Restoration of patient accelerating anterior teeth wear by loss of posterior support: Case report (구치부 지지 소실에 의한 전치부 마모 가속화 환자의 수복: 증례보고)

  • Increased anterior teeth mastication following posterior teeth loss leads to greater anterior occlusal force. It may cause greater attrition of anterior teeth, traumatic force occlusion (TFO), also often followed by antagonist extrusion and occlusal disharmony. This clinical report describes the treatment for a 67-year-old female patient diagnosed with loss of both maxillary and left mandibular posterior teeth, severe attrition of maxillary and mandibular anterior teeth and extrusion of multiple teeth. A diagnostic cast was mounted on articular in centric relation (CR) position to evaluate vertical dimension (VD) and interspace. To provide adequate space for the prosthetic reconstructions, VD was increased by 3 mm on the anterior pin. And then diagnostic wax-up was completed upon that VD. Wax-up was converted to provisional restorations and verified in the patient's mouth and the final restorations were delivered. Clinical follow up examination held 3 months after temporary restoration owing to changes in vertical dimension revealed proper function in mastication without evidence of temporo-mandibular joint (TMJ) disorders. This clinical report presents successfully restoring severe attrition case with increasing vertical dimension resulting in satisfaction in esthetics and function.

  • Purpose: The purpose of this study was to examine the effect of tooth-brushing oral care on oral health and ventilator-associated pneumonia of patients in Intensive Care Unit (ICU). Methods: A total of 74 participants were recruited from a medical, surgical, or neurosurgical ICU at S Hospital in Seoul, Korea, from September of 2010 to January of 2011. An experimental group (n= 36) received oral care with tooth-brushing while a control group (n=38) received oral care with cotton ball and gauze. In both group, the oral care was done three times a day at least one minute for 7 days. Oral health was examined by the Modified Oral Assessment Guide before the intervention and each night. Results: There is no difference between the groups in aspect of passage of time (p = .603), interaction between time and intervention (p =.300), the performance intervention (p = .766), and the incidence of VAP (p = .486). Conclusion: The effect of oral care with tooth-brushing on oral health and VAP was not different from that of usual oral care in ICU. However, further study is necessary due to high attrition in this study.

  • Talon cusp is cusp-like structure that develops additionally in crown of anterior tooth. And it is one of dental anomalies. Talon cusp can cause various diagnostic, functional, aesthetic problems depending on the size and configuration of cusp. there is a tendency for caries to occur in the developmental grooves, and advanced attrition, periodontal problems, irritation the tongue and temporomandibular pain, occlusal interference and displacement of affected tooth may result. Therefore, early diagnosis and appropriate treatment of each case is important so that it minimize local problems, such as caries, periodontal diseases, and malocclusion. These cases which are in permanent and primary anterior teeth are about gradual reduction forming reparative dentine and complete reduction of talon cusp and root canal therapy which is an alternative and effective form of treatment when gradual reduction of talon cusp may not be possible.

  • Dens evaginatus is an anomalous tooth development arising during morphodifferentiation. It is most often reported in premolar, and familial occurrence has been reported. The primary dental complication of dens evaginatus is fracture or wear of the tubercle which leads to pulp exposure, pulpal necrosis and periapical infection. Pathosis of the pulp can occur before complete root formation with cessation of root development. A rational and conservative approach to the management of dens evaginatus in vital teeth includes early diagnosis and treatment to prevent fracture or attrition of the tubercle. This treatment would include careful sequential grinding, pulp capping, preventive resin restoration. When presented with a case of dens evaginatus in a nonvital tooth with incomplete root development, the treatment of choice has been extraction, apexification. We report two cases of dens evaginatus that appear in two sisters. In the elder, a periapical lesion on radiographs is shown, and it is treated by calcium hydroxide apexification. The other sister is early recognized of dens evaginatus, it is treated using glass ionomer cement reinforcing with sequential grinding.

Full mouth rehabilitation of the patient with severely worn dentition and limited vertical dimension (심한 치아 마모와 수복공간 부족을 보이는 환자에서의 완전 구강회복 증례)

  • Severely worn dentition causes various complications such as loss of tooth structure, discoloration, pulp complications and loss of function and aesthetics. In this case, the patient showed particularly severe attrition in the anterior teeth and lack of space for restoration. The amount of vertical dimension was determined based on the diagnostic wax up, and the patient's adaptation was evaluated by using a removable occlusal splint for 6 weeks. Thereafter, the coordination of the muscular nervous system, aesthetics, temporomandibular joint were re-evaluated for 3 months by restoring the fixed provisional restoration. Through the above treatment process, the final restoration was completed with full mouth fixed prosthesis using monolithic zirconia, and functionally and aesthetically stable results were obtained.

