보완대체의학의 정의 변화와 국내 관련 연구 분석 (Changing Definitions of Complementary and Alternative Medicine and Related Research Analysis in Korea)

  • Objective : There is a growing interest on Complementary and Alternative Medicine(CAM). But the term 'CAM' often causes confusion, for the perception of CAM is determined by how the relationship between CAM and Conventional Medicine is established. Methods : In this paper, we researched the concept of CAM through the change in the terminology and definition of CAM throughout history. Furthermore, we also analyzed related papers in Korea. Results : International studies show that the relationship between Conventional Medicine and CAM has become more cooperative. Meanwhile, in Korea, Korean Medicine is categorized as a type of Conventional Medicine, not CAM. Conclusion : Though Korean Medicine is included in Conventional Medicine in Korea, the research methodology is still restricted in ways of bio-medical science. Further study on the original research method of Korean Medicine is needed.

요통에 대한 매선 임상연구의 중국 현황 분석 - 중국 내(內) 출판 저널을 중심으로 (Research Trends on the Thread Embedding Therapy of Low back pain in Traditional Chinese Medicine - Focusing on published articles in China)

  • About 60% to 90% of a total population experience low back pain at least once of life, and about 35% to 79% among them experience a recurrent and chronic low back pain. thread-embedding therapy is mainly used to improve appearance or treat obesity in early stage, but recently it is also used to treat musculoskeletal pain. This study aimed to search Chinese study using thread-embedding therapy on low back pain and to analyse their methodology. Three Chinese database(CNKI(www.cnki.net), WANFANG(www.wanfangdata.com), WEIPU(www.cqvip.com)) were searched for clinical study of thread-embedding therapy up to March 2016. The characteristics of included studies and regimen of thread-embedding in those studies were analyzed. The total 21 studies (4 case studies, 16 non-randomized controlled trials, 1 randomized controlled trial) were included. All studies on thread embedding treatment of low back pain reported that its effectiveness was very good. The most frequently used acupoints was Ashi acupoints and acupoints on bladder meridian(BL) or governor vessel(GV). Thread-embedding therapy is considered very useful for low back pain in Traditional Chinese medicine. Further studies are needed to investigate the effect of thread-embedding therapy and to expand its application. This study is limited in that the literature search in the Chinese database were restricted.

원발성 월경곤란증에 대한 침구치료 관련 무작위배정 임상시험의 체계적 고찰: 한국 문헌을 중심으로 (Acupuncture and Moxibustion for Primary Dysmenorrhea in Korean Literatures: A Systematic Review of Randomized Controlled Trials)

  • Objectives : Primary dysmenorrhea is one of the most common female gynecological diseases. Acupuncture and moxibustion therapy have been used to treat dysmenorrhea in Korea. The aim of this review was to examine the effectiveness of acupuncture and moxibustion therapy for primary dysmenorrhea as described in studies in Korea. Methods : A total of 8 databases were searched, with the search concluding February 15, 2015. These were the Oriental Medicine Advanced Searching Integrated System, DBpia, Korean Studies Information Service System, National Digital Science Library, Korean Traditional Knowledge Portal, Research Information Sharing Service, and Pubmed. Randomized controlled Trails(RCTs) comparing acupuncture or moxibustion therapy with non acupoints stimulation or medication were selected. Data abstraction and assessment of methodology was conducted by authors and disagreements were resolved by discussion. Results : 7 trials were included in this review, with a total of 308 participants. 4 trials reported on acupuncture, 1 trial reported on acupress by magnet, 1 trial reported on pharmacopuncture, and the other trial reported on moxibustion. Quality of methodology was low. 2 trials showed that experimental therapy was effective for pain relief compared to the controlled group. However, 5 trials did not show a significant difference in pain relief. Conclusions : Acupuncture and moxibustion therapy may reduce period pain, however, it is needed for well designed RCTs in Korea.

현대 한의학의 이해 - 한의학의 정체성 문제 고찰을 위한 예비 연구 (Understanding Current Traditional Korean Medicine - Preliminary Study for Discussion on the Identity Issue of TKM)

