경구강 CO2 레이저를 이용하여 치료한 설갑상설관 낭종 1예 (A Case of Lingual Thyroglossal Duct Cyst Treated by CO2 Laser via Transoral Approach)

  • A lingual thyroglossal duct cyst(LTGDC) is a rare congenital anomaly that account for only 0.5% to 2% of total thyroglossal duct cyst. LTGDC is frequently associated with respiratory problem in infants and pharyngeal foreign body sensation or dysphagia in adults. Because of its location and characteristics, lingual thyroid, dermoid cyst, and vallecular cyst should be included in differential diagnosis. Standard treatment for thyroglossal duct cyst is sistrunk's operation, but in terms of LTGDC, because of its location and cosmetic reasons, different kinds of treatments such as electrical cauterization, $CO_2$ laser, Robort surgery via transoral approach have been introduced. Recently authors encountered 21 years old woman with LTGDC and the mass was removed successfully via transoral approach using $CO_2$ laser. We report the clinical course with review of the literature.

제 4 새성 기형 1예 (A Case of Fourth Branchial Cleft Cyst)

  • The branchial anomaly is a lateral neck mass commonly seen by otolaryngologists. Depending on its anatomic location, branchial anomaly can be classified into first, second, third and fourth. The fourth branchial cleft anomaly is very rare entity and until now, only 35cases have been reported worldwide. It may present as neck cyst, recurrent neck abscess, thyroiditis. Combined with barium swallow esophagogram and computed tomography scan can aid in diagnosis of this rare disease entity. Complete excision of the entire epithelial tract combined with ipsilateral thyroid lobectomy remains the mainstay of treatment. Authors experienced a case of lateral neck mass which was anatomically presumed to be the fourth branchial cleft cyst. We report this case with the related literature.

악성 및 양성 갑상선 질환의 조직에서 면역조직학적 검사법에 의한 $Na^+/I^-$ symporter의 발현율 비교 (Comparison of $Na^+/I^-$ Symporter Expression Rate in Malignant and Benign Thyroid Diseases: Immunohistochemical Study)

  • 목적 : 기존의 연구는 갑상선 악성 종양에서 NIS 단백질 발현에 대해 충분한 증례에서 일관성 있는 결과를 보이지 못하고 있다. 이에 본 연구에서는 원발성 갑상선 악성 종양의 조직에서 NIS 발현율의 분포를 알아보고, 이를 양성 갑상선 질환에서의 NIS 발현율의 분포와 비교하였다. 대상 및 방법 : 악성 갑상선 종양이나 양성 갑상선 질환으로 수술을 시행한 환자들을 대상으로 후향적 분석을 하였다. 환자들은 총 119명(남자 15명)이며. 나이는 $48{\pm}3$세(범위 20-75세) 였다. 이들에게서 얻은 조직 표본은 총 205개 였다. 이 표본들은 악성 질환(총 153개)으로 유두상 종양이 90개, 여포상 종양이 4개, 수질상 종양이 2개, 전이성 임파절이 57개 였으며, 양성 질환(총 52개)으로 갑상선 종대가 36개, 갑상선염이 11개, 여포선종이 5개였다. 단클론 항체인 mouse anti-NIS Ab를 사용하였다. 염색된 조직을 현미경으로 관찰하여 양성 등급은 염색 강도에 따라 약하게 염색되면 등급 1, 중등도로 염색되면 등급 2, 강하게 염색되면 등급 3으로 나누었으며, 음성인 경우는 등급 0으로 표시하였다. 개개의 질환에 있어서 NIS의 발현율은 전체의 개수에 대하여 등급 2와 3이 차지하는 비율을 백분율로 표시하였다. 결과: 갑상선 악성 질환에서 NIS의 발현율은 유두상 종양의 원발성 종양에서 63%, 전이성 임파선에서 81%를 보였다. 여포상 종양과 수질상 종양의 NIS의 발현율은 각각 71, 100% 였다. 갑상선 양성 질환에서 NIS의 발현율은 갑상선 종대에서 53%, 갑상선염에서 64%, 여포선종에서 40%였다. NIS의 발현율은 악성 종양에서 양성 질환보다 더 높았다(71% vs 54%) 증례수가 많은 유두상 종양과 갑상선 종대의 경우에 한쪽 등급에만 치우침이 없이 모든 등급에 걸쳐 분포를 보이는 특징을 보였다. 갑상선 질환을 보이는 병변 조직의 주위에 있는 정상 조직의 염색 정도는 다양한 등급을 보여서 일관성을 보이지는 않았으며, 병변조직의 염색 정도와도 연관성을 보이지 않았다. 결론: 갑상선 유두상 종양에서 면역조직화학검사에 기초한 NIS의 발현율은 불균질성 분포를 나타내며 증가하는 양상을 보였고, 악성 질환과 양성 질환 사이에 NIS의 발현율이 큰 차이를 보이지 않았다.

경부 피하조직내에 발생한 기관지성 낭종 (Cervical Bronchogenic Cyst)

  • Bronchogenic cysts are thought to be uncommon developmental anomalies. They develop from small buds or diverticuli that separate from the foregut in the formation of the tracheo-bronchial tree. They are nearly always located near the tracheal bifurcation; However, these lesions can occur anywhere along the tracheo-bronchial tree. We experienced a case of cervical bronchogenic cyst presenting sorely as an anterior neck mass in a 26-year-old woman. The neck ultrasonography showed as $1.4{\times}1.1cm$ sized hypoechoic lesion with a well-defined margin on the isthmic portion of the thyroid gland. Excision of the mass was carried out. The mass was superfical to the strap muscle and was contained within the subcutaneous tissue in the midline without any connection to the trachea. Grossly, the mass was an oval-shaped cystic lesion which measured 1.5 cm in the greatest diameter. The cyst was filled with thick, yellow, jelly-like material and the inner surface was smooth and glistening. Microscopically, the cyst showed a lining of ciliated columnar epithelium, beneath which was a loose areolar stroma containing plaques of mucous glands and mature cartilage. We thought this cervical bronchogenic cyst appeared to represent an expression of complete aberrent accessory lung bud detachment from the primitive foregut.

