A parametric study of settlement and load transfer mechanism of piled raft due to adjacent excavation using 3D finite element analysis

  • The urbanization and increasing rate of population demands effective means of transportation system (basement and tunnels) as well as high-rise building (resting on piled foundation) for accommodation. Therefore, it unavoidable to construct basements (i.e., excavation) nearby piled foundation. Since the basement excavation inevitably induces soil movement and stress changes in the ground, it may cause differential settlements to nearby piled raft foundation. To understand settlement and load transfer mechanism in the piled raft due to excavation-induced stress release, numerical parametric studies are carried out in this study. The effects of excavation depths (i.e., formation level) relative to piled raft were investigated by simulating the excavation near the pile shaft (i.e., He/Lp=0.67), next to (He/Lp=1.00) and below the pile toe (He/Lp=1.33). In addition, effects of sand density and raft fixity condition were investigated. The computed results have revealed that the induced settlement, tilting, pile lateral movement and load transfer mechanism in the piled raft depends upon the embedded depth of the diaphragm wall. Additional settlement of the piled raft due to excavation can be account for apparent loss of load carrying capacity of the piled raft (ALPC). The highest apparent loss of piled raft capacity ALPC (on the account of induced piled raft settlement) of 50% was calculated in in case of He/Lp = 1.33. Furthermore, the induced settlement decreased with increasing the relative density from 30% to 90%. On the contrary, the tilting of the raft increases in denser ground. The larger bending moment and lateral force was induced at the piled heads in fixed and pinned raft condition.

Seismic behavior of caisson-type gravity quay wall renovated by rubble mound grouting and deepening

  • Caisson-type structures are widely used as quay walls in coastal areas. In Korea, for a long time, many caisson-type quay walls have been constructed with a low front water depth. These facilities can no longer meet the requirements of current development. This study developed a new technology for deepening existing caisson-type quay walls using grouting and rubble mound excavation to economically reuse them. With this technology, quay walls could be renovated by injecting grout into the rubble mound beneath the front toe of the caisson to secure its structure. Subsequently, a portion of the rubble mound was excavated to increase the front water depth. This paper reports the results of an investigation of the seismic behavior of a renovated quay wall in comparison to that of an existing quay wall using centrifuge tests and numerical simulations. Two centrifuge model tests at a scale of 1/120 were conducted on the quay walls before and after renovation. During the experiments, the displacements, accelerations, and earth pressures were measured under five consecutive earthquake input motions with increasing magnitudes. In addition, systematic numerical analyses of the centrifuge model tests were also conducted with the PLAXIS 2D finite element (FE) program using a nonlinear elastoplastic constitutive model. The displacements of the caisson, response accelerations, deformed shape of the quay wall, and earth pressures were investigated in detail based on a comparison of the numerical and experimental results. The results demonstrated that the motion of the caisson changed after renovation, and its displacement decreased significantly. The comparison between the FE models and centrifuge test results showed good agreement. This indicated that renovation was technically feasible, and it could be considered to study further by testbed before applying in practice.

Division of a single free flap in multiple digit reconstruction

  • Background Single free flaps are a commonly used reconstructive method for multiple soft tissue defects in digits. We analyzed the flap size, division timing, and degree of necrosis in cases with various types of flap division. Methods We conducted a retrospective review of the medical charts of patients who had undergone single free flap reconstruction for multiple soft tissue defects across their digits from 2011 to 2020. The flap types included were the lateral arm free flap, venous forearm free flap, thenar free flap, hypothenar free flap, anterolateral thigh free flap, medial plantar free flap, and second toe pulp free flap. Flap size, anastomosed vessels, division timing, and occurrence of flap necrosis were retrospectively investigated and then analyzed using the t-test. Results In total, 75 patients were included in the analysis. The success rate of the free flaps was 97.3%. All flaps were successfully divided after at least 17 days, with a mean of 47.17 days (range, 17-243 days) for large flaps and 42.81 days (range, 20-130 days) for the medium and small flaps (P=0.596). The mean area of flap necrosis was 2.38% in the large flaps and 2.58% in the medium and small flaps (P=0.935). Severe necrosis of the divided flap developed in two patients who had undergone flap division at week 6 and week 34. Conclusions In cases where blood flow to the flap has been stable for more than 3 weeks, flap division can be safely attempted regardless of the flap size.