  • This is a preliminary study for examining the identity issue of the current Traditional Korean Medicine(TKM). In order to examine the identity issue, it is necessary to understand "what" current TKM is. The current TKM has been formed in the complicated historical settings and the newly formed academic geography of modern times, completely different from those of the traditional era. This paper took diachronic and synchronic approaches in order to understand the current TKM. In the process of modernization and scientization of the TKM-which had begun in the early twentieth century-the western medical knowledge merged into the TKM. Also, as the College education of the TKM began after the Liberation from the Japanese colonial rule, the TKM scholars accepted the western medical department system to develop the TKM into professional knowledge. Meanwhile, since the late 1970s, the TKM has been influenced by the modern Traditional Chinese Medicine (TCM), and the TCM knowledge also merged into the TKM. And recently, the research methodology of the complementary and alternative medicine (CAM), and the clinical research technique of the western medicine-like Evidence based Medicine (EBM)-are affecting the scientization and clinical study of the TKM. The current TKM has been formed as a result of the hybridization of these different knowledges. These changes in the TKM were inevitable for the survival of it in the fast changing world. But these changes brought with them problems. Among those, the identity issue is the most important and crucial.

인체에서 방사되는 생물광자(生物光子)에 대한 소고 (Review of Biophoton of Human Beings - Domestic Papers)

  • Objectives : The purpose of this study is to review research papers published by Korean research groups on biophoton of human beings based on experiment subject and methodology. Methods : In order to review human biophoton research executed in Korea, all the papers published in Korean and international journals by Korean research groups were selected. Several key foreign papers were also reviewed for clarification of this study. Based on experiment subjects, experiment methodology, and interpretation of experiment results were analyzed. On each experiment, its original interpretation was directly quoted. Issues on the experiment methodology and interpretation were expressed at the end of each subject. Results and Conclusions : We found that experiments on human biophoton were compelling. However it seemed that more experiments, especially on their sample sizes, are needed to demonstrate its clinical application. Interpretations based on Korean traditional medicine also need to be elaborated more. In order to do accomplish clinical application of biophoton, interdisciplinary works are required. Some suggestions on biophoton experiments were made.

한국전통사상으로 살펴본 양생기공에 대한 고찰 (Literature for YangsaengKigong)

  • Although the demand on 'Ki' training because of the increased interest in health, there has not been an established theory and system in 'Ki' training from the perspective of oriental medicine. Therefore, many training associations are in operation without sound basis. The general use of the terminology 'Kigong' is the influence of China. As it was distributed to the general public through the translation and introduction of Chinese Kigong theory, the genuine Korean YangsaengKigong was not emphasized but it was included in or considered as the second-class to the Chinese Kigong. The purpose of the study was to establish and review YangsaengKigong based on our traditional ideologies. Korean Yangsaengkigong can be defined as the training method nurturing mind, energy, body based on the Sambeob training method such as Pause of thinking, Modulation of breath and Prohibition of sensibility with the goal of Yangsaengkigong, good health and long life and the maximum service for the benefit of humanity. Although this study can identify the differecnes between Korean Yangseangkigong and Chinese kigong through Korean traditional ideology, we could not develop this study to the necessity of ideological background of medical Kigong and concrete methodology of Yangsaengkigong and our traditional ideology because of scarce literature on Korea and Korean traditional ideologies. For the purspose of the study, we need cooperation between Studies of Korea, Studies of National History, and Oriental Medicine.

자궁경부 이형증과 관련한 한의약 연구 고찰 - 자궁경부암 및 자궁경부 이형증 논문을 중심으로 - (A Review on Korean Traditional Medicine Research against Cervical Dysplasia - Focused on Studies of Cervical Cancer and Cervical Dysplasia-)

  • Purpose: This review is to figure out evidence that suggest effectiveness of Korean Medicine treatments against cervical dysplasia. Methods: Studies on cervical cancer and cervical dysplasia were searched through 6 databases: Korean Studies Information Service System(KISS), National Discovery for Science Leaders (NDSL), Korea Institute of Science and Technology Information (Korean TK), Oriental Medicine Advanced Searching Integrated System (OASIS), the Journal of Korean Medicine, and the Journal of Korean Obstetrics & Gynecology. After that, the articles were extracted with reference point of Korean Traditional Medicine. Results: 37 articles were included lastly according to selection criteria. 3 of them were case reports on cervical dysplasia, and 34 were in-vitro studies on Human Papilloma Virus (HPV) positive cancer cell. In case reports, acupuncture, moxibustion, medical herbs and pharmacoacupuncture were used for treatments of cervical dysplasia with about 3 months. Experimental studies on cervical cancer cell showed that several herbs function with clear heat, eliminate stasis (淸熱解毒, 化瘀消腫) have anti-cancer effects inducing apoptosis. Conclusions: The results of articles are not enough to use in practice. Therefore, we indicates more advanced research methodology as follows: development of Korean Medicine treatment protocol with oral and external, in-vivo experimental study, and evaluation immunity index.

