The Experimental Study on Antifungal effects of mixture of Sophorae Subprostratae Radix, Acoti Radix, and Hibisci Syriaci Cortex by the Three Types of Extraction on three species of the dermatophytes. (다양한 山豆根·川烏·木槿皮 혼합물 용액이 足部白癬菌에 미치는 영향)

  • This experimental study was performed to investigate the antifungal effects of mixture of Sophorac Subprostratae Radix, Aconti Radix, and Hibisci Syriaci Cortex by the three types of extraction in the dermatophytes such as Trichophyton rubrum, Trichophyton mentagrophytes, and Epidermophyton floccosum. The following results were obtained by using Minimum Inhibition Concentration(MIC) and Inhibition Zone. 1. MIC on Trichophyton mentagrophytes in Sample S-2, Sample S-3, and Sample-Y were 10${\mu}l$, respectively, showing the highest antifungal effect. 2. MIC on Epidermophyton floccosum in Sample S-2, Sample S-3, and Sample-Y were 10㎕ respectively, showing the highest antifungal effect. 3. MIC on Trichophyton rubrum in Sample S-3, Sample-Y were 10${\mu}l$ respectively, showing the highest antifungal effect. 4. The size of inhibition zone on Trichophyton mentagrophytes for Sample S-3 were 22.3mm in 10${\mu}l$, showing the highest antifungal effect. 5. The size of inhibition zone on Epidermophyton floccosum for Sample S-3 was 34.3mm in 10${\mu}l$, showing the highest antifungal effect. 6. The size of inhibition zone on Trichophyton rubrum for Sample-Y was 26.5mm in 10${\mu}l$, showing the highest antifungal effect. Whereas. in extractions, that of Sample S-3 was 12.5mm in 10${\mu}l$, showing the highest antifungal effect. 7. From the above results, Sample S-3 on Trichophyton mentagrophytes, Epidermophyton floccosum and Sample-Y on Trichophyton rubrum showed the highest antifungal effects. This results suggests that vinegar extraction excels other extraction method using water or ethanol to measure the antifungal effect on dermatophytes. In addition, the result shows that the extract of herbal medicines can be used to tinea pedis and if further study is performed, the use of the extract of herbal medicines will be valuable and beneficial in the clinical medicines. Incidence Trends of Dermatophytoses Isolated in Children (소아에서 분리된 피부진균증의 발생경향)

  • From January 2 to December 30, 2017, out of 691 children diagnosed with dermatophytoses infection, 179 children had infections caused by Trichophyton among the total 13,093 patients who visited the Dermatology department. According to a survey, the patients included 317 adolescents, 203 elementary school age children, and 171 infants. In seasonal variation, the diagnosis was higher in summer (195 cases, 28%) and winter (191, 28%) and comparatively lower in spring (165, 24%) and autumn (140, 20%). The infection among the patients was caused by, from maximum to minimum, T. pedis (351, 51%), T. corporis (91, 13%), and T. unguinum (77, 11%). In all age groups, the highest number of patients were infected with T. pedis. After T. pedis, in descending order, the infection was caused by T. corporis, T. unguinum, T. manus and Tinea capitis in the infants and elementary school age children and by T. corporis, T. unguinum and T. cruris in adolescents. Among infections caused by Trichophyton, T. rubrum was the highest isolated causative agent. This data analysis confirmed the distribution of dermatophytoses and the different species according to paediatric age and will aid the study of paediatric dermatomycosis through continuous research.

  • A dermatophyte is a zoonotic infection that causes infection on the skin, hair, and nails and is transmitted through contact. The species of Microsporum, Trichophyton, and Epidermophyton are the major dermatophytes that infect humans. Tinea capitis is a dermatophyte infection in the scalp, and it may progress to kerion celsi, including severe redness, swelling, and pus formation. Kerion celsi is sometimes misdiagnosed as a bacterial infection or a tumor. Trichophyton verrucosum is a zoophilic dermatophyte that mainly causes infection in cattle. It can be spread to dairy farmers or ranchers who have frequent contact with infected cattle. We report a pediatric case who received scalp tumor excision and a split-thickness skin graft for extensive and severe inflammatory scalp tumors that occurred after contact with cattle with ringworm. Finally, the patient was diagnosed with kerion celsi caused by T. verrucosum infection. This was based on the patient's medical history, clinical manifestations, and histopathologic findings.

