Cone-beam computed tomography findings of impacted upper canines

  • Purpose: To describe the features of impacted upper canines and their relationship with adjacent structures through three-dimensional cone-beam computed tomography (CBCT) images. Materials and Methods: Using the CBCT scans of 79 upper impacted canines, we evaluated the following parameters: gender, unilateral/bilateral occurrence, location, presence and degree of root resorption of adjacent teeth (mild, moderate, or severe), root dilaceration, dental follicle width, and presence of other associated local conditions. Results: Most of the impacted canines were observed in females (56 cases), unilaterally (51 cases), and at a palatine location (53 cases). Root resorption in adjacent teeth and root dilaceration were observed in 55 and 47 impacted canines, respectively. In most of the cases, the width of the dental follicle of the canine was normal; it was abnormally wide in 20 cases. A statistically significant association was observed for all variables, except for root dilaceration (p=0.115) and the side of impaction (p=0.260). Conclusion: Root resorption of adjacent teeth was present in most cases of canine impaction, mostly affecting adjacent lateral incisors to a mild degree. A wide dental follicle of impacted canines was not associated with a higher incidence of external root resorption of adjacent teeth.

수종 근관확대 기구를 이용한 근관 형성후의 근관형태 변화에 관한 연구 (A STUDY OF ROOT CANAL SHAPE FOLLOWING CANAL PREPARATION OF VARIOUS CANAL ENLARGING INSTRUMENTS)

  • The purpose of this study was to evaluate the resulting root canal angulation and shape change after using various enlarging instruments. The mesial canals (120) of extracted human mandibular 1st and 2nd molars were randomly divided into 6 groups; Control, K-type me, Heliapical me, Canal Master me, sonic and ultrasonic instrumentation group. Vertical angulation of each canal was determined by a straight line through the long axis of canal and another straight line through the apical foramen to intersect at the point where the canal began to leave the long axis of the tooth. By recording and comparing the measured angles of the each set of pre-and postinstrumentation. Then, the roots were sectioned horizontally in the apical, middle, coronal thirds and the canal shapes examined, as was the mesiodistal canal diameter as it relates to the external root surface. The results were as follows: 1. Instrumentation using K-type me group resulted in the highest mean change in angulation ($9.900^{\circ}$) (p < 0.005), while Sonic Air MM 3000 group resulted in the least degree of straightening canals ($8.250^{\circ}$) (p < 0.005). 2. Canal Master file group resulted in the best canal shape at the three levels (P < 0.005). 3. Measured minimal mesial root width produced Heliapical me group at the apical 1/3 level, Sonic Air MM 3000 group at the middle 1/3 level, Heliapical me group at the coronal 1/3 level (P < 0.005). 4. Measured minimal distal root width produced Sonic Air MM 3000 group at the apical 1/3 level (P < 0.05), Heliapical me group at the middle 1/3 level (P < 0.005), Canal Master me group at the coronal 1/3 level (P < 0.005). 5. HeIiapical me group produced more increased canal diameter than any other groups (P < 0.005).

  • 유성기어의 설계는 모듈 등의 이산변수, 잇수 등의 정수 변수, 치폭, 종횡비 등의 연속 변수가 혼재된 복잡한 문제로 이를 해결하기 위해서는 최적 설계 기법이 적용되어야 한다. 본 연구에서는 유전 알고리즘(Genetic algorithm)을 이용한 최적 설계를 유성기어 설계에 적용하였다. 유성기어 설계시 기본이 되는 기어 잇수, 모듈, 압력각, 치폭 등과 같은 매크로 지오메트리(Macro-geometry)를 이용하여 이뿌리/치면 강도에 대한 강도 평가를 수행하였으며, 상용 프로그램과의 비교를 통해 검증하였다. 유전 알고리즘을 이용하여 기어의 체적을 최소화하기 위한 최적 설계를 수행하였으며, 이를 통하여 설계자는 초기 설계시 시행착오를 줄여 설계 시간을 단축시킬 수 있었다.

