Purpose: The objective of this study was to determine the effects of coronavirus disease 2019 (COVID-19) on the volume of trauma patients, the number of orthopedic trauma operations, and the severity of injuries. We also investigated the correlations between social distancing and these variables. Methods: This was a retrospective review of trauma patient cases at a single focused training center for trauma in Korea from January 2017 to April 2021. The COVID-19 group included patients treated from January 1 to April 30 in 2020 and 2021, and the control group included patients treated during the same months from 2017 to 2019. The volume of trauma patients according to the level of social distancing was evaluated among patients treated from August 2, 2020 to November 23, 2020. Results: The study included 3,032 patients who presented to the emergency department with traumatic injuries from January to April 2017 to 2021. The average number of patients was 646.7 and 546.0 in the control and COVID-19 groups, respectively. The percentage of patients injured in traffic accidents (TAs) decreased from 25.0% to 18.2% (P<0.0001). The proportions of in-car TAs and pedestrian TAs also decreased from 6.7% and 10.8% to 3.5% and 6.0%, respectively (P=0.0002 and P<0.0001). The percentage of bicycle TAs increased from 2.4% to 4.0% (P=0.0128). The proportion of patients with an Injury Severity Score above 15 and the mortality rate did not change significantly. As the level of social distancing increased, the number of trauma patients and the number of trauma injuries from TAs decreased. The number of orthopedic trauma operations also depended on the social distancing level. Conclusions: The number of trauma patients presenting to the emergency department decreased during the COVID-19 period. The volume of trauma patients and orthopedic trauma operations decreased as the social distance level increased.
Objectives This survey study is designed to check the awareness and satisfaction of Korean herbal medicine in patients suffering from acute traffic accidents. Methods From May 1 to 20, 2020, patients who visited the hospital within 7 days after the accident and received prescription of Korean herbal medicine for the first-time symptoms were selected. Survey was conducted 10 days after the first visit to confirm the perception of Korean herbal medicine and expected to be used as a prior study to improve the quality of Korean medicine led by Korean herbal medicine. Results Most patients answered that they were aware of the prescribed Korean herbal medicine's dosage and treatment effects. 65% of the patients said they would like to receive other forms of prescription if possible. Most patients answered that portability is convenient and tastes better than normal. 48% of the respondents said that it was the first to time to take Korean herbal medicine, and 68% said that they were willing to take the Korean herbal medicine later. At the time of the first visit, the health condition assessed using EuroQol visual analogue scale (EQ-VAS) was 46.84, but 10 days after the treatment began, it increased significantly to 61.93. Conclusions The improvement of symptoms and high satisfaction with Korean medicine medical services contributed to the patient's willingness to visit the Korean medicine hospital again and to respond that they were willing to take further medication. EQ-VAS elevation suggests that overall patients' subjective health conditions have improved since treatment.
This study was based on a series of 369 patients with Oral and Maxillofacial injuries treated at Kumi Hospital, College of Medicine, Soon-Chun-Hyang University from Jan. 1992 to Dec. 1994. The results obtained were as follows : 1. The number of male patient was 310 and that of female was 59, leading to 5.3 : 1 of male-female ratio, and the 3rd decade was the highest age group in incidence. 2. Weekly incidence was the highest in Sun. & Sat. and monthly incidence was the highest in Nov.& Oct. 3. Causes as follows : traffic accident 41.9%, slip& fall down 25.4%, human trouble 16.5%, industrial accident 7.5%, sports 6.7%, etc. 4. Site distribution as follows : mandible fracture 32.3%, maxilla fracture 4.8%, zygoma fracture 21.4%, nasal bone fracture 34.1%, orbital& ethmoidal fracture 4.6%. 5. The most common site of mandible was symphysis & angle, and the ratio of OR & CR was 1.3 : 1. 6. The most common site of maxilla was Le Fort 1, 2, and the ratio of OR & CR was 3 : 1. 7. The most common site of zygoma was body, the ratio of OR & CR was 3.3 : 1. 8 . The mean period of intermaxillary fixation was 4.33weeks. 9. Combined injury in facial fracture was 35.8% : The facial fracture were most frequently combind with head & neck(47.0%), upper extremities and abdomen(9.8%). 10. The mean elapsed time from injury to hospital was 1.9days, and that to operation was 5.1days. 11. The mean number of combined teeth injury was 0.6, and percent of combined soft tissue injury of face was 51.3%. 12. Post-operative complication occurred in 4 out of 323 cases. all of that was infection.
