혈관화된 쇄골을 이용한 기관재건술 (Laryngotracheal Reconstruction Using The Vascularized Clavicle)

  • One of the most difficult airway problems is a subglottic stenosis combined with a tracheal stenosis. Laryngotracheal stenosis has not been completely solved with classic methods like cricotracheal split with autogenous cartilage graft, thyrotracheal anastomosis, and free hyoid or hyoid-sternohyoid myo-osseous flap reconstruction. A 28-near-old male patient who had had laryngotracheal stenosis was successfully treated by laryngotracheal reconstruction using vascularized clavicle. This method has a several advantages: the vascularized clavicle harvesting in the same operative fields, one stage operation, less restenosis due to plentyful blood supply, md prevention of airway collapse due to a clavicle provided framework. It is too early to make a definite conclusion, but the authors suggested that this method could become one of the most effective methods of laryngotracheal stenosis.

결핵성 임파결절에 의한 기관 협착증 치험 1 (Surgical treatment of tracheal stenosis due to paratracheal tuberculous lymphadenitis.*)

  • ; A 15 years old girl was operated due to tuberculous paratracheal lymph node enlargement resulted in marked narrowing of lower part of trachea. She had suffered from sputum expectoration difficulty, which was developed dyspnea and cyanosis. 1st operation was simple removal of enlarged paratracheal lymph node through Rt. thoracotomy but restenosis sign developed 2 months later in spite of Anti-Tbc. medication. 2nd operation was resection of constricted tracheal portion [2.5cm] 8 end to end anastomosis under Extra Corporeal Circulation. Under Extra Corporeal Circulation trachea reconstruction offered many conveniences during operation. Till now, she has been follow up checked for one year, she was very good post-operation state without any restenosis signs.

단단문합술에 의한 기관재건의 임상적 고찰 (Clinical Analysis of Cases of Laryngotracheal Reconstruction Treated with End to End Anastomasis)

  • From 1988 to 1995, 59 patients with laryngotracheal stenosis were operated with resection of stenotic segment and end to end anastomosis in Seoul national university. Among these patients, 36 were tracheal stenosis, 17 were combined lesion of subglottis and trachea and 5 were subglottic stenosis. The success of procedure was determined by stoma closure or decannulation and an overall 89.8% success rate was achieved following 165 procedures. The success rate showed correlation with severity of stenosis and the number of procedures done was related to the site of stenosis. There was no mortality and granulation formation, unilateral vocal cord palsy were the common complication of end to end procedures.

  • Successful regeneration of a cartilage framework using perichondrium has been reported by several authors but there are some arguments surrounding mucosal regeneration. Some authors report that regeneration of mucosa is completed by ingrowth from neighboring tissue but others insist that it occurs via metaplasia from the squamous epithelium. This study was designed to investigate the differences, especially in mucosal regeneration between nonvascularized and vascularized flaps via immunohistochemical study. A morphologic study was carried out to elucidate the characteristics o( the regenerated mucosa which was sutured on the vascularized perichondrium and fabricated in a rabbit ear. A nonvascularized perichondrial-mucosal composite flap with the same dimension was transferred in the control group. BrdU was labelled on both normal mucosa and grafted mucosa in the experimental group without my statistically significant differences. In cytokeratin stain, it was regarded that mucosal coverage of the control group occurred by ingrowth from the neighboring mucosa. It can be conceived that metaplasia of the grafted mucosa occurs in a vascularized composite flap transferred group.

기관삽관에 후발한 기관협착증의 외과적 치료 (The Surgical Treatment of the Tracheal Stenosis Following Tracheostomy and Intubation)

  • Eight patients underwent tracheal resection and reconstruction for tracheostomy and postintubation injuries from 1971 to early 1981. The ages ranged from 12 years to 59 years. The patients had 7-cuff stenosis and one stomal lesion in whom intubated long. Four male and four female patients were treated. Cervical approach was used in one, cervicomediastinal in 3 and transthoracic in four. The longest length of resection extended to 4 cm in whom cervico-upper half mediastinal incision and neck flexion were applied. Techniques for obtaining tension-free anastomosis included cervical flexion or division of the inferior pulmonary ligament and mobilization of the right hilum. Concurrent tracheostomy was not needed in all. There was one death at the end of emergency operation from anesthetic accident. Granulations at the anastomosis line, necessitating bronchoscopy, were noted in two and the lesion did not recurred after removal. No restenosis or other complications occurred during long follow-up.

횡격막에 발생한 신경섬유종 1례 (Primary neurofibroma of the Diaphragm)

