Two Cases of Hand-Foot-Mouth Disease with Neurologic Manifestations (신경학적 증상을 동반한 수족구병 2례)

  • Hand-Foot-Mouth disease, which has a various enanthem-exanthem complex at the tongue, buccal mucosa, hands and feets and buttock area with febrile illness, is usually caused by Coxscakie virus type A(16). Generally, this disease shows self limited course and good prognosis without neurologic manifestations. However, enterovirus 71, which was newly discovered and reported in 1974, can cause the striking features of Hand-Foot-Mouth disease outbreaks and has neuropathogenic potentials of polio-like paralytic illness including aseptic meningitis, meningoencephalitis and respiratory disease. We experienced a case of Hand-Foot-Mouth disease with polyradiculitis manifestations, and a case of Hand-Foot-Mouth disease with meningoencephalitis. Therfore, we report these cases with brief review of related literatures.

Treatment of the Intraarticular Calcaneal Fractures and its Complications (관절내 종골 골절 및 합병증에 대한 치료)

  • Purpose: The treatment of calcaneal fractures remains a controversy in orthopaedic field because of its complications. The purpose of this study is to evaluate the efficacy of the treatment of calcaneal fractures and its complications. Materials and Methods: Clinical and radiological results were retrospectively analyzed in 28 patients, 34 cases with fractures of calcaneus which were treated in our department from September 1998 to march 2003. Results: According to the Creighton-Nebraska Foundation Assessment score, there were 3 excellent, 11 good, and 12 fair 6 poor results. Bohler angle was corrected from $8.3^{\circ}$ to $18.3^{\circ}$, Gissane angle was corrected from $121^{\circ}$ to $135^{\circ}$, and calcaneal width was corrected from 46.8mm to 37mm. Conclusion: Open reduction and internal fixation for joint depression type calcaneal fractures thought to be a good method of treatment. Closed reduction and percutaneous axial pinning should be chosed in selected cases of tongue type fractures. In treatment of complicated calcaneal fractures as malunion, subtalar distraction arthrodesis and lateral wall exostectomy will reduce disability of the disease.

  • Thyroglossal duct cyst is most common type of congenital cervical tumor. The suprahyoid thyroglossal duct cyst is a subtype of thyroglossal duct cyst and has different clinical characteristics. This subtype shows recurrent submental swelling and no tumor movement during swallowing or tongue protrusion. Because of these clinical differences, it is important to consider the possibility of thyroglossal duct cyst of in diagnosis and surgery of submental tumor. Recently, we have experienced two cases of suprahyoid type thyroglossal duct cyst with submental swelling and treated successfully by Sistrunk's operation.

Type II and III Taste Bud Cells Preferentially Expressed Kainate Glutamate Receptors in Rats

  • Glutamate-induced cobalt uptake reveals that non-NMDA glutamate receptors (GluRs) are present in rat taste bud cells. Previous studies involving glutamate induced cobalt staining suggest this uptake mainly occurs via kainate type GluRs. It is not known which of the 4 types of taste bud cells express subunits of kainate GluR. Circumvallate and foliate papillae of Sprague-Dawley rats (45~60 days old) were used to search for the mRNAs of subunits of non-NMDA GluRs using RT-PCR with specific primers for GluR1-7, KA1 and KA2. We also performed RT-PCR for GluR5, KA1, $PLC\beta2$, and NCAM/SNAP 25 in isolated single cells from taste buds. Taste epithelium, including circumvallate or foliate papilla, express mRNAs of GluR5 and KA1. However, non-taste tongue epithelium expresses no subunits of non-NMDA GluRs. Isolated single cell RT-PCR reveals that the mRNAs of GluR5 and KA1 are preferentially expressed in Type II and Type III cells over Type I cells.

  • The following is the conclusion drawn from a review on the clinical cases cured by Jaeumkangwha-tang in perspective of Hyungsang medicine. Jaeumkangwha-tang originated from the modified Jibaiksamul-tang in 'Manbyunghuichun(萬病回春)' and applied to the diseases induced by flaming fire due to deficiency of Yin. In 'Donguibogam', there are two kinds of Jaeumkangwha-tang. One in the chapter of kidney is mainly prescribed to replenish the Jung of kidney and applicable to the cases with configuration and symptom of kidney along with fire. The other one in the chapter of fire is applied to the symptoms of flaming fire due to deficiency of Yin. The characteristics of the patients treated by Jaeumkangwha-tang from the viewpoint of Hyungsang medicine can be classified as follows : ① configuration: Dam type, Shin type, inverted triangular type(:天垂象), bird type, round eyes, thin lips, slender waist, thin leg, pretty face, sparkling eyes, prominent upper lip and upward eyes and nose. ② color : dark red complexion, red cheek bone, red lips and red glabella. ③ pulse : fine and fast pulse and pulsation on the Chuk(尺) in man and so forth. ④ symptoms: agility, talkativeness, pimpled face, crooked back, weakness to the heat of summer, light eating, timidity, blood-shot eyes, night sweat, cough, abundant phlegm, hemoptysis, bloody spittle, enervation, emaciation, back pain, flaccidity of lower limbs, involuntary emission, nocturnal emission, heat sensation in the chest palms and soles, anger, flush on cheek bone, red lips and dry mouth, reddish tongue, stiff excrement, scanty yellow urine, etc.