Purpose: For trauma patients, an early-transport and an organized process which are not delayed in hospital stage are necessary. Our hospital developed a procedure, the trauma Critical Pathway (CP), through which a traumatic patient has the priority over other patients, which makes the diagnostic and the therapeutic processes faster than they are for other patients. Methods: The records of patients to whom Trauma CP were applied from January 1, 2011 through April 15. 2012. were reviewed. We checked several time intervals from ER visiting to decision of admission-department, to performing first CT, to applying angio-embolization, to starting emergency operation and to discharging from ER. In addition, outcomes such as duration of ICU stay, hospital stay and mortality were checked and analyzed. Results: The trauma CP was applied to a total of 143 patients, of whom, 48 patients were excluded due to pre-hospital death, ER death, transferring to other hospital and not severe injury. Thus 95 patients (male 64, 67.3%) were enrolled in this study. Fifty-nine patients(62.1%) were injured by the traffic accident. The mortality rate was 10.5% and the mean Revised Trauma Score (RTS) of the patients was $6.4{\pm}2.0$. After visiting ER, decision making for admission was completed, on average, in 3 hours 10 seconds. The mean time intervals for the first CT, angio-embolization, surgery and discharge were 1 hour 20 minutes, 5 hours 16 minutes, 7 hours 26 minutes and 6 hours 13 minutes, respectively. Conclusion: The trauma CP did not show the improvement of time interval outcome, as well as mortality rate. However, this test did show that the trauma CP might be able to reduce delays in procedures for managing trauma patients at the university-based hospitals. To find out the benefit of CP protocol, a large scaled data is required.
This is a retrospective study on 452 patients with maxillofacial bone fracture. The patients were treated at the Dept. of Oral and Maxillofacial Surgery, WON KWANG UNIV. HOSPITAL from Aug. 1, 1984 to Sept. 30, 1988. The results were as follows. 1. The facial bone fractures occured most frequently in the twenties (35.3%)and male were predominant(75%) than female. 2. The most frequent etiologic factor was traffic accident. 3. The most common location of facial bone fracture was mandible(57%). Zygomatic bone & arch(39.9%) were most common on middle 1/3 of the face and symphysis(37.1%) were most common on the mandible. 4. The time from injury to treatment was variable from under the 1 week (65%) to over the 1 week(35%). In most case of the patients were treated beyond the 3 days(51.5%). 5. The relationship of intermaxillary fixation period and treatment method was an intimate relationship. The IMF period was reduced by the use of plate and screw osteosynthesis. 6. The weight loss was proportioned to intermaxillary fixation period.
Purpose: The current study seeks to identify the effect of neck muscle strengthening exercise using proprioceptive neuromuscular facilitation (PNF) on the swallowing ability of patients diagnosed with dysphagia due to stroke. Methods: As a single case study, the current research conducted neck muscle strengthening exercise using PNF on the patient with dysphagia for 40 minutes, four times per week for a six-week period. At the same time, typical rehabilitation therapy for dysphagia was provided. This type of therapy included food-swallowing and the relation of the muscles surrounding the neck. The functional dysphagia scale and the penetration-aspiration scale were used to assess swallowing ability. Results: After the therapy, the functional dysphagia scale and the penetration-aspiration scale decreased by 18 points and 3 points, respectively, which proves the effectiveness of this type of therapy for dysphagia. Conclusion: The results of the current study indicate that neck muscle strengthening exercise using PNF reduces penetration-aspiration in patients with dysphagia, and that PNF can be clinically utilized to improve the swallowing ability of dysphagic patients.
