교정력 및 외인성 전류가 고양이 치조골의 prostaglandin $E_2$에 미치는 영향에 관한 연구 (A STUDY ON THE EFFECT OF ORTHODONTIC FORCES AND EXOGENOUS ELECTRIC CURRENTS ON $PGE_2$ CONTNET OF ALVEOLAR BONE IN CATS)

  • This experiment was performed to explore the effect of electric currents and orthodontic forces on bone $PGE_2$ content and orthodontic tooth movement on cats. Stainless steel electrodes were connected a power pack consisting of five miniature batteries, a transistor, and a resistor. The current $(10{\pm}2{\mu}A)$ was provided by a constant source encased in a palatal acrylic plate. In first experiment, the cathode was placed mesial to the right maxillary canine tooth and the anode was positioned distal to the tooth, Sham electrodes were placed new the left cuspid, to serve as control. Nine cats were divided into three groups evenly. Groups of three animals were treated with electric currents only-for 1, 3 and 7 days, respectively. In second experiment, electric currents and the orthodontic forces of about 80 gm were applied to the right maxillary canine, and the orthodontic forces only were applied to the left maxillary canine. 3 groups of three cats each were treated in this experiment-for 1, 3 and 7 days, respectively. Alveolar bone samples were obtained from sites of tension and compression as well as from contralateral sites. Bone samples were extracted by homogenization in $40\%$ ethanal. The supernatant partitioned twice with 2 volumes of petroleum ether to remove neutral lipids and the aqueous supernatant partitioned in ethyl acetate. After drying the solvent, $PGE_2$ was measured by radioimmunoassay technique. The obtained results were as follows. 1. Teeth treated with combined force and electricity moved faster than those treated with force alone. 2. Alveolar bone $PGE_2$ content of electric stimulation was increased at both electrodes. 3. Alveolar bone $PGE_2$ content of mechanical stimulation at compression sites was gradually increased at all time period. At tension site, $PGE_2$ content increased after 1 day of mechanical stimulation remained elevated at all time period. 4. Alveolar bone $PGE_2$ content of compression sites was increased more than that of tension sites from mechanical stimulation as well as electrical stimulation.

치아재식시 골형태형성단백이 치주조직 치유에 미치는 영향 (EFFECTS OF BONE MORPHOGENETIC PROTEIN ON THE HEALING OF PERIODONTIUM AFTER TOOTH REPLANTATION OF THE RAT)

  • The purpose of this study is to evaluate the influences of the bone morphogenetic protein (BMP) on the healing of periodontal ligament and alveolar bone after replantation of tooth, and to examine the possibility of its clinical application. 45 Sprague Dawley rats weighted about 100 gram were divided into 3 experimental groups by different dose of BMP. All the upper right and left 1st molar were extracted after 5 days feeding of 0.4% ${\beta}$-aminopropionitrile, and right molar were used as experimental group and left molar were used as control group. The root surface of experimental molar were treated with 25,50 and l00ng/ml of human recombinant Bone morphogenetic protein-4 (rh-BMP-4) with micropipet, and 1M Sodium hypochloride were used on control root surface. All the experimental animals were sacrificed as 1, 2, 4, 7 and 14 days after autoreplantation of upper 1st molar into their own position. The maxilla were disected included both side of 1st molar. The collected tissue were processed from demineralization to paraffin embeding as usual procedure, and the specimens were prepared with Hematoxylin-Eosin stain for the light microscopic evaluation. The results of this study were as follows : 1. There was no significant differences between control and experimental site on 1 and 2 days after replantation of tooth. In the case of 4th days, the evidence of tissue regeneration were observed on experimental site to compare the controls. New osteoid were revealed on high concentration of BMP at 7 days after replantation, and it became more obvious at 14 days, 2. The effect of the rh-BMP-4 coated on root surface was revealed slight influences for the prolifertion of cells of periodontium and tissue regeneration as dose-dependent pattern. 3. Bony ankylosis was observed between alveolar bone and root surface due to the remarkable amount of osteoid formation on the 14 days after replantation of root. In the conclusion, it was suggested that topical application of the rhBMP-4 on the root surface has influence on the periodontal ligament and alveolar bone. The application method of BMP on the root should be designed with calculation of proper concentration.

