중국 연변 조선족자치주 성인과 한국 구리지역 성인의 식생활 양식 비교 (Comparative Study on Dietary Patterns of Korean - Chinese and Koreans)

  • This study was conducted to investigate dietary patterns and assimilation of dietary culture of Korean descendants in Yanbian, China compared to Korean. A dietary survey was conducted using one 24-hour recall method from 730 adults over 30 years of age in Yanbian, China and 695 adults over 25 years of age in Kuri, Korea. The average number of food items consumed per day was 14 in Yanbian and 20 in Kuri and there was a significant difference between the two regions(p<0.001). The foods consumed most frequently were rice, salt, the M.S.G. in Yanbian and rice, green onion and garlic in Kuri. The foods consumed most by amount were rice, cucumber, Soju in Yanbian and rice, Kimchi and grape in Kuri. The average number of dishes consumed per day was 6.4 in Yanbian, 9.4 in Kuri and the difference between the two regions was significant(p<0.001). Total number of dishes appeared in the survey was 253 in Yanbian and 494 in Kuri. The dishes consumed most frequently were cooked-rice, beverages, Kimchi, stir-fried eggplant in Yanbian and cooked-rice, Kimchi, coffee, soybean paste stew, in Kuri. The dish groups appeared most frequently were rice, stir-fried foods, vegetables and Kimchi in Yanbian and rice, Kimchi, beverages and fruits in Kuri. Male subjects in Yanbian Consumed alcohol frequently while adults in Kuri consumed coffee more frequently than Yanbian. The major meal patterns were rice+side dish, rice+soup, rice+side dish+Kimchi in Yanbian and rice+soup+side dish+Kimchi, rice+side dish+Kimchi, rice+soup+Kimchi in Kuri. Stir-fried foods, which were consumed 46.6% of all the meals, were dominant side dish in Yanbian, but stir-fried food, seasoned vegetables and grilled food appeared aver 10% in Kuri. There results show that variety of diet of Korean-Chinese in Yanbian was lower than Kuri. Korean-Chinese tend to keep traditional Korean dietary patterns of consuming rice as staple but were adapted to Chinese dietary patterns of consuming stir-fried side dishes. Efforts should be directed toward preserving Korean traditional patterns of dietary culture among Korean-Chinese population.

섬수 추출물에 의한 T24 인체 방광암세포의 증식억제에 관한 연구 (Growth Arrest by Bufonis Venenum is Associated with Inhibition of Cdc2 and Cdc25C, and Induction of p21WAF1/CIP1 in T24 Human Bladder Carcinoma Cells)

  • Bufonis venenum (dried toad venom; Chinese name, Chan su) is a traditional Chinese medicine obtained from the skin venom gland of the toad. It has long been used in treating arrhythmia and other heart diseases in China and other Asian countries. Additionally, Bufonis venenum has been reported to selectively inhibit the growth of various lines of human cancer cells. In the present study, it was examined the effects of extract of Bufonis venenum (EBV) on the growth of human bladder carcinoma cell line T24 in order to investigate the anti-proliferative mechanism and induction of apoptosis by EBV. Treatment of T24 cells to EBV resulted in the growth inhibition, morphological change and induction of apoptotic cell death in a dose-dependent manner. Flow cytometric analysis revealed that EBV treatment caused G2/M phase arrest of the cell cycle and down-regulation of cyclin A, cyclin B1 and Cdc2, which was associated with a marked up-regulation of cyclin-dependent kinases (Cdks) inhibitor p21 (WAF1/CIP1) in a p53-independent manner. The Cdc25C expression was also significantly inhibited by EBV treatment, however Wee1 kinase expression was not affected. The induction of apoptotic cell death by EBV was connected with down-regulation of anti-apoptotic Bcl-XS/L expression without alteration pro-apoptotic Bax expression. Taken together, these findings suggest that EBV may be a potential chemotherapeutic agent for the control of human bladder carcinorma cells and further studies will be needed to identify the active compounds that confer the anti-cancer activity of EBV.

