본원에서는 우측 흉곽내 폐용적이 감소하면서 심장이 우측으로 이상 회전을 하게 되어 기관기관지의 협착이 유발된 3세 여환아를 우측 폐의 확장을 유도하여 심장의 이상 회전을 교정해 줌으로써 기관기관지 협착을 치료하였기에 이에 보고하는 바이다.

The extent of resection and release of the trachea is important for successful anastomosis. Bilateral bronchial dissection is one of the release techniques for resection of the lower trachea. We present the experience of cervical video-assisted mediastinoscopic bilateral bronchial release for long segmental resection and anastomosis of the lower trachea.

Tracheo-innominate artery fistula (TIF) is a rare, life-threatening complication of tracheostomy that makes it difficult to secure the airway due to massive bleeding, constituting a medical emergency. Therefore, most successful surgical treatments include innominate artery debridement and tracheal fistula repair. Herein, we report a case of successful surgical treatment of a TIF while maintaining cerebral blood flow through an artificial vascular graft.

The authors report a case of double aortic arch associated with complete transposition of the great arteries. On 7th, Feb. 1985, Rastelli operation was performed for transposition using extracardiac valved conduit. Postoperative course was complicated by persistent right lower lobe atelectasis which resulted from tracheal compression by double aortic arch. On 20th, Mar. 1985, left arch was divided distal to the left subclavian artery followed by complete resolution of the atelectasis. To the best of our knowledge, this is the first case ever reported in Korea.

59세 여자환자에서 매우 드문 기관의 소세포암을 치험하였기에 보고한다. 진단은 병리조직학적 소견 및 면역조직화학적 검사로 확진하였고, 외과적 절제후 보조적 항암요법을 한 뒤 호전이 있었으나 술후 6개월째 다발성 전이를 보이면서 사망하였다.

The tracheobronchial rupture from blunt trauma is an uncommon injury with a variety of clinical presentations. Most of traumatic tracheobronchial tears occur within 2.5cm of the tracheal carina; lobar or segmental bronchi are rarely affected. Recently we experienced one case of traumatic rupture of right middle lobar bronchus in a patient who had fallen from a bicycle. Bronchoscopy showed linear disruption of the right middle lobe bronchus at its bifurcation site. Emergency right middle lobectomy was performed. The patient was recovered without any sequelae.

Posterior mediastinal goiter extending to carotid sheath posteriorly is rare. Recently we experienced two cases of posterior mediastinal goiter presenting dyspnea due to tracheal compression. The one was a 48-year-old female with mediastinal tumor shadow on chest roentgenogram . The other was a 54-year-old female with palpable mass on neck and huge mediastinal mass. These masses were resected completely through the right posterolateral thoracotomy and median sternotomy respectively. The postoperative courses were uneventful.

A tracheo-esophageal fistula following from blunt chest trauma is one of less common lesion and few guidelines are available to direct its optimal management. Herein, we report a 24-year-old man injured in a motor vehicular accident sustained a nonpenetrating double blowout injury of the thorax and large tracheoesophageal fistula occurred. Tracheal defect required resection and reconstruction, of which the membranous portion underwent closure with borrowed adjacent esophageal wall primarily and substernal left colon interposition was performed 4 weeks later.

12세된 여자 환자에서 한국에서는 처음으로 심장-폐 동시이식이 성공적으로 이루어 졌다. 환아는 심실 중격결손증이 없는 폐동맥 폐쇄증으로 좌폐부전과 함께 우심실의존성 관상동맥혈류를 가지고 있었으며 개심술에 의한 일반적인 심장 수술은 불가능하였다. 장기 제공자는 9세된 남아로 교통사고에 의한 뇌출혈이 사망의 원인이었다. 심장폐이식은 1997년 4월 20일 이루어 졌다. 이식시 수술 방법은 소위 아리조나 방법으로 양측 폐문부를 환아의 횡격막 신경위로 놓는 방법을 사용하였는데 수술시 지혈에 큰도움이 되었다. 수술후 환자는 두가지 큰 합병증이 발생하였는데 하나는 발열이고 둘째는 기관 문합부위 협착을 들수가 있다. 수술후에 계속되었던 발열은 결핵약 투여후 극적으로 소실되었으며 기관 협착에 의한 호흡 곤란은 수술후 제 71일째 실시한 금속성 그물망에 의한 확장으로 치유 되었다. 환자는 수술후 7개월째 호흡 곤란은 보이지 않고 있으며 운동 및 일상 생활에 지장이 없이 양호한 상태로 지내고 있다.