  • In spite of great advances in surgical treatment during past several decades, surgery of the trachea failed to develop correspondingly, partly because of relative rarity of the tracheal lesions and partly because of difficulties in surgical technique and anesthesia. Surgical diseases of the trachea are largely obstructions due to neoplasm or cicatrical stenosis and tracheal malacia. The present treatment of respiratory failure, using cuffed endotracheal and tracheostomy tubes, has produced, apparently with increasing frequency, tracheal stenosis, tracheomalized tracheal erosion. Surgery is presently the only reasonable way to treat stenotic lesions of the tracheobronchial tree. In the case of tumors, the current trend has been that of radical excision. Primary end-to--end reconstruction of the trachea has been generally recognized as the ideal method of repair following resection. However, for decades it was believed that a maximum of four tracheal rings only might be excised and primary healing achieved with safety. A great variety of procedures, developed by numerous investigations and directed at tracheal substitution, have almost invariably met with discouraging results. A meticulous study done by Grillo and associates on autopsy specimens has shown that an average 6.4cm of mediastinal trachea can be safely resected by full mobilization of the right lung and transplantation of the left main bronchus into the bronchus intermedius. Recently, we experienced a case of successful resection of a tumor of the tracheal carina and primary tracheo-left main bronchial anastomosis at the Department of Thoracic & Cardiovascular Surgery, the National Medical Center in Seoul. The patient, a 29-year-old man, was admitted to the hospital with complaints of dyspnea and cough. On admission, chest film showed hydropneumothorax on the right. After closed thoracostomy, hydropneumothorax disappeared, but hazy densities, developed in the right middle and lower lung fields, resisted to treatment. Bronchoscopy uncovered irregular tumor covering the carina and the right main bronchus, and biopsy indicated well differentiated squamous Cell carcinoma. Operation was performed on July 2, 1975. A right postero-lateral thoracotomy was used. Excision involved the lower trachea, the carina, the left main bronchus and the right lung. This was followed by direct anastomosis between the trachea and the left main bronchus. Bronchography was done on 17th postoperative day revealed good result of operation without stricture at the site ofanastomosis. About one month after the operation symptoms and signs of bronchial irritation with dyspnea developed, and these responded to respiratory care. On 82nd postoperative day, sudden dyspnea developed at night and the patient expired several hours later. Autopsy was not done and the cause of death was uncertain.

  • 기관내 삽관 후 발생한 기관협착환자의 치료는 아직 완전한 치료법이 정립되어 있지 않아 다양한 치료법들이 이용되고 있는 실정이며 경직성 기관지경이나 풍선도관을 이용한 물리적인 확장, 혹은 레이저시술 후 스텐트 삽입등이 수술적인 치료 전에 할 수 있는 치료법이다. 저자등은 레이저시술에 이은 풍선 도관으로 확장을 시행한 기관내 삽관 후 발생한 기관협착 1례를 문헌고찰과 함께 보고하는 바이다.

기관 절제 및 단단문합술의 성적 고찰 (Result of Tracheal Resection and End-to-end Anastomosis)

  • 기관 협착의 치료에는 주기적인 확장, 기관내시경을 통한 협착 조직의 제거, T-tube 삽입 등의 보존적 방법과 수술적 방법이 있다. 그러나, 최근 수술을 통한 기관 절제 및 단단문합술이 기능적으로나 해부학적으로 정상적인 상기도 확보에 효과적인 것으로 보고되고 있어 그 결과를 알아보고자 한다. 대상 및 방법: 1990년 3월부터 2002년 7월까지 울산대학교 의과대학 서울아산병원에서 기관 절제 및 단단문합술을 시행한 환자 41예를 대상으로 의무기록을 참조하여 후향적으로 분석하였다. 결과: 원인 질환으로는 postintubation stenosis 26예,기관에 발생된 원발성 기관종양이 10예(양성 3예, 악성 7예), 기관내 결핵 1예, 외상 2예, 그리고 갑상선암이 2예였다. 기관 절제 및 단단문합술을 시행한 환자 41예 중 29예는 기관 절제 및 단단 문합술을 시행하였고, 12예는 갑상연골 또는 윤상연골 절제가 동반된 후두 기관 문합술을 시행하였다. 후두 기관 문합술을 시행한 11예 중 4예에서 후두분리술 (supralaryngeal release)을 시행하였다. 절제된 기관의 길이는 평균 $3.6{\pm}1.0$cm였다. 기관 절제 및 단단 문합술을 시행한 41예의 환자 중 수술 후 상태가 양호한 환자는 30명(73.2%)이었고, 수술 직후 일부문합 부위에 육아종 증식 또는 감염이 있었으나 보존적 치료 후 호전된 환자는 8명(19.5%)으로 전체환자 중 92.7%에서 수술 후 양호한 기도의 확보가 가능하였다. 합병증으로는 육아종 증식을 보여 보존적 치료를 시행한 환자가 7예, 창상감염 2예, 문합부 dehiscence 2예, 운동 시만 호흡곤란을 보이면서 특별한 치료없이 지내는 재협착이 1예, 수술 후 반복적인 기도 흡인으로 재기관절개술을 시행한 환자가 1예 있었다. 기관 절제 및 단단문합술과 직접 관련된 30일 내의 조기사망은 없었고, 원내사망이 3예 있었다 결론: 기관 절제 및 단단문합술은 높은 성공률과 낮은 유병률 및 사망률을 보여 병변의 길이가 적절한 경우, 기관 수술의 표준이 될 수 있을 것으로 생각한다 그러나, 수술부위 육아조직 증식이 가장 심각한 합병증으로 이를 방지하기 위한 지속적인 연구와 노력이 필요할 것으로 생각한다.

흉부둔상에 의한 기관식도루의 치험 1례 (A Large T-E Fistula Following Blunt Chest Trauma -A Case Report-)

  • A tracheo-esophageal fistula following from blunt chest trauma is one of less common lesion and few guidelines are available to direct its optimal management. Herein, we report a 24-year-old man injured in a motor vehicular accident sustained a nonpenetrating double blowout injury of the thorax and large tracheoesophageal fistula occurred. Tracheal defect required resection and reconstruction, of which the membranous portion underwent closure with borrowed adjacent esophageal wall primarily and substernal left colon interposition was performed 4 weeks later.
