여성 성의학의 새로운 지평(地平) (New Spectra in Female Sexology in Korea)

  • The World Association for Sexology (WAS) will adopt a Declaration of Sexual Rights as basic and fundamental human rights August this year in Hong Kong. Two years ago WAS has been developing a Declaration of Sexual Rights in order to further promote sexual health and protect the sexual rights of everyone. Undoubtedly sexuality is one of the most intriguing subjects in the area of human behavior and psychology, and has been so since antiquity. The influence of sexual factors of human on all aspects of the society has been immense. The varieties of marriage, from traditional to the open marriage model are presented in front of our sight. The influences of women's movement in Korea, which has produced sexual, psychological, economic, political, familial and social changes look so small and the changes are not that much in America. But the profound effects of the women's movement affect on Korean society being realized as man's view of woman and woman's view of herself undergo constant change and reevaluation. With increased sexual awareness on the part of the public owing to mass media, for example AH-U-SEONG aired in TV and with increased emphasis on sexual matters in daily life, more and more physicians are being asked to deal with and manage sex-related problems in their daily practice. Yet, despite the obvious need for informed sexual counseling, doctors are often uncomfortable about sexual matters and many physicians have no special expertise in this area. So physicians concerned in sexology especially such as gynecologists, urologists, psychiatrists and endocrinologists must realize well about sex-related knowledge which is needed for counseling the patient as medical consumer and gap between reality and ideality in sexual diagnosis and treatment. Establishment of management system for the people who have sexual problems is strongly needed to promote sexual health and protect the sexual rights of everyone and for the Sexual Rights as basic and fundamental human rights.

다용 한약처방 투여가 흰쥐의 간기능에 미치는 영향 - 십전대보탕, 육미지황탕, 보중익기탕 및 오적산을 중심으로 - (Effects of High Frequency Herbal Medication Administrations on the Liver Functions in Rats - Focusing on Sipjeondaebo-tang, Yukmaijihwang-tang, Bojungikgi-tang, and Ojeoksan -)

  • Background : Traditional herbal medicine is used extensively among the Korean populations, and other Asian countries employ similar therapies as well. In recent years, extensive focus was laid on adulteration of the herbal medicine with liver damage. The use of herbal preparations as remedies for various medical conditions has continuously increased in Korea. Large proportions of Korean patients use herbal medicinal products, folk remedies, and health food. However, studies on the safety of herbal products are conducted on a less than sufficient basis even in the countries like Korea where herbal medicine is being used extensively. Some of the reports on the safety of herbs were done by the doctors of western medicine but lack of knowledge and misclassification led to misunderstandings. Objectives : This study aims to verify the evidences on safety of frequently used 4 herbal medications (Sipjeondaebo-tang, Bojungikgi-tang, Ohjeok-San, Yukmaijihwang-tang) in the lab animal model. Methods : Sprague-Dawley rats was treated by 4 herbal medications during 31 days. After 1 month, we checked body weight, liver weight, and serum enzyme associated with liver function. Results : There is no significant difference in body weight and liver weight after 1 month of administrations. In all experimental groups, no abnormal findings was observed in histotogical research and lab liver Functions test(AST, ALT etc). Conclusions : These four herbal medications, frequently used in oriental medicine clinics and hospitals, are safe from hepatoxic events in the lab animal model.

족부백선의 한의학 논문에 대한 고찰 (Review on Studies of Korean Medicine about Tinea Pedis)

