소아 환자에서의 외상성 간 손상 (Traumatic Liver Injury in Pediatric Patients)

  • Purpose: Trauma is an important cause of death in children. In particular, the liver is the second most commonly organ injured by blunt abdominal trauma. Treatment of patients with liver injury is has changed, and non-operative treatment is the major treatment method at present. In this study, we reviewed traumatic liver injury in pediatric patients. Methods: Seventy-seven patients younger than 16 years of age with traumatic liver injury were assessed for 10 years from July 1999 to June 2009 at Wonju Christian hospital. Records of the patients were reviewed retrospectively. Demographic and clinical data were analyzed. Results: The median age was 6 years, and the male-to-female ratio was 1.2 : 1. The most common injury grade was grade I. The majority of injuries were caused by was traffic accidents, and the second most common cause of injuries was falls. Twenty-four patients had liver injuries alone, and the most common accopaning injury was a lung injury. The average hospital stay was 20.7 days, and the average ICU stay was 4.8 days. Four patients died (5.2%). There were 6 patients with under 10 points on the Glasgow coma scale (GCS). Among these patients, three died. All mortality cases had over 16 points on the Injury Severity Score (ISS). Two patients were treated surgically, one of whom died. Of the 75 patients with non-operative management, three died due to associated injuries. Conclusion: Most pediatric patients with liver injury have good results with non-operative management. Associated injuries and hemodynamic instability are predictive of patient outcome, and those with isolated liver injuries can be successfully managed non-operatively.

소방차량의 안전벨트 설계 및 정책제안 (Design and Policy of Seat Belt for fire Engines)

  • According to the National Fire Agency, there were 61 traffic accidents of Fire engines in 2021. That is an increase of about 80% compared to the previous year. And the number of injuries and deaths increased by more than 300%. According to the survey of this study, less than 25% of firefighters wore seat belts of Fire engines during dispatch, contrary to the rapid increase in injuries and deaths. About the reasons for why firefighters do not wear seat belts, It can be found in aspects that are related to personal consciousness, organizational culture, and firefighting dispatch. The number of dispatches will increase in the future, this is considered to be a serious problem for the stability of the firefighting organization. In this study, a seat belt should be designed newly and proposed to improve the seat belt wearing policy.

외상 후 사지 혈관손상의 치료 (Management of Vascular Injuries to the Extremities after Trauma)

  • 배경: 사지의 혈관손상은 적절한 치료를 하지 않을 경우 사지 손실과 사망에 이르게 하는 치명적인 결과를 가져올 수 있다. 근래에 와서 산업과 교통수단이 발달함에 따라 산업재해와 교통사고에 의한 혈관손상이 증가 하고 있다. 혈관손상은 빠른 진단과 치료가 좋은 경과를 가져온다. 대상 및 방법: 1998년 1월부터 2006년 12월까지 둔상과 관통상으로 응급 수술한 43예의 환자를 대상으로 후향적으로 조사 하였다. 결과: 남자 38예, 여자 5예였으며, 평균 나이는 $42.0{\pm}16.8$세(17~77)였다. 혈관손상의 원인으로 교통사고 28예(65%), 산업재해 6예(14%), 유리에 의한 손상 6예(14%), 그리고 칼에 의한 자상 3예(7%)였다. 입원에서 수술시간까지 평균시간은 $319.0{\pm}482.2$분(27~2,400)이었고, 평균 입원기간은 $53.1{\pm}56.0$일(2~265)이었다. 손상부위는 대퇴동맥 16예(37%), 슬와동맥 8예(19%), 상완동맥 8예(19%), 그리고 쇄골하동맥과 액와동맥 7예(16%)였다. 동반손상으로 골절 23예(53%), 근육손상 18예(42%), 신경손상 5예(12%) 그리고 정맥손상 11예(26%)였다. 수술방법으로 20예(46%)에서 단단문합, 16예(36%)에서 조직이식술, 2예(5%)는 패취 혈관성형술, 5예(12%)에서는 결찰술과 혈전제거술을 시행하였다. 절단율과 사망률은 3예(7%), 4예(9%)였다. 결론: 사지 생존을 높이기 위해서는 허혈 시간을 최소하게 하는 것이 중요한 요소이다. 그러므로 빠른 진단과 치료가 절단율과 사망률을 감소 시킬 수 있다.