경부 낭종의 형태로 발현한 전이암 2례 (Two Cases of Metastatic Cancer Presenting as A Cervical Cyst)

  • Metastatic cancer presenting as a cervical cyst is uncommon, and often misdiagnosed as branchial cleft cyst. Authors experienced two cases which presented clinically with features of branchial cleft cyst, but were subsequently found to be metastatic cancer. Radiologic examination and fine needle aspiration biopsy proved to be non-diagnostic, and pathologic findings after surgical excision showed metastatic cancer. Further evaluation and examination were made to find out the primary focus, which revealed tonsillar squamous cell cancer and thyroid papillary cancer in each case. When cervical cysts are noted in aged patients, it is mandatory to rule out metastatic cancer until it is proven otherwise. Surgical excision and pathologic diagnosis should be always accompanied to make correct diagnosis and further treatment.

  • Branchial cleft cysts typically are characterized as lateral swellings anterior to sternocleidomastoid muscle in upper third of the neck. However, cysts have been reported in unusual location such as the thymus, oral cavity, parotid gland, pancreas, and thyroid. Perithyroidal branchial cleft cysts are also rare and preoperative diagnosis is very difficult. Recently we have experienced a case of intrathyroidal branchial cleft cys and a case of perithyroidal branchial cleft cyst, which were diagnosed preoperatively as the parathyroid cyst. So, we report these two cases with review of the literatures.

갑상선 및 부갑상선 수술시 배액관 삽입술에 대한 검토 (Thyroid and Parathyroid Surgery without Wound Drains)

  • Traditionally, wound drainage after thyroid or parathyroid surgery has been widely used to prevent airway obstruction due to accumulation of hematoma or seroma within the paratracheal dead space. Recently, however, the routine use of drains after thyroid or parathyroid surgery has become a matter of controversy. To determine whether the rouine use of drains after thyroid or parathyroid surgery is warranted, a prospective study on the complications after various types of thyroid or parathyroid surgery without wound drains was conducted. Three hunded sixty-six consecutive patients underwent thyroid or parathyorid surgeries by one surgeon from January through December 1994 were included in this study. Of these, only 38 patients (10.4%) required the wound drains. Indications for drainage included the patients with a large dead space(n=9) or wet operative field at the conclusion of surgery(n=11), and patients with radical neck disection(n=18). In the remaining 328 patients(89.6%), the wounds were closed without drains after thyroid lobectomy and isthmusectomy(n=226), bilateral subtotal thyroidectomy(n=21), total or near-total thyroidectomy(n=62), isthmusectomy(n=9) and parathyroid surgery(n=l0). Histologic findings revealed benign tumors in 214(65.2%), carcinoma in 89(27.1%), Graves' disease in 15(4.7%), hyperparathyroidism in 7(2.1%) and parathyroid cyst in 3(0.9%). Among the 328 patients without drain used, wound related complications were seen in only 15 patients(4.6%); 12 patients with seroma and 3 patients with hematoma. All but one complications could be controlled by two or three aspirations, and the remaining one patient required re-exploration. There were no instances of laryngeal nerve palsy or wound infection. The mean length of hospital stay after surgery was 2.8 days with a range of 1 to 11 days. These results support the routine use of drains is not warranted in most thyroid or parathyroid surgeries.

기도협착을 유발한 흉선낭종 (Thymic Cyst Causing Tracheal Stenosis : one case report)

  • Thymic cysts are uncommon tumors which usually occur in the neck and mediastinum. It is known to arise from embryonic remnants of the thymopharyngeal duct or from inflammation of thymic tissues. Patients with thymic cyst are often asymptomatic and identified after surgical removal and histologic examination. We experienced a 73 year-old man with recently developed dyspnea. During the examination, chest CT showed a $5\times6cm$ sized cystic mass causing deviation of the trachea. It was located in between the right thyroid gland and anterior mediastinum. It also caused tracheal narrowing noted by bronchoscopy. Right anterior cervical incision and removal of the mass was performed and a histological diagnosis of thymic cyst was confirmed. The patient was discharged without complication.

소아에서의 갑상선설관낭종의 임상적 고찰 (Thyroglossal Duct Lesions in Childhood)

  • Thirty-four consecutive cases of thyroglossal duct lesions in children were reviewed at the Department of Surgery, Chonbuk National University Hospital. Twenty patients were males, and the most prevalent age of discovering was 2 to 4 years (52.9%). Resection was performed within 2 years after discovering the lesions in 19 cases, but was delayed untill 4 to 10 years in 6 cases. Cystic lesions (85.3%) were 5.8 times more common than fistulas. A midline upper neck mass was found in every cystic cases, and a draining sinus at hyoid region was noticed in fistula patients. The location of the lesion was on the hyoid bone in 34 cases, at the midline in 31 cases, slightly to the left in 2 cases, and slightly to the right in 1. Two cases were misdiagnosed as lymphadenopathy, and a single case of ectopic thyroid gland was misinterpreted as a thyroglossal duct cyst. Modified Sistrunk operation was performed in twenty-three cases(67.6%), Sistrunk operation in 9(26.5%), and cyst excision in 2(5.9%). Postoperative complications occurred in 4 cases(11.8%); 2 wound infections and 2 recurrences. One of recurrences was a fistula treated by modified Sistrunk operation, and the other was a cyst treated by cyst excision.
