  • Purpose : To describes the important aspects of the foot and ankle movement and function used when git and balance strategy. Method : The foot and ankle was a very important roles in the lower limb movement and gait. This study summarizes the physiologic movement of knee to the PNF lower extremity patterns. Result : The ankle joint composed of the talocural joint, the subtalarl joint, transverse tarsal joint, talocalcaneonavicular joint. The onset of dorsiflexion muscle activity starts in pre swing gait patterns. First contract muscle is the extensor hallucis. Activity of tibialis anterior and extensor digitorum longus quickly follows in mid swing gait phase. During stance phase, the soleus and gastrocnemius muscle provided plantar flexor torque, which muscle reacts quickly to restrain ankle dorsiflexion, and contributes modulated control of the ankle motion in gait patterns. Conclusions : The understanding of ankle kinematics, could provide a good therapeutic approach for improving gait patterns in patients with various pathological condition.

  • Background: The purpose of this study was to investigate the changes of trunk and lower extremity muscle activity according to the compensation of arm during bridge movement of healthy subject. Design: Cross-sectional Study. Methods: Twenty healthy subjects participated in this study. The subjects performed bridge exercise with 3 different arm positions(arm abduction 45°, 90° and cross-arms) and measured the muscle activity of the trunk and lower extrimity. During bridge exercise with 3 different arm positions, trunk (rectus abdominis, erector spinae) and lower extrimity muscle activity (gluteus medius, biceps femoris, tibialis anterior) were measured using wireless surface EMG. Results: Rectus abdominis and gluteus medius muscle were most activated during bridge exercise with arm abduction 90° and erector spinae and biceps femoris muscle were most activated during bridge exercise with arm abduction 45°. In addition, tibialis anterior muscle was most activated during bridge exercise with arm cross. However, these difference in muscle activity according to the arm position was not statistically significant. Conclusion: As a result of this study, we think that the change in arm position does not induce sufficient instability to increase the muscle activity of the trunk and lower extremity muscles. Therefore, various approaches for inducing instability of the support surface for increasing muscle activity when applying bridge movement in clinical practice should be explored.

  • The purpose of this study was to analyze the effects of three different pelvic tilts on a sit-to-stand (STS) and to suggest a new assessment approach based on biomechanical analysis. The three difrent pelvic tilts were: (1) comfortable pelvic tilt sit-to-stand (CPT STS), (2) posterior pelvic tilt sit-to-stand (PPT STS) and (3) anterior pelvic tilt sit-to-stand (APT STS). To determine the onset time of muscle contraction surface electrodes were applied to the rectus femoris muscle (RF), vastus lateralis muscle (VL), biceps femoris muscle (BF), tibialis anterior muscle (TA), gastrocnemius muscle (GCM), and soleus muscle (SOL). The ICC was used for functional linkage analysis. The findings of this study were as follows. First, significant differences were found in kinematic variables and in muscle activation pattern among the three activities. Second, the results of functional integrated analysis revealed that recruited muscle activation patterns changed when the thigh-off was viewed as a reference point. Third, there were independent functional units between the thigh-off and the VL and between the thigh-off and the RF in the functional linkage analysis. The VL and RF acted as prime mover muscles, and more postural adjustment muscle recruitment was required as the demand of postural muscle control increased (PPT STS, APT STS, and CPT STS in order). In conclusion, the findings of this study suggest the following evaluative and therapeutic approach for STS activity. APT STS can be introduced for movement efficiency and functional advantage when abnormal STS is treated. However, excessive APT would change the muscle activation patterns of BF and SOL and require additional postural muscle control to cause abnormal control patterns.

  • Purpose: The purpose of this study was to investigate the change of muscle activities during level walking, stairs and ramp climbing in hemiplegic patients. Methods: Eight hemiplegic patients were recruited and agreed to participate in this study. Muscle activity was measured by MP100 system (BIOPAC System Inc., Santa Barbara, CA, USA). Statistical analysis was used as a one-way repeated measure of ANOVA to know the difference according to the gait conditions (level walking, stairs and ramp ascending). Results: In the swing phase, muscle activity of rectus femoris muscle, with the side lower extremities affected, were generally significantly different in the stair and ramp ascending. In addition, biceps femoris muscle with unaffected side lower extremity was generally significantly different in the ramp ascending. In the swing phase, muscle activity of tibialis anterior muscle with unaffected side lower extremities was generally showed a significant difference in the ramp ascending. In the stance phase, climbing stairs and ramps showed an increase in the muscle activities. Further, climbing the stairs increased muscle activities of the gastrocnemius muscle. Conclusion: These findings indicate that compared with the level walking climbing stairs, ramps and muscle activities of lower extremity during each showed different results. It can be seen that in accordance with the terms of gait are different muscles group recruitment.

  • Purpose: This study investigated whether the strength imbalance between two muscles can affect the score of EMG based biofeedback game, and whether the EMG based biofeedback game score can be used as predictable indicator of the degree of muscle balance alternating the conventional strength measuring equipment. Methods: 40 participated in this study. Biodex was used to measure the peak torque/weight in order to calculate the muscle strength balance index between plantar flexor and dorsiflexor of ankle joint. And muscle balance index (MBI) was calculated. The EMG biofeedback game scores of dorsiflexor and plantar flexor were acquired, so that the EMG electrodes were attached at tibialis anterior and gastrocnemius. The relationship between the game score and the muscle balance index were analyzed. Results: There was negative correlation between the muscle balance index between plantar flexor and dorsiflexor and the peak torque/weight of plantar flexor (r=-0.70). And there was negative correlation between the muscle balance index between plantar flexor and dorsiflexor and the game score of plantar flexor (r=-0.83). Conclusion: The EMG biofeedback game score had significant relationship with muscle imbalance at ankle joint, so it seems that the game score can be used for predicting the degree of muscle imbalance as a parameter.
