  • Purpose: The purpose of this study was to investigate sleep patterns and predictors of excessive daytime sleepiness (EDS) in university students. Methods: Participants were 120 university students who were attending two universities in S-city and C-city. Data were collected from May 20 to June 15, 2012 using self-report questionnaires which included Johns' Epworth Sleepiness Scale, Yi's Sleep Quality Scale, and Beck Depression Inventory. Data were digitalized and analyzed using frequency, percentages, means and standard deviations, Wilcoxon rank sum test, Chi-square test, Fisher's exact test, and multiple logistic regression with SAS 9.0. Results: Mean total sleep time was 6.6 hours on weekdays, 8.1 hours on weekends. Mean sleep latency was 19.1 minutes and the score for mean sleep quality was 22.6. Prevalence of EDS was 12.5%. Depression was significantly different between EDS and Non-EDS students (t=2.17, p=.030). Multiple logistic regression showed that the only factor associated with EDS was depression (adjusted odds ratio of depression=5.33, 95% Confidence Interval=1.49-19.04). Conclusion: Results of this study indicate that university students experience short sleep time, low sleep quality, and common EDS with depression, suggesting that students with complaints of EDS should be completely assessed for depression as well as sleep problems.

  • Background: This cross-sectional study assessed the sleep quality using the ActiGraph and investigated the relationship between the parameters of sleep assessment and the type of shift work in Korean firefighters. Methods: The participants were 359 firefighters: 65 day workers (control group) and 294 shift workers (shift work group: 77 firefighters with 3-day shift, 72 firefighters with 6-day shift, 65 firefighters with 9-day shift, and 80 firefighters with 21-day shift). Sleep assessments were performed using the ActiGraph (wGT3X-BT) for 24 hours during day shift (control and shift work group) and night shift and rest day (shift work group). The participants recorded bed time and sleep hours during the measurement period. Results: Sleep efficiency, total sleep time, and percentage of wake after sleep onset during night work were lower in the shift work group than control group (p < 0.05). Sleep efficiency decreased in night shift and increased in rest day, whereas wake after sleep onset increased in night shift and decreased in rest day (p < 0.05). Among shift work groups, sleep efficiency of 6-day shift was higher in day shift, and sleep efficiency of 21-day shift was lower in night shift than other shift groups (p < 0.05). Conclusion: We found that the sleep quality in night shift of the shift work group was poorer than the control group. As to the type of shift work, sleep quality was good in 6-day shift and poor in 21-day shift. Thus, fast rotating shift such as 6-day shift may be recommended to improve the sleep quality of the firefighters.

  • Background: There is a lack of statistical analysis investigating the relationship between sleep problems and commute time in Korea. We aimed to analyze the association between representative health symptoms, sleep disturbances, and commute time according to working hours in Korea. Methods: The 4th Korean Working Conditions Survey data were used for analysis, and unpaid family workers and workers who work fewer than three days in a week were excluded. Commute time, working hours, and sleep hours were assessed using self-reported questionnaires. Odds ratios (ORs) with 95% confidence intervals (CIs) for sleep problems were calculated using a multivariate logistic regression model with ≤10 min commute time as the reference group. Results: Among a total of 28,804 workers (men = 14,945, women = 13,859), 2.6% of men and 3.2% of women experienced sleep problems. In both sexes, long commute time (51-60 minutes and >60 minutes) showed an increased OR [men, 2.03 (CI = 1.32-3.13) and 2.05 (CI = 1.33-3.17); women, 1.58 (CI = 1.05-2.39) and 1.63 (CI = 1.06-2.50), respectively]. In stratification analysis of working hours, long commute time (51-60 and > 60 minutes) showed an increased OR in men working >40 hours/week [2.08 (CI = 1.16-3.71) and 1.92 (CI = 1.08-3.41), respectively]. Furthermore, long commute time (41-50, 51-60, and >60 minutes) showed an increased OR in women working >40 hours/week [2.40 (CI = 1.27-4.55), 2.28 (CI = 1.25-4.16), and 2.19 (CI = 1.17-4.16), respectively]. Moreover, commute time >60 minutes showed an increased OR in women working ≤40 hours/week [1.96 (CI = 1.06-3.62)]. Conclusion: This large cross-sectional study highlights that long commute time is related to sleep problems in both sexes. Shorter commute times and decreased working hours are needed to prevent sleep problems in workers.

