Endoscopic Ultrasound Staging of Upper Gastrointestinal Malignancies

  • Since 1980, endoscopic ultrasound (EUS) has been used as an important tool for the evaluation of malignant diseases in hollow viscus and bilio-pancreas, as well as sub-epithelial tumors. The high-resolution capacity and low penetration depth of EUS make it possible to obtain highly detailed images of the gastrointestinal wall and immediate surroundings to a depth of 4-5 cm. Thus, over the past 35 years, EUS succeeded to modify management in significant number of cases and is now considered a gold standard tool for many gastrointestinal diseases, especially in the pancreatico-biliary tract, and adjuvant needle insertion now allows access to remote lesions that were difficult to reach in the past. With the growing spectrum of indications, tissue sampling for diagnostic purposes has become common. In this review, we aim to highlight the expanding spectrum of EUS indications and uses in staging of upper gastrointestinal malignancies, especially esophageal, gastric and ampullary tumors.

Quantitative Analysis of Epicardial fat (심외막 지방의 정량분석)

  • Subepicardial space is one of the regions in which fat in moderate amount is normally deposited. When excess deposits of epicardial fat occur, penetration or infiltration of fat into the connective tissues between muscle bundles is observed. In some cases with very extensive involvement of epicardial fat,myocardial insufficiency and conduction system defect such as heart block have been implicated. More fat is deposited over the right ventricle than over left and the anterior surface contains more fat than dose the posterior surface. We studied 40 patients who underwent open heart surgery under the cardiopulmonary bypass. This study was designed to evaluate the percentage of the area of epicardial fat of the right ventricular anterior surface, which is more accessible area during the operative procedure. Other variables included thickness of subcutanous tissue of anterior chest wall, the ratio of waist to hip circumference, body weight and age. In conclusion, the amount of the epicardial fat correlates with thickness of the subcutaneous fat of the anterior chest wall and simple linear regression analysis indicates good correlation with body weight. Increased age results in large amount of epicardial fat.

Changes of Salt and Calcium Concentration in Radish during Salting (무우의 소금절임 과정 중 소금의 침투와 칼슘의 용출)

  • Salt penetration, moisture effluence and elution of calcium of radish were investigated during salting in $3{\sim}10%$ salt solution. The desirable salt concentration of kimchis were also evaluated by sensory method. Among 20 samples collected from domestic and commercially prepared kimchis, 8 samples were found to be desirable salt concentration having the concentration range of $1.2{\sim}2.2%$. Moisture and calcium diffused from radish tissue were increased as the salt concentration increased. The proper salting time were estimated to be two hours at 3%, one hour at 5% and 30 minutes at 10% of salt at $20^{\circ}C$. Salting at high concentration shortened the salting time but brought a significant calcium loss.

THE EFFECT OF ND : YAG LASER ON DENTINAL TUBULE SEAL AFTER ROOT END RESECTION (치근단절제시 노출된 상아세관의 밀폐효과에 대한 Nd : YAG 레이저의 효과)

  • If root and resection is done during surgical endodontic treatment, newly exposed dentinal tubules form pathways between the canal and the peripheral tissue. Nd : YAG laser was used to block this phenomenon, and its effect was studied with dye penetration and SEM techniques. 40 intact single rooted teeth were divided into 4 groups(10 each) : control group and test groups, in which retrograde cavity surface, cutting surface, retrograde cavity surface & cutting surface were treated with laser(1 watt 15pps) and finally retrograde filling with IRM was conducted. After that, they were stained with 2 % methylene blue, sectioned and evaluated by the maximum infiltration depth. And to observe surface change, they were prepared for SEM. The results were as follows ; 1. All experimental groups showed microleakage with variation in amount. 2. The 2nd group which treated both the retrograde cavity and cutting surface showed significantly less microleakage than the other groups(p<0.05). There was no significant difference between groups treated on one side only. 3. As a result of SEM observation of dentin surface, obstruction of dentinal tubules with marble shaped granules, which were different from normal dentin could be seen. Cracks could be seen also. 4. In summary of this experiment, it is thought that effort to obstruct the exposed dentinal tubules as well as retrograde cavity after root end resection is needed.