치주 지지가 감소된 소구치에서 포스트가 치근 응력 분포에 미치는 영향에 대한 3차원 유한요소법적 연구 (STRESS DISTRIBUTION OF PERIODONTALLY INVOLVED TEETH RESTORED WITH VAR10US POSTS -THREE-DIMENSIONAL FINITE ELEMENT STUDY-)

  • Statement of problem: The endodontically treated tooth is generally restored with post and core, owing to the brittle and the loss of large amount of tooth structure. As periodontal treatment was developed, there are many cases that periodontally involved teeth used in prosthetic treatment. Purpose: The purpose of this study was to analyze the stress distribution in the dentin and post structures by the various post materials and the amount of remaining alveolar bone height. Material and method: The 3-dimensional finite element models of mandible 1st premolars were divided into six types according to the various amount of remaining alveolar bone and post type. All types were modeled using equal length, diameter and shape of the post. Three types of post and core materials were used: prefabricated titaniumpost and amalgam core, prefabricated stainless steel post and amalgam core, and cast gold post and core. 300 Newton force was applied to functional cusp of mandible 1st premolar. Results: The results were as follows: First, there was no apparent difference in the pattern of stress distribution according to the alveolar bone condition concentrate on the post middle area. Second, there was difference in pattern of stress distribution according to the core materials, gold post and core generated same than amalgam core. Third, there was no apparent difference in the pattern of stress distribution within the dentin according to the post and core materials. But a cast gold post and core generated the lowest maximum stress value, a stainless steel post generated the highest maximum stress value. Fourth, in the reduced alveolar bone model, maximum stress value is 1.5 times than that of the normal alveolar bone model. Conclusion: Within the limitations of this study, to provide minimal stress to the root with alveolar bone reduced, the post length may be as long as apical seal was not destroyed. To prevent fracture of tooth, it is rational to use gold alloy which material was good for stress distribution for post materials.

Current Methods for the Treatment of Alveolar Cleft

  • Alveolar cleft is a tornado-shaped bone defect in the maxillary arch. The treatment goals for alveolar cleft are stabilization and provision of bone continuity to the maxillary arch, permitting support for tooth eruption, eliminating oronasal fistulas, providing an improved esthetic result, and improving speech. Treatment protocols vary in terms of the operative time, surgical techniques, and graft materials. Early approaches including boneless bone grafting (gingivoperiosteoplasty) and primary bone graft fell into disfavor because they impaired facial growth, and they remain controversial. Secondary bone graft (SBG) is not the most perfect method, but long-term follow-up has shown that the graft is absorbed to a lesser extent, does not impede facial growth, and supports other teeth. Accordingly, SBG in the mixed dentition phase (6-11 years) has become the preferred method of treatment. The most commonly used graft material is cancellous bone from the iliac crest. Recently, many researchers have investigated the use of allogeneic bone, artificial bone, and recombinant human bone morphogenetic protein, along with growth factors because of their ability to decrease donor-site morbidity. Further investigations of bone substitutes and additives will continue to be needed to increase their effectiveness and to reduce complications.

Mouse의 치아 발육시 Runx2의 발현 양상 (EXPRESSION PATTERN OF RUNX2 IN MURINE TOOTH DEVELOPMENT)

  • Runx2는 runt gene family에 속하는 전사조절 인자로써 뼈의 형성과 골아세포의 분화에 중요한 역할을 담당하고 있다. Runx2-haploinsufficency는 쇄골의 저형성 및 두개 봉합의 지연을 특징으로 하는 쇄골두개 이형성증을 일으키며, 치아에 있어서는 법랑질의 저형성, 영구치 맹출지연 등을 보인다. 이에, 치아의 발육 및 맹출에 미치는 Runx2의 영향을 알아보기 위해 in situ hybridization 방법으로 태생 1, 4, 7, 14, 21일 된 쥐의 하악 및 제1대구치를 사용하여 실험을 실시하였다. Runx2-full length는 태생 1일과 4일에 치낭 및 그 주위조직에 보이지만 Runx2-typeII는 보이지 않았다 Runx2-full length는 태생 7일에 치관 교합면 부위의 법랑모세포에 발현하였고, 1주일 후인 태생 14일에는 백악법랑경계 상방의 치관인접면 법랑모세포에서 발현되었다. 이에 반해 Runx2-typeII는 법랑모세포에서 발현하지 않았다. 또한 태생 21일에서는 두 가지 이성질체가 모두 하악골에서 발현을 보였다 이런 결과를 종합해볼 때, Runx2-full length는 치아의 맹출과 연관이 있으며, 법랑모세포의 분화 및 이로 인한 법랑질형성에 영향을 주지만 Runx2-typeII는 하악골의 형성에만 영향을 미치는 것으로 사료된다.