Surgical Treatment Using a Lateral Approach in Intra-articular Fractures of the Calcaneus (외측 도달법을 이용한 관절내 종골 골절의 수술적 치료)

  • Purpose: We report the radiologic and the clinical results for surgical treatment of calcaneal fractures involving the subtalar joint. Methods: We evaluated the findings of radiographs and computed tomographs of 39 patients (40 cases) with intra-articular calcaneal fractures. The fractures were treated with open reduction via an extended lateral approach and internal fixation using a plate. We assessed the radiologic results, such as the $B{\ddot{o}}hler$ angle, the Gissane angle, and the height/width ratio. We assessed the clinical results based on the critieria of Salama et al. Results: According to the Essex-Lopresti classification, 9 cases were classified as tongue type and 31 cases as joint depression type. According to the Sanders classification, 10 cases were classified as type IIA, 4 cases as IIB, 16 cases as IIIAB, 4 cases as IIIAC, and 6 cases as type IV. The mean preoperative $B{\ddot{o}}hler$ angle was 7.7, the mean postoperative $B{\ddot{o}}hler$ angle was 21.1, and the mean last follow-up $B{\ddot{o}}hler$ angle was 16.8. Clinical results classified as 10 excellent, 13 good, 11 fair, and 6 poor. Conclusion: In the treatment of intra-articular fractures of the calcaneus, open reduction via an extended lateral approach seems to be a useful method in that it can provide direct exposure of the subtalar joint with little morbidity.

Study on the 'Diagnosis and Treatment of Terror and Palpitation due to Fright and the Several Hemorrhagic Diseases' in Synopsis of Golden Chamber (금궤요략${\cdot}$경계토뉵하혈흉만어혈병맥증병치 제십육(第十六)에 대(對)한 연구(硏究))

  • This Chapter mentioned Terror and Palpitation due to Fright(驚悸) and Hemorrhagic disease(血證). Terror and Palpitation due to FrightAcctually Terror(驚) is different from Palpitation(悸). Terror(驚) is one of the seven emotions. But in this case, It refer to the Palpitation and the uneasiness of mind due to one's hearing a strange sound of seeing a strange. Tremulous Pulse can be appear. So Terror(驚) is caused by Exopathic Factors(外因) and belongs to Excess syndrome(實證). Palpitaion(悸) is the sensation of plamus, palpitation and unrest not because of being frightened. It is usurally caused by the deficiency of Ki(氣) and blood(血). So Deep, Thready and weak pulse can be appear. So Palpitaion(悸) is caused by Endopathic Factors and belongs to Deficiency Syndrome. In this Chapter, Terror and Palpitation due to Fright(驚悸) treat with the Kyeji-ke-jakyak-ka-chokchil-moryu-yongkol-kuyuk-tang(桂枝去芍藥加蜀漆牡蠣龍骨救逆湯) and Banha-mahwang-hwan(半夏麻黃丸). There are two type in Hemorrhagic disease(血證). One is bleeding(出血) and another is blood stasis(瘀血). The contents which relate with the Hemorrhagic disease(血證) are Hematemisis(吐血), Rhinorrhagia, Hemafecia(下血). In hemorrhage pathological mechanisms, there are two mechanisms. One is that Fire and Heat(火熱) pressure blood. Another is that cold and deficiency(虛寒) disable Ki(氣) from keeping blood flowing within vessels. Blood stasis(瘀血) can be called Extravasated Blood(惡血), Coagulated Blood, Blood retention(蓄血,積血), Dead Blood(死血) and Emaciation due to Blood disorder(乾血). It refer to a morbid state of unsmooth circulation and blood stagnancy often resulting from Ki(氣) stagnation, Ki(氣) deficiency and accumulation of pathogenic coldness. The symptom of Blood stasis are 'Fullness sensation in the chest, Lip Flaccidity, Cyonotic Tongue and Dryness of Mouth'. And the man who have Blood stasis, want to rinse his mouth with the water, but he can't drink the water because there isn't interior Heat of Excess Type. The symptom of Cyonotic Tongue(舌靑) had influence on diagnosing Blood stasis(瘀血) in offspring.

