Surgical intervention in the posterior maxillary region requires detailed knowledge of maxillary sinus anatomy and the possible anatomical variations. This study evaluated the incidence, location of maxillary sinus septa by using radiographic (panoramic radiography and computed tomography) findings and comparison of panoramic radography with CT in antral anatomical variation. This study was based on data from 70 sinuses in partial dentate maxilla. The sample consisted of 61 patients(25 women and 36 men, with ages ranging between 19 and 77 years and a mean age of $49.4{\pm}11.3$ years) who were being treatment-planned to receive implant-supported restorations. First, the panoramic images were examined for the presence of antral septa by radiologist and examiner who don't know about CT findings. And incidence of antral septa was evaluated using an axial plane of CT image. The incidence of septa was compared between panoramic radiography and CT. The accuracy of the incidence was compared between radiologists and dentists. A total of 20 septa were found in 70 sinuses on CT image and the prevalence of one or more septa per sinus was found to be 28.6%. The assumed incidence of septa on panoramic radiography was $27.6%{\pm}2.2%$ in radiologist and $31.9%{\pm}5.8%$ in dentists. Erroneous diagnosis rate was 11.42% in radiologist and 15.96% in dentists. 40% of antral septa were located in the anterior(premolar) region, 30% of septa were located in the middle(first molar) and posterior(second molar) region separately. Prior to implant placement, it seems appropriate to consider panoramic radiography as a standard radiographic examination and periapical radiographs may be used to complete the findings in regions not sharply depicted in the panoramic radiograph. And cross-sectional imaging should be used in sites with severe bone loss and close proximity of the maxillary sinus.

Pulmonary Langerhans cell histiocytosis is an uncommon diffuse cystic lung disease in adults. In rare cases, it can involve extrapulmonary organs and lead to endocrine abnormalities such as central diabetes insipidus. A 42-year-old man presented with polyphagia and polydipsia, as well as a dry cough and dyspnea on exertion. Magnetic resonance imaging of the hypothalamic-pituitary system failed to show the posterior pituitary, which is a typical finding in patients with central diabetes insipidus. This condition was confirmed by a water deprivation test, and the patient was also found to have type 2 diabetes mellitus. Computed tomographic scanning of the lungs revealed multiple, irregularly shaped cystic lesions and small nodules bilaterally, with sparing of the costophrenic angles. Lung biopsy through video-assisted thoracoscopic surgery revealed pulmonary Langerhans cell histiocytosis. On a follow-up visit, only 1 year after the patient had quit smoking, clinical and radiological improvement was significant. Here, we report an uncommon case of pulmonary Langerhans cell histiocytosis that simultaneously presented with diabetes insipidus and diabetes mellitus.

A 2-year-old, 2 kg Maltese dog was evaluated for progressive tetraparesis and neck pain. The dog had been treated with steroids for the preceding 2 weeks after unknown trauma but was deteriorating progressively and had become tetraparetic. The dog was presented with a non-ambulatory tetraparesis. Radiographic and computed tomographic examinations revealed a transverse C2 fracture with subluxation of the atlantoaxial joint and C2-C3. In addition, hydrocephalus was observed on magnetic resonance imaging. Stabilization of C1-C3 using screws, wires, and polymethyl methacrylate (PMMA) was performed. Application of ventral screws, wires, and PMMA resulted in improvement of the clinical signs after 4 weeks, and the dog could walk as before the tetraparesis 6 weeks after the operation. This stabilization method is an effective surgical treatment for management of cervical instability.
Computed tomography (CT) has the problem of having more radiation exposure compared to other radiographic apparatus. There is a low-dose imaging technique for reducing exposure, but it has a disadvantage of increasing noise in the image. To compensate for this, various noise reduction algorithms have been developed that improve image quality while reducing the exposure dose of patients, of which the median modified Wiener filter (MMWF) algorithm that can be effectively applied to CT devices with excellent time resolution has been presented. The purpose of this study is to optimize the mask size of MMWF algorithm and to see the excellence of noise reduction of MMWF algorithm for existing algorithms. After applying the MMWF algorithm with each mask sizes set from the MASH phantom abdominal images acquired using the MATLAB program, which includes Gaussian noise added, and compared the values of root mean square error (RMSE), peak signal-to-noise ratio (PSNR), coefficient correlation (CC), and universal image quality index (UQI). The results showed that RMSE value was the lowest and PSNR, CC and UQI values were the highest in the 5 x 5 mask size. In addition, comparing Gaussian filter, median filter, Wiener filter, and MMWF with RMSE, PSNR, CC, and UQI by applying the optimized mask size. As a result, the most improved RMSE, PSNR, CC, and UQI values were showed in MMWF algorithms.
