고혈압 환자의 구강건강과 삶의 질에 관한 연구 (A study on oral health and quality of life in patients with hypertension)

  • Objectives: This study investigated the oral health and quality of life of patients with hypertension using the raw data of the 8th National Health and Nutrition Examination Survey, the second year (2020) and 1,269 subjects were analyzed. Methods: The data underwent frequency analysis, descriptive statistics, general linear model analysis, and linear regression analysis using SPSS Statistics 26.0. Results: The quality of life (EuroQol-5 dimension: EQ-5D) of patients with hypertension was surveyed at 0.92 points/1 point. Quality of life according to general characteristics showed significant differences in gender (p<0.001), age (p<0.001), household income (p<0.001), and education level (p<0.001). Quality of life according to oral health showed significant differences in chewing discomfort (p<0.001), speaking discomfort (p<0.001), oral examination (p=0.004), toothache experience (p=0.020), subjective oral health (p<0.001) and oral care product use (p=0.006). Factors affecting quality of life were gender, age, household income, education level, chewing discomfort (B=-0.027, p=0.018), speaking discomfort (B=-0.049, p=0.026), toothache experience (B=-0.018, p=0.027) and subjective oral health (B=0.022, p<0.001). Conclusions: Based on the above results, it was confirmed that the oral health of patients with hypertension is a factor influencing the quality of life; thus, it is considered that it will be used as basic data for research on measures to improve oral health and quality of life.

일부지역 노인복지관 노인들의 주관적 구강건강상태에 따른 사회적 효능감 및 구강건강관련 삶의 질에 관한 연구 (A Study on Social Efficacy of Senior Citizens in Welfare Centers in Some Areas according to Their Subjective Oral Health and Their Quality of Life related to Oral Health)

  • 본 연구는 인구사회학적 특성에 따른 주관적인 구강건강상태와 주관적인 구강건강상태인식에 따른 사회적 효능감과 삶의 질, 구강건강 관련 삶의 질을 알아보고자, 2013년 1월 9일부터 2013년 6월 5일까지 대구광역시 7개의 구에 있는 6개의 노인복지관을 이용하는 노인 582명의 설문지를 분석하여 다음과 같은 결론을 얻었다. 성별은 치통과 구강건조증, 연령층별은 구강건강상태, 음식저작 장애, 치주문제, 구강건조증, 최종학력에서는 구강건강상태, 음식저작 장애, 치통, 치주문제, 동거가족 수에서는 음식저작 장애, 치통, 치주문제, 구강건조증, 구취에서 통계적으로 유의하였다. 구강건강상태에 따른 사회적 효능감 항목에서 '나의 구강은 건강하지 않다'가 3.11점이었고, '나는 턱에서 소리가 나거나 아프다'가 1.99점이었다. 구강건강상태 인식에 따른 사회적 효능감에서 건강은 2.05점, 삶의 질에서 건강은 3.41점이었다. 구강건강관련 삶의 질 점수에서 건강의 경우 기능적 제한은 2.13점, 신체적 동통은 2.53점, 심리적 불편은 2.17점, 신체적 제한은 2.31점, 심리적 능력저하는 2.06점, 사회적 능력저하는 1.81점, 사회적 불리는 1.99점이었다. 노인구강건강증진을 위한 지속적인 구강보건교육의 기회제공과 예방프로그램의 개발을 통하여 구강건강이 진행되지 않도록 적극적인 방안모색이 충분히 고려될 때 노인의 구강건강 뿐만 아니라 삶의 질까지 향상될 수 있을 것으로 사료된다.

Anti-Gastritis and Anti-Oxidant Effects of Chenopodium album Linne Fractions and Betaine

  • Chenopodium album Linne (CAL) is a fast-growing weedy annual plant. The leaves and young shoots may be eaten as a leafy vegetable. In oriental medicine, CAL has been used for treatment of skin disease, fever, stomach ache, toothache, and paralysis. After a preliminary screening of CAL ethanol extract and its fractions obtained from CAL leaves for anti-gastritic and anti-Helicobacter pylori (H. pylori) activity, the butanol (BuOH) fraction was found to have the most significant effect. We also examined antioxidative properties of the total CAL extract and its fractions, and also betaine as an ingredient of the BuOH fraction. To investigate the antioxidant effects of CAL on gastritis, the reducing power, free radical scavenging activity using 1,1-diphenyl-2-picrylhydrazyl (DPPH), and lipid peroxidation effects were determined. Additionally, the BuOH fraction reduced cell viability in a concentration dependent manner in human gastric cancer cell lines. The results of this study revealed that CAL has excellent antioxidant activity, and may be useful in treating gastritis and gastric cancer.

왕초피나무 Zanthoxylum coreanum Nakai의 성분연구(成分硏究)(I) (Studies on the Constituents of Zanthoxylum coreanum Nakai)

  • Pericarp of Zanthoxylum coreanum Nakai have been used as a crude medicine for the treatment of ozena, rheumatoid, nasal sinusitis, meno-xenia, dyspepsia, toothache, sore throat, pains in the limbs, etc. in Korea. A sterol fraction and two compounds, compound I and II were isolated from the roots, stems, leaves of Zanthoxylum coreanum Nakai. The sterol fraction was identified as a mixture of ${\beta}-sitosterol$ and campesterol. Compound I, colorless prismatic crystals, $C_{13]H_{13}NO_4$, mp. $175{\sim}177^{\circ}$, was proved to be skimmianine which is one of the alkaloids and compound II, $C_{14}H_{28}O_2$, mp. $53{\sim}55^{\circ}$, white powdered crystals was identified as myristic acid.

