Preventive dentistry for children (소아 예방치과)

  • Dental caries is the first disease to cause the pathological extraction of teeth in children. The complete prevention of dental caries is not achieved by toothbrushing alone. The use of fluoride and pit and fissure sealant is regarded as key tools to prevent dental caries. Fluoride-containing tablets or multi-vitamins and community water fluoridation can be used as systemic application techniques. Professional fluoride application, fluoride iontophoresis on teeth, fluoride mouth rinsing and fluoride-containing toothpaste can be used as local application techniques. Pit and fissure sealant is mainly used to prevent dental caries on occlusal surfaces of premolars and molars. Sweeteners not to occur dental caries has been developed to substitute sucrose. Dental erosion increases according to the high consumption of acidic soft drink or beverages. The appropriate use of fluoride and pit and fissure sealant are recommended to prevent dental caries effectively and the education is required to reduce the consumption of acidic soft drinks or beverages to decrease dental erosion.

  • 본 연구조사결과 영유아 세치제 패키지 디자인의 중요도는 포장디자인(일러스트, 패턴)는 약간중요가 28%, 포장디자인(라벨) 약간중요가 42%, 제품의 신뢰성 약간중요 46%, 제품의 안정성 약간중요 46% 로 나타났다. 영유아 세치제 패키지디자인에 색채 분석결과 유기농 제품의 경우 KS 6.25Y 2/2라는 안정된 색상을 45.37%사용하였고 딸기 ? 세치제는 KS 1.25YR 7/2 색상, KS 계통색 연한 자줏빛 분홍을 43.2% 사용하여 캐릭터와 딸기라는 것을 패키지 디자인으로 드러냈다. 본 연구 결과 영유아 세치제 패키지 디자인 색채분석을 통해서 영유아 세치제의 구매 동향 분석과 포장디자인의 개선을 위한 후속연구가이루어 져야 할 것이다.

Caries Control for infants based on Health Plan 2010 in Korea (국민건강종전종합계획에 근거한 영유아 치아우식 관리 방안)

  • We examined progress in oral health of Korean 5 year olds children and dental public health infrastructure since the National Health Promotion Plan 2010 Oral Health Objectives were issued. We summarize trends in the prevalence of dental caries and trends in national public oral health program activities and budget. The Oral Health Objectives were achieved in 2006. Oral health in Korean preschool children improved considerably by improving of lifestyle and consumption of fluoride containing toothpaste. Although the number of public oral health center and the budget of oral health education were increased, the impact of public oral health program for preschool children was not influential. New oral health program for infants such as fluoride varnish application and strengthening of existing public oral health program should be performed for continuing improvement of oral health in Korea.

치주질환 발생기전 및 예방에 관한 실험적 연구

  • 64 dental students who had brushed their teeth with scrubbing method, using 'W' dentifirice for a long lime, were made to practise either scrubbing or Bass method. using one of the 'W','X','Y','Z' toothpaste instead of only 'W' paste for recent 3 weeks. Dental plaque on the teeth of every students were carefully examined compared the results. The simplified oral hygiene index was applied for this purpose and following conclusions were achieved. 1) The Scrubbing method of tooth-paste 'X', Bass method of tooth-paste 'X', and the Bass method of tooth-paste 'Z' had an obvious effect to inhibition of accumulation of plaque. 2) In the tooth brushing method, the scrubbing and Bass are both effective in reducing the plaque accumulation 3) The tooth paste 'X' has the most effectiveness in reducing the plaque accumulation among four tooth pastes 4) The difference in the value of plaque index between the tooth pastes, 'W' and 'X' was only significant statisticallay

Is it Possible to predict the Characteristics of Calcific Deposition in Calcific Tendinitis of Shoulder Joint? (단순 방사선 검사를 이용하여 석회성 건염에서 발생하는 석회 침착물의 성상에 대한 예측이 가능한가?)

  • Purpose: The purpose of current study was to evaluate the validity of the existing radiological classifications as a diagnostic modality for predicting characteristics of calcific deposition in calcific tendinitis of the shoulder joint. For that purpose, we determined the inter-observer reliability for evaluating diagnostic precisions of the classification and also evaluated diagnostic accuracy of predicting the toothpaste type calcific deposition. Materials and Methods: We performed retrospective study with total 26 patients surgically treated with calcific tendinitis of the shoulder joint from March 2010 to October 2013. Two independent observers reviewed preoperative radiographs of shoulder joints, and classified the characteristics of calcific depositions according to the criteria of Gartner, DePalma and Patte. Cohen's kappa were calculated for each classifications to evaluate inter-observer reliability. Sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio were determined for type of calcific depositions with Gartner type III, DePalma type I, and Patte type II for predicting toothpaste type calcific deposition. Results: The values of Cohen's kappa were the highest in the classification of Patte, 0.62, and the values for the classifications of DePalma and Gartner were 0.56 and 0.36, respectively. The sensitivities for predicting toothpaste type calcific deposition in Gartner Type III, DePalma type I and Patte type II were 83.3%, 91.7%, and 58.3%, respectively. Specificities were 85.7% 50.0% and 64%, positive likelihood ratios were 5.833, 1.833 and 1.633, negative likelihood ratios were 0.194, 0.167 and 0.648, and diagnostic odds ratios were 30.00, 11.00 and 2.52, respectively. Conclusion: There were no radiologic classifications of calcific tendinitis which has both high precision and accuracy. Further studies with other diagnostic modalities such as ultrasonography are needed for predicting characteristics of calcific deposition in calcific tendinitis of the shoulder joint.

