Clinical Characteristics and Distribution of the Tongue-Related Diagnoses of Patients with Tongue Symptoms

  • Purpose: This study was conducted to analyze the clinical characteristics and distribution of the tongue-related diagnoses in patients with tongue symptoms. Methods: The subjects were patients who visited Chosun University Dental Hospital with tongue symptoms from January 1, 2021, to December 31, 2021. We analyzed 491 patients (139 males and 352 females) with tongue-related diagnostic code names in the Korean Standard Classification of Disease (KCD) in the Electronic Medical Record (Dentopinformation Technology Co., Seoul, Korea). Results: On analyzing the diagnoses of tongue diseases using the KCD, glossopyrosis (44.8%) and painful tongue (15.2%) showed high rates. Of the 491 patients, 139 (28.3%) were males and 352 (71.7%) were females, and the average age was 58.1 years. As for systemic diseases, there were many patients with diseases of the circulatory system (27.0%) and endocrine, nutritional, and metabolic diseases (13.7%). Tongue symptoms were discomfort in 58 patients, pain in 329 patients, paresthesia in 10 patients, burning sensations in 222 patients, dysgeusia in 25 patients, dryness in 110 patients, fissures in 57 patients, and other symptoms in 72 patients. Areas with tongue symptoms were the anterior area in 102 patients, the lateral area in 140 patients, the posterior area in 12 patients, the dorsal area in 140 patients, the ventral area in 42 patients, and which area in 126 patients overall. Patients with burning mouth syndrome and oral candidiasis had significantly more tongue symptoms. Conclusions: Patients with tongue symptoms exhibited a variety of tongue diseases. Tongue symptoms were more common in women over 50 years of age and were frequently seen in patients with burning mouth syndrome and oral candidiasis.

  • Objective: A case report on improving the symptoms of patients with fissured tongues by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 61-year-old Korean female who not only felt pain during her daily life, but also had difficulty eating due to a fissured tongue that lasted for three years. Results: The practice of Ortho-Cellular Nutrition Therapy (OCNT) improved the patient's pain and xerostomia caused by a fissured tongue. Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) can be effective in relieving the symptoms of patients with fissured tongues.

  • Objectives The purpose of this study is to analyze the types of childhood simple obesity and suggest clinical symptoms, treatment principles, and traditional Chinese medicine (TCM) treatments for each type. Methods All kinds of literature published by the China National Knowledge Infrastructure (CNKI) up to August 20, 2022 were analyzed. We extracted information about types of childhood simple obesity, relevant clinical symptoms, treatment principles and TCM treatments. Results 25 studies were included. Spleen deficiency with dampness obstruction, gastrointestinal dampness-heat, internal excess of phlegm-dampness were the most reported. Spleen deficiency with dampness obstruction has symptoms of powerless, heavy limbs, pale tongue, teeth-marked tongue, sunken and slippery pulse. As a treatment, herbal medicine (HM) like modified Banggihwanggitang and acupoint like Joksamri were mainly reported. Gastrointestinal dampness-heat has symptoms of thirst, constipation, edacity, rapid hungering, heavy limbs, red tongue, slippery and rapid pulse. HM like Modified Xiehuangsan to clear heat was mainly reported. Internal excess of phlegm-dampness has symptoms of heavy limbs, lack of strength, tongue with white slimy fur, slippery pulse. Modified Ijintang to dry dampness to resolve phlegm was mainly reported. Conclusions This study analyzed types of pattern, clinical symptoms, treatment principles, and TCM treatments of childhood simple obesity. Based on this study, it is necessary to derive a standardized dialectical information that reflects the domestic situation.

