Purpose: This study purposed to analyze the spatial accessibility of mental health institutions in Ganwon-Do using Geographic Information System and to suggest policy implications. Methodology: Network analysis was applied to assess the spatial accessibility of mental health institutions in Gangwon-Do. To perform the network analysis, network data set was built using administrative district map, road network, address of mental health institutions in Gangwon-Do. After building network data set, Two network analysis methods, 1) Service area analysis, 2) Origin Destination cost matrix were applied. Service area analysis calculated accessive areas that were within specified time. And using Origin Destination cost matrix, travel time and road travel distance were calculated between centroids of Eup, Myeon, Dong and the nearest mental health institutions. Result: After the service area analysis, it is estimated that 19.63% of the total areas in Gangwon-Do takes more than 60 minutes to get to clinic institutions. For hospital institutions, 23.08% of the total areas takes more than 60 minutes to get there. And 59.96% of Gangwon-do takes more than 30 minutes to get to general hospitals. The result of Origin-Destination cost matrix showed that most Eup Myeon Dong in Gangwon-Do was connected to the institutions in Wonju-si, Chuncheon-si, Gangneung-si. And it showed that there were large regional variation in time and distance to reach the institutions. Implication: Results showed that there were regional variations of spatial accessibility to the mental health institutions in Gangwon-Do. To solve this problem, Several policy interventions could be applied such as mental health resources allocation plan, telemedicine, providing more closely coordinated services between mental health institutions and community mental health centers to enhance the accessibility.

Purpose: The study examined the poisoned patients' characteristics nationwide in Korea by using data from the National Emergency Department Information System (NEDIS). Methods: Among the patients' information sent to NEDIS from January 1, 2013 to December 31, 2015, the included subjects' main diagnosis in ED showed poisoning according to the 7th edition of the Korean Standard Disease Classification (KCD-7). We analyzed the patients' gender, age, initial vital signs, visit time, stay time of staying in ED, results of ED care, main diagnosis in ED, length of hospitalization, and results of hospitalization. Results: A total of 106,779 ED visits were included in the analysis. There were 55,878 males (52.3%), which was more than the number of females. The number of intentional poisoning was 49,805 (59.6%). 75,499 cases (70.8%) were discharged, and 25,858 cases (24.2%) were hospitalized. The numbers of poisoning patients per 1,000 ED visits were 14 in Chungnam and 11.9 in Jeonbuk. The most common cause of poisoning, according to the main diagnosis, was venomous animals. It was the same for hospitalized patients, and pesticide was next. Pesticide was the most common cause of mortality in ED (228 cases, 46.1%) and after hospitalization (584 cases, 54.9%). The incidence of poisoning by age group was frequent for patients in their 30s to 50s, and mortality in ED and post-hospitalization were frequent for patients in their 60s to 80s. Conclusion: This study investigated the characteristics of poisoning patients reported in the past 3 years. Pesticide poisoning had a high mortality rate for patients in ED and in-hospital. For mortality, there was a high proportion of elderly people over 60. Thus, policy and medical measures are needed to reduce this problem. Since it is difficult to identify the poison substance in detail due to nature of this study, it is necessary to build a database and monitoring system for monitoring the causative substance and enacting countermeasures.
Objectives: The purpose of this study is to report the clinical effectiveness and safety of herbal medicine treatment for postmenopausal osteoporosis. Methods: Researchers searched papers through 5 online databases including The Cochrane Library Central, Pubmed, China Academic Journal (CAJ), Oriental medicine Advanced Searching Integrated System (OASIS) and Korean studies Information Service System (KISS). Randomized controlled trials (RCTs) that used herbal medicine as treatment were included. Results: Twenty three studies were selected by the selection and exclusion criteria. The treatment group was treated with herbal medicine alone or with herbal medicine and conventional medicine. The control group was treated with conventional medicine. Most common evaluation index was Bone Mineral Density (BMD) followed by total efficacy rate and level of bone metabolism markers, level of sex hormones, etc. Compared with the control group, the treatment group was more effective and safer in all of 23 studies. Conclusions: Herbal treatment alone could be an effective and safe option in treating postmenopausal osteoporosis. Moreover herbal treatment with conventional medicine could improve its therapeutic effect on postmenopausal osteoporosis as well.