Effect of Fluoride Recharging on Fluoride Release and Surface Properties of Orthodontic Bracket Adhesives (불소 적용 후 교정용 브라켓 접착제 종류에 따른 불소 재흡수성과 표면 변화에 관한 연구)

  • The aim of this study was to compare fluoride release and surface changes according to different orthodontic bracket adhesives the application of fluoride products. We used non-fluoridated composite resin Transbond fluoridated composite resins Blugloo and LightBond, resin-modified glass ionomer Rely $X^{TM}$ Luting 2, and conventional glass ionomer Fuji $I^{(R)}$. Fluoride release of five orthodontic bracket adhesives and fluoride release ability after application of three fluoride products (1.23% acidulated phosphate fluoride gel, Tooth Mousse $Plus^{(R)}$, Fluor Protector, and a toothbrush with sodium fluoride-containing toothpaste) were measured using a fluoride electrode that was connected to an ion analyzer. After 4 weeks of fluoride application, the surface roughness and surface morphology were examined using a surface roughness tester and field emission scanning electron microscopy. The amounts of fluoride release were observed not only on application of Tooth Mousse $Plus^{(R)}$ and Fluor Protector on resin-modified glass ionomer Rely $X^{TM}$ Luting 2 and Fuji $I^{(R)}$, but also during tooth brushing using fluoride-containing toothpaste. After application of Tooth Mousse $Plus^{(R)}$, except Transbond XT, the surface roughness increased, and all orthodontic adhesives showed a partial drop of micro-particle filler. On application of 1.23% acidulated phosphate fluoride gel on all orthodontic bracket adhesives, their surface roughness increased. To bond the orthodontic bracket, resin-modified glass ionomer Rely $X^{TM}$ Luting 2 and Fuji $I^{(R)}$ adhesives are highly recommended if the amount of fluoride release is considered to confer a preventative effect on dental caries, and among the fluoride products, Tooth Mousse $Plus^{(R)}$ and Fluor Protector are better than 1.23% acidulated phosphate fluoride gel, and these are expected to prevent dental caries even during tooth brushing with fluoride-containing toothpaste.

  • This study was intended to prepare the basic data required for the development of oral products for the patient with diabetes by analyzing the relation between diabetes and oral health and the actual condition of oral care from the senior citizens diagnosed with diabetes out of the senior citizens over 65. In this study, 8 senior citizens who visited the senior welfare center in D city, were organized into the group of 4 senior citizens and the interview approximately for 40 minutes per group was conducted. In the results of this study, 5 core topics of 'Diabetes and Oral Health", 'Oral Discomfort', 'Oral Care Method', 'Visit to Dentist' and 'Demand for Oral Care' were drawn. For the relation between diabetes and the oral health, the most of research subjects answered that they heard about it for the first time, They answered that for the oral health care, they brushed the teeth using toothpaste 3 ~ 4 times a day and for the visit to dentist, they visited the dentist only when they had oral problem. For what is necessary for oral care, they answered that they wish that the oral care education customized for patient with diabetes and diverse oral care products such as toothpaste, etc. would be developed. Considering the results of this study, the development of oral care management program and the development of oral products in future are deemed to be necessary.

Determination of Preservatives in Pharmaceuticals and Personal Care Products (의약품 및 개인위생·생활용품 중 보존제 함유량 분석)

