The purpose of this study is to observe the effect of formocresol and glutaraldehyde to tooth germs and periapical tissues after perforation of interradicular portion of pulpal floor and application of physiological saline solution in control groups, formocresol and glutaraldehyde in experimental groups. The following results were obtained 1. In control groups, normal healing processes were seen, and, on the sixteenth day, the epithelization of injured areas was completed. Inflammatory reactions were limited to the injured surface, and the underlying alveolar bone were normal and successive tooth germs were normal. 2. In both formocresol groups and glutaraldehyde groups, tissue reactions were identical. Inflammatory reactions were slightly compared with control groups, but the surface epithelizations were delayed compared with control group. 3. In both formocresol and glutaraldehyde groups, necrosis was seen in superficial tissue of bone marrow, and, at 24th day, center area of bone marrow on the successive tooth germs were losed and replaced with connective tissue, and superficial soft tissue of the injured area was connected with soft tissue on the successive tooth germ. In remaining alveolar bone, osteoclastic reaction was remarkable. 4. In both formocresol and glutaraldehyde groups, there is no injury to the successive tooth germs. 5. In both formocresol and glutaraldehyde groups, periodontal membrane was normal, but the partial resorption of cementum and dentin near the injured area were seen.
To study the effect of dentin permeability on a tooth with wear from tooth brushing after application of desensitizing agent, extracted teeth free from caries were chosen. Coronal dentin discs with thickness of 1mm were prepared. Using the split chamber device developed by Pashely, hydraulic conductance, scanning electron microscope images(SEM) and atomic force microscope images(AFM) were compared and contrasted before and immediately after the application of desensitizing agent and after equivalent tooth brushing of 1 week, 2 weeks, and 6 weeks. Four commercially available desensitizing agents were used in this study ; they were Gluma, Seal & Protect, All-Bond 2 and MS Coat. The results of this study are as follows. 1. On all specimens, the hydraulic conductance decreased after the application of tooth desensitizing agent. 2. Except the specimens treated with MS Coat, the remaining specimens had an increase in dentin permeability after tooth brushing for 1 and 2 weeks but a decrease after 6 weeks. 3. The specimens treated with MS Coat had statistically significant increase in the dentin permeability regardless with the duration of tooth brushing. 4. On examination of SEM and AFM, the dentinal tubule diameter had decreased after treatment of desensitizing agents. The specimens other than those treated with MS Coat, smear layers were noted after tooth brushing. It is not always consistant but the hydraulic conductance correlated with the images from SEM and AFM.
Background: A Teachable Machine is a kind of machine learning web-based tool for general persons. In this paper, the feasibility of Google's Teachable Machine (ver. 2.0) was studied in the diagnosis of the tooth-marked tongue. Methods: For machine learning of tooth-marked tongue diagnosis, a total of 1,250 tongue images were used on Kaggle's web site. Ninety percent of the images were used for the training data set, and the remaining 10% were used for the test data set. Using Google's Teachable Machine (ver. 2.0), machine learning was performed using separated images. To optimize the machine learning parameters, I measured the diagnosis accuracies according to the value of epoch, batch size, and learning rate. After hyper-parameter tuning, the ROC (receiver operating characteristic) analysis method determined the sensitivity (true positive rate, TPR) and specificity (false positive rate, FPR) of the machine learning model to diagnose the tooth-marked tongue. Results: To evaluate the usefulness of the Teachable Machine in clinical application, I used 634 tooth-marked tongue images and 491 no-marked tongue images for machine learning. When the epoch, batch size, and learning rate as hyper-parameters were 75, 0.0001, and 128, respectively, the accuracy of the tooth-marked tongue's diagnosis was best. The accuracies for the tooth-marked tongue and the no-marked tongue were 92.1% and 72.6%, respectively. And, the sensitivity (TPR) and specificity (FPR) were 0.92 and 0.28, respectively. Conclusion: These results are more accurate than Li's experimental results calculated with convolution neural network. Google's Teachable Machines show good performance by hyper-parameters tuning in the diagnosis of the tooth-marked tongue. We confirmed that the tool is useful for several clinical applications.
Purpose: Vitamin D deficiency may cause bone loss and increased inflammation, which are well-known symptoms of periodontal disease. This study investigated whether serum 25-hydroxyvitamin D (25(OH)D) levels are associated with periodontal disease status and tooth loss. Methods: Cross-sectional data from 5,405 individuals aged ≥50 years (2,253 males and 3,152 females) were obtained from the 2008-2010 Dong-gu study, a prospective cohort study of risk factors for chronic diseases. Periodontal examinations were conducted to evaluate the number of remaining teeth, the periodontal probing depth (PPD), the clinical attachment level (CAL), and bleeding on probing. The percentages of sites with PPD ≥4 mm and CAL ≥4 mm were recorded for each participant. The severity of periodontitis was classified using the Centers for Disease Control and Prevention and the American Academy of Periodontology case definitions. Serum 25(OH)D levels were classified as reflecting severe deficiency, deficiency, insufficiency, or sufficiency. Multivariate linear regression analysis was performed to assess the associations of serum 25(OH)D levels with periodontal parameters and the number of remaining teeth after adjusting for confounders including age, smoking status, alcohol consumption status, month of blood collection, and physical activity. Multivariate logistic regression was used to evaluate the association between serum vitamin D levels and severe periodontitis. An overall statistical analysis and a stratified analysis by sex were performed. Results: Overall, the rates of severe deficiency, deficiency, insufficiency, and sufficiency were 6.5%, 67.9%, 22.4%, and 3.2%, respectively. After adjustment for confounders, vitamin D levels were directly associated with the number of remaining teeth, an association that was significant in males, but not in females. Sufficient serum 25(OH)D was associated with a low frequency of severe periodontitis. Conclusions: This population-based cross-sectional study indicates that low serum 25(OH) D is significantly associated with tooth loss and severe periodontitis in Koreans aged 50 years and older.