Purpose: Quantitative analysis through count measurement in nuclear medicine planar images is limited by analysis techniques that are useful for obtaining various clinical information or by organ overlap or artifacts in actual clinical practice. On the other hand, the use of SPECT tomography images is quantitative analysis using volume rather than planar, which is not only free from problems such as projection overlap, but also has excellent quantitative accuracy. In the use of developing SPECT quantitative analysis technology, this study aims to compare the accuracy of quantitative analysis between ROI of the conventional planar images and VOI of the SPECT tomographic images in evaluating the count change happened by the volume change of the source. Materials and Methods: A 99mTcO4- source(200.17 MBq) was filled with sterilized water in the syringe to create a phantom with an inner diameter volume of 60 cc, and a planar image and a SPECT image were obtained by reducing the volume by 15 cc (25%) respectively. ROI and VOI(threshold: 1~45%, 5% interval) were set for each image obtained to estimate true count and measure the total count, and compared with the preseted volumetric change rate(%). Results: When volume changes of 25%, 50%, and 75% occurred in the initial volume of 60 cc(100%) of the phantom, the average count changes of the measured planar image were 26.8%, 53.2%, 77.5%, and the average count changes of the SPECT image were 24.4%, 50.9%, and 76.8%. In this case, the VOI size(cm3) set showed an average change rate of 25.4%, 51.1%, and 76.6%. The highest threshold value for the accuracy of radioactive concentration by VOI size (average error -1.03%) was 35%, and the VOI size of the same threshold had an error of -17.1% on average compared to the actual volume. Conclusion: On average, the count-based volumetric change rate in nuclear medicine images was able to track changes more accurately using VOI than ROI, but there was no significant difference with relatively similar value. However, the accuracy of radioactive concentration according to individual VOI sizes did not match, but it is considered that a relatively accurate quantitative analysis can be expected when the size of VOI is set smaller than the actual volume.

The objective of this study was to measure densities in various areas of the normal canine lung with computed tomography (CT) depending on influences of gravity and the degree of lung inflation and to determine optimal positions and positive end expiratory pressure of canine lung for CT scanning. In each eight normal Beagle and Shihtzu dogs, a respiratory breathhold maneuver without spontaenous breathing at different positive end expiratory pressure (PEEP) of 0 mmHg, 10 mmHg and 20 mmHg was applied with the position of right and left lateral recumbency, sternal recumbency, and dorsal recumbency and spiral-CT scans of the total lung were acquired. Slices were selected at three levels through the apex, middle and basal lung at the aortic arch, carina and just above the diaphragm and lung density was measured in the dorsal, ventral, and lateral portions of the peripheral lung field. Lung density in dependent areas was higher than in nondependent areas (p < 0.05) regardless of species, positions, anatomic locations at the PEEP of 0 mmHg and 10 mmHg. However, no significant difference of lung density was found at PEEP of 20 mmHg in both species except the dorsal recumbency in Shihtzu dogs. This density gradient in the dependent areas is strongly influenced by PEEP (p < 0.05). In the four positions on the CT gantry, the lung density at the dependent and nondependent location of the lung was greater at the aortic arch than at the base (p < 0.05). Lung density decreased on identical location according to increase of PEEP (p < 0.05). There was no significant difference between right and left lung density at sternal and dorsal recumbency and no significant difference of the dorsal, ventral, and lateral portions of lung density at the right and left recumbency under identical pressure. It is implied that during chest CT scan with 20 mmHg of positive end expiratory pressure with right or left lateral recumbency, canine lung density do not influenced by gravity or anatomic location.
Background: It is very important to determine an accurate staging of the non-small cell lung cancer(NSCLC) for an assessment of operability and it's prognosis. However, it is difficult to evaluate tumor involvement of mediastinal lymph nodes accurately utilizing noninvasive imaging modalities. PET is one of the sensitive and specific imaging modality. Unfortunately PET is limited use because of prohibitive cost involved with it's operation. Recently hybrid SPECT/PET(single photon emission computed tomography/positron emission tomography) camera based PET imaging was introduced with relatively low cost. We evaluated the usefulness of coincidence detection(CoDe) PET in the detection of metastasis to the mediastinal lymph nodes in patients with NSCLC. Methods: Twenty one patients with NSCLC were evaluated by CT or MRI and they were considered operable. CoDe PET was performed in all 21 patients prior to surgery. Tomographic slices of axial, coronal and sagittal planes were visually analysed. At surgery, mediastinal lymph nodes were removed and histological diagnosis was performed. CoDe PET findings were correlated with histological findings. Results: Twenty of 21 primary tumor masses were detected by the CoDe PET. Thirteen of 21 patients was correctly diagnosed mediastinal lymph node metastasis by the CoDe PET. Pathological N0 was 14 cases and the specificity of N0 of CoDe PET was 64.3%. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of N1 node was 83.3%, 73.3%, 55.6%, 91.7%, and 76.2% respectively. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of N2 node was 60.0%, 87.5%, 60.0%,87.5%, and 90.0% respectively. There were 3 false negative cases but the size of the 3 nodes were less than 1cm. The size of true positive nodes were 1.1cm, 1.0cm, 0.5cm respectively. There were 1 false positive among the 12 lymph nodes which were larger than 1cm. False positive cases consisted of 1 tuberculosis case, 1 pneumoconiosis case and 1 anthracosis case. Conclusion: CoDe PET has relatively high negative predictive value in the enlarged lymph node in staging of mediastinal nodes in patients with NSCLC. Therefore CoDe PET is useful in ruling out metastasis of enlarged N3 nodes. However, further study is needed including more number of patients in the future.