Subungual Squamous Cell Carcinoma of the Left Fifth Toe: A Case Report (족지 조갑하 편평세포암 증례 보고)

  • Purpose: Subungual squamous cell carcinoma is a very rare malignant tumor of the digitus especially in toe making it very difficult to differentiate with tinea unguium, verruca vulgaris, eczema, pyogenic granuloma, and malignant melanoma due to their clinical similarities. Therefore this article reports on subungual squamous cell carcinoma of left 5th toe. Methods: A patient visited at our hospital due to pain and incurable inflammation on left 5th toe with no improvement for four months. Despite of antibiotics and dressing for a week, treatment was ineffective. After excisional biopsy, he was diagnosed with subungual squamous cell carcinoma. And ray amputation and prophylactic inguinal lymph node dissection was done. Results: The biopsy was showed irregular cells made up of anaplastic and squamous cells in the dermis and epidermis. And the inguinal lymph node showed no metastatsis. The patient had been treated without significant complications and recurrence for a year. Conclusion: This article emphasizes the need of diagnostic biopsy on the subungual lesions that have resistance to the conventional treatments. And if that is subungual squamous cell carcinoma, complete resection of the lesion and prophylactic inguinal lymph node dissection is essential.

  • I have come to conclusion as follows about Bangpungtongsungsan after literatural study. 1. Bangpungtongsungsan is the medical treatment of heat, wind and dryness. 2. Bangpungtongsungsan is used in head-wind, dizziness, both eye disease, tinnitus, both ear deafness, nasal polyp, uriticaria, sajuabi, beard and hair falling, apoplexy, paralysis of hands and feet, dull mentality, tetanus, epidemic disease characterized by swelling and redness of face, carbuncle, daepungchang, pustule, syphilis, tinea capitis and so on. 3. Bangpungtongsungsan is used in the disease of cerebral hemorrhage, hypertension, arteriosclerosis, constipation, rosacea, hemorrhoids, cutaneous disease, empyema, eye disease, diabetes, asthma, obesity, beriberi, erysipelas, baldhead, fatty heart, chronic nephritis and so on. 4. Bangpungtongsungsan fits in Taeumin who has much wetness-heat and those who have much heat or have much heat but don't give off well. And it doesn't fit in the disease of intolerance to cold and fever in the form of weakness headache, asthenia of the spleen and stomach, cold by internal disorder and so on.

"Study for the orthotics & shoes of diabetic among Korean adult" (한국 성인에서의 당뇨병성 족부 질환의 신발 및 족부 질환 연구)

  • Purpose: To provide baseline data for prevention and education of diabetic foot in Korea. Materials and Methods: 216 diabetic patients are included in this survey. The average age is 58.64 year old with average 9.92 years of diabetes mellitus periods. Among them, 41% used insulin injection, 42% oral medication, 14% both and 3% diet were used for their blood sugar control. The diabetic foot survey was done. Results: The survey of patients symptom and disease of their feet results: 49% had sensory decrease in their feet. Associated disease in foot were callus(15%), heel pain(10%), bunion(4%), toe deformity(2%), Tinea pedis were 9%, tingle sensation were 17%, and swelling on feet were 2%, The survey of patients privious history of foot problems are: 8% had previous operation history on their feet and 17% had history of infection on their feet, 23% had bullae history, The survey of patients prevention and education are: Those who have had more than one diabetic foot education were 45.7%, and among them 24% had routine regular foot check up. Only 5% wear special diabetic shoe and 3% had custom orthotics. Conclusion: Interests to diabetic foot care is increasing in diabetic patients in Korea. However still we need more education, and efforts for prevention. Special education and diabetic shoe is still low, The survey data could be good baseline for diabetic prevention and education.