치과 응급 환자의 유형 분석 및 일차 응급처치

  • Recently dental emergency patients tend to increase and diversify because of increased living quality and acknowledgement of health care. We performed this study to understand the pattern of dental emergency and perform the adequate diagnosis and treatment. From August 1998 to July 1999, we inverstigated the rate of dental emergency, distribution according to disease pattern, monthly distribution, and types of emergency treatment from all the emergency patients that visited emergency medicine of Daejin Medical Center, Jesaeng Hospital. There were 266 dental patients(0.978%) from total 27,192 emergency patients. Types of emergency situations included lip lacerations, teeth fracture, teeth missing or avulsion, TMJ contusion, mandible fracture, tongue laceration, toothache, teeth luxation, and so forth. Dental emergency had highest frequency in July. The most common treatments included primary closure of soft tissue lacerations, fixations of luxated teeth and medications.

민간약 때죽나무의 생약학적 연구 (Pharmacognostical Studies on the Korean Folk Medicine "DdaeJukNaMu")

  • Korean Folk Medicine 'DdaeJukNaMu' has been used mainly to cure toothache and neuralgia. With regard to the botanical origin of 'DdaeJukNaMu', it has been considered to be Styrax species of Styracaceae, but there was no pharmacognostical confirmation on it. To clarify the botanical origin of 'DdaeJukNaMu', the anatomical characteristics of the branch of Styrax species growing wild in Korea, Styrax japonica and Styrax obassia were studied. As a result, it was clarified that 'DdaeJukNaMu' was the branch of Styrax japonica.

한국산 토룡의 기초성분 분석 -Lamnodrilus gotai Hatai를 대상으로- (Analysis of Basic Constituents in Korean Earthworm About Lamnodrilus gotai Hatai)

  • The earthworm(Annelid) is a herbicine which has traditionally been used in the treatment of infectious fever, jaundice, infection of middle ear, laryngitis, pharyngitis, nephritis, headache, toothache and certain urinary tract infections from the olden times. Before the isolation and purification of biologically active components we analyzed the basic constituents(proteins, amino acids, mineral, etc.) with lyophilized powder of Lamnodrilus gotai Hatai. The results were as follows: Minerals detected and quantitatively analyzed were $Ca^#$, $Mg^#$, Fe, Zn, Mn, Cu, Co, Cr, Ni, Ge and Se. Amino acids detected were alanine, phenylalanine, leucine, glutamic acid, tyrosine, threonine, arginine, aspartic acid, methionine, lysine, serine, histidine, isoleucine, glycine, proline and etc. The constituents of proteins, fat, fiber, ash and phosphorous were measured. These constituents were compared and discussed with those of other investigations.

담쟁이덩굴엽의 플라보놀 배당체 (Flavonol Glycosides from Parthenocissus tricuspidata Leaves)

  • For the investigation of medicinal resources the studies were carried out to evaluated the pharmaco-constituents in the Leaves of Parthenocissus tricuspidata(Vitaceae), of which leaves have been used in Korea as folk remedies for the treatments of arthritis, jaundice, toothache, neuralgia, and etc. From 1-butanol fraction of the MeOH extract, Compound I ($C_{21}H_{18}O_{13}$, Quercetin-3-O-$\beta$-D-glucuronopyranoside), Compound II ($C_{21}H_{20}O_{12}$, Quercetin-3-O-$\beta$-D-glucopyranoside) and Compound III ($C_{25}H_{28}O_{12}$, Quercetin-3-O-(6"-n-butyl)-$\beta$-D-glucuronopyranoside) were isolated by column chromatographic separation using Sephadex LH-20 and ODS gel. Their structures were elucidated through instrumental analyses, such as $^{1}H$-NMR, $^{13}C$-NMR, IR, UV, El-Mass, FAB-Mass and GC. Especially compound III was Flavonol glycoside and named parthenosin.

Clinical Diagnosis and Treatment of Herpes Zoster in an Immunocompromised Dental Patient: A Case Report

  • Herpes zoster (HZ) is an acute, unilateral inflammatory viral infection characterized by a rash with painful blisters in a localized area of the body. HZ is often associated with intense pain in the acute phase and presents postherpetic neuralgia in the chronic phase. During the prodromal stage of the HZ from the trigeminal nerve, however, the only presenting symptom may be odontalgia, which could be particularly difficult to diagnose. This distinctive syndrome occurs predominantly in the immunocompromised or elderly individuals. In this article, we report a case of HZ developed in the trigeminal nerve of a 60-year-old immunocompromised female patient, whose symptoms including atypical, non-odontogenic odontalgia had improved after series of antiviral treatments.

Rare Odontalgia of Mandibular Teeth Associated with Migraine: A Case Report

  • A 39-year-old male presented with severe pain in right posterior mandibular teeth and temporal area. Initially, the pain in the mandibular teeth was moderate, but the concomitant headache was unbearably severe. His medical history was non-contributory. The clinical and radiographic examination failed to reveal any pathology in the region. There was no tenderness to palpation in the temporalis and masseter muscles or temporomandibular joints. The clinical impression was migraine. The pain in the teeth and headache were aborted using ergotamine tartrate and sumatriptan succinate. Atenolol prevented further pain, while amitriptyline and imipramine had no effect. Migraine can present as non-odontogenic pain in the mandibular teeth, although not as frequently as in the maxillary teeth. A correct diagnosis is essential to avoid unnecessary dental treatments and to manage pain effectively. Clinicians should be able to identify migraine with non-odontogenic dental pain and establish a proper diagnosis through a comprehensive evaluation.