Purpose : To intra-individually compare diagnostic accuracy of high-resolution contrast-enhanced magnetic resonance angiography (CE-MRA) with computed tomography angiography (CTA) and digital subtraction angiography (DSA) for the assessment of supraaortic steno-occlusive disease. Materials and Methods : Twenty-eight patients (20 men, 8 women, 53-79 years of age) underwent supraaortic CE-MRA, CTA and DSA. CE-MRA was performed on two 1.5T MR scanners (voxel dimension: $0.66{\times}0.66{\times}1.1$ or $1.2\;mm^3$), and CTA on 64-slice CT scanners (voxel dimension: $0.42{\times}0.42{\times}0.63\;mm^3$). All the three examinations were completed within 40 days (median 19 days; range 1-40 days). Retrospective evaluation and measurement of diameter of 6 extracranial and 9 intracranial arterial segments was done by 2 experienced radiologists. Results: A total of 420 arterial segments were examined by CE-MRA, CTA and DSA. On DSA, 34 stenoocclusive lesions were noted at extracranial (n= 19) and intracranial (n = 15) vessels. For extracranial stenosis greater than 70%, sensitivity, specificity, positive predictive value (PPV) and negative predictive values (NPV) were 94.7%, 98.7%, 90.0% and 99.3% on CE-MRA, and 94.7%, 99.3%, 94.7% and 99.3% on CTA. For intracranial stenosis greater than 50%, sensitivity; specificity, PPV and NPV were 93.3%, 98.3%, 77.8%and 99.6% on CE-MRA, and 86.7%, 97.9%, 72.2% and 99.1 % on CTA, with DSA as the standard of reference. Conclusion : Supraaortic CE-MRA is as reliable as CTA in depicting the arterial stenosis, and is effective in screening of significant stenosis of both extracranial and intracranial arterial stenosis.
Purpose: We investigated whether the diagnostic performance of SPECT scintimammography (SMM) can be improved by adding computer-aided diagnosis (CAD) of ultrasonography (US). Materials and methods: We reviewed breast SPECT SMM images and corresponding US images from 40 patients with breast masses (21 malignant and 19 benign tumors). The quantitative data of SPECT SMM were obtained as the uptake ratio of lesion to contralateral normal breast. The morphologic features of the breast lesions on US were extracted and quantitated using the automated CAD software program. The diagnostic performance of SPECT SMM and CAD of US alone was determined using receiver operating characteristic (ROC) curve analysis. The best discriminating parameter (D-value) combining SPECT SMM and the CAD of US was created. The sensitivity, specificity and accuracy of combined two diagnostic modalities were compared to those of a single one. Results: Both SPECT SMM and CAD of US showed a relatively good diagnostic performance (area under curve = 0.846 and 0.831, respectively). Combining the results of SPECT SMM and CAD of US resulted in improved diagnostic performance (area under curve =0.860), but there was no statistical differerence in sensitivity, specificity and accuracy between the combined method and a single modality. Conclusion: It seems that combining the results of SPECT SMM and CAD of breast US do not significantly improve the diagnostic performance for diagnosis of breast cancer, compared with that of SPECT SMM alone. However, SPECT SMM and CAD of US may complement each other in differential diagnosis of breast cancer.
Tc-99m-MIBI(Sestamibi) myocardial SPECT along with T1-201 tomographic imaging has demonstrated wide application and high image qualify sufficient for the diagnosis of myocardial perfusion defect, which consequently reflects regional myocardial blood flow, The qualitative values of myocardial SPECT with Tc-99m-MIBI as well as the quantitative cases depend in some degree on the reconstruction techniques of multiple projections. Filtered backprojection(FBP) is the common standard for reconstruction rather than the complicated and time-consuming arithmetic methods. In FBP it Is known that the distribution of radioactivity in reconstructed transverse slices varies with the selected filter parameters such as cutoff frequencies and order(Butterworth case). The cutoff frequencies basically remove and decrease the true radioactive distribution and alter the pixel counts, which lead to underestimation of true counts in specific myocardial regions. In this study, we have investigated the effect of cutoff frequencies of reconstruction filter on the artifactually induced perfusion defects, which are often demonstrated near inferior and/or inferoseptal cardiac walls due to the intense hepatic uptake of Tc-99m-MIBI. A computerized method for identifying the relative degree of artifactual perfusion defect and for comparing those degrees along with the relative amount of hepatic uptake to myocardium was developed and patient images were studied to observe the quantitative degree of underestimation of myocardial perfusion, and to propose some reasonable thresh-old of cutoff frequency in the diagnosis of perfusion defect quantitatively. We concluded that from the quantitative viewpoint cutoff frequencies may be used as high as possible with the sacrifice of homogeneity of image quality, and those frequencies lower than the common 0.3 Nyquist frequency would reveal severe degradation of radio-active distribution near inferior and/or Inferoseptal myocardium when applying Butterworth or low pass filter.
