표리한열의 설 특성에 관한 정량적 연구 (Quantitative Study on Tongue Images according to Exterior, Interior, Cold and Heat Patterns)

  • Tongue diagnosis is an important diagnostic method in traditional Oriental medicine. It has been especially accepted that quantitative analysis of tongue images allows the accurate diagnosis of the exterior-interior and cold-heat patterns of a patient. However, to ensure stable and reliable results, the color reproduction of such images must first be error-tree. Moreover, tongue diagnosis is much influenced by the surrounding illumination and subjective color recognition, so it has to be performed objectively and quantitatively using a digital diagnostic machine. In this study, 457 tongue images of outpatients were collected using the Digital Tongue Inspection System. Through statistical analysis, the result shows that the heat and cold patterns can be distinguished clearly based on the hue value of the tongue images. The average hue value (1.00) of the tongue's image in the cold pattern is higher than that in the heat pattern (0.99).

  • 설진은 한의학에서나 서양의학에서 진단에 많이 사용되고 있는 방법이다. 그 이유는 인간의 혀는 다양한 정보를 제공하기 때문이다. 혀를 분석하면 다양한 질병이나 건강정보를 알 수 있다. 모바일 환경을 이용한 설진기는 사용자에게 진단의 편리함을 제공한다. 이는 언제 어디서나 설진기를 통해서 질병이나 건강정보를 확인할 수 있다. 이런 부분은 의료기기의 새로움 패러다임을 보여줄 수 있다.

백태 중 후태 및 박태 분류 판별함수 설계 (Design of discriminant function for thick and thin coating from the white coating)

  • Introduction: In Oriental medicine, the status of tongue is the important indicator to diagnose one's health, because it represents physiological and clinicopathological changes of inner parts of the body. The method of tongue diagnosis is not only convenient but also non-invasive, so tongue diagnosis is most widely used in Oriental medicine. By the way, since tongue diagnosis is affected by examination circumstances a lot, its performance depends on a light source, degrees of an angle, a medical doctor's condition etc. Therefore, it is not easy to make an objective and standardized tongue diagnosis. In order to solve this problem, in this study, we tried to design a discriminant function for thick and thin coating with color vectors of preprocessed image. Method: 52 subjects, who were diagnosed as white-coated tongue, were involved. Among them, 45 subjects diagnosed as thin coating and 7 subjects diagnosed as thick coating by oriental medical doctors, and then their tongue images were obtained from a digital tongue diagnosis system. Using those acquired tongue images, we implemented two steps: Preprocessing and image analyzing. The preprocessing part of this method includes histogram equalization and histogram stretching at each color component, especially, intensity and saturation. It makes the difference between tongue substance and tongue coating was more visible, so that we can separate tongue coating easily. Next part, we analyzed the characteristic of color values and found the threshold to divide tongue area into coating area. Then, from tongue coating image, it is possible to extract the variables that were important to classify thick and thin coating. Result : By statistical analysis, two significant vectors, associated with G, were found, which were able to describe the difference between thick and thin coating very well. Using these two variables, we designed the discriminant function for coating classification and examined its performance. As a result, the overall accuracy of thick and thin coating classification was 92.3%. Discussion : From the result, we can expect that the discriminant function is applicable to other coatings in a similar way. Also, it can be used to make an objective and standardized diagnosis.

한의학과 아유르베다에서의 설진(舌診)에 대한 비교 연구 (A Comparative Study on the Tongue Diagnosis between Korean medicine and Ayurveda)

  • Objectives: The purpose of this study is to compare and analyze Tongue Diagnosis of Korean medicine with that of Ayurveda. Methods: In this article, first we introduced concept of Tongue Diagnosis based on physiological view. Further, we also reviewed published works including books and articles. Then, we reviewed Ayurveda to find similar concepts to Vicera Assignment on Tongue, and analyzed the comparison between the relevant contents of Korean medicine and Ayurveda. Results: In Ayurveda, they divided vicera into two part (right and left) and some elements of vicera were assigned to either side of divided tongue. In Korean medicine, a tongue is divide into three parts of Sangcho(上焦, Shang Jiao), Jungcho(中焦, Zhong Jiao) and Hacho(下焦, Xia Jaio) ; similarly, in Ayurveda, they divide a tongue into three regions of VATA, PITTA, KAPHA. Conclusion: It can be inferred from the relationship between portions of a tongue and vicera of a body that Korean medicine and Ayurveda have a close relationship.

정상 설색의 수치적 기준에 관한 고찰 (An investigation of normal range of tongue color in numerical coordinate)

  • Objectives Although tongue diagnosis is one of major diagnostic methods in east Asian traditional medicine, the standard of normal tongue color have not established. So we tried to suggest the method to establish the standard of normal tongue color range and the pilot data about the normal range. Materials and methods: 22 precedent study papers that presented the numerical data of normal (light red) tongue color were analyzed. At the same time, 46 adult people tongue color data were also collected and analyzed. Results Precedent studies showed remarkable fluctuations of the range of normal tongue color. Collected tongue color data of the 46 people showed mean Hue value 2 and standard deviation 14 in HSB system. Additionally, 3 ways of standard establishment for normal tongue color were suggested. Conclusion We suggest statistical method as a reasonable method for tongue color standard establishment and $2{\pm}14$ as a reference Hue value for normal tongue color.

