The infraocclusion usually occurs in the mixed-dentition stage, and it is commonly accepted that the cause of the infraocclusion is ankylosis. The treatment options for patient with infraocclusion of primary molars are observation, restoration or surgical removal of the affected teeth. If the successors are present, most of the infraoccluded and ankylosed primary molars may occur normally. However, when the permanent successors are absent and the progression of infraocclusion is found, affected teeth may need to be extracted. In the case of infraocclusion which can cause vertical alveolar defect due to ankylosis, extraction before growth spurt should be performed for the future prosthetic treatment. A six-year-old female had the ankylosis and infraocclusion of multiple primary molars and congenital missing of premolars. The affected primary molars were extracted before growth spurt to avoid a significant vertical ridge defect and to promote the vertical development of alveolar bone, and the result was observed for many years. The purpose of this report is to report the management of multiple infraoccluded primary molars without permanent successors in a young patient.

The developmental process of a tooth is being presented as an important study subject to analyze formation of normal dental arch and tooth. The purpose of this study was to see the formation of tooth from a tooth bud transplanted in a white rat regarding that the jawbone could be used as a new donor site of a trasplant. The first molar of a matured white rat was extracted and the tooth bud of a 13.5 day rat embryo was transplanted. The histological and radiographical results after 4 and 8 weeks respectively are as the following. 1. Calcification in dentin, cementum, pulp and periodontal ligament was formed from the tooth bud transplanted in the alveolar socket. 2. The development of hard and soft tissue was delayed compared to the normal tooth formation and abnormal histologic features such as ankylosis and osteodentin were found. 3. The formed hard tissue did not erupt into the jaw within 8 weeks.
Impaction with a severely dilacerated root is seldom reported, especially in the maxillary incisor. It is probably because of the high clinical difficulty associated with bringing the dilacerated tooth into proper position, and the high chance of failure due to ankylosis, external root resolution, and root exposure after orthodontic traction. Even the successful cases may need periodontal surgery to improve the unesthetic gingival shape. However, it has previously been reported that an impacted maxillary central incisor was successfully treated by proper crown exposure and orthodontic traction. This article presents a case of an invertedly impacted maxillary right central incisor with a developing dilacerated root, which was aligned into proper position after orthodontic traction composed of two stages of a closed eruption technique.

Most of impacted teeth reported are permanent teeth. Impaction of primary tooth caused by primary failure of eruption is rare and the most of impacted primary tooth are second primary molars. The etiology of a primary molar impaction would appear to be a physical barrier, early ankylosis of the tooth, abnormal development of the primary molar germ or malposition of the successor bud. Transposition has been described as an interchange in the position of two teeth within the same quadrant of the dental arch. It is most commonly seen with canine and lateral incisor teeth, but is rarely associated with the primary dentition. The two cases show the transposition of impacted primary molar and the tooth germ of second premolar in their clinical and radiographic examinations. In case 1, lower second primary molar was deeply impacted inferiorly to the tooth germ suspected to be a second premolar. In case 2, impacted all second primary molars were positioned inferiorly to the tooth germs suspected to be the successors. We extracted all second primary molars under general anesthesia and the extracted tooth germs appeared to be second premolars.

Extraction of impacted third molars is a routine procedure performed by oral and maxillofacial surgeons. However, extractions in middle-aged or elderly individuals are not easy, and there are several factors that need to be considered. These factors include decreased healing potential and the risk of complications increasing with age. In addition, third molars can often be fully or deeply impacted in middle-aged individuals, and pathologic changes, such as cysts, caries, or periodontitis of the adjacent second molar, can develop. Furthermore, the rate of ankylosis and systemic disease increases after middle age. It is considered that these factors acting alone or in combination increase the difficulty of extraction.

Traumatic intrusive luxation, an occurrence common in the primary dentition but one that occurs rarely in the permanent dentition, has a poor prognosis. There have been many treatment approaches such as allowing the tooth to reerupt spontaneously, surgical repositioning and immediate luxation, surgical luxation, and orthodontic repositioning; but all have their own drawbacks. Meanwhile, Turley et al. (1987) have proposed surgical and orthodontic combination therapy to treat intrusion. Surgical and orthodontic combination therapy means to apply the orthodontic traction force immediately after surgical luxation. If ankylosis occurs, orthodontic force may be applied after re-luxation repeatedly. But in cases of complete intrusive luxation, it would be not feasible to bond an orthodontic button or bracket on the tooth directly. Thus, in this case, traction of the tooth was attempted after surgically repositioning it close to the probable original socket site to promote better healing.