  • Objectives: Preservatives are commonly used in pharmaceuticals, cosmetics and other products to extend the expiration date and prevent the growth of microorganisms. Preservatives are generally effective in controlling mold and inhibiting yeast growth, and against a wide range of bacterial attacks as well. They also adversely affect the quality of sperm and cause precocious puberty in children. This study was performed to analyze seven preservatives used in pharmaceuticals and personal care products. Methods: Five kinds of pharmaceuticals and personal care products (PPCPs) were examined for analysis with a high performance liquid chromatography-diode array detector. Each sample was homogenized and the targeted compounds were extracted with methanol. The suspended particulate was removed by syringe filter. Next, the sample was injected into an HPLC system. The separation of the seven preservatives was achieved with a C18 column and gradient mode. The accuracies were between 73% and 120% and precision was lower than 11.58% (RSD). Results: All of the calibration curves showed good linearity with a coefficient of determination ($r^2$) over 0.999. Among the PPCP samples, the detection rate of preservatives was 32.5% for pharmaceuticals, 44.8% for toothpaste, 76.9% for mouthwash, 40.0% for body lotion and 56.0% for wet tissues. The average concentrations of the preservatives in PPCPs were BA 1141.0 mg/kg, MP 709.8 mg/kg, EP 624.9 mg/kg, PP 216.9 mg/kg, BP 167.8 mg/kg, and TCS 538.2 mg/kg. The most frequently detected preservatives in pharmaceuticals and personal care products were BA, MP and PP. The concentrations of preservatives exceeded Korean regulatory standards in 11 samples of medicines, three of mouthwash and two of body lotion. Conclusion: We found that most of the PPCP samples contained various preservatives. It is necessary to identify which preservatives were used and to determine the level of preservatives in PPCPs and to assess the health risk to susceptible populations such as children.

A study on practice behavior of toothbrushing in preschool children by observing survey (미취학 아동의 잇솔질 실천 행동에 관한 관찰조사 연구)

  • Objectives : The purpose of this study was to examine the toothbrushing practice behavior of preschool children by observing children's actual toothbrushing behavior and further to offer basic data available for being reflected in direction and goal establishment of the systematic and organized oral health education in consideration of individual characteristic. Methods : It targeted 225 preschool children from May 6, 2010 to June 8, and analyzed by having the survey results of totally 208 people, except 17 who are noncooperative with the observation survey. As for the collected materials, it carried out frequency analysis and $x^2$-test in order to grasp subjects' general characteristics and toothbrushing practice behaviors and to examine correlation, by using SPSS 12.0 for window. Results : 1. A grasp of toothbrush was indicated 47.1% for "proper" and 52.9% for "improper." The position of holding a toothbrush was indicated to be the highest with 59.6% for "the center in grip." 2. Toothpaste amount was indicated to be 49.0% for" tip in toothbrush, "followed by 38.9% for "half in toothbrush" and 12% for "every side of toothbrush" in order. Significant difference was shown according to age($x^2$=19.125. p<.05). 3. Average toothbrushing time per once was surveyed to be $106{\pm}56$ seconds. 4. As for a toothbrushing method by region, the labial and the baccal surface were indicated to be horizontal scrub with 63% and fones method 11.5%. Significant difference was shown according to gender($x^2$=10.275. p<.05). The anterior lingual surface wasn't washed with 72.6%. Significant difference was shown according to appearance of education($x^2$=6.056. p<.05). 5. A case of requiring exchange because of being widened toothbrush was indicated to be 59.2%. The toothbrush replacement in the highest ratio was needed in kindergarten teachers. Conclusions : The actual toothbrushing practice attitude of preschool children was surveyed to be not right as a whole such as toothbrushing time, toothbrushing method, toothpaste amount, and replacement of toothbrush. Accordingly, the oral health education will need to be performed in order for a change into positive attitude and for improvement in practice level. In addition, it is considered to be likely necessary for development in substantial oral health educational program available for enhancing knowledge level of oral health care in mothers and kindergarten teachers.

A CONTROL OF PLAQUE, GINGIVITIS AND ANTI-CARIES EFFECT OF DENTIFRICE CONTAINING HORSERADISH EXTRACTS (서양산 고추냉이 추출물이 함유된 세치제의 효능에 대한 임상적 실험연구)

  • The purpose of this study was to find out the effect of dentifrice containing Horseradish (Armoracia rustica) root extracts on improvement of oral hygiene. The clinical evaluation was done participated by 80 adults volunteers who provided a informed consent for their participation. The participants were divided into two groups ; control group (using toothpaste without Horseradish extracts) and experimental group (using toothpaste containing Horseradish extracts). The checklist of the experiment includes the plaque index, gingival index and anti-caries activity. The anti-caries activity was measured by cultivating Streptococcus mutans for 48 hours then estimating the number of increased bacteria using Dentocult SM. The double-blind method was used in this study. The results of this study can be summarized as follows; 1. The plaque formation decreased 28% in control group, 58% in experimental group using dentifrice with Horseradish extracts compared to baseline data. 2. For gingival index, control group and experimental group showed 26% and 40% decrease compared to baseline data, respectively. 3. The score of caries activity test decreased by 4% for control group and 69% for experimental group compared to baseline data. According to the results, a dentifrice containing Horseradish extracts can improve oral hygiene.