외상으로 유착된 영구 전치에서의 치관 절제술 (DECORONATION ON ANKYLOSED PERMANENT INCISOR AFTER DENTAL TRAUMA)

  • 외상으로 치주 인대의 손상이 심한 치아에서는 치아-치조골 유착이 흔히 나타난다. 성장기 아동에서의 치아 유착의 경우 치조골의 성장 중단을 초래하여 저위 교합이 나타나며 이로 인하여 청소년기에 심미적 불만족이 생길 뿐 아니라 인접치의 경사가 발생한다. 이는 교정 또는 보철 치료를 어렵게 만들 수 있으므로 유착된 치아는 비교적 조기에 치료한다. 하지만 저위 교합된 치아를 발치할 경우 치조제의 상당한 소실을 유발할 수 있으며 특히 상악 전치부의 얇은 순측골의 위축을 가져와 심미적인 보철물을 형성하기가 어려워진다. 이러한 치조골의 소실을 방지하기 위하여 치아의 치관부는 제거하고 유착된 치근은 골에 의해 대체되도록 치조골에 남겨두는 치관절제술을 행하는 것이 바람직할 것으로 생각된다. 본 증례에서는 만 9세와 10세에 상악 전치부 치아의 외상을 받은 후 치근 유착이 발생하여 저위 교합을 보이는 성장기 남아에서 추후 예상되는 보철적, 교정적 합병증의 가능성을 감소시키기 위하여 치관절제술을 시행하였다. 저위교합이 2-3 mm 정도 진행되었을 때 술식을 행하였으며, 환아의 성장을 고려하여 한국인의 최대 성장 완료 시기인 16세 이전에 술식을 행하였다. 계속적인 임상적, 방사선학적 검사 결과 수평적 치조골의 소실이 방지될 뿐 아니라, 치조골의 수직적 증가를 보이며, 남겨진 치근은 서서히 흡수되는 양상을 보였기에 이를 보고하고자 한다.

실험적 치조백선 병소부에 관한 방사선학적 연구 (A RADIOGRAPHIC STUDY ON THE ARTIFICIALLY INDUCED LESIONS INVOLVING LAMINA DURA)

  • This study was performed to simulate various sizes of periapical lesions and condensing osteitis in bone and to clarify that condensing osteitis would give a greater radiographic contrast because of the more radiopacity immediately adjacent to the lesion and what the lamina dura on radiogram is in bone. For this study, two dry and wet human adult mandible and a cadaver were used. Each mandible was sectioned longitudinally to split lingual and buccal cortical plates to gain better access to the apex of tooth. Lesions were gradually enlarged and bordered with tooth ash that burned out at 950℃ for the reproduction of condensing osteitis in sequence. The serial radiograms were taken under the constantly maintained condition and compared with the actual lesions.

  • In recent years, a number of special treatment procedures have been introduced to reestablish new tooth supporting tissues with varying degrees of success including guided tissue regeneration(GTR), bone grafting(BG) and the use of enamel matrix derivative(EMD). EMD is an extract of enamel matrix and contains amelogenins of various molecular weights. Emdogain(EMD) might have some advantages over other methods of regenerating the tissue supporting teeth lost by gum disease, such as less postoperative complications. Emdogain contains proteins(derived from developing pig teeth) believed to regenerate tooth attachment. The decrease in probing depth after EMD treatment is achieved primarily by clinical attachment gain and bone regeneration and only to a minor extent by gingival recession. In conclsion, EMD seems to be safe, was able to regenerate lost periodontal tissues in previously diseased sites based on clinical parameters.

성인 구순구개열환자의 교정치료 및 관리 (Orthodontic treatment and management of adult patient with cleft lip and palate)

  • Patients with cleft lip and palate require interdisciplinary treatment to achieve successful rehabilitation. However, there are special difficulties in orthodontic treatment of adult cleft lip and palate patients: 1. Lack of Tissue, Bone, and Soft tissue; 2. Heavy Scar Tissue, Vestibule, and Palate; 3. Severe Anteroposterior discrepancy and Impaired Maxilla; 4. Distortion of Alveolar Ridge; 5. Abnormal Eruption Path and Malalignment of Tooth. Solving these problems, orthodontist should have differential diagnosis on extent of cleft site and residual deformities of adult cleft lip and palate patient. The tooth missing area in cleft site was commonly treated with a removable or fixed prosthesis, but this method is not stable to retain maxillary arch shape. To establish the more stable arch shape in cleft lip and palate, endosseous implants in the alveolar clefts with bone graft is helpful for management of adult cleft lip and palate patient.

Immediate implant placement in fresh extraction sockets

  • Immediate implant placement (IIP) in fresh extraction sockets exhibits similar survival and success rates to delayed implant placement in healed sockets. Several advantages of IIP involve shortened total treatment time, reduction of the number of invasive surgeries, and subsequent reduction of patient discomfort due to lack of additional surgeries. The major shortcomings in IIP, however, include the inability to obtain early bony support, presence of a gap between the extraction socket and fixture, and the inability to cover the fixture with soft tissue, leading to increased risk of infection and implant loss. When IIP is performed, atraumatic or minimally traumatic extractions, conservation of the septal bone in molars, minimal flap elevation or flapless surgery, bone grafting the gap between the fixture and the extraction socket, and coverage with soft tissue or a membrane must be considered.