하악골 골절의 임상통계학적 연구 (ANALYSIS OF 334 CASE REPORTS OF MANDIBULAR FRACTURE)

  • We retrospectively reviewed 334 inpatients who sustained a total of 518 mandibular fractures and who ewer treated in our department between l980-1990. This results were obtained as follows : 1. In respect of incidence, there were the highest frequency in July, and the lowest frequency in May. The number of patients has not been increased year after year due to competition with other department in our hospital. 2. The age frequency was the highest in the 2nd decade(38.9%) and the ratio of man to women was 4.9 : 1. 3. The most frequent cause of mandibular fracture was traffic accident(43.4%), and the next was fall down(24.3%), fist blow(71%), industrial accident(21%) and others in order. In the traffic accident, autobicycle accident was 14.1%. 4. The most common location of mandibular fracture was symphysis(38.8%), condyle(20.7% ), angle(19.9%) and body(15.1%) were next in order of frequency. The classification by location of fracture, the frequency of single fracture was 54.8%. 5. In 334 patients of mandibular fracture, the frequency of associated injuries was facial laceration(58.4%), teeth injuries(37.7%), extremity injuries(13.2%) were next in order of frequency. 6. The patients arrived in hospital immediately within 24 hours after accident wee 61.4% of all. In respect of treatment, open reduction was 68.7% of all. 7. Complications including infection were present 11.1% of patient. Other complications inclued delayed healing malocclusion, malocclusion and neurologic problem.

Incidence and management of mandibular fractures in a low-resource health facility in Ghana

  • Objectives: The mandible and other parts of the maxillofacial region suffer significant morbid injuries following road traffic accidents. Our study gives epidemiological description of mandibular fractures in Ghana and also evaluates the relevance of closed reduction and indirect fixation for managing mandibular fractures in low-resource health facilities in low-income countries like Ghana. Patients and Methods: This is a retrospective study involving 268 patients who reported to the Department of Oral and Maxillofacial Surgery of the Sunyani Regional Hospital with mandibular fractures from January 2010 to December 2019. Patient medical records were assessed for information on age, sex, fracture etiology, anatomic location of fracture, time of day of road traffic accident, and other associated injuries. Results: A total of 268 patients were included in this study (males, 216 [80.6%]; females, 52 [19.4%]). Motor vehicular accident (MVA) was the leading cause of mandibular fractures (202 injuries, 75.4%). Other etiologies included assault (39, 14.6%), gunshot (13, 4.9%), falls (12, 4.5%), and industrial accidents (2, 0.7%). Of the 161 male cases caused by MVA, 121 (75.2%) occurred at night and in the evening while the remaining 40 (24.8%) occurred in the morning and afternoon. Among all managed 222 patients, 212 (79.1%) were treated with closed reduction and indirect fixation technique while 10 (3.7%) were treated with open reduction and direct fixation. Conclusion: Closed reduction with indirect fixation could successfully be used to manage mandibular fractures in low resourced health facilities, especially in low-income countries. The poor lightening system on roads in Ghana is a major contributory factor to motor vehicular accidents.

Abdominal Impalement Injury Caused by Scaffolding Pipe following a Traffic Accident - A Case Report

  • Traffic accidents involving agricultural machinery have been decreasing, but mortality is still high due to a lack of safety devices such as seat belts. Furthermore, secondary damage, such as abdominal impalement injury caused by loaded materials, is more likely to occur, and this type of injury leads to a poor prognosis. Impalement with pipes is often more fatal than other penetrating injuries because the diameter of the pipe is usually larger in size than other loaded materials. We report a case of a 72-year-old man with secondary abdominal impalement injury caused by a scaffolding pipe following a traffic accident.

  • Modern traffic accidents are a complex occurrence. Various indicators are needed to analyze traffic accidents. Countries that have been investigating traffic accidents for a long time accumulate various data to analyze traffic accidents. The Korean In-Depth Accident Study (KIDAS) database collected damaged vehicles and severity of injury caused by Collision Deformation Classification code (CDC code), Abbreviated Injury Scale (AIS), and Injury Severity Score (ISS). As a result of the investigation, data relating to the injuries of the occupants can be easily obtained, but it was difficult to analyze human severity based on the information of the damaged vehicle. This study suggests a method to measure the speed change at the time of an accident, which is one of the most important indicators in the vehicle crash database, to help advance KIDAS research.