Study on Preparation and Drug Release of Sulconazole Nitrate Gels (질산술코나졸겔의 제조 및 약물방출에 관한 연구)

  • Sulconazole nitrate(SCN), an imidazole derivative which has been effective in the treatment of dermatophytosis, tinea versicolor and candidiasis, was formulated as a gel containing drug, poloxamer 407, ethanol and propylene glycol. The resulting SCN gels were evaluated with respect to their viscosity, drug release rate, skin permeation rate. The apparent viscosity of SCN gel increased in proportion to poloxamer 407, drug and propylene glycol concentration. In case ethanol was added, the apparent viscosity decreased. The drug release rate of SCN gel increased in proportion to temperature and ethanol concentration. But the drug release rate decreased as the concentration of poloxamer 407 increased. The increase of drug concentration induced nonlinear increase of drug release rate. When propylene glycol was added at the level of 10%, the drug release rate increased but from 15% it decreased. The skin permeation rate decreased in high concentration of poloxamer 407. The skin permeation rate of SCN gel containing 15% ethanol increased about twice than that of gel without ethanol. The increase of drug concentration induced nonlinear increase of skin permeation rate. When propylene glycol was added at the level of 10%, the skin permeation rate increased but from 15% it decreased.

An Etiology Report for Burns Caused by Korean Folk Remedies

  • Background In this modern era of science-based medicine, some people still accept folk remedies as an alternative form of medicine. However, misinformation and misuse of folk medicines can result in dangerous complications. Among the possible complications of folk remedy use, this study focused on the clinical characteristics of burns caused by folk remedies. Methods We retrospectively reviewed the clinical records of patients who had been treated for burns caused by folk remedies from May 2015 to April 2022. Data were collected on patients' age and gender, type of folk remedy, reason for using the folk remedy, the severity of the burn, the number of wounds, lesion type, and type of treatment. Results We found 59 patients with burns due to folk remedies. Most were female (76.3%) and ≥ 60 years old (72.9%). The most common type of folk remedy was moxibustion (74.6%), followed by the use of glacial acetic acid (20.3%). The reasons for using folk remedies were arthralgia relief (39%), health improvement (18.6%), and treatment of tinea pedis (11.9%). Most patients had multiple wound sites and had burns that were considered severe, requiring surgical treatment (72.9%). The majority of lesions were on the lower extremity, including the foot. Conclusion This study described the risk of burns caused by folk remedies and the clinical characteristics of the wounds. The results emphasize the need for greater public awareness of the risk of burn injuries when using folk remedies.

  • Eczema is a general term for skin diseases showing itching, vessiculopapular eruption, and erythema when acute, or lichenification and scale if chronic. It is classified as irritant dermatitis, atopic dermatitis, seborrheic dermatitis, neurodermatitis, nummular eczema, statis dermatitis, infective dermatitis, xerotic eczema, and pompholyx by afflicted regions and symptoms. Therapy for eczema is allopathic, and includes application of ice- pack to erythema, and application of emollients and steroids to the affected parts during subacute or chronic stages. Eczema is understood as ulceration(瘡), tinea(癬), wind syndrome(風證), and dryness syndrome(燥證) in terms of oriental medicine. Xerotic eczema is understood in dryness syndrome. Nourishing blood(養血) therapy is used. This is a case study on which xerotic eczema occurred on a patient's afflicted left palm and fingers after stroke. The patient's xerotic eczema was attributed to dryness syndrome. Therefore, Samul-tang(四物湯) and Saenghyeoryunbu-eum(生血潤膚飮) were used for nourishing blood. This therapy was effective on eczema and accompanying symptoms such as dizziness and numbness of the left hand.