합착용 복합레진시멘트로 합착한 Machinable Ceramic과 상아질 사이의 인장강도에 대한 실험적 연구 (TENSILE STREGNTH BETWEEN MACHINABLE CERAMIC AND DENTIN CEMENTED WITH LUTING COMPOSITE RESIN CEMENTS)

  • In the case of CAD/CAM ceramic inlay restorations, if isthmus width is widened too much, it may cause fracture of remaining tooth structure or loss of bonding at the luting interface because of excessive displacement of buccal or lingual cusps under occlusal loads. So to clarify the criterior of widening isthmus width, this study was designed to test the tensile bond strength and bond failure mode between dentin and ceramic cemented with luting composite resin cements. Cylindrical ceramic blocks(Vita Cerec Mark II, d=4mm) were bonded to buccal dentin of 40 freshly extracted third molars with 4 luting composite resin cements(group1 : Scotchbond Resin Cement/Scotchbond Multi-Purpose, group2 : Duolink Resin Cement/ All-Bond 2, group3: Bistite Resin Cement/Ceramics Primer, and group4:Superbond C&B). Tensile bond test was done under universal testing machine using bonding and measuring alignment blocks(${\phi}ilo$ & Urn, 1992). After immersion of fractured samples into 1 % methylene blue for 24 hours, failure mode was analysed under stereomicroscope and SEM. Results: The tensile bond strength of goup 1, 2 & 4 was $13.97{\pm}2.90$ MPa, $16.49{\pm}3.90$ MPa and $16.l7{\pm}4.32$ MPa, respectively. There was no statistical differences(p>0.05). But, group 3 showed significantly lower bond stregnth($5.98{\pm}1.l7$ MPa, p<0.05). In almost all samples, adhesive fractures between dentin and resin cements were observed. But, in group 1, 2 & 4, as bond strength increased, cohesive fracture within resin cement was observed simultaneously. And, in group 3, as bond strength decreased, cohesive fracture between hybrid layer and composite resin cement was also observed. Cohesive fracture within dentin and porcelain adhesive fracture were not observed. In conclusion, although adhesive cements were used in CAD/CAM -fabricated ceramic inlay restorations, the conservative priciples of cavity preparation must be obligated.

치근단 방사선 사진에서 Paewinsky 연령추정법 적용에 대한 연구 (The Application of Paewinsky et al.'s Age Estimation Method to Periapical Radiographs)

  • There have been many radiographic studies on age estimation that evaluate reduction in size of dental pulp cavity with secondary dentin formation. The Paewinsky method reported high accuracy in estimating ages by measuring the width of the pulp cavity in panoramic radiographs. The aim of this study was to evaluate the application of the Paewinsky method to digital periapical radiographs. This study was conducted on 103 cases that reported to the Section of Human Identification of the National Forensic Service. The age was calculated by applying the Paewinsky method that measures the root and pulp canal width at three points in a tooth. The estimation results were compared with those calculated by the Johanson method. When the Paewinsky models were applied to digital periapical radiographs, the errors were significantly greater as compared to the original study. The errors of the maxillary second premolar and mandibular lateral incisor were greater than those of the maxillary central incisor, lateral incisor, mandibular canine, and first premolar. Furthermore, errors of the age estimation models in level C were greater than those in levels A and B. This study could be a reference for the application of the Paewinsky method to digital periapical radiographs.