Purpose: The aim of the present study was to investigate the effect of a PNF orofacial exercise program on diadochokinesis in subacute stroke patients with dysarthria. Methods: Thirteen subacute stroke inpatients with dysarthria were recruited for the study. All participants were randomly assigned to two groups: an experimental group (n=7) and a control group (n=6). The experimental group performed a PNF orofacial exercise program, and the control group carried out an orofacial self-exercise. Both groups received the treatments 30 minutes per day, 5 days per week, for 4 weeks. The outcome measures were assessed before and after the intervention, including an alternating motion rate (AMR) and sequential motion rate for diadochokinesis. Results: After the intervention, both groups showed significant improvement in diadochokinesis (p<0.05). In the change values for both groups, the experimental group showed a more significant improvement than the control group did in /pə/ of AMR (p<0.05). Conclusion: These findings suggest that PNF orofacial exercise programs may have positive effects on the improvement of diadochokinesis in subacute stroke patients with dysarthria.
Objective: In patients with temporomandibular joint disorders, air insoles are used to investigate functionality and pain changes in the temporomandibular joint when walking in daily life. Intervention: Sixty-five patients with temporomandibular joint disorder were recruited: 34 as a control group who walked more than 7,000 steps a day in daily life, and 31 as an experimental group who were instructed to take at least 7,000 steps every day while wearing their air insoles. Measurements: To determine the effects of air insoles on temporomandibular joint pain, steady-state pain, maximum mouth opening, average pain, and the most severe pain were measured before and after the experiment. In addition, to evaluate functionality, the ability to open the mouth in a comfortable state, pain when opening the mouth, and the point of sound and maximum degree to which the mouth could be opened were evaluated before and after the experiment. Results: Pain, mouth openness, and sound points showed significant differences from the control group after the experiment, and the maximum mouth opening range showed no significant difference. Conclusion: When air insoles were used by patients with temporomandibular joint disorder, the functionality of the temporomandibular joint was improved and pain was decreased.
Purpose: This study aimed to investigate the use of insoles in patients with temporomandibular joint disorders to assess the function and changes in pain when walking in daily life. Methods: Sixty-five patients with temporomandibular joint disorder, were selected, with 34 assigned to the control group and 31 to the experimental group. Conclusion: The application of air insoles to patients with temporomandibular joint disorder confirmed the function of the temporomandibular joint and its positive effects on pain.
Purpose: The coronavirus disease 2019 (COVID-19) pandemic has had major effects worldwide, including sudden and forceful setbacks to the healthcare system. The COVID-19 pandemic has also led to changes in the plastic and reconstructive management of emergency cases, including those due to road traffic accidents. This study analyzed changes in patterns of plastic surgery emergencies and modifications in consultation policies to minimize the exposure of healthcare workers. Methods: Data on plastic surgery emergency calls received from the trauma and emergency department were collected for a period of 2 months before and during lockdown. The data were then analyzed with respect to the cause, mechanism, and site of the injury, as well as other variables. Results: During lockdown, there was a 40.4% overall decrease in the plastic surgery emergency case volume (168 vs. 100). The average daily number of consultations before lockdown was 2.8 as compared to 1.6 during lockdown. Road traffic accidents remained the most common mechanism of injury in both groups (45.8% vs. 39.0%) but decreased in number during the lockdown (77 vs. 39). Household accidents, including burns, were the second most common cause of injury in both phases (7.7% vs. 20.0%), but their proportion increased significantly from 7.7.% to 20.0% in the lockdown phase (P=0.003). The percentage of minor procedures done in the emergency department increased from 53.5% to 72.0% during lockdown (P=0.002). Procedures in the operating room decreased by 73.1% during lockdown (67 vs. 18, P=0.001). Conclusions: The COVID-19 pandemic and lockdown orders in India greatly influenced trends in traumatic emergencies as observed by the plastic surgery team at our tertiary care center. Amidst all the chaos and limitations of the pandemic period, providing safe and prompt care to the patients presenting to the emergency room was our foremost priority.