전치부 외상성 손상시 치아고정술 후의 예후에 관한 연구 (A STUDY ON THE PROGNOSIS AFTER STABILIZATION OF THE TRAUMATIZED ANTERIOR TEETH)

  • This study was to evaluate the nature of traumatic injuries on anterior teeth of 530 patients and the prognosis of stabilized cases of 102 patients, including possible 18-follow up cases among them, treated at Dental Hospital, Yonsei Medical Center from Mar. 1983 to Feb. 1988. The obtained results were as follows. 1. The peak of monthly distribution of trauma to the anteriors was June and was the lowest in November. The ratio of male to female was 2.6 : 1. The most common incidence of the trauma to the anteriors was seen in the 3rd decades. The most chiefly involved teeth were central incisors in both upper and lower jaws. The causes of trauma were as followed in descending order by first blows, traffic accidents, falls, slip downs, and sports. The types of traumatic injuries of anterior teeth consisted of periodontal tissue injuries(55.9%), the injuries to the hard dental tissues and pulp(31.3%), and alveolar bone injuries(12.8%). 2. 35.1% stabilized anteriors were treated with root canal therapy. The success rate of stabilization was 95.3%, and 92.3% in replantation of avulsed teeth. 3. External root resorption, internal root resorption with calcification of pulp chamber or root canal, ankylosis of root, and other periapical pathosis were seen in follow-up radiographic findings. 4. Success rate of stabilization of the traumatized anteriors after 3 years showed 71.4% average.

비장 손상의 임상적 치료 결과 (Clinical Outcomes of Splenic Injury)

  • Purpose: The management of splenic injuries has shifted from a splenectomy to splenic preservation owing to immunity. The purpose of this study was to assess the kinds of management and outcomes through a review of our experience with splenic injuries. Methods: We retrospectively reviewed 47 patients with traumatic splenic injuries using by electronic medical records from Jan. 2007 and Dec. 2011. Splenic injuries were classified according to the American Association for the Surgery of Trauma (AAST) grading system. Results: There were 11 falls, 11 traffic accidents, 10 motorcylcle accidents, 10 pedestrian accidents and 5 abdominal blunt traumas. Low-grade injured patients (${\leq}$ Grade III) were 29 of 43(61.7%), and High-grade injured patients (${\geq}$ Grade IV) were 18 of 43(38.3%). In 34 patients, non-surgical treatment was performed, and 14 patients underwent a splenectomy. There were relatively more high-grade in older patients, and the high-grade-injury group showed need for a transfusion (p=0.002), more need for a splenectomy (p<0.001), a longer mean hospital stay (p=0.036), a longer ICU stay (p=0.045) and more combined organ injury (p=0.036). Conclusion: Conservative treatment should be considered in low-grade-injury patients (${\leq}$ Grade III). A Splenectomy was performed on 56% of the patients with Grade IV injuries, so a splenectomy should be considered carefully in such patients. In patients with a grade V injury, we think surgical treatment may be needed.

흉부 손상의 임상적 고찰 (Clinical Evaluation of Chest Trauma)

  • A clinical analysis was performed on 404 cases of the chest trauma who were admitted and treated at department of thoracic and cardiovascular surgery, Chung Ang University, Yong San Hospital during the past 8 years from July 1984 to April 1992. The results were as follows. 1. The sex ratio was 3: 1 with male predominence. 2. The common age groups were 3rd, 4th, 5th and 6th decades. 3. The most common chief complaint was chest pain[58.2%]. 4. Of 404 cases of chest trauma, 50 cases were resulted from penetrating injuries whereas 354 cases were from non penetrating injuries. The most common cause of the non penetrating injuries was traffic accident[234/354, 66.1%] and of the penetrating injuries were stab wound[47/50, 94%]. 5. The left thorax was the preferred site of chest trauma. 6. The range of hospital stay was from less than 1 week to over 6 weeks and the average duration was about 2 weeks. 7. The common chest trauma was rib fracture[51.6%] and others were simple contusion [18.8%], hemothorax[14.6%], hemopneumothorax[14.9%] and pneumothorax[8.7i]. The rib fracture was prevalent between 4th to 9th rib laterally. 8. There were 92 cases of associated injuries which were bone fracture[66/92, 71.7%], head injury[17/92, 18.5%] and abdominal injury[9/92, 9.8%]. 9. The methods of treatment were conservative management[58.6%], closed tho-racostomy[23.3%], open thoracotomy[3.4%] and others. 10. There were 28 cases[6.9%] of complication, such as pneumonia, atelectasis, emp-yema, respiratory failure and others. 11. The overall mortality was 2.5%[10 cases] and causes of death were hypovolemic shock, acute renal failure, adult respiratory distress syndrome, sepsis and multiple organ failure.