  • Purpose: The purpose of this study was to investigate sleep quality in older adults in nursing home with objective data collection. Methods: Participants included 74 older adults in nursing homes in Korea aged 65 years or above. Data were collected using a wearable device (Fitbit), salivary melatonin level and Sleep Disorder Inventory (SDI). The Pearson correlation coefficient was calculated to examine whether there was any correlation between sleep-related variables such as Total Sleep Time (TST), Rapid Eye Movement (REM) sleep, shallow sleep, deep sleep, salivary melatonin level and SDI. Results: There were distortion of sleep structure, as TST comprised short REM sleep (15.93±5.47%), long shallow sleep (74.18±8.08%) and short deep sleep (9.89±5.03%). Also, salivary melatonin levels were low (15.06±7.77 pg/mL). Moreover, we found than melatonin was significantly associated with TST (r = .251, p= .044), REM sleep (r= .294, p= .020) and deep sleep (r= .391, p= .002). But there was no correlation between SDI and other sleeprelated variables. Conclusion: These findings highlight that insufficient sleep structure is associated with the salivary melatonin level among older adults in nursing home. We suggest developing programs to promote sleep quality of older adults in nursing homes.

  • The effects of the timing of daily bathing on sleep in winter were studied. Eight healthy male subjects were assigned to three sleep conditions: bathing just before sleeping (Condition J), bathing 2 h before sleeping (Condition T0 and no bathing before sleeping (Control). We can found that slow wave sleep and REM sleep were increased, and sleep onset latency and wake after sleep onset were shortened in Condition T compared with Condition J. Rectal and mean skin temperatures n both bathing conditions were the same levels after the first half of sleep. Furthermore, subjective sleep sensation was the highest value in Condition T. These results suggest that bathing done before going to bed in winter was good for sleep; moreover, bathing 2 h before going to bed was more effective than bathing immediately before going to bed.

  • Sinomenium acutum has been long used in the preparations of traditional medicine in Japan, China and Korea for the treatment of various disorders including rheumatism, fever, pulmonary diseases and mood disorders. Recently, it was reported that Sinomenium acutum, has sedative and anxiolytic effects mediated by GABA-ergic systems. These experiments were performed to investigate whether sinomenine (SIN), an alkaloid derived from Sinomenium acutum enhances pentobarbital-induced sleep via ${\gamma}$-aminobutyric acid (GABA)-ergic systems, and modulates sleep architecture in mice. Oral administration of SIN (40 mg/kg) markedly reduced spontaneous locomotor activity, similar to diazepam (a benzodiazepine agonist) in mice. SIN shortened sleep latency, and increased total sleep time in a dose-dependent manner when co-administrated with pentobarbital (42 mg/kg, i.p.). SIN also increased the number of sleeping mice and total sleep time by concomitant administration with the sub-hypnotic dosage of pentobarbital (28 mg/kg, i.p.). SIN reduced the number of sleep-wake cycles, and increased total sleep time and non-rapid eye movement (NREM) sleep. In addition, SIN also increased chloride influx in the primary cultured hypothalamic neuronal cells. Furthermore, protein overexpression of glutamic acid decarboxylase ($GAD_{65/67}$) and $GABA_A$ receptor subunits by western blot were found, being activated by SIN. In conclusion, SIN augments pentobarbital-induced sleeping behaviors through $GABA_A$-ergic systems, and increased NREM sleep. It could be a candidate for the treatment of insomnia.