The anti-hypertensive effect of ginseng in patients with mild hypertension

  • Ginseng has been traditionally used to recover vital energy from Qi deficiency in oriental countries. Recent reports suggested that ginseng could regulate blood pressure (BP), but much controversy still remain. Therefore, we intended to assess the anti-hypertensive effect of some ginseng species on Koreans and Chinese. This is a randomized, double blinded controlled clinical trial. The study subjects were recruited from the mild hypertensive patients who belonged prehypertension(120/80 to 139/89 mmHg) and stage I hypertension (140/90 to 159/99 mmHg) in Korea and China. After assigning the subjects into a Korean, a Chinese, and an American ginseng group by randomization, we prescribed ginseng with the dose of 4.5 g per a day for 4 w. To assess the anti-hypertensive effect, we compared the mean of systolic and diastolic BP between before and after ginseng medication by 24 h Ambulatory Blood Pressure Monitor (24 h ABPM). We also monitored adverse effect and laboratory findings to secure the subjects' safety. There were 64 subjects treated with Korean ginseng, 58 treated with Chinese ginseng, and 64 treated with American ginseng. All of the ginseng species reduced subjects' BP. Especially, Korean and Chinese ginseng showed more excellent effects. The secondary analysis on the subjects' nationality revealed that all of the ginseng species showed more significant anti-hypertensive effect in Chinese than in Koreans. We suggest ginseng could be useful for mild hypertension regardless of its species. And it would be safe within the dosage of 4.5 g per a day.

Abiraterone for Treatment of Metastatic Castration-resistant Prostate Cancer: a Systematic Review and Meta-analysis

  • Introduction: Although most prostate cancers initially respond to castration with luteinizing hormonereleasing analogues or bilateral orchiectomy, progression eventually occurs. Based on the exciting results of several randomized controlled trials (RCTs), it seems that patients with metastatic castration-resistant prostate cancer (mCRPC) might benefit more from treatment withabiraterone. Therefore we conducted a systematic review to evaluate the efficacy and toxicity of abiraterone in the treatment of mCRPC. Methods: Literature was searched from Embase, PubMed, Web of Science, and Cochrane Library up to July, 2013. Quality of the study was evaluated according to the Cochrane's risk of bias of randomized controlled trial (RCT) tool, then the Grading of Recommendations Assessment, Development and Evaluation (GRADE) System was used to rate the level of evidence. Stata 12.0 was used for statistical analysis. Summary data from RCTs comparing abiraterone plus prednisone versus placebo plus prednisone for mCRPC were meta-analyzed. Pooled hazard ratios (HRs) for overall survival (OS), radiographic progression-free survival (RPFS) and time to PSA progression (TTPP); Pooled risk ratios (RR) for PSA response rate, objective response rate and adverse event were calculated. Results: Ten trials were included in the systematic review; Data of 2,283 patients (1,343 abiraterone; 940 placebo) from two phase 3 trials: COU-AA-301 and COU-AA-302 were meta-analyzed. Compared with placebo, abiraterone significantly prolonged OS (HR, 0.74; 95% confidence interval [CI], 0.66 to 0.84), RPFS (HR, 0.59; 95% CI, 0.48 to 0.74) and time to PSA progression (HR, 0.55; 95% CI, 0.43 to 0.70); it also significantly increased PSA response rate (RR, 3.63; 95% CI, 1.72 to 7.65) and objective response rate (RR, 3.05; 95% CI, 1.51 to 6.15). This meta-analysis suggested that the adverse events caused by abiraterone are acceptable and can be controlled. Conclutios: Abiraterone significantly prolonged OS, RPFS and time to progression patients with mCRPC, regardless of prior chemotherapy or whether chemotherapy-na$\ddot{i}$ve, and no unexpected toxicity was evident. Abiraterone can serve as a new standard therapy for mCRPC.

후박나무에서 분리한 Meso-dihydroguaiaretic acid의 tyrosinase 저해활성 (Tyrosinase Inhibitory Activities of Meso-dihydroguaiaretic Acid from Machilus thunbergii)

  • 후박나무(녹나무과)는 한국과 일본 등지에 서식하는 상록 교목으로 한국, 중국, 일본에서 부종, 복통, 복부 팽만 등의 질병 치료를 위해 오랫동안 사용되어오고 있다. 본 연구에서는 후박나무 껍질을 메탄올에 추출하고 메탄올 추출물을 헥산, 클로로포름, 부탄올에 순차적으로 분획하였다. 클로로포름 분획물로부터 2종의 화합물을 분리하였으며 분리된 화합물1과 2의 구조는 1H-, 13C-NMR과 참고 문헌 데이터에 의해 dibenzylbutane lignin 화합물인 macelignan (1)과 meso-dihydroguaiaretic acid (2)로 동정되었다. 분리된 화합물들의 tyrosinase 저해 활성을 측정한 결과, 화합물 2는 tyrosinase 저해 활성 중 monophenolase (IC50 = 10.2 μM)에 대해 높은 저해활성을 나타내는 경쟁적 저해제였으며 효소에 결합하는 화합물 2의 저해 상수(Ki 값)는 4.8 μM였다. 따라서 meso-dihydroguaiaretic acid (2)는 멜라닌 생합성과 관련된 피부 질환 치료를 위한 잠재적 후보가 될 수 있을 것으로 판단된다.