A Clinical Study on Syndrome Differentiation of Male with Teeth-Mark Tongue (남자(男子) 치흔설(齒痕舌) 변증에 관한 임상적 고찰)

  • Objectives : The purpose of this study was to analyze the propensity and find out the Syndrome Differentiation of teeth-mark tongue by taking survey and body examinations with 178 male patients. 164 patients out of 178 were checked up on Heart Rate Variability (HRV), Accelerated Photoplethysmograpy (APG), Body Composition. This study was also planned to find out the distinctive characteristics of teeth-mark tongue diagnosis and compare differences between Qi-Deficiency and Accumulation of Dampness and Phlegm patients group. Methods : The questionnaire was carried out targeting 178 male with teeth-mark tongue respondents among who had Oriental Health Examination and patients from the 3rd oriental-internal medicine department in Dongeui Hospital from $1^{st}$, March 2005 to $30^{th}$, April 2010. Only 164 patients were checked on HRV, APG and Body composition examinations. Results : It showed that 86 patients had Qi-Deficiency and 78 had Dampness and Phlegm but 14 couldn't be categorized. The major symptoms of Qi-Deficiency compared to Dampness and Phlegm were 'Frequent running nose', 'Soft stool', 'Chronic fatigue', and 'Eyestrain'. On the contrary, Dampness and Phlegm's dominant symptoms were 'Chest discomfort', 'Feeling bloated', 'Back pain', 'Feeling sluggish', and 'Itchy skin'. However, all symptoms were not matched with the Syndrome Differentiation of Qi-Deficiency or Dampness and Phlegm. It also showed that teeth-mark tongue patients' frequent symptoms were 'Stuffy nose', 'Feeling bloated', 'Oliguria', 'Shoulder pain', 'Chronic fatigue' 'Eyestrain' and these symptoms were matched with the Syndrome Differentiation of Qi-Deficiency and Dampness and Phlegm. In the results from this study, there were no significant differences between Qi-Deficiency and Dampness and Phlegm. Conclusions : It is hard to conclude that teeth-mark tongue could be only one to diagnose Qi-Deficiency or Dampness and Phlegm with 3 examinations.

  • In traditional Chinese medical (TCM) science, tongue images can be observed for medical diagnosis; however, the tongue diagnosis of TCM is influenced by the subjective factors of doctors, and the diagnosis results vary from person to person. Quantitative TCM tongue diagnosis can improve the accuracy of diagnosis and increase the application value. In this paper, digital image processing and pattern recognition technologies are employed on mobile device to classify tongue images collected in different health states. First, through grayscale integral projection processing, the trough is found to localize the tongue body. Then the tongue body image is transferred from RGB color space to HSV color space, and the average H and S values are considered as the color features. Finally, the diagnosis results are obtained according to the relationship between the color characteristics and physical symptoms.

Effect of Botulinum Toxin Injection and Physical Therapy to Reduce Tongue Pain and Discomfort: Case Reports

  • The causes of tongue pain and discomfort include systemic disease, malnutrition, mental illness, fungal infection, and neuropathy. Three postmenopausal women reported burning sensations and stiffness of the tongue for various periods, from one month to four years. There were no objective etiological factors to cause the tongue pain and discomfort. Muscular tenderness upon palpation of masticatory muscles, sternocleidomastoid, trapezius, and tongue were observed. Physical therapy approaches such as moist hot pack, ultrasound, and myomonitor were performed on three patients with tongue pain, just as for temporomandibular joint disease. Additional botulinum toxin injection therapy was applied to one patient who displayed a clenching habit. All three patients showed a marked improvement in their tongue symptoms after the muscle relaxation and botulinum toxin injection therapy.

  • Background: Schwannomas (or neurilemmomas) of the tongue are benign, usually solitary, encapsulated masses derived from Schwann cells. Clinical evidence indicates that schwannoma is painless and slow growing. In general, schwannoma is treated by surgical excision. Here, we describe a case of schwannoma of the tongue, include a review of the literature from 1955 to 2016, and provide data on age, gender, location, presenting symptoms, size, and treatment methods. Case presentation: A 71-year-old female patient presented with a swelling at the base of the tongue of unknown duration. Magnetic resonance images (MRI) showed a large well-circumscribed solid mass and no significant lymph node enlargement. The mass was excised without removing overlying mucosa. Conclusions: The authors report a case of lingual schwannoma that was completely removed intraorally without preoperative biopsy. No sign or symptoms of recurrence were observed at 12 months postoperatively.