Dynamic Threshold MOS 스위치를 사용한 고효율 DC-DC Converter 설계 (The design of the high efficiency DC-DC Converter with Dynamic Threshold MOS switch)

  • 본 논문에서는 DTMOS(Dynamic Threshold voltage MOSFET) 스위칭 소자를 사용한 고 효율 전원 제어 장치 (PMIC)를 제안하였다. 높은 출력 전류에서 고 전력 효율을 얻기 위하여 PWM(Pulse Width Modulation) 제어 방식을 사용하여 PMIC를 구현하였으며, 낮은 온 저항을 갖는 DTMOS를 설계하여 도통 손실을 감소시켰다. 벅 컨버터(Buck converter) 제어 회로는 PWM 제어회로로 되어 있으며, 삼각파 발생기(Saw-tooth generator), 밴드갭기준 전압 회로(Band-gap reference circuit), 오차 증폭기(Error amplifier), 비교기(Comparator circuit)가 하나의 블록으로 구성되어 있다. 삼각파 발생기는 그라운드부터 전원 전압(Vdd:3.3V)까지 출력 진폭 범위를 갖는 1.2MHz 발진 주파수를 가지며, 비교기는 2단 연산 증폭기로 설계되었다. 그리고 오차 증폭기는 70dB의 DC gain과 $64^{\circ}$ 위상 여유를 갖도록 설계하였다. Voltage-mode PWM 제어 회로와 낮은 온 저항을 스위칭 소자로 사용하여 구현한 DC-DC converter는 100mA 출력 전류에서 95%의 효율을 구현하였으며, 1mA이하의 대기모드에서도 높은 효율을 구현하기 위하여 LDO를 설계하였다.

차폐막 노출이 발치 후 치조제 보존술의 결과에 미치는 영향에 관한 임상적 연구 (The influence of membrane exposure on post-extraction dimensional change following ridge preservation technique)

  • Purpose: Following tooth extraction caused by severe periodontitis, alveolar ridge dimension lose their original volume. To reduce the alveolar ridge dimension, the ridge preservation technique has been introduced and tested in many clinical studies with membrane alone or membrane plus graft, achieving reduced ridge loss compared to extraction only. The aim of the present clinical study was to compare the post-extraction dimensional changes in the membrane exposure group to non-exposure group during healing period following ridge preservation technique. Methods: Ridge preservation was performed in 44 extraction sites. After extraction, deproteinized bovine bone mineral coated with synthetic oligopeptide (Ossgen-$X15^{(R)}$) or deproteinized bovine bone mineral (Bio-$Oss^{(R)}$) was implanted into the socket. A collagen membrane (Bio-$Gide^{(R)}$) was trimmed to cover the socket completely and applied to the entrance of the socket. Four clinical parameters were compared between baseline and 6 months. Results: During healing period, membrane exposure was observed at 19 sites. At the re-entry, hard newly formed tissue were observed at the ridge preservation site. The grafted socket sites were well preserved in their volume dimension. In both groups, horizontal ridge width was reduced and vertical height was increased. There were not statistically significant differences in horizontal (-1.32 mm vs -1.00 mm) and vertical ridge change (2.24 mm vs 2.37 mm at buccal crest, 1.36 mm vs. 1.53 mm at lingual crest) between two groups. Conclusions: The ridge preservation approach after tooth extraction effectively prevented resorption of hard tissue ridge in spite of membrane exposure during healing period.

Precision-TPA를 이용한 구치부 회전시 근심 및 원심삽입법에 따른 동적인 치아이동 양상에 관한 실험 연구 (An experimental study on the dynamic teeth movement of 3 types of the insertion method of Precision-TPA for derotating the posterior teeth)

  • 회전된 구치를 개선시키기 위해 사용하는 precision-TPA(transpalatal arch)를 삽입 방법들-근심삽입법, 원심삽입법, 확장력을 부여한 근심삽입법(이하 근심확장법) -에 따라 동적인 치아이동의 양상과 치열궁의 변화를 비교하기 위해 본교실에서 개발, 제작한 Calorific machine을 이용하여 실험한 후 회전된 치아와 고정치간의 이동양상을 계측하여 통계처리한 결과 다음과 같은 결론을 얻었다. 1. 구치간 폭경은 근심삽입법에 의해 감소하였고, 원심삽입법과 근심확장법에 의해서는 증가하였다. 2. 원심삽입법에 의해서 치열궁 장경이 가장 증가하였고, 근심확장법, 근심삽입법 순으로 증가하였다. 3. 근심삽입법의 회전중심은 근심소와(mesial pit)와 중심소와(central pit)사이에 위치하였고, 원심삽입법에서는 원심소와(distal pit)와 원심설측교두(distolingual cusp)사이에, 근심확장법은 원심소와(distal pit)에 위치하였다.

