauma patients in hypovolemic shock, a massive transfusion protocol (MTP) should be used (1B). The decision to implement MTP should be made based on hemodynamic status and initial responses to fluid resuscitation, not only the patient's initial condition (1B). The ratio of plasma to red blood cell concentration should be at least 1:2 for trauma patients requiring massive transfusion (1B). When a trauma patient is in life-threatening hypovolemic shock, vasopressors can be administered in addition to fluids and blood products (1B). Early administration of tranexamic acid is recommended in trauma patients who are actively bleeding or at high risk of hemorrhage (1B). For hypovolemic patients with coagulopathy non-responsive to primary therapy, the use of fibrinogen concentrate, cryoprecipitate, or recombinant factor VIIa can be considered (2C). Conclusions: This research presents Korea's first clinical practice guideline for patients with traumatic shock. This guideline will be revised with updated research every 5 years.