골다공증 유발 동물모델에서 녹용 추출물의 경구 투여 효과: 체계적 문헌고찰 및 메타분석 (The Effects of Oral Administration of Deer Antler Extracts on an Osteoporosis-induced Animal Model: A Systematic Review and Meta-analysis)

  • Objectives This study aimed to assess the effects of oral administration of deer antler extracts on an osteoporosis-induced animal model. We analyzed the results of using deer antler single extracts on animal models with osteoporosis through a systematic review and meta-analysis. Methods We included osteoporosis studies in animal experiments that administrated deer antler extracts orally. We searched the following 13 databases without a language restriction: PubMed, EMBASE, Cochrane Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), China National Knowledge Infrastructure (CNKI), Wanfang, Korean Medical Database (KMbase), National Digital Science Library (NDSL), Korean Traditional Knowledge (Koreantk), Oriental Medicine Advanced Searching Integrated System (OASIS), Research Information Sharing Service (RISS), Korea Institute of Science and Technology Information (KISTI), and Koreanstudies Information Service System (KISS). We used Systematic Review Centre for Laboratory Animal Experimentation's risk of bias tool for assessing the methodological quality of the included studies. Results A total of 299 potentially relevant studies were searched and 11 were included for a systematic review. Nine studies used a single deer antler extract. A study compared the effects of single extracts of deer antler and antler glue, while another study compared the effects of three single extracts of deer antler, old antler, and antler glue. For evaluating the intervention effect, bone mineral density (BMD) was measured as the primary outcome, while the histomorphometric indicators of the bone and serum alkaline phosphatase and osteocalcin levels were used as the secondary outcome variables. On conducting a meta-analysis of studies on single deer antler extract, BMD was observed to be significantly increased compared to that in control group (standardized mean difference [SMD]=2.11; 95% confidence interval [CI]=1.58~2.65; Z=7.75; p<0.00001; I2=56%). As a result of meta-analysis, according to the concentration of deer antler, the group with high concentration showed statistically significantly higher BMD than the group with low concentration (SMD=1.28; 95% CI=0.74~1.82; Z=4.63; p<0.00001; I2=9%). Conclusions The research shows that the deer antler extracts have significant anti-osteoporotic effects on the osteoporosis-induced animal model. However the studies included in this research had a high methodological risk of bias. This indicates the requirement of considerable attention in the interpretation of the study results.

견수증후군에 대한 침치료 임상연구의 동향 고찰 (Review of Clinical Studies on Acupuncture for Shoulder-hand syndrome)

  • Objectives : Despite various treatment for Shoulder-hand syndrome being presented, only a few studies that examine the direct efficacy of acupuncture exist. The aim of this study is to establish a base for further research by reviewing studies on acupuncture treatment for Shoulder-hand syndrome. Methods : Clinical studies involving the effects of acupuncture for Shoulder-hand syndrome, published on July 19th 2016, were obtained from Pubmed, RISS, OASIS, the Journal of Korean Acupuncture and Moxibustion, the Journal of Korean institute of pharmacopuncture, the Korean Journal of Acupuncture, NDSL, Korean Traditional Knowledge Portal, DBpia, and KISS. Selected studies were evaluated by the Jadad scale and FEAS. Results : Among a total of two hundred and fifteen articles, thirteen articles (RCT 9, SR 1, Meta analysis 1, CCT 1 article, Case series 1) that meet the criteria were selected. Among 13 articles, one article was published in Korea. And apart from this one article, twelve studies were published in China. All studies reported that the experimental group treated by acupuncture had significantly better effects compared to the control group. However, the overall quality assessment of the studies is low. Conclusion : Despite several limitations, various studies to prove limited yet effective acupuncture treatment on Shoulder-hand syndrome provides much significance. Subsequent studies conducted by the complementary systematic review of the studies and well-designed clinical trials using the methodological quality will be needed to more firmly validate the therapeutic effect of acupuncture on Shoulder-hand syndrome.

황금(黃芩) 에탄올 추출물에 의한 인체 신세포암 Caki-1 세포의 자가세포사멸 유도 (Induction of Apoptosis by Ethanol Extract of Scutellaria baicalensis in Renal ell Carcinoma Caki-1 Cells)