  • Objectives : The purpose of this study is to analyze research trends on tinea pedis in studies of Korean medicine.Methods : We searched papers using NDSL, KISS, RISS and KTKP(Korean Traditional Knowledge Portal). The first search used the keyword "Tinea pedis" in NDSL, KISS, RISS and KTKP. Used searching duration was not specified.Results : Studies found in NDSL, KISS and RISS were 122 and 118 studies were excluded. Studies found in KTKP were five papers and four studies of them were excluded. Finally five studies were selected and analyzed. Two studies of five selected ones were experimental researches and three studies were clinical researches. Among 2 researches of experimental researches, one of them was about antifungal efficacy of herbal medicines and ethahol extract of the mixture of Sophorae Subprostratae Radix, Aconiti Radix and Hibisci Syriaci Cortex and hot water extract of Phellodendri Cortex were effective. The other was about antifungal effect of the medicinal herb extraction method and vinegar extract was effective. Among 3 researches of clinical researches, there were one clinical study and two case studies. Functional soap containing herbal medicines and bee venom therapy were effective.Conclusions : As we looked for five researches, which were two experimental studies, one clinical study and two case studies. It is possible to treat tinea pedis with korean medical approach by conclusions of 5 researches. We expect that further researches will be proceeded and following results can be actively used as clinical treatments.

한국미션건축에 있어서 장로교 소속 개척선교사들의 건축활동에 관한 사적 고찰 (A Historical Study on the Activities of the Presbyterian Pioneer Missionaries in Korea Protestant Mission Architecture)

  • The objective of this study is to clarify the early state of modern architecture, specifically mission architecture, with the introduction of the Christianity, and for the goal it deals with the architectural activities of pioneer missionaries. Especially it is a piece in a series of those researches after on Graham Lee, and it is focused on the other representative missionary, R. M. Wilson, A. M. Sharrocks, and C. A. Clark. Although coming in the capacity of a medical doctor or an educator, they should play all kinds of roles, such as carpenters, educators, and officers. For this mission, even they didn't have a professional knowledge for construction, they should carried on to establish mission compounds, only with their experience and informations. In this process they needed to be assisted by Korean carpenters, surely. The architectures born by them-selves are designed with a unique eclectic style compounded with those of Korean traditional and occidental. That is to say, the pioneer missionaries made a profound influence to create a new tradition of modern architecture in Korea, which is called as Korean-occidental eclectic style in the period of transition. Moreover, some pioneer missionary, such as Sharrocks, took a part in education of architectural skills. The educated carpenters seemed to be called from outside, and the unique styles could get spread away. On this point this paper has the meaning to clarify historically the continuous development of the Korean architecture through the survey on their activities, on the contrary of the existed recognition that Korean modern architecture has been evaluated to be imported from outside; therefore having a crevice with the its own architectural tradition.

Development of HPTLC Fingerprinting and Phytochemical Study of a Polyherbal Unani Formulation

  • Plants produce a wide range of active principles, making them a rich source of different types of medicines. Without any knowledge of the phytoconstituents or active principles the medicinal plants itself or in the form of polyherbal formulations, were used by all society of human being from ancient times for prevention and cure of disease, but most of the traditional formulations including the formulation of Ayurveda and Unani have not been scientifically validated in order to establish the pharmacopoeial standards to improve the efficacy. Globally, the people become conscious that uses of synthetic drugs for a long period is not safe; the trend of medical society at large is looking at alternatives from natural, safe sources to combat diseases. Due to this comprehension, it has been increased the demand for plant-derived medicine, and on the other side, it is extremely important to standardize the polyherbal formulations and validate them scientifically to improve their safety and efficacy. The polyherbal Unani formulation Safuf-e-Muallif is widely used and recommended in Unani system of medicine (USM) as a spermatogenic agent, semen thickening agent, etc. to treat sexual disorders viz. premature ejaculation, nocturnal emission, etc. The study mainly deals with phytochemical screening for the detection of nature of phytoconstituents and analytical technique like High-performance thin-layer chromatography (HPTLC) for developing fingerprint of Safuf-e-Muallif revealing specific identities of the drug. The phytochemical screening and HPTLC fingerprint profile for SM reported here may be used as a reference standard for quality control purpose in future.