The aim of this study was to measure the regional micro-shear bond strength of dentin bonding agents to dentin, and to investigate the relationship between the micro-shear bond strength and two dentinal characteristics ; Vickers hardness and remaining dentin thickness. Twenty-four freshly extracted, noncarious human molars were selected for this study. The materials tested in this study consisted of two commercially available dentin bonding agents (MAC-BOND, ONE-STEP) and two restorative light-cured composite resins (AELITEFIL, Z100). The occlusal or side surface of tooth crown was sectioned to expose dentin, and the exposed surface was finally polished with # 600 sandpaper. Four groups of application methods were used combining the filling materials and the dentin bonding agents. The composite resin-attached tooth specimens were embeded in a cold cure acrylic resin, and were cut with a low speed diamond saw to the dimension of 1mm $\times$ 1mm. Nine specimens were obtained from each tooth. The cut specimens were divided into three groups depending on the position of the dentin bonding surface. The micro-shear bond strength, remaining dentin thickness, and dentinal hardness were measured. Experimental results were then statistically analyzed with ANOVA. t-test, Scheffe test, and regression analysis. From this experiment, the following results were obtained : 1. In the case of occlusal surface bonding, the pooled micro-shear bond strength of ONST-AELIT group (16.62 MPa) was significantly higher than that of MACB-AELIT group (9.91 MPa) (p<0.05). However, there was no significant difference in the micro-shear bond strength depending on the dentin position (p>0.05). 2. In the case of side surface bonding of crown, the pooled micro-shear bond strength of four different bonding groups was not significantly different among each other (p>0.05). However, in three of the test groups (ONST-AELIT, MACB-Z100, ONST-Z100), the micro-shear bond strength to the lower 1/3(III) position was significantly lower than that to middle 1/3(II) position of surface (p<0.05). 3. In the ONST-AELIT bonding group, the pooled micro-shear bond strength to the occlusal surface was significantly lower than that to the side surface of crown (p<0.05). 4. There was no significant correlation between the micro-shear bond strength and dentin hardness / remaining dentin thickness (p>0.05).
유치열에서 드물게 관찰되는 삼중치는 세 치아의 비정상적인 융합으로 나타난다. 몇몇 문헌들에서 이중치에 대해 보고하고 있지만, 삼중치에 대한 보고는 매우 드물다. 이러한 다중치는 유치열에서 더 흔하게 관찰되며, 몇 가지 임상적 문제점을 나타낸다. 심미적인 문제 뿐만 아니라 치아 우식증 및 파절에 취약함, 영구 치배의 결손, 교정적, 치주적 문제를 야기할 수 있으며, 복잡한 치아 구조로 인해 치수 치료를 어렵게 한다. 첫 번째 증례는 만 1세 7개월 남아의 상악 유전치부에 나타난 삼중치로, 깊은 우식으로 치수 치료가 필요할 것으로 예상되었고, Mineral Trioxide Aggregate (MTA)를 이용하여 부분 치수 절단술을 시행하였다. 두 번째 증례는 만 1세 9개월 남아의 하악 유전치부에 나타난 삼중치로 파절을 주소로 내원하였다. 파절편 조각을 제거한 뒤, 잔존 치아에서 치수 절제술을 시행하였다. 각각 24개월, 6개월간의 정기 검진시까지 특별한 합병증은 나타나지 않았으며, 수복물 또한 비교적 양호하게 유지되었다.
Fracture of cusp, on posterior teeth, especially those carious or restored, is major cause of tooth loss. Inappropriate treatments, such as unnecessarily wide cavity preparations, increase the potential of further trauma and possible fracture of the remaining tooth structures. Fracture potential may be directly related to the stresses exerted upon the tooth during masticatory function. The purpose of this study is to evaluate the fracture resistance of tooth, restored with composite resin inlay. In this study, MOD inlay cavity prepared on maxillary first premolar and restored with composite resin inlay. Three dimensional finite element models with eight nodes isoparametric solid element, developed by serial grinding-photographing technique. These models have various occlusal isthmus and depth of cavity, 1/2, 1/3 and 1/4 of isthmus width and 0.7, 0.85 and 1.0 of depth of cavity. The magnitude of load was 474 N and 172 N as presented to maximal biting force and normal chewing force. These loads applied onto ridges of buccal and lingual cusp. These models analyzed with three dimensional finite element method. The results of this study were as follows : 1. There is no difference of displacement between width of occlusal isthmus and depth of cavity. 2. The stress concentrated at bucco-mesial comer, bucco-disal comer, pulpal line angle and the interface area between internal slopes of cusp and resin inlay. 3. The vector of stress direct to buccal and lingual side from center of cavity, to tooth surface going on to enamel. The magnitude of vector increase from occlusal surface to cervix. 4. The crack of tooth start interface area, between internal slop of buccal cusp and resin inlay. It progresses through buccopulpal line angle to cervix at buccomesial and buccodistal comer. 5. The influence with depth of cavity to fracture of tooth was more than width of isthmus. 6. It would be favorable to make the isthmus width narrower than a third of the intercuspal distance and depth of cavity is below 1 : 0.7.