Seasonal Distribution of Oceanic Conditions and Water Mass in the Korea Strait and the East China Sea: Correction of Atmosphere Cooling Effect (대한해협과 동중국해의 해황과 수괴의 계절분포: 대기에 의한 냉각효과 보정)

  • Water mass classification was conducted using the data of 1985 and 1986 in the East China Sea and the Korea Strait. Kuroshio water (type K) and mixed water (type I) were broadly distributed at 50 m depth in winter and spring, and mixed waters (type I to IV) were distributed in summer and autumn. At 100 m depth of the East China Sea, and mixed water (type I) was broadly distributed in winter and spring, and mixed waters (type I to III) were in summer, and type I was in autumn. Water mass in summer is the most influenced from the Chinese coastal water. In the Korea Strait, the Kuroshio water (type K) was the main water mass in winter and spring, and mixed waters (type I to IV) were in summer and autumn. If temperatures are corrected to remove the cooling effect from the atmosphere, the Kuroshiowater region was diminished, however the mixed water region was expanded in winter and spring. This shows that although the Kuroshio water appears to be a main water mass of the East China Sea and the Korea Strait in winter andspring, in reality the mixed water (type I) which is slightly changed from the Kuroshio water (type K) widely distributed. The tongue-shaped distribution of low density surface water indicates that the water mixed with the Chinese coastal water flows to the Korea Strait and the Okinawa in summer.

Operative Treatment with the Plate Fixation in Intraarticular Calcaneal Fractures (관절 내 종골 골절에서 금속판 고정을 이용한 수술적 치료)

  • Purpose: We evaluated the results of operative treatment with F or H plates and screws using extensile lateral approach in intraarticular calcaneal fractures. Materials and Methods: From August 2003 to July 2006, twenty intraarticular calcaneal fractures which were operated with open reduction and internal fixation with F or H plates and screws were evaluated retrospectively. According to the Essex-Lopresti classification, 3 cases were tongue type and 16 were joint depression type. With the Sanders classification, 2 cases were IIA type, 7 were IIB, 5 were IIIAB, 2 were IIIAC and 3 were IV. We have analysed the $B{\ddot{o}}hler$ angle, Gissane angle, and calcaneal width in radiologic evaluation, and evaluated clinical result according to the Creighton-Nebraska Health Foundation Score. Results: Radiologic changes showed as follows: $B{\ddot{o}}hler$ angle improved from $5.8^{\circ}$ to $25.9^{\circ}$, Gissane angle from $119.0^{\circ}$ to $113.3^{\circ}$, and calcaneal width from 50.4 mm to 37.8 mm. In the clinical results, excellent cases were noted in 8 cases, good in 8 cases, fair in 2 cases, poor in 1 case. Conclusion: Operative treatment with F or H plates and screws using extensile lateral approach in intraarticular calcaneal fractures was thought to be a useful operative method allowing anatomical reduction.

REPULSIVE EFFECT AND PALATABILITY OF DIETARY PHENYLALANINE IN LAYING HENS

  • Food intake of birds can be affected by particle size as well as diet composition, In order to investigate whether food intake of diets including excessive amount of phenyalanine(Phe) was influenced by diet types. a series of experiments were conducted in growing chicks and laying hens. Growing chicks significantly decreased food intake in a semipurified excessive Phe diet compared with a semipurified control diet, while laying hens fed a practical diet including excess Phe did not significantly reduce their food intake over a corn starch-substituted control diet. In an attempt to find out whether diet type affects food intake in layers, the semipurified type diet with excess Phe greatly reduced food ingestion, but the effect was delayed in the practical type diet. Moreover, under choice feeding regimes between the Phe and either starch, tyrosine(Tyr) or fiber diets in order to investigate whether the decreased food intake in the presence of an excess of dieary Phe in laying hens is involved in the palatability for the diet, there was no significant difference between Phe and starch diet while a preference for the Phe diet tended to be increased when birds were offered Tyr. Laying hens ingested significantly more the Phe diet than the fiber diet within 1 h after feeding. For supporting the idea that preference for the Phe diet may be affected by manipulating taste sense, an anaesthetic or saline was intramuscularly administered under the tongue just before a choice feeding. Preference for the Phe diet was not significantly different from that for the fiber reference diet within 1 h in the anaesthetized birds while the birds preferred the Phe diet in the saline treated group. It is suggested that because birds are able to select a diet, the decreased food intake induced by dietary excess Phe may be due to the repulsive effect of Phe after ingestion but not the taste of Phe.