임플란트 환자의 분포 및 식립부 유형 (The Distribution of Implant Patients and the Type of Implant Site)

  • Nowdays, the awareness of implant treatment has grown rapidly among dentists and patients alike in Korea, as it becomes a widely accepted treatment. The reason is that unlike crown and bridge or denture treatment, implant treatment helps preserve existing bone and improve masticatory functions. So, It is needed understanding about the type, distribution of implant patient. The following results on patient type and implant distribution were compiled from 4433 implant cases of 1596 patients treated at the periodontal dept. of Y University Hospital during 1992 to 2004. 1. There are no dissimilarities between men and women, with patients in their 40, 50s accounting for 52.5% of patients and 57.5% of implant treatments; the largest share of patients and implant treatments. 2. Mn. posterior area accounted for 54.9% of implant treatments followed by Mx. posterior area(27.6%), Mx anterior area(11.9%) and Mn anterior area(5.6%). 3. Partial edentulous patients treated by single crown and bridge-type prosthesis accounted for 97.5% and fully edentulous patient accounted for the remaining 2.5%. 4. The major cause of tooth loss is periodontal disease, followed by dental caries, trauma and congenital missing. Also, older people are more likely to suffer from tooth loss due to periodontal disease rather than dental caries. 5. In the distribution of bone quality for maxillae, type III was most, followed by type II, r type IV and r type I. As for mandible, type II was most, followed by type III, type IV and for type I. 6. In the distribution of bone quantity for maxillae, type C was most, followed by type B, type D, type A, and for type E. As for mandible, type B was 52% most, followed by type C, type D, type A and type E. 7. The majority of implants were those of 1O-14mm in length (85.2%) and regular diameter in width (64%). The results provided us with basic data on patient type, implant distribution, bone condition, etc. We wish that our results coupled with other research data helps assist in the further study for better implant success/survival rates, etc.

매복된 상악 견치의 자가치아이식을 통한 치험례 (AUTOTRANSPLANTATION OF IMPACTED MAXILLARY CANINES)

  • 매복치의 적절한 맹출 유도를 위해서는 유치의 상태, 치조골과 치아와의 상관관계, 환자의 치령, 치료에 대한 환자의 협조도 및 경제적 상태의 고려 등 많은 요소에 좌우되는 복잡한 치료계획이 필요하다. 만약 맹출장애의 원인이 발견되었다면, 맹출 유도를 위해서는 원인의 제거와 다양한 술식이 사용될 수 있다. 특히 치아이식법은 매복된 치아의 위치가 외과적인 노출과 후 교정적인 견인이 어려운 곳에 존재하는 경우, 발치에 앞서 사용할 수 있는 유용한 방법이다. 본 증례는 매복 상악 견치의 자가이식을 시행한 3증례를 보고한 것으로, 자가이식 치료 후 관찰 결과 다음과 같은 결론을 얻었다. 공여 치아 제거시 가해지는 외상이 최소가 되게 하여야 한다. 치근단이 완성된 치아를 이식할 경우에는 치수괴사의 가능성이 높으므로 치수치료를 고려하여야 한다. 복잡한 매복증례에서 교정치료시 소요되는 경비 및 시간을 고려하여 치아이식을 시행할 수도 있다. 수용부의 치조와는 공여 치아의 치근보다 약간 커야 하며, 공여치아의 치근 크기와 치조골의 순설측 폭경에 따라 open 또는 closed procedure로 형성할 수 있다.