화병 진료지침 개발을 위한 한약 임상시험 방법론 연구 (The Systematic Review of Clincal Trial(s) Intervening Traditional Korean Herbal Medicine: Extension of Developing Clinical Guideline for Hwa-byung)

  • Objectives : To address most probable and suitable method for designing clinical trial intervening Traditional Korean Herbal Medicine on hwa-byung. Study Design : A systematic review of research studies of complementary and/or alternative medical(CAM) treatment of depression, and of domestic clinical trials of Traditional Korean Medicine, and of Chinese clinical trials of Traditional Chinese Medicine. Methods : Randomized, controlled trials(RCTs) of treatment of depression intervening herbal medicine were searched through MEDLINE, Cochrane Library, and CNKI databases. Also, domestic RCTs intervening Traditional Korean Herbal Medicine were searched through Korean Traditional Knowledge Portal and Korean studies Information Service System(KISS). Studies were evaluated using Iadad scale and self-designed tool for this study. Results : Thirty four RCT studies( 10 from MEDLINE, 16 from CNKI, 8 domestic studies) of herbal medicine met inclusion criteria. Mean Jadad score of studies published in English was 2.8${\pm}$0.79, in Chinese 1.94${\pm}$0.77, and in Korean 2.75${\pm}$0.71. Twenty one percent of studies included pattern differentiation in their inclusion criteria. Twenty nine percent of studies used combined treatment of herbal and conventional medicine. Among studies on depression, 9% included Complementary Medical assessing tools. Conclusions : There is shortage of domestic clinical trial involving herbal medicine. In China, studies tend to focus on investigating effect of Combined treatment of herbal and conventional medicine on depression. Clinical trial(s) of hwa-byung should provide good internal validity by describing methodology for randomization, double-blinding, and attrition. Also, specific guideline for clinical trial, including Traditional Korean Medical aspects across inclusion criteria, and assessing tools is needed.

  • The theory of Hwa Acupuncture Therapy (HAT), called Hwa Chim Therapy, is a part of the Five Elements Theory unique to Korea. HAT, created and developed by Jaehoon Song, integrates the victor-vanquished as well as the son-mother relationship of the Five Elements of breakdown and restoration of balance between yin and yang. And also, it provides resources and data on The seventy fifth Difficulty(75難) of Classic on Difficulty - Nan Jin 75. HAT establishes objectiveness and accuracy of diagnosis based upon the traditional method and procedure of pulse taking. In HAT, a person's state of illness is diagnosed by applying the comparative examination of the palpitation of the pulse. It is fact that the pulse varies according to the state, and that HAT treatment has proven the positive results by using the victor-vanquished relationship of Classic on Difficulty-Nan Jin 75. Despite a concise and simple theory, Hwa Chim is very effective in treating a wide range of illnesses, and thus it has gained an increasing attention of many scholars and practitioners in the field of traditional Korean oriental medicine. However, it is the first theoretical attempt to the clinical research and scientific methodology of Ohaeng Hwa Chim, and more active Hwa Acupuncture R&D is being conducted nationwide.

스위스에서의 국민투표에 의한 보완의학 건강보험 급여화 사례 연구 (A case study on benefit coverage of complementary medicine in public health insurance by the referendum in Switzerland)

  • Background : Efforts towards increasing insurance coverage for traditional Korean medicine (TKM) are being continued. However, various difficulties are faced in generating evidence for TKM due to limited financial support and the low quality of research methodology. Objectives : The objectives of this study were to review the Swiss evaluation program for complementary and alternative medicine (CAM) and assess the expansion in public health insurance coverage of complementary medicine as approved by referendum in Switzerland. Methods : The regulations of CAM in the European Union were assessed. Research articles, reports, government publications and websites which deal with the 'Programm Evaluation $Komplement{\ddot{a}}rmedizin$ (PEK)' and the referendum in Switzerland were searched for and analyzed. Results : The PEK was conducted from 1998 to 2005. The PEK evaluated the efficacy, utilization and cost-effectiveness of anthroposophical medicine, homeopathy, neural therapy, phytotherapy and traditional Chinese medicine. However, clear conclusions could not be drawn from the evaluation according to the PEK Report. Later, a referendum was implemented in which 5 therapies would be added to the Switzerland Constitution with the support of the public. The coverage of CAM was approved by Swiss a plebiscite with an approval rate of 67.0%. Conclusions : The reason for the successful referendum is suggested to be public support and the solidarity with CAM experts and politicians. It may be surmised that recognition of the political efforts and scientific aspects required to expand insurance coverage of TKM, and towards obtaining public support, is necessary.