A Study on the Measurements of Optical Parameters in Photosensitizer by Light Scattering (농도가 진한 매질에서 광증감제에 의한 광학적 파라미터측정에 관한 연구)

  • The study of wave propagation and scattering in biological media has become increasingly important in recent years. The propagation of light within tissues is an important problem that confronts the dosimetry of therapeutic laser delivery and the development of diagnostic spectroscopy. In the clinical application of photodynamic therapy(PDT) and in photobiology, the photon deposition within a tissue determines the spatial distribution of photochemical reactions. Scattered light is measured as a function of the distance (r) between the axis of the incident beam and the detection spot. Consequently, knowledge of the photosensitizer(Chlorophyll-a) function that characterizes a phantom is measured. To obtain the results of scattering coefficients(${\mu}s$) of a turbid material from diffusion described by experimental approach. It was measured the energy fluency of photon radiation at the position of penetration depth. From fluorescence experimental method obtained the analytical expression for the scattered light as the values of $(I/I_o)_{wavelength}$ vs the distance between the center of the incident beam and optical fiber in terms of the condition of "in situ spectroscopy(optically thick)" and real time by fluorometric measurements. The result was compromised with transport of intensities though a random distribution of scatters.

  • The fluoroquinolones are an important group of antibiotics widely used in the treatment of various infectious diseases in adults, as a result of an excellent spectrum of activity, good tissue penetration and convenient ways of administration. In recent decades, there has been extensive development, clinical investigation, licensure and use of fluoroquinolone antibiotics. However, the use of fluoroquinolones in children has been limited because of their potential to induce arthropathy in juvenile animals. Despite class label warnings against use in children, prescriptions for quinolone antibiotics to treat infections in children have become increasingly prevalent. The main use of fluoroquinolones in pediatrics should be, understandably, in serious life-threatening infections for which other antibiotics therapies are not effective or available. While most of the published studies failed to detect an increased rate of articular adverse effects in children treated with fluoroquinolones, an increase in the use of these compounds, particularly in community-acquired lower respiratory infections, could accelerate the emergence of multidrug-resistant (including fluoroquinolone) pneumococcal strains. This review will discus the main issues related to the use of fluoroquinolones in children, the major problems of resistance developing among these compounds, with special emphasis on the potential side effects and skilled use of these alternative potent drugs in pediatric infection.

  • The purpose of this study was to investigate the effect of hydrocortisone in the fusion mechanism of the secondary palatine shelves in the rat. Pregnant rats were injected with 5.0 mg/100 gm body weight of hydrocortisone for 3 days between the 7th day and the 9th day of pregnancy. Between the 14th day md the 18 day of pregnancy, the fetuses were removed and decapitated to be immersed in $10\%$ formalin and Carney's solution. Preparations were stained with alizarin red S, hematoxylin-eosin and alcian blue respectively, and partly were treated for Periodic acid-Schiff reaction. The results were as followings: 1. The incomplete fusion of the palatal epithelium took place though the polarity of epithelial cells. 2. At the edge of shelves the differentiation of mesenchymal cells was observed, but the inter penetration of mesenchyme was not shown. 3. It was considered that the phenomenon of hypocalcification in matrix had relation to the decrease stainability to alizarin red S in shelves. 4. It might be concluded that the connective tissue under epithelium showed the decrease tendency to alcian blue and PAS reaction due to the inhibition of the synthesis of the mucopolysaccharide.

The Use of Fluoroquinolones in Children: Recent Advances (소아에서의 Fluoroquinolones 사용: 최근 경향을 중심으로)

  • Fluoroquinolones have been widely used since the development of nalidixic acid because of the excellent spectrum of activity, superior tissue penetration, and convenient route of administration. Despite such advantages, the usage in children is limited by concerns of toxicity to cartilage, damage to joints in animal studies, and an increase in antibiotic resistance. However, pediatric use has been increasing under certain circumstances with no report of such adverse effects. Further study is required prior to recommendation for general use of fluoroquinolones in children. The careful administration of fluoroquinolones is recommended only in limited circumstances, such as failure of primary antibiotics and lack of alternative antibiotics.

  • Fluoroquinolones are an important class of antibiotics that are widely used in adult patients because of their broad spectrum of activity, good tissue penetration, and oral bioavailability. However, fluoroquinolone use in children is limited because juvenile animals developed arthropathy in previous experiments on fluoroquinolone use. Indications for fluoroquinolone use in patients younger than 18 years, as stated by the U.S. Food and Drug Administration, include treatment of complicated urinary tract infections and postexposure treatment for inhalation anthrax. In Korea, the systemic use of fluoroquinolones has not been approved in children younger than 18 years. Although concerns remain regarding the adverse musculoskeletal effects of fluoroquinolones in children, their use in the pediatric population has increased in many circumstances. While pediatricians should be aware of the indications and adverse effects of fluoroquinolones, recent studies have shown that the risk for musculoskeletal complications in children did not significantly increase following fluoroquinolone treatment. In addition, fluoroquinolones may be particularly helpful in treating multidrug-resistant infections that have not responded to standard antibiotic therapy in immunocompromised patients. In the present article, we provide an updated review on the safety and current recommendations for using fluoroquinolones in children.