조갑 주변의 후천성 수지 섬유각화종 (Acquired Digital Fibrokeratoma over Nail)

  • 후천성 수지 섬유각화종(acquired digital fibrokeratoma)은 비교적 드문 피부 양성 종양으로 손가락과 발가락 말단부에 주로 발생한다. 일반적으로 성인에서 단독으로 발생하는 과각화성의 돔 형태(dome-shaped) 종괴로 관찰되며 때로는 잔류 과다지(rudimentary supernumerary digit) 양상이거나 손톱(accessory nail)이 여러 개인 양상, 혹은 사마귀와 같은 보다 일반적인 상태로 오진되는 경우가 있다. 발생기전은 아직 확실하지 않으나 물리적인 외상이 가장 흔한 원인으로 알려져 있다. 저자들은 36세 남자 환자에서 좌측 제2수지의 외상 후 발생한 잔류 과다지 형태의 후천성 수지 섬유각화종을 진단하고 절제 생검술을 시행한 후 조직학적 검사를 통해 확진한 1예를 문헌 고찰과 함께 보고하고자 한다.

랜덤워크 모델을 이용한 토석류 산사태 피해범위 산정기법 제안 (A Random Walk Model for Estimating Debris Flow Damage Range)

  • 본 연구에서는 산사태 발생시 붕괴토사량을 산정하기 위하여 산사태 발생면적과 붕괴토사량의 상관관계를 활용하고, 붕괴토사량의 총 이동거리를 예측하기 위하여 붕괴토사량과 붕괴사면의 높이 및 붕괴토사의 도달거리를 활용하였다. 그리고 토석류의 이동경로를 예측하기 위하여 붕괴토사량의 유동 및 퇴적특성을 경사도의 인자로 단순화시킨 랜텀워크 모델을 적용하였다. 산사태 발생지점에서 이동경로 및 피해범위를 산정하기 위하여 토석류 이동 확률계산을 몬테카를로 시뮬레이션을 통하여 10,000회 반복적으로 수행하였다. 이때 계산된 다양한 랜텀워크의 궤적을 피해영역으로 제시하였다. 제안된 랜텀워크 모델을 이용한 산사태 피해범위 산정기법의 정확도를 확인하기 위하여 지리산 천왕봉 일대에서 발생된 산사태 발생이력을 적용하였다. 제안된 모델의 적용성을 검토한 결과 비교적 정확하게 피해범위를 산정하는 것으로 나타났으며, 10 m × 10 m 크기의 셀을 활용하는 것이 실제 피해범위에 대한 정확한 재현이 가능한 것으로 확인되었다.

이침을 병행한 고유수용성신경근촉진법이 뇌졸중 환자의 다리 근활성도에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation Combined with Auricular Acupuncture on Activation of the Leg Muscles of Strok e Patients)

  • PURPOSE: This study analyzed how proprioceptive neuromuscular facilitation (PNF) combined with auricular acupuncture affected the activation of the leg muscles of stroke patients and measured the effects of this combination to provide clinical data. METHODS: The subjects were divided randomly into experimental group I, which received PNF combined with auricular acupuncture, and experimental group II, which received PNF alone. Each group had ten members. A 30-minute intervention was performed four days a week for six weeks. Before the six weeks of intervention, pre-tests were conducted to measure muscle activation in the legs. After six weeks, post-tests were also conducted to measure muscle activation in the legs. RESULTS: Experimental group I showed a statistically significant difference in muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus. Experimental group II also showed a statistically significant difference in the muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus (p < .05). In a between-group comparison of the changes, a statistically significant difference was observed between the two groups in terms of muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus (p < .05). CONCLUSION: Intervention in experimental group I increased the activation of the leg muscles more effectively because auricular acupuncture was applied to various spots on the ear corresponding to the spleen, liver, kidney, pelvis, knee, ankle, and toe. Auricular acupuncture is expected to be used more widely in the future because it is a safe way of stimulating muscle activation.

The Effect of Induced Weight Bearing Method Using a Cane on Weight Bearing Distribution in Children with Spastic Hemiplegic Cerebral Palsy

  • Purpose: This study was undertaken to determine whether the position of cane use affects the distribution of weight-bearing on both feet of children with hemiplegic cerebral palsy in a standing posture. Methods: Twenty participants with cerebral palsy were recruited as volunteers for this study. Using the Zebris FDM-System, weight-bearing distribution according to the method of using a cane was measured under three conditions in randomized order: (1) standing unaided (no cane); (2) standing with the affected side using the cane; and (3) standing with the non-affected side using the cane. The cane was matched by measuring length-from-floor to the greater trochanter of the subject, and was placed 15 cm outward from the little toe on the supporting side. Results: Evaluating the method of using a cane under the three conditions, we determined that pressure of the foot on the affected side was higher in the order: standing with affected side using cane > standing unaided (no cane) > standing with non-affected side using cane (p<0.05). In the post-hoc analysis, a significant difference was observed between (i) standing unaided (no cane) and standing with the affected side using cane, and (ii) standing with affected side using cane and standing with non-affected side using cane (p<0.05). Conclusion: This study suggests that induced weight-bearing methods using a cane on the affected side could increase the weight-bearing capacity on the affected side in children with spastic hemiplegic cerebral palsy, which will have a positive effect on reducing asymmetry weight support.