  • The purpose of this study was to evaluate the stability of external stain that long term attrition caused by tooth brushing affected the color variation of external stain of IPS e.max Press Porcelain. The specimen was made by IPS e.max Press LT ingots, and treated the shade of external staning as Orange, A, B, C, D After conducting horizontal toothbrush strokes about 11,000 / 22,000 / 44,000 / 66,000 / 88,000, Color changers of the external stained layer was measured with SpectroShade$^{tm}$ MICRO(MHT, Italy). The result of this study was obtained as follows: 1. After 11,000 strokes, the color changes of shade were greater in Orange shade than the other 4 shade groups. And even though toothbrush strokes were increased until 88,000, there were no statistical significant color changes about the external stain shade variation. 2. The amount of color variation about increasing of toothbrushing strokes was irregular(no consistent increasing, or decreasing), and it was no statistical significant changes According to these results, the long term tooth-brushing doesn't affect the changes of external stain about IPS e.max press porcelain. And it doesn't reduce the stability of external stain clinically. Later, it will be needed to study other factors affecting the external stain.

  • Tooth wear refers to the loss of dental hard tissue caused by various physiological and pathological causes, and excessive pathological wear can cause complications such as pathological changes in dimensions, occlusal disharmony, loss of function, and aesthetic problems. The cause of tooth wear can be caused by attrition, abrasion, corrosion and abfraction, and it is known to act in a multifactorial etiology in interocclusal activity. In patients with excessive pathological wear, it is important to determine whether or not the vertical dimension of occlusion is reduced, and complete oral rehabilitation should be achieved with the adaptation of the neuromuscular and temporomandibular joint through accurate diagnosis and analysis. The patient in this case was a 63-year-old male patient, who presented discomfort to cold beverage due to severe tooth wear. After analysis of the patient's vertical dimension of occlusion, a full mouth rehabilitation was performed with increasing vertical dimension of occlusion. The goal of treatment was to improve the occlusal plane with the equal-intensity contact of all teeth, harmonious anterior guidance and immediate disclusion of all posterior contacts. After rehabilitation, the patient was satisfied with function and esthetic appearance.

The influence of Advanced Adult Periodontitis on the pulp (진행성 치주염이 지수 조직에 미치는 영향)

  • The purpose of this study was to observe histopathologically the influence of advanced periodontitis on pulp tissue, and to conclude the correlation between the results with clinical manifestations. The samples were teeth with over 7mm pocket depth and over 50% radiographic bone loss. These were diagnosed to have very poor prognosis and thus planned to be extracted. Those with any of following conditions were excluded from the samples, loss of vitality, periapical pathology, restoration or prosthesis, dental caries, and attrition or abrasion. It was because these conditions could affect pulp without any correlation with periodontal disease. For the experiment, 17 teeth from 11 patients were selected. Average age of patient was 47. Each tooth was examined for following categoris; pocket depth, gingival recession, electric pulp test, mobility, percussion test, sensitivity test. The extracted teeth were fixed buffered neutral formalin solution. It was decalcified using 4% nitric acid. Sliced histological samples observed using light microscope, for pulp status, and severeity of inflammation. 4 samples were excluded due to histologic sample discrepency. Thus 13 samples were subject to observation. 4 showed normal conditions. Focal reversable pulpitis was shown in 5 samples. Chronic pulpitis was observed 1 sample. Pulpal abscess was observed in 3 samples.

  • The Purpose of this study was to know the differences between the occlusal plane angles formed by F-H plane on cephalograms and the occlusal plane angles by the upper margin of the articulator, and to ind the contributing factors to the difference of occlusal angles. for this study, 39 young adults (20 men, 19 women) were selected who had normal occlusion, no severe attrition, no missing tooth, not been under orthodontic treatment and occlusal equilibration and no temporomandibular disorders. The maxillary casts were mounted with Hanau 159-1 ear piece type face-bow and measured the occlusal plane angle with prototype occlusal plane projector. The following results were obtained; 1. The mean value of the ear rod F-H plane occlusal angle was 14.75˚, articulator occlusal angle was 9.26˚ and strong positive correlation between these angles, 2. It was almost same angle between the ear rod F-H plane angle (14.75˚) and the beyron point to infraorbital rim-6mm. occlusal angle (14.46˚) 3. The mean distance from the ear rod porion to the Byron point was 5.67mm. 4. It was no significant correlation between the distances Po-Beyron point, Or-infraorbital rim and articulator occlusal angle.

  • Pathologic attrition leads to pathologic damage on occlusal plane, functional disorders, occlusal disharmony, esthetic problems, pulpal lesion, temporomandibular joint (TMJ) disorder. In this case, treatment plan should be considered for possibility of vertical dimension loss, occlusal pattern, esthetics, phonetics, amount of vertical dimension increase. This case report was a 71-year-old man who had severely worn dentition. Full mouth rehabilitation was carried out with vertical dimension increase due to limited space for prosthesis. After evaluation of provisional restorations for patient's compliance, final restorations were fabricated and routine clinical assessments were made. This case presents that a satisfactory clinical result was achieved by restoring the worn dentition.