Objectives: Localization of the mandibular canal (MC) and measurement of the height and width of the available alveolar bone at the proposed implant site in the posterior segment of the mandible using cone-beam computed tomography (CBCT) in patients with a single missing tooth. Materials and Methods: A cross-sectional study was performed where CBCT scans of the patients with a single missing tooth in the posterior segment of the mandible-premolar, I (1st) molar, and II (2nd) molar were used. The scans were assessed using OnDemand3D software (version 1.0; CyberMed Inc., Seoul, Korea) for localization of the MC asnd remaining alveolar bone both vertically (from the superior position of the MC to the crest of the alveolar ridge) and horizontally (buccolingual, 3 mm below the crest of the alveolar ridge). The findings were statistically analyzed using independent t-test. Results: A total of 120 mandibular sites (40 sites for each of the three missing premolar, I molar, and II molar) from 91 CBCT scans were analyzed. The average heights (from the alveolar crest to the superior margin of the MC) at the premolar, I molar, and II molar areas were $15.19{\pm}2.12mm$, $14.53{\pm}2.34mm$, and $14.21{\pm}2.23mm$, respectively. The average widths, measured 3 mm below the crest of the alveolar ridge, at the premolar, I molar, and II molar areas were $6.22{\pm}1.96mm$, $6.51{\pm}1.75mm$, and $7.60{\pm}2.08mm$, respectively. There was no statistically significant difference between males and females regarding the vertical and horizontal measurements of the alveolar ridges. Conclusion: In the study, the measurements were averaged separately for each of the single missing teeth (premolar, I molar, or II molar), giving more accurate information for dental implant placement.
Purpose: To quantify artifacts from different root filling materials in cone-beam computed tomography (CBCT) images acquired using different exposure parameters. Materials and Methods: Fifteen single-rooted teeth were scanned using 8 different exposure protocols with 3 different filling materials and once without filling material as a control group. Artifact quantification was performed by a trained observer who made measurements in the central axial slice of all acquired images in a fixed region of interest using ImageJ. Hyperdense artifacts, hypodense artifacts, and the remaining tooth area were identified, and the percentages of hyperdense and hypodense artifacts, remaining tooth area, and tooth area affected by the artifacts were calculated. Artifacts were analyzed qualitatively by 2 observers using the following scores: absence (0), moderate presence (1), and high presence (2) for hypodense halos, hypodense lines, and hyperdense lines. Two-way ANOVA and the post-hoc Tukey test were used for quantitative and qualitative artifact analysis. The Dunnet test was also used for qualitative analysis. The significance level was set at P<.05. Results: There were no significant interactions among the exposure parameters in the quantitative or qualitative analysis. Significant differences were observed among the studied filling materials in all quantitative analyses. In the qualitative analyses, all materials differed from the control group in terms of hypodense and hyperdense lines (P<.05). Fiberglass posts did not differ statistically from the control group in terms of hypodense halos(P>.05). Conclusion: Different exposure parameters did not affect the objective or subjective observations of artifacts in CBCT images; however, the filling materials used in endodontic restorations did affect both types of assessments.
Purpose: The purpose of this study was to evaluate severe periodontitis with tooth loss as a modifiable risk factor for Alzheimer dementia (AD), vascular dementia (VaD), and mixed dementia (MD) using the National Health Insurance Service-National Health Screening Retrospective Cohort database with long-term follow-up over 14 years. Methods: Multivariate Cox hazards regression analysis was applied to a longitudinal retrospective database, which was updated in 2018, to evaluate the association between severe periodontitis with few remaining teeth and dementia after adjusting for potential risk factors, including sociodemographic factors and comorbid diseases. Results: Among 514,866 individuals in South Korea, 237,940 (46.2%) participants satisfying the inclusion criteria were selected. A total of 10,115 age- and sex-matched participants with severe periodontitis and 10,115 periodontally healthy participants were randomly selected and evenly assigned. The results showed that the risks of AD (hazard ratio [HR], 1.08), VaD (HR, 1.24), and MD (HR, 1.16) were significantly higher in patients with severe periodontitis with 1-9 remaining teeth after adjustment for sociodemographic factors, anthropomorphic measurements, lifestyle factors, and comorbidities. Conclusions: Severe periodontitis with few remaining teeth (1-9) may be considered a modifiable risk factor for the development of AD, VaD, and MD in Korean adults.