To identify the changes in professional care patterns after the introduction of medical insurance in Korea, professional care in hospitals and clinics of two succeeding years were compared. The hospitals and clinics selected for this study were those which located in Seoul city. Hospitals were classified into 3 categories: university hospital, general hospital and hospital. The diseases selected for this study were acute appendicitis and normal delivery. They were selected because their disease courses are considered to be fairly stable. The variables used for this study were length of stay, total hospital costs, costs of each components of cares. The information used for this study was obtained from the official forms requested by the medical facilities to the Korea Medical Insurance Corporation. The two periods studied were 3 months of each year from March 1st to May 31st in 1979 and 1980, The total number of normal delivery studied was 289 in 1979, 301 in 1980 respectively and the acute appendicitis was 92 and 111 respectively. In order to compare the quantity of medical care between 2 study periods the insurance price scores of 1979 were converted to prices of 1980. For statistical test of difference between 2 periods T-test and Welch's test were used. The result of the study were briefly summarized in below. 1. No significant difference was observed in the average length of stay of both disease between two study periods in all types of hospitals. 2. No significant difference was observed in the average total hospital costs of both diseases in all types of hospital, but in the private clinic the average clinic costs was rather decreased significantly in 1980. 3. More cost decrease were seen than cost increase in 1980 in all types of facilities, More cost changes by items were seen in acute appendicitis than in normal delivery between two study periods. The total hospital costs can be devided into 2 portions: charges for drug and material and for physician. In normal delivery, costs for physician's charges was significantly decreased in almost all the hospitals and costs for drug and material were not changed significantly in all the hospitals in 1980. In the university hospitals, however, the costs for drug and material were increased significantly in 1980. The cost decrease for physician's charge were mainly due to the decrease in the costs of laboratory test, treatment and physical therapy. The increase in the costs for the drug and material in the university hospitals was mainly due to the increase in the cost for drugs for oral administration and injection. 4. The proportion of components of medical care in the hospital has not been changed significantly, however, the cost for injection in normal delivery was characteristically increased in 1980 in all hospitals studied. In general the proportion of the costs for drug and material was tended to increase and the costs for physician was tended to decrease in 1980. The increase in the costs for drug and material were considered to be due to increase in the cost for drugs for oral administration and injection. The decrease in the costs for physician were due to decrease in the costs of laboratory test, treatment and physical therapy. Above mentioned changes in hospital and clinic care patterns are considered to be mostly influenced by the review criteria set by the K.I.C. for the assessment of the fee request made by clinics and hospitals.

A single phase of Bi2212 can be fabricated via the co-deposition process. While it cannot be obtained by the layer-by-layer process. Ultra-low growth rate in our ion beam sputtering system brings out the difference in Bi element adsorption between the two processes and results in only 30% adsorption against total incident Bi amount by layer-by-layer deposition, in contrast to enough Bi adsorption by co-deposition.

Tn the modern integrated healthcare environment, digital technology plays a critical role in the success of its operation. For overcoming the deteriorated healthcare management environment is to improve business service ability, cost and management efficiency through information processing of the business service process and the decision support system. For a solution of these problems, the Hospital Information System(HIS) was introduced. In this paper we proposed HIS system through the HL7-based with legacy system that improves both the efficiencies of medical of office and medical treatments. The implementation of HIS system enables combination of legacy systems including HL7 based OCS and PACS. And we proved the effective HIS development method provided by analysis of total module.
Objectives To systematically explore the effects of acupuncture treatment for rotator cuff disorders and review the clinical trials. Methods We searched 9 electronic databases (PubMed, Cochrane central, Embase, China National Knowledge Infrastructure [CNKI], Korea Institute of Science and Technology Information [KISTI], National Digital Science Library [NDSL], Korean studies Information Service System [KISS], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS]) to find randomized controlled trials that used acupunture treatment for rotator cuff disorders. We assessed the designs of the randomized controlled trials and the method of acupuncture treatment according to the Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA). The methodological quality of randomized controlled trials were assessed using the Cochrane Risk of Bias (RoB) tool. Results Total 5 trials were reviewed. 4 out of 5 randomized clinical trials reported meaningful effects of acupuncture treatments compared to control group. However risk of bias seemed high. Conclusions Although the results suggest that acupuncture treatment has favorable effects for rotator cuff disorders, most of the studies included methodologically high risk of bias. Thus, well designed randomized clinical trials which evaluate the effects of acupuncture treatment for rotator cuff disorders should be encouraged.