파킨슨병의 한약 치료에 대한 최신 임상연구 동향 고찰 - 2010년부터 2014년까지 중국 논문을 중심으로 - (Review of Current Clinical Studies for Herbal Medicine of Parkinson’s Disease in Traditional Chinese Medicine)

  • The objective of this study was to review the current clinical studies about the effect of herbal medicine for Parkinson's disease in China over the last 5 years and then to suggest the foundation for treatment and further studies. We had searched for studies in China National Knowledge Infrastructure(CNKI, http://www.cnki.net) and PubMed from January 2010 to December 2014. Key words were the various combinations of '帕金森', '湯', '丸', '散', '中醫藥', 'Parkinson's disease', and 'herbal medicine'. Total 53 clinical studies were selected and analyzed. The most frequently used diagnostic criteria of Parkinson's disease was the Unified Parkinson's Disease Rating Scale(UPDRS). The most frequently used medical herb was Paeoniae Radix alba(白芍藥) more 30 times and the highest amount was Astragali radix(黃芪) 100g per day. The most frequent syndrome differentiation was liver kidney yin deficiency(肝腎陰 虛). We found out there are many clinical studies of herbal medicine for Parkinson's disease in China. These studies would be able to provide the basis of clinical research on Parkinson's disease, and also applied to the treatment of Parkinson's disease in Korea.

Antibacterial potential of the extracts derived from leaves and in vitro raised calli of medicinal plants Pterocarpus marsupium Roxb., Clitoria ternatea L., and Sanseveiria cylindrica Bojer ex Hook

  • Pterocarpus marsupium, Clitoria ternatea, and Sanseveiria cylindrica are some of the important and endangered medicinal plant species of India. Despite of medicinal properties, antibacterial potential of the plants have not yet been explored. The present study was designed to optimize the in vitro technique for micropropagation and to screen the extracts from leaves and in vitro raised calli for antibacterial properties. Excised leaf-explants from the parent plants were surface sterilized and cultivated on Murashige & Skoog's (MS) medium containing $N^6$-benzyladenine (BA) in concentrations of 1, 2, 5, and $10{\mu}M$. Optimal growth of calli was noticed at a concentration of $5{\mu}M$, therefore the extracts from calli grown at this concentration were further studied for antibacterial activity. Both alcoholic and aqueous extracts from leaves of respective plants, and their in vitro raised calli were tested for antibacterial activity by agar well diffusion method against a range of Gram-positive and Gram-negative bacteria. Aqueous extracts showed antibacterial activity against limited number of bacterial species; notably the extracts of C. ternatea which showed antibacterial activity against Streptococcus pyogenes, Bacillus subtilis and Bacillus cereus. Alcoholic extracts of all three plants showed antibacterial activity against a wider range of bacteria. Among the Gram-positive bacteria, extracts from C. ternatea showed strong antibacterial activity against Bacillus spp., whereas the extracts of S. cylindrica showed good antibacterial potential for Staphylococcus aureus, S. epidermidis and S. pyogenes. The extracts from all three plants showed antibacterial activity against Gram-negative bacteria, including, Salmonella spp. and Shigella dysenteriae; organisms causing enteric fever and dysentery. In most of the cases, the extracts from respective calli showed comparable, and in some cases better, result in comparison to the extracts from parent leaves. To the best of our knowledge this is the first preliminary report on antibacterial potential, especially through calli extracts, of these plants; and in vitro cultivation of the explants may be used to obtain phytotherapeutic compounds.

코로나-19 무증상 감염 환자의 한약 치료 방안에 대한 고찰 (A review on the treatment of Herbal Medicine for asymptomatically infected COVID-19 patients)