영역 특징 학습을 이용한 혀의 자동 영역 분리 및 한의학적 설진 시스템 (Automatic segmentation of a tongue area and oriental medicine tongue diagnosis system using the learning of the area features)

  • 본 논문에서는 고가의 디지털 설진 장비와 특별한 장치 없이 누구나 손쉽게 사용할 수 있는 디지털 설진 시스템의 첫 단계로 미각 영역별 균열 유무를 판별하는 시스템을 제안한다. 훈련 DB는 한방 병원에서 수집한 사진 261장을 바탕으로 Haar-like feature, Adaboost 학습을 하였다. 학습된 결과를 통하여 입력영상으로부터 혀 후보영역을 검출하고, 검출된 혀 후보영역으로부터 혀 영역만을 분리하기 위하여 261장의 훈련 DB의 HSV 컬러모델의 Hue 성분 평균 값을 산출하였다. 검출된 혀 윤곽으로부터 Connected Component Labeling을 통하여 혀 영역을 분리 하였다. 분리된 혀 영역의 상대적 너비와 높이를 이용하여 미각 영역별 로 분할하였다. 분할된 미각 영역별 영상은 Gray영상으로 변환하고, 각각의 영역별 평균 밝기를 산출하여 이진화하였다. 이진화 영상에 Connected Component Labeling을 통하여 균열 유무를 판별하였다.

설진 유효 분석을 위한 혀의 기하정보 추출 방법 (Extraction Method of Geometry Information for Effective Analysis in Tongue Diagnosis)

  • 한의학에서 혀의 상태는 인체의 건강 상태를 진단하는 중요한 지표로 활용된다. 이러한 혀의 상태를 진단하는 설진은 편리할 뿐 아니라 비침습적이므로, 한의학에서 널리 활용되고 있다. 그러나 설진은 객관화와 표준화라는 관점에서 문제가 있으며, 한의사의 의사결정에 도움을 줄 수 있는 도구도 부족한 실정이다. 본 논문은 이러한 문제점을 해결하기 위해, 설진 유효 분석을 위한 혀의 기하정보를 자동으로 계산하는 방법을 제안한다. 제안된 방법은 개선된 스네이크(Snake) 방법을 통해 혀를 검출하고 컨벡스 헐(Convex Hull)과 인페인팅 방법을 이용하여 객관적인 기하 정보를 추출하였다. 제안 알고리즘의 성능평가로 치흔의 경우 7.2%, 균열의 경우 8.5%의 영역 차이 비율로 안정적인 결과가 도출되었다.

설진과 종양의 관계에 대한 연구 (Study on relation between tongue diagnosis with cancer : a review of literature)

  • Recently, the occurrence and death rates of cancer have increased rapidly. In oriental medicine, the tongue gives some kind of special physiological information on human body. Oriental medical doctors have used information about the color, degree of wetness and shape of the patient's tongue to determine patient's disease and body condition. This fact leads us that the tongue image is one of the most important clinical data for helping doctor's decision making. It also has significant meaning to cancer patients related with traditional theory of oriental medicine. In diagnosis and treatment of cancer, tongue diagnosis can give some prognosis and change o cancer. It also detect the sign of early stage cancer, but it cannot diagnose the kind of cancer and/or benign or malignant. Nowadays BioTechnology(BT) has developed rapidly, but there are a lot of limits(economy, accuracy, clinical significance, etc). Tongue diagnosis is very economic and practical way of diagnose and has a lot of possibility of development. The need for doctors and it also helps the development of tongue diagnosis related with cancer.

다차원 컬러벡터 기반 백태 및 황태 분류 판별함수 설계 (Design of Discriminant Function for White and Yellow Coating with Multi-dimensional Color Vectors)

The method of tongue diagnosis is not only convenient but also non-invasive, therefore, tongue diagnosis is one of the most widely used in Oriental medicine. But tongue diagnosis is affected by examination circumstances a lot. It depends on a light source, degrees of an angle, doctor's condition and so on. So it is not easy to make an objective and standardized tongue diagnosis. As part of way to solve this problem, in this study, we tried to design a discriminant function for white and yellow coating with multi-dimensional color vectors. There were 62 subjects involved in this study, among them 48 subjects diagnosed as white-coated tongue and 14 subjects diagnosed as yellow-coated tongue by oriental doctors. And their tongue images were acquired by a well-made Digital Tongue Diagnosis System. From those acquired tongue images, each coating section were extracted by oriental doctors, and then mean values of multi -dimensional color vectors in each coating section were calculated. By statistical analysis, two significant vectors, R in RGB space and H in HSV space, were found that they were able to describe the difference between white coating section and yellow coating section very well. Using these two values, we designed the discriminant function for coating classification and examined how good it works. As a result, the overall accuracy of coating classification was 98.4%. We can expect that the discriminant function for other coatings can be obtained in a similar way. Furthermore, if an automated segmentation algorithm of tongue coating is combined with these discriminant functions, an automated tongue coating diagnosis can be accomplished.

국내외 설진기를 활용한 인간 대상 연구현황 (A review on Clinical Trials by Using a Computerized Tongue Diagnosis System)

  • Objectives The purpose of this study was to survey the status of clinical use of a computerized tongue diagnosis system (CTDS) Methods We searched domestic/international articles using the CTDS from online medical databases including OASIS, NDSL and pubmed. We selected articles on clinical application or reliability of CTDS but excluded articles on mechanical design or software programming for developing a new CTDS. Finally we found 15 articles and classified the articles according to the study purpose. Results Out of the 15 articles, 8 were focused on the clinical application including halitosis, cold/heat syndrome, lung cancer, xerostomia etc. Other 5 articles were aimed at evaluating and improving reliability of CTDS. The other 2 articles were studied for development of differential diagnostic criteria on tongue coating thickness. Conclusion We found out that until now the researches on clinical application of CTDS mainly had been performed for producing a variety of CTDSs. Considering the importance of the tongue color in the traditional Korean medicine, we suggest that at first standard operating procedure for CTDS be developed and researches to develop differential diagnostic criteria on tongue body/coating color be performed and then explore its applications.