  • 꿀풀과(Labiatae)에 속하는 황금(黃芩, S. baicalensis)은 한국, 중국, 몽골 및 시베리아 동부 등지에 분포하는 여러해살이 초본식물로서 예로부터 민간처방 약재로 사용되었으며, 한방에서는 뿌리 말린 것을 이질, 발열 및 황달의 치료제로 사용되고 있다. 또한 최근 연구에 따르면 황금 추출물은 항염증, 항당뇨, 항균, 항알레르기, 항바이러스, 항고혈압, 항산화 및 항암 효능을 가지는 것으로 알려져 있으나 신세포암에서의 항암효능 및 분자생물학적 기전에 대해서는 명확히 밝혀져 있지 않다. 본 연구에서는 인체 신세포암 Caki-1 세포에서 황금 에탄올 추출물(ethanol extract of S. baicalensis, EESB)이 유발하는 항암효과 및 항암기전을 조사하였다. 본 연구의 결과에 의하면 EESB 처리에 의한 Caki-1 세포의 증식억제는 apoptosis 유발과 밀접한 연관이 있었으며, 이는 DR4 Fas ligand 및 Bax 단백질의 발현 증가와 Bid, XIAP 및 cIAP-1의 발현 억제와 관련이 있었다. EESB는 또한 미토콘드리아의 기능 손상과 caspase-3의 기질단백질인 PARP, ${\beta}$-catenin 및 $PLC{\gamma}$-1 단백질의 단편화를 유발하였다. 그러나 EESB 처리에 의하여 유발되었던 apoptosis가 pan-caspases inhibitor인 z-VED-fmk를 이용하여 caspases의 활성을 억제하였을 경우 현저하게 감소되어, EESB에 의한 apoptosis 과정에 caspase의 활성 증대가 중요한 역할을 한다는 것을 알 수 있었다. 이러한 결과들은 황금의 항암작용을 이해하는데 중요한 자료가 될 것이고 나아가 향후 수행될 추가 실험을 위한 기초 자료로서 그 가치가 매우 높을 것으로 생각된다.

동양에 있어서의 인삼재배 역사 (The history of ginseng cultivation in Orient)

  • Ginseng has been recognized as a lifespan extending medicine which has been regarded as one of the medicines classified as top medicines, as the Boncho (medical herbs) study which is influenced by the idea of guidance's costume and food concept mainly in China is gaining its bona fide form. As the demand for ginseng has been expanded to other levels, the demand for ginseng has been increasing. Ginseng from the nature reached its supply chain limit due to its extinction and difficulty of picking, so it translated into ginseng cultivation of economy rather than harvesting in nature. After the start of ginseng cultivation, the ginseng cultivation was further enhanced by the rapid development of processing methods such as white-ginseng and red-ginseng, and the surge of consumption due to the traditional belief in ginseng drug efficacy and support of scientific research. In the Joseon Dynasty, the name Gasam (cultivated ginseng) had been created as ginseng was cultivated on farmland after the stage of SanYang (wild cultivated ginseng), the purpose of the new name Gasam is to differentiate from natural ginseng, and natural ginseng lost its firm position as the genuine ginseng as the Gasam replaced the genuine ginseng, and the natural ginseng got a new name of SanSam (wild ginseng). Because the real ginseng substance concept dissipated, and as Gasam is being called ginseng, the name Gasam was also disappeared. As a result, it was possible to grow large quantities according to the arrival of the Gasam era, and it was possible to supply the demand for ginseng, and it could become one agricultural industry. In this ginseng cultivation, in Japan where ginseng did not grow naturally, it was difficult to obtain ginseng from Joseon and faced with a shortage of ginseng at all times. Therefore, the shogun cultivated the Gasam systematically at the national level by the inside of the shogunate. However, since the natural ginseng is native to China and Korea, there is a concern about the deterioration of the quality of natural ginseng due to the incorporation of cultivated ginseng (Gasam). To protect the interests, the cultivation of ginseng was subject to control. For this reason, the lack of historical information on Gasam cultivation, which had to be started secretly, would be a natural result. In this paper, althouh not sufficient enough, the historical informations were used to summarize the history of ginseng cultivation in China, Japan and Korea.

조선왕조실록 갈피에서 발견된 잎 조각의 실체 및 천궁의 식물학적 기원 (Taxonomic Identity of Leaf Fragments Found in the Annals of the Joseon Dynasty and Botanical Origin of a Herbal Medicine 'Cheongung')

  • Tiny leaf fragments were found in the Annals of the Joseon Dynasty, which were compiled about 500 years ago. The records describing the detailed process of compiling the Annals indicate that silk bags packed with the powders of 'Cheongung' and 'Changpo', which have been used as traditional herbal medicines in the northeast Asian countries such as China and Japan as well as Korea, were put in the wooden storage boxes together with the volumes of the Annals. However, there is no record that parts of plants were used in the process of compiling the Annals. The botanical origin of leaf fragments was identified as Ligusticum sinense 'Chuanxiong' by the analysis of trnK of chloroplast DNA as well as the examination of leaf surface with SEM. The comparative analysis of trnK sequences showed that the chloroplast DNA haplotype of 'Tocheongung', a triploid species cultivated in Korea, was identical with Cnidium officinale, but different from L. sinense 'Chuanxiong'. The molecular results provide a new suggestion on the botanical origin of crude drugs used as 'Cheongung', which has been disputed in Korea.