  • Objectives: Objective: The purpose of this study is to investigate the treatment of herbal medicine for patients with COVID-19 asymptomatic infections. Method: In English, search engines such as 'PubMed', 'Science Direct', and 'Cumulative Index to Nursing and Allied Health Literature (CINAHL)' were used. In Chinese, search engines such as 'China National Knowledge Infrastructure (CNKI)' and Wanfang were used. The keywords for the search engines were 'COVID-19', 'asymptomatic infection', 'Korean Medicine', 'traditional Chinese medicine', 'herbal medicine', and etc. Only clinical studies using herbal medicine for patients without fever or respiratory symptoms were selected. We excluded the cases that do not fit the research topic. Results: A total of 5 studies were finally selected. Among them, four of them used integrated herbal medicine and Western medicine, and one of the studies treated exclusively for herbal medicine. There were a total of seven prescriptions for herbal medicine used in the study. Outcome variables were used as following: lab test, nucleic acid conversion time, hospitalization period, chest CT, and etc. In the RCT study, herbal medicine and Western medicine decreased nucleic acid conversion time, average hospitalization time compared to the control group, but it was not statistically significant. No other adverse reactions were reported in all studies. Conclusion: According to the results, integrated herbal medicine and Western medicine might be an effective treatment for patients with COVID-19 asymptomatic infection reducing hospitalization period, time of nucleic acid turning negative. No severe adverse effects were reported. However, it is thought that better-designed research will be needed in the future.

  • This study introduces and experimentally validates a novel approach that combines Instruction fine-tuning and Low-Rank Adaptation (LoRA) fine-tuning to optimize the performance of Large Language Models (LLMs). These models have become revolutionary tools in natural language processing, showing remarkable performance across diverse application areas. However, optimizing their performance for specific domains necessitates fine-tuning of the base models (FMs), which is often limited by challenges such as data complexity and resource costs. The proposed approach aims to overcome these limitations by enhancing the performance of LLMs, particularly in the analysis precision and efficiency of national Research and Development (R&D) data. The study provides theoretical foundations and technical implementations of Instruction fine-tuning and LoRA fine-tuning. Through rigorous experimental validation, it is demonstrated that the proposed method significantly improves the precision and efficiency of data analysis, outperforming traditional fine-tuning methods. This enhancement is not only beneficial for national R&D data but also suggests potential applicability in various other data-centric domains, such as medical data analysis, financial forecasting, and educational assessments. The findings highlight the method's broad utility and significant contribution to advancing data analysis techniques in specialized knowledge domains, offering new possibilities for leveraging LLMs in complex and resource-intensive tasks. This research underscores the transformative potential of combining Instruction fine-tuning with LoRA fine-tuning to achieve superior performance in diverse applications, paving the way for more efficient and effective utilization of LLMs in both academic and industrial settings.

A Randomized trial to investigate the effect of Asparagus racemosus root tablet in Relieving Postmenopausal Hot Flashes

  • Background and Objectives: As per the available data, 75% peri and postmenopausal women experience hot flashes having negative effect on their quality of life. Using herbal medicines are stepping forward to alleviate hot flashes as the available hormonal medications have been reported the presence of side effects. This study was planned with the intention to investigate the effect of Asparagus racemosus root in the treatment of postmenopausal hot flashes in comparison to the placebo. Methods: This prospective patient blinded clinical trial randomly assigned 40 postmenopausal women with symptoms of hot flashes to receive either test drug (n=20) comprising Asparagus racemosus tablet of 1g in the dosage of 2 tablet thrice in a day for the duration of 60 days continuously or to receive placebo tablet prepared from roasted wheat flour in the same dosage and duration as test drug. The efficacy was determined by reduction in the frequency and intensity of hot flashes assessed by daily diary along with the improvement in quality of life assessed through Hot Flash-Related Daily Interference Scale. Statistical analysis was accomplished by student t test and Chi-square/Fisher Exact test. Results: Test drug significantly (p <0.001) reduced the intensity and frequency of hot flashes as well as improved the quality of life without showing any side effect. Discussion & Conclusion: According to Unani scriptures, the treatment is to treat the underlying cause, such as aberrant temperament and psychological and environmental issues. Unani drugs have emmenagogue, anti-inflammatory, analgesic, and neuroprotective qualities that can help alleviate premenstrual symptoms. Unani herbs such as C. sativus, Vitex, agnus castus, P. vulgare, N. Jatamansi, M. officinalis, and Z. officinalis have been clinically demonstrated to be effective in PMS. Thus, traditional knowledge authentication and conservation are vital for future research and appreciated for application in the modern day. Furthermore, randomised controlled trials, comprehensive reviews, and meta-analyses are suggested.