Expression and Characterization of the Human Lactoferrin in the Milk of Transgenic Mice

  • Human lactoferrin (hLF) is an 80 kDa iron-binding glycoprotein that is expressed in high concentration in milk and in lesser amount in the secondary or specific granules of neutrophils and in plasma, LF is classically considered to be related to the binding, transport, and storage of iron. The transgenic mice carrying the human hLF gene in conjunction with the bovine $\beta$-casein promoter produced the human hLF in their milk during lactation. To screen transgenic mice, PCR was carried out using chromosomal DNA extracted from tail or toe tissues. In this study, stability of germ line transmission and expression of hLF were monitored up to generation Fl7 of a transgenic line. When female mouse of generation F9 was crossbred with normal male, generation F9 to Fl7 mice showed similar transmission rates ($66.0 \pm 12.57%, 42.0 \pm 14.98%, 72.2 \pm 25.45%, 50.0 \pm 16.70%, 65.7 \pm 6.45%, 48.6 \pm 14.65%, 54 1 \pm 18 11%, 57.8 \pm 16.16% and 48.6 \pm 20.66$, respectively), implying that the hLF gene can be transmitted stably up to long term generation in the transgenic mice For ELISA analysis, hLF expression levels were determined with an hLF ELISA kit in accordance with the supplier's protocol. Expression levels of human hLF from milk of generation F9 to Fl3 mice were $ 3.2 \pm 0.69 mg/ml, 3.1 \pm 0.81 mg/ml, 4.6 \pm 1.38 mg/ml, 3.1 \pm 0.42 mg/ml, and 4.5 \pm 1,48 mg/ml$, respectively. These expression levels were lower than that of founder (6.6 mg/$m\ell$) mouse. We concluded that transgenic mice faithfully passed the transgene on their progeny and successively secreted target proteins into their milk through several generations.

Rebalancing SMAD7/SMAD3 Signaling Reduces Adhesion Formation during Flexor Tendon Healing

  • Transforming growth factor-β is a key factor in regulating adhesion formation during tendon healing. We investigated the effectiveness of SMAD family members, SMAD7 and SMAD3, in the TGF-β/Smad signaling during flexor tendon repair. Mouse flexor toe deep tendon rupture anastomosis models were made. On days 3, 7, 14, 21, and 28, the expressions of smad7 and smad3 in flexor tendon tissues were detected by RT-qPCR and western blot. Furthermore, postoperative intraperitoneal injections of SMAD7 agonists or SMAD3 antagonists were given. The degree of tendon healing was evaluated by adhesion testing and biomechanical experiments. Hematoxylin and eosin (HE) staining was used to observe the pathological changes. Immunohistochemistry was used to evaluate the expressions of collagen III, SMAD3, and SMAD7. The mRNA levels of matrix metalloproteinases, Mmp2 and Mmp9, and scleraxis (SCX) in flexor tendon tissue were detected by RT-qPCR. Smad3 expression increased and Smad7 expression decreased in flexor tendon tissue after injury. In addition, the SMAD7 agonist blocked SMAD3 phosphorylation. SMAD7 agonist and SMAD3 antagonist both improved adhesion formation during flexor tendon healing, and decreased the expressions of collagen III, Mmp9, and SCX, while increasing Mmp2 expression. This study provides a possible theoretical basis for the SMAD7-SMAD3 signal cascade during flexor tendon adhesion healing.

암반에 근입된 현장타설말뚝의 선단하중전이거동 분석 (End Bearing Load Transfer Behavior of Rock Socketed Drilled Shafts)

  • 본 연구에서는 3차원 유한차분해석을 통하여 말뚝직경, 암반물성, 불연속면의 간격 및 방향 등 주요 인자가 암반근입 현장타설말뚝의 선단거동에 미치는 영향에 대한 분석을 수행하였으며, 이를 토대로 초기기울기 및 극한 단위선단지지력을 변수로 하는 쌍곡선형태의 선단하중전이(q-w)함수를 제안하였다. 제안식의 국내 암반지반에의 적용을 위하여 국내에서 수행된 14개 현장 23본의 시험말뚝의 재하시험자료를 토대로 제안식의 경험계수를 산정하였으며, 더불어 기존 초기기울기 및 극한단위선단지지력 산정식의 타당성 검토도 수행하였다. 현장재하시험 사례와의 비교분석 결과, 본 연구에서 제안된 산정식은 국내 암반지반에 근임된 현장타설말뚝의 선단거동을 비교적 잘 예측 가능하고, 말뚝지지력 및 침하량 예측값이 실측값에 가까움을 알 수 있었다.