Objective: This study aimed to investigate the effect of pre-applied orthodontic force on the regeneration of periodontal ligament (PDL) tissues and the underlying mechanisms in tooth replantation. Methods: Orthodontic force (50 cN) was applied to the left maxillary first molars of 7-week-old male Sprague-Dawley rats (n = 32); the right maxillary first molars were left untreated to serve as the control group. After 7 days, the first molars on both sides were fully luxated and were immediately replanted in their original sockets. To verify the effects of the pre-applied orthodontic force, we assessed gene expression by using microarray analysis and real-time reverse transcription polymerase chain reaction (RT-PCR), cell proliferation by using proliferating cell nuclear antigen (PCNA) immunofluorescence staining, and morphological changes by using histological analysis. Results: Application of orthodontic force for 7 days led to the proliferation of PDL tissues, as verified on microarray analysis and PCNA staining. Histological analysis after replantation revealed less root resorption, a better arrangement of PDL fibers, and earlier regeneration of periodontal tissues in the experimental group than in the control group. For the key genes involved in periodontal tissue remodeling, including CXCL2, CCL4, CCL7, MMP3, PCNA, OPG, and RUNX2, quantitative RT-PCR confirmed that messenger RNA levels were higher at 1 or 2 weeks in the experimental group. Conclusions: These results suggest that the application of orthodontic force prior to tooth replantation enhanced the proliferation and activities of PDL cells and may lead to higher success rates with fewer complications.

This study aimed to analyze the prevalence and severity of infraoccluded primary molars (IPM) and their correlation with the agenesis of permanent successors (APS). A total of 4,011 panoramic radiographs from children aged 4 to 11 years who had all 8 primary molars were examined. The prevalence of APS was analyzed based on the severity and tooth type of IPM. The prevalence of IPM was 13.0%, and there was no difference between genders. The majority of the children had two IPMs each. In all tooth types, mild infraocclusion was the most prevalent, whereas severe infraocclusion was the least frequent. Among the types of primary molars, the lower primary first molar (62.3%) was the most affected, followed by the lower primary second molar (27.7%), the upper primary second molar (5.8%), and the upper primary first molar (4.2%). Infraocclusion was 11.2 times more prevalent in the mandible than in the maxilla, with no significant difference between the left and right sides. The correlation between IPM and APS was also investigated. Among the subjects, 177 had one or more agenesis of premolars, of whom 54 (30.5%) had IPM. Additionally, among the 521 individuals with IPM, 54 individuals exhibited APS (10.4%). This study identified a noteworthy prevalence of infraocclusion, with notable variations among molar types. The LPFM was particularly affected. The majority of cases were classified as mild in severity. Furthermore, a compelling association between IPM and APS was established. Understanding this connection may enhance treatment strategies for infraoccluded teeth and tooth agenesis.

Impacted molars are not usual with the prevalence rate of less than 1%, however problems such as infraocclusion, extrusion of the opposite tooth and lack of space are caused. If molars are impacted, radiographic check-up and observations are needed as they play an important role in skeletal development and mastication. Causes of impaction are underdevelopment of roots, supernumerary teeth, cysts, odontomas, ankylosis and etc. In our first case, we present impacted molar which was surgically exposed and orthodontically traded. As the developmental state of the root was less than half at first visit, we observed until the root was 2/3 in its length and traction was carried out. The second case is orthodontic traded molar with compleate enucleation of the existing cysts. When trading impacted molars, direct bonding of attachments to the tooth and light continuous forces are recommended so as to ankylosis, external absorption and periodontal attachment loss avoid. Fixed appliances tract the teeth on arch and obtain eruption spaces without patient's cooperation. We report surgically exposed and orthodontically traded molars which resulted in good occlusion and patients satisfaction.

Tooth impaction refers to situations in which the eruption is inhibited by some physical barriers in eruptive path and the tooth remains unerupted beyond the normal time of eruption. The etiology of impacted tooth is controversial, but ankylosis has been suggested probably as a leading role. Impacted primary molars may cause several problems such as space loss, tipping of adjacent teeth, supra-eruption of the antagonists, dislocation of succeeding premolar, cystic change and infection. As one of conventional treatments of impacted primary molars, early tooth extraction or surgical extraction following space regaining when there is space loss has been suggested. However, when they are in normal formation and not ankylosed, orthodontic traction following surgical exposure can be the choice of treatment. In this case, a 3-year-old boy was referred to the department of pediatric dentistry for the unerupted mandibular right second primary molar. After surgical removal of gingiva on the occlusal surface, orthodontic traction was performed. After treatment, we could get normal alignment of primary teeth and the opportunity for normal development of permanent teeth.