Background: External-cause mortality is an important public health issue worldwide. Considering its significance to workers' health and inequalities across industries, we aimed to describe the state of external-cause mortality and investigate its difference by industry in Republic of Korea based on data for 2018. Methods: Data obtained from the Statistics Korea and Korean Employment Information System were used. External causes of death were divided into three categories (suicide, transport accident, and others), and death occurred during employment period or within 90 days after unemployment was regarded as workers' death. We calculated age- and sex-standardized mortalities per 100,000, standardized mortality ratios (SMRs) compared to the general population and total workers, and mortality rate ratios (RRs) across industries using information and communication as a reference. Correlation analyses between income, education, and mortality were conducted. Results: Age- and sex-standardized external-cause mortality per 100,000 in all workers was 29.4 (suicide: 16.2, transport accident: 6.6, others: 6.6). Compared to the general population, all external-cause and suicide SMRs were significantly lower; however, there was no significant difference in transport accidents. When compared to total workers, wholesale, transportation, and business facilities management showed higher SMR for suicide, and agriculture, forestry, and fishing, mining and quarrying, construction, transportation and storage, and public administration and defense showed higher SMR for transport accidents. A moderate to strong negative correlation was observed between education level and mortality (both age- and sex-standardized mortality rates and SMR compared to the general population). Conclusion: Inequalities in external-cause mortalities from suicide, transport accidents, and other causes were found. For reducing the differences, improved policies are needed for industries with higher mortalities.
Purpose: The purpose of this study was to describe the current status of utilization and costs of home health nursing care by the levels of medical institutes in Korea. Method: A secondary analysis of existing data was used from the national electronic data information(EDI) of 148 home health agencies for 6 months from May to Oct 2005 in total. Result: The 148 agencies had multiple services in cerebral infaction, essential hypertension, sequoia of cerebrovascular disease, type 2 diabetes mellitus, etc.. The highest 10 rankings of 76 categories of home health nursing services were composed of 96.4% of the total services, such as simple treatment, inflammatory treatment, urethra & bladder irrigation, inserting indwelling catheter etc., in that order. The highest 20 rankings of 226 categories of home examination services were composed of 77.0% of the total home examination services. In addition, the average cost of home health care per visit was 46,088 Won (${\fallingdotseq}$ 48 $, 1 $=960 Won). The costs ranged from 74,523 Won (${\fallingdotseq}$78 $, loss of chronic kidney function, N18) to 32,270 Won (${\fallingdotseq}$34 $, other cerebrovascular diseases, 167). Conclusion: Results suggest that client characteristics of hospital based HHNC are not different from community based HHNC or visiting nursing services for elderly. The national results will contribute to baseline data used to establish a policy for the home health nursing care system and education.

Objectives This study was conducted to analyze the effect of using acupotomy for piriformis muscle syndrome. Methods The databases used for the search were PubMed, Embase, Cochrane library, China National Knowledge Infrastructure (CNKI), Koreanstudies Information Service System (KISS), Research Information Sharing Service (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS), KMbase, and National Digital Science Library (NDSL). The literature was searched using a total of 9 domestic and foreign databases. randomized controlled trial using only acupotomy as an intervention was selected. The selected studies are analyzed the risk of bias through Cochrane risk of bias, and meta-analysis was performed using Revman. Results As a result of meta-analysis of 11 selected studies, acupotomy was more effective than nerve block and acupuncture. In terms of visual analog scale and immediate analgesic effect, acupotomy did not have statistically significant results compared to other treatment methods. meta-analysis of electro-acupuncture and acupotomy showed no significant results. Conclusions When acupotomy is applied as a treatment for piriformis muscle syndrome, it is more effective than nerve block and acupuncture. but most of the studies have been judged as 'unclear' in the risk of bias assessment. Due to characteristics of treatment group and control group's intervention, It is judged that the blinding of the participant was not properly implemented. Therefore, it is considered that high-quality studies are necessary to supplement the problems in the future.