  • Objective : I would define the differences of sentences that 'A tongue can detect five basic tastes' and 'A mouth can perceive the five cereals' in Youngchu Macdo(靈樞 脈度). Methods : 1. I refer to literatures for the sentences in Youngchu Macdo 2. I look into the symptoms which may be caused by heart and pancreas troubles, respectively. Result : Human being can not detect a specific taste on his heart trouble. That is, acid, bitter, sweet, hot, and saltness can be all one. Human being may lose his appetite on his pancreas trouble. Even though the viscera and the tongue are connected to on-meridians, the function of a tongue is managed and controlled by a heart. Such a tongue perceives five basic tastes. Conclusion : A healthy tongue appreciates five basic tastes clearly and a healthy mouth may stimulate his appetite.

  • Macroglossia can cause dentomusculoskeletal deformities, instability of orthodontic and orthognathic surgical treatment, and create masticatory, speech and airway management problems. To determine whether a reduction glossectomy is necessary, it will important to identify the signs and symptoms of macroglossia. Development of dentoskeletal changes directly related with tongue size, such as an anterior open bite or a Angle Class III malocclusion tendency, would indicate that reduction glossectomy may be beneficial. For reduction glossectomy, several techniques have been reported. However, in most techniques the tip of tongue is removed. So its excision causes the loss of most mobile and sensitive portion of the tongue, and creates ankylosed, globular tongue. To avoid such problems, central tongue reduction technique have been proposed. This article will introduce central tongue reduction for anterior openbite case associated with macroglossia.

A Literature study on the Sasang Constitutional Tongue Diagnosis based on Experience Prescriptions(經驗方) in the Donguisusebowon (Longevity and Life Preservation in Oriental Medicine) (동의수세보원(東醫壽世保元) 경험방(經驗方)에 근거한 사상체질별 설진(舌診)의 문헌연구)

Objectives: This study was performed to research the relationship between Sasang Constitution and characteristics of tongue diagnosis based on Donguisusebowon's Experience prescriptions(經驗方). 2. Methods: We searched tongue diagnosis factors in Donguisusebowon and classified this. Based on Donguisusebowon's Experience prescriptions(經驗方), we find tongue diagnosis factors in 6 contemporary Oriental medicine prescription books. 3. Results: Most So-eumin has white thin tongue coating and clear redish tongue color. Yellow tongue coatings are found in symptoms for Yinjinho-tang(Yinchenhao-tang), Soseungki-tang(Xiaochengqi-tang), Daeseungki-tang(Dachengqi-tang). Shortened Tongue is found in So-eumin's experience prescription and So-yangin's treat histroy. It didn't shown that So-eumin's Semen Crotonis included 6 prescritions' factors of tongue diagnosis. So-yangin's white tongue coating is found in Hyungbangpaedok-san(Jingfangbaidu-san), Sipjo-tang(Shizao-tang), Sosiho-tang(Xiaocihu-tang), Shinki-hwan(Shenqi-wan). Most of other So-yangin's tongue coatings are Yellowish and tongue color is deep red. Tae-eumin have thin white tongue coating at the Mahuang-tang(Mahuang-tang). Red tongue color and drying tongue are found at Jowiseungki-tang(Tiaoweichengqi-tang)Daesiho-tang(Dacihu-tang)Saengmaek-san(Shengmai-san) 4. Conclusions: Following So-eumin's Experience prescriptions, So-eumin's tongue color is clear red and tongue coatings are thin white and greasy. Following So-yangin's Experience prescriptions, So-yangin's tongue color is deep red and tongue coatings are yellowish and scarce except Exterior cold disease. And So-yangin has specific symptom of irritability, oral sores. Tae-eumin's tongue has two sides of white thin tongue coatings and Red and drying tongue.
